In the Colombian    Caribbean region, fish contamination with methyl-mercury has been found. </p>     <p>Key Words: omega-3 fatty acids, ventricular arrhythmia, fish    oil, sudden death, methyl-mercury. </p> <hr size="1">     <p>La historia sobre la importancia de los &aacute;cidos grasos    poliinsaturados omega (w)-3, el &aacute;cido eicosapentanoico (EPA) y el &aacute;cido    docosahexanoico (DHA) se report&oacute; mucho tiempo atr&aacute;s; de hecho,    se conoce que los habitantes de la zona &aacute;rtica presentan escasas enfermedades    de las arterias coronarias, a pesar de su dieta rica en grasa. Posteriormente,    se demostr&oacute; que el efecto positivo se deb&iacute;a a su dieta rica en    omega-3, y es as&iacute; como su manejo se convierte en una alternativa coadyuvante    para disminuir las enfermedades cardiovasculares (1-3). </p>     <p>El por qu&eacute; estos aceites de pescados ricos en omega-3,    previenen las enfermedades coronarias, ha sido delineado en cientos de experimentos    animales y c&eacute;lulas cultivadas y ensayos cl&iacute;nicos y poblacionales    (4-6).</p>     <p>Todos estos recientes estudios correlacionan la ingesti&oacute;n    de &aacute;cidos omega-3 con un coraz&oacute;n sano. &iquest;Pero con qu&eacute;    frecuencia se deben ingerir, qu&eacute; cantidad y cu&aacute;l origen alimentario    es adecuado para obtener dicho efecto cardioprotector? Esto a&uacute;n es confuso    y son pocas las investigaciones que evidencian estos factores positivos. De    otro lado, cabe preguntarse si los peces de Colombia poseen la cantidad suficiente    de omega-3 y las caracter&iacute;sticas fisicoqu&iacute;micas correctas para    una disposici&oacute;n fisiol&oacute;gica efectiva que conlleve a disminuir    el riesgo de muerte s&uacute;bita cardiaca y mantener sano el coraz&oacute;n.</p>     <p>Los omega-3 son &aacute;cidos grasos poliinsaturados de cadena larga que no    pueden ser sintetizados por el organismo, por lo que se denominan esenciales    (7, 8). La caracter&iacute;stica bioqu&iacute;mica que distingue a los &aacute;cidos    grasos esenciales, es que presentan un doble enlace dentro de los &uacute;ltimos    siete carbonos de la cadena carbonada, a partir del grupo metilo terminal, ocupando    para este caso la posici&oacute;n 3 (serie n-3 u omega 3, en cuya familia se    destacan el &aacute;cido a linol&eacute;nico (acido D9,12,15 octadecatrienoico)    (9, 10) (<a href="#figura1">Figura 1</a>).</p>     <p>    <center><a name="figura1" id="figura1"><img src="/img/revistas/rcca/v13n3/a7f1.jpg"></a></center></p>     ]]></body>
<body><![CDATA[<p>Los &aacute;cidos grasos de la serie omega 3, son constituyentes    de los aceites de pescado y se encuenntran en mayor concentraci&oacute;n en    los &aacute;cidos eicosapentaenoico (EPA, C20:5, n-3), conocido tambi&eacute;n    como &aacute;cido timnod&oacute;nico, y en el docosahexaenoico (DHA, C22:6,    n-3), tambi&eacute;n denominado &aacute;cido cerv&oacute;nico o clupanod&oacute;nico;    ambos se pueden sintetizar a partir del &aacute;cido linol&eacute;nico (11).    Los &aacute;cidos grasos omega 3 cumplen una funci&oacute;n vasodilatadora y    reguladora de la pared de los vasos sangu&iacute;neos, lo cual los convierte    en potenciadores de la funci&oacute;n endotelial. Esto se explica por la producci&oacute;n    de PGI3 y TXA3 que favorece la vasodilataci&oacute;n y la anti-agregaci&oacute;n    plaquetaria (<a href="#figura2">Figura 2</a>). </p>     <p>    <center><a name="figura2" id="figura2"><img src="/img/revistas/rcca/v13n3/a7f2.jpg"></a></center></p>     <p>El EPA se acumula principalmente en el tejido adiposo, mientras    que el 90% o m&aacute;s del DHA, constituye el tejido nervioso y el cerebro    de los peces, en la forma de fosfatidil serina y fosfatidil etanolamina. De    esta manera, ejemplares como el at&uacute;n, la caballa, la sardina, el jurel,    el salm&oacute;n y la anchoa constituyen importantes fuentes de EPA y DHA, consumidos    como tal o a trav&eacute;s de los productos de su industrializaci&oacute;n (harina    y aceite principalmente) (11).</p>     <p>Cabe resaltar que esta acci&oacute;n potenciadora de la funci&oacute;n    endotelial est&aacute; apoyada por diversos estudios experimentales, cl&iacute;nicos,    epidemiol&oacute;gicos y de cohorte, que han reportado los beneficios de una    dieta rica en &aacute;cidos grasos poliinsaturados. La Asociaci&oacute;n Americana    del Coraz&oacute;n reporta como mecanismos de cardioprotecci&oacute;n de los    &aacute;cidos grasos omega-3, las siguientes funciones coadyuvantes (12-14):</p>     <p>- Disminuci&oacute;n del riesgo de arritmias y por tanto de    muerte s&uacute;bita.    <br>   - Disminuci&oacute;n del riesgo de trombosis, lo que conduce a contrarrestar    accidente cerebro-vascular.     <br>   - Reducci&oacute;n de triglic&eacute;ridos y lipoprote&iacute;nas LDL circulantes.    <br>   - Disminuci&oacute;n del engrosamiento de la placa ateroscler&oacute;tica.    <br>   - Mejor&iacute;a de la funci&oacute;n endotelial.    ]]></body>
<body><![CDATA[<br>   - Reducci&oacute;n de la presi&oacute;n sangu&iacute;nea.    <br>   - Aumento del tiempo de coagulaci&oacute;n.    <br>   - Disminuci&oacute;n de la quimotaxis de leucocitos polimorfos nucleares.    <br>   - Reducci&oacute;n de la respuesta inflamatoria.</p>     <p> M&uacute;ltiples estudios cl&iacute;nicos en animales y experimentales,    muestran que los &aacute;cidos grasos poliinsturados omega-3, disminuyen el    riesgo de arritmias y por ende de muerte s&uacute;bita, a trav&eacute;s de la    modificaci&oacute;n de los canales i&oacute;nicos de calcio y sodio, los cuales    cambian el potencial de acci&oacute;n del coraz&oacute;n. Esto conlleva una    disminuci&oacute;n de la vulnerabilidad del miocardio y baja el riesgo de fibrilaci&oacute;n    ventricular. Los estudios realizados en animales experimentales, tambi&eacute;n    reportan que las dietas enriquecidas con omega-3 durante tres meses versus dieta    saturada, disminuyen la presencia de fibrilaci&oacute;n ventricular (15-20).    Ser&iacute;a preciso agregar, que la trombosis es una importante complicaci&oacute;n    de la aterosclerosis coronaria que conlleva al infarto del miocardio. </p>     <p>Los &aacute;cidos omega-3 de aceites de pescado, tienen alta    acci&oacute;n antitromb&oacute;tica ya que el EPA inhibe la s&iacute;ntesis    de tromboxanos A2 a partir del &aacute;cido araquid&oacute;nico en la placa;    los tromboxanos A2 causan agregaci&oacute;n plaquetaria y vasoconstricci&oacute;n,    por tanto la ingesti&oacute;n de omega 3 mejora la anti-agregaci&oacute;n plaquetaria    y disminuye el riesgo de trombosis, adem&aacute;s el TXA3, posee menor actividad    biol&oacute;gica, formada desde EPA celular en plaquetas humanas (21).</p>     <p> Es de resaltar que el mayor metabolito urinario end&oacute;geno    de PGI3, activo vasodilatador inhibidor de la agregaci&oacute;n plaquetaria,    es excretado en individuos que han ingerido aceite de h&iacute;gado de bacalao.    Estos resultados demuestran que las c&eacute;lulas endoteliales responsables    de la formaci&oacute;n de PGI3, f&aacute;cilmente convierten EPA a PGI3 y que    los omega-3 no alteran la liberaci&oacute;n y ciclooxigenaci&oacute;n del &aacute;cido    araquid&oacute;nico a PGI2 (22, 23).</p>     <p> Los LTB5 sintetizados a partir de EPA, poseen 1/30 de la potencia    quimot&aacute;xica y agregatoria de LTB4. Se ha comprobado que los LTB5 se forman    f&aacute;cilmente a partir de EPA celular por leucocitos polimorfos nucleares    despu&eacute;s de una suplementaci&oacute;n con aceite de h&iacute;gado de bacalao,    aumentando su concentraci&oacute;n. En contraste, la formaci&oacute;n de LTB4    desde &aacute;cido araquid&oacute;nico, no se ve alterada despu&eacute;s de    cuatro semanas de suplementaci&oacute;n (22, 23).</p>     <p> Pero hay m&aacute;s; el EPA y DHA contenidos en el aceite    de pescado, inhiben el desarrollo de aterosclerosis por acciones de disminuci&oacute;n    en la concentraci&oacute;n de colesterol en plasma. Esta acci&oacute;n puede    asociarse con la inhibici&oacute;n de la migraci&oacute;n de monocitos dentro    de la placa, con lo cual reduce las citoquinas y la producci&oacute;n de interleukinas    a y adem&aacute;s la estimulaci&oacute;n en la producci&oacute;n de &oacute;xido    n&iacute;trico (24, 25).</p>     <p> No cabe duda de que los efectos del aceite de pescado en la    hiperlipidemia, est&aacute;n bien documentados en estudios que han implicado    dietas ricas en aceite de salm&oacute;n en contraste con dietas ricas en aceite    vegetal y dietas ricas en grasas saturadas. De esto se concluye que una dieta    rica en aceite de pescado omega-3, disminuye los triglic&eacute;ridos en plasma    porque inhibe la s&iacute;ntesis de los mismos y el VLDL en el h&iacute;gado.    Despu&eacute;s del consumo de aceite de pescado, tambi&eacute;n disminuye la    producci&oacute;n de apo B y aumenta la s&iacute;ntesis de fosfol&iacute;pidos    en la membrana celular (26, 27).</p>     ]]></body>
<body><![CDATA[<p> Por lo anterior, en nuestra opini&oacute;n, existe suficiente    soporte cient&iacute;fico que demuestra la importancia de la ingesti&oacute;n    de &aacute;cidos grasos omega-3 para aumentar los fosfol&iacute;pidos en plasma    y por consiguiente, disminuir el riesgo de muerte s&uacute;bita cardiaca. No    obstante, cuestionemos por qu&eacute; algunos estudios de cohorte no han observado    asociaci&oacute;n en la frecuencia de la ingesti&oacute;n de pescado y el riesgo    de enfermedad cardiovascular coronaria fatal o no fatal (Leaf et al) (19, 28,    29).</p>     <p> Bien, en principio pareciera, por todo lo expuesto, que el    consumo de &aacute;cidos grasos omega-3 ser&iacute;a el fin de todos los pesares    cardiacos. Pero con sano criterio, es preciso considerar que el contenido elevado    de metil-mercurio en los peces, contrarresta los efectos ben&eacute;ficos de    los &aacute;cidos grasos omega-3, disminuyendo as&iacute; su acci&oacute;n cardioprotectora.    El mercurio como metal de transici&oacute;n, promueve la formaci&oacute;n de    radicales libres mitocondriales, adem&aacute;s es un estimulador de la peroxidaci&oacute;n    lip&iacute;dica y un amante de los grupos tioles (30-33).</p>     <p>Estudios epidemiol&oacute;gicos revelan que una ingesti&oacute;n    de grasa de pescado a la semana, disminuye la muerte s&uacute;bita en 40% a    50%. Las recomendaciones del consumo de pescados ricos en &aacute;cidos grasos    omega-3 por la American Heart Association es de dos veces por semana y anotan    que fre&iacute;r el pescado a altas temperaturas disminuye la acci&oacute;n    omega-3 para lograr el efecto cardioprotector. Aunque existen estudios que muestran    efectos positivos para el coraz&oacute;n con bajas dosis de omega-3, nuestra    recomendaci&oacute;n es consumirlo por lo menos una vez por semana que es mejor    que nada, pero teniendo presente que sean pescados con buen contenido omega-3    y no relacionados con contaminantes ambientales como el mercurio (34-39).</p>     <p>Por tanto, puede ser que los estudios epidemiol&oacute;gicos    y otros que han reportado acciones no tan positivas con la ingesti&oacute;n    de pescado, lo hagan debido a las cantidades de metil-mercurio de dichos pescados,    que a nuestra manera de ver no se tienen en cuenta. Es as&iacute; como algunas    poblaciones consumidoras de pescado no presentan disminuci&oacute;n del riesgo    por enfermedad cardiovascular coronaria fatal. </p>     <p>Diversos estudios realizados por Olivero, indican que la contaminaci&oacute;n    por mercurio en peces en algunas regiones de la Costa Atl&aacute;ntica colombiana    ba&ntilde;ada por las aguas de los r&iacute;os Magdalena y Cauca, es elevada,    y concluye que, por ejemplo, en la Ci&eacute;naga Grande de Achi, ubicada en    el sur de Bol&iacute;var, las especies mojarra amarilla, moncholo y doncella    poseen concentraciones de mercurio superiores al l&iacute;mite m&aacute;ximo    internacional aceptado para especies de consumo humano (0,5 mg Hg/g) (WHO 1991)    (40). Por esta raz&oacute;n, nuestra hip&oacute;tesis es que el papel de los    &aacute;cidos grasos omega-3 de nuestros pescados, ser&iacute;a dudoso y no    existe correlaci&oacute;n del contenido de omega-3 versus el contenido de mercurio    de nuestros pescados y de su efecto cardioprotector. </p>     <p>La revisi&oacute;n de las Gu&iacute;as Alimentarias que en    el a&ntilde;o 2000 efectu&oacute; la Asociaci&oacute;n del Coraz&oacute;n de    los Estados Unidos (AHA), incluy&oacute; una recomendaci&oacute;n para que se    consumiera m&aacute;s pescado (como por ejemplo, at&uacute;n enlatado), por    los beneficios que aporta a la salud cardiaca. Esta gu&iacute;a tambi&eacute;n    menciona los efectos ben&eacute;ficos que tienen los &aacute;cidos grasos omega-3    que contiene el at&uacute;n fresco y enlatado, y el salm&oacute;n, en aquellas    personas que padecen enfermedades inflamatorias y autoinmunes.</p>     <p>En los pescados tambi&eacute;n existe gran variabilidad, por    ello hay especies magras como la merluza, el congrio y la corvina, y especies    con mayor contenido graso como el salm&oacute;n, la trucha y el jurel entre    otros. Los pescados ricos en grasas tienen la ventaja de ser una fuente importante    de &aacute;cidos grasos omega-3. Por sus efectos en el organismo, un consumo    adecuado y frecuente de pescados ricos en &aacute;cidos grasos omega-3, contribuye    a la prevenci&oacute;n de enfermedades cr&oacute;nicas, especialmente de las    cardiovasculares.</p>     <p>El pescado es un alimento muy poco presente en nuestra dieta.    Otros pa&iacute;ses, como por ejemplo Espa&ntilde;a, se sit&uacute;an en los    primeros puestos en el consumo mundial de productos pesqueros, justo detr&aacute;s    de Jap&oacute;n y Portugal. Este alimento posee indudables ventajas: es apto    para todas las edades, su consumo contribuye a prevenir enfermedades y su aporte    cal&oacute;rico es, en general, moderado.</p>     <p>Pr&aacute;cticamente, todos los pescados y mariscos contienen    rastros de mercurio, por lo tanto, las personas que consumen pescado quedan    expuestas al metilmercurio. Pese a que el consumo de pescado no causa un problema    grave a la salud, los altos niveles de mercurio en el torrente sangu&iacute;neo    pueden tener un efecto nocivo.    <br>   Finalmente, es necesario considerar la calidad de los peces de nuestro pa&iacute;s    y fomentar pol&iacute;ticas para lograr disminuir las concentraciones de mercurio    y otros contaminantes, y con lo anterior obtener los beneficios de los &aacute;cidos    omega-3 en el sistema cardiovascular.</p>     ]]></body>
