Both cases were evaluated by three-dimensional transesophageal echocardiography in real time.</p>     <p> <i><b>Keywords</b></i>: transesophageal echocardiography, thrombosis, prosthetic heart valves.</p> <hr size="1">   <font size="3" face="Verdana"><b>Introducci&oacute;n</b></font>     <p> La trombosis de las pr&oacute;tesis valvulares biol&oacute;gicas es un evento infrecuente pero con mortalidad elevada. Su diagn&oacute;stico representa un reto para los cl&iacute;nicos; no obstante, la disponibilidad de tecnolog&iacute;a como la ecocardiograf&iacute;a transesof&aacute;gica tridimensional, se vislumbra como una nueva herramienta para la valoraci&oacute;n y el diagn&oacute;stico de la disfunci&oacute;n de v&aacute;lvulas prot&eacute;sicas. </p>   <b><font size="3" face="Verdana">Primer caso</font>   </b>     <p>Paciente de g&eacute;nero masculino, de 72 a&ntilde;os, con antecedente de implantaci&oacute;n de pr&oacute;tesis valvular biol&oacute;gica en posici&oacute;n a&oacute;rtica hace dos a&ntilde;os, quien ingres&oacute; al servicio de urgencias por dolor tor&aacute;cico; al examen f&iacute;sico se encontr&oacute; hipotensi&oacute;n y taquicardia. Se realiz&oacute; ecocardiograf&iacute;a transesof&aacute;gica que document&oacute; trombosis de la pr&oacute;tesis biol&oacute;gica en posici&oacute;n a&oacute;rtica con gradientes elevados. Fue llevado a cirug&iacute;a de emergencia y se realiz&oacute; un reemplazo exitoso de la v&aacute;lvula (Figuras <a href="#figura1">1</a> y <a href="#figura2">2</a>).</p>       <p>    <center><a name="figura1"></a>    <br>   <img src="img/revistas/rcca/v18n5/v18n5a10f1.gif"></center></p>         ]]></body>
<body><![CDATA[<p>    <center><a name="figura2"></a>    <br>   <img src="img/revistas/rcca/v18n5/v18n5a10f2.gif"></center></p>  <font size="3" face="Verdana"><b>Segundo caso</b></font>     <p>Paciente de g&eacute;nero femenino, de 64 a&ntilde;os de edad, con antecedente de implantaci&oacute;n de pr&oacute;tesis valvular biol&oacute;gica en posici&oacute;n mitral hace cinco a&ntilde;os por valvulopat&iacute;a reum&aacute;tica. Ingres&oacute; remitida de un hospital de segundo nivel por cinco d&iacute;as de evoluci&oacute;n de tos con expectoraci&oacute;n mucopurulenta, fiebre, malestar general, mialgias, dolor en hemit&oacute;rax izquierdo y disnea progresiva; posteriormente sufri&oacute; deterioro progresivo de su condici&oacute;n cl&iacute;nica, con hipotensi&oacute;n y requerimientos de soporte inotr&oacute;pico, vasopresor y ventilatorio. Se diagnostic&oacute; shock s&eacute;ptico secundario a neumon&iacute;a adquirida en la comunidad. Evolucion&oacute; a falla org&aacute;nica m&uacute;ltiple y edema pulmonar, que se consider&oacute; de origen cardiog&eacute;nico. Durante su valoraci&oacute;n se realiz&oacute; ecocardiograf&iacute;a transesof&aacute;gica que mostr&oacute; trombosis de la v&aacute;lvula biol&oacute;gica; el trombo compromet&iacute;a ambas valvas con una prolongaci&oacute;n hacia la aur&iacute;cula con &aacute;rea mayor a 5 cm2 e incremento severo de los gradientes. En vista del estado cl&iacute;nico previo y en com&uacute;n acuerdo con la familia, no se realiz&oacute; intervenci&oacute;n quir&uacute;rgica. La paciente falleci&oacute; veinticuatro horas despu&eacute;s de su ingreso (Figuras <a href="#figura3">3</a>, <a href="#figura4">4</a> y <a href="#figura5">5</a>).</p>       <p>    <center><a name="figura3"></a>    <br>   <img src="img/revistas/rcca/v18n5/v18n5a10f3.gif"></center></p>       <p>    <center><a name="figura4"></a>    <br>   <img src="img/revistas/rcca/v18n5/v18n5a10f4.gif"></center></p>       ]]></body>
<body><![CDATA[<p>    <center><a name="figura5"></a>    <br>   <img src="img/revistas/rcca/v18n5/v18n5a10f5.gif"></center></p> <font size="3" face="Verdana"><b>Discusi&oacute;n</b></font>     <p>Existen varias causas de disfunci&oacute;n prot&eacute;sica de las v&aacute;lvulas biol&oacute;gicas y de &eacute;stas, el deterioro estructural intr&iacute;nseco es la m&aacute;s frecuente. Otras causas incluyen endocarditis infecciosa, formaci&oacute;n de pannus y trombosis. La trombosis de las v&aacute;lvulas biol&oacute;gicas es una entidad sumamente infrecuente, con una incidencia de 0,034% a 0,15% por a&ntilde;o (1, 2). En la mayor&iacute;a de los casos se presenta en los tres primeros meses pos-implantaci&oacute;n. La posici&oacute;n mitral, los episodios tromb&oacute;ticos previos y la suspensi&oacute;n del tratamiento anticoagulante, se consideran factores que predisponen a su aparici&oacute;n (3). Igualmente, la trombocitopenia inducida por heparina se ha asociado con la trombosis temprana de la pr&oacute;tesis biol&oacute;gica (4, 5). </p>       <p>A fin de prevenir esta complicaci&oacute;n las gu&iacute;as actuales recomiendan anticoagular durante los primeros tres meses luego de la implantaci&oacute;n de una v&aacute;lvula prot&eacute;sica biol&oacute;gica (6). De igual forma, el compromiso infeccioso puede crear el sustrato adecuado para la trombosis; por ejemplo, la serie reportada por Shapira y colaboradores (7) document&oacute; hallazgos histol&oacute;gicos sugestivos de endocarditis bacteriana no sospechada en 0,9% de los pacientes programados para remplazo valvular electivo en casos con degeneraci&oacute;n de la pr&oacute;tesis valvular biol&oacute;gica. As&iacute; mismo, en la serie publicada por Renzulli (8), en cuanto a histolog&iacute;a se document&oacute; la degeneraci&oacute;n de la biopr&oacute;tesis en la mayor&iacute;a de los pacientes que presentaron trombosis y en uno de ellos se prob&oacute; endocarditis no sospechada previamente. </p>       <p>En los casos que se reportan en este art&iacute;culo, la trombosis ocurri&oacute; de manera tard&iacute;a y en el primer paciente no se logr&oacute; documentar ning&uacute;n factor de riesgo asociado con &eacute;sta. En el segundo caso el evento ocurri&oacute; dentro del contexto de una enfermedad infecciosa sist&eacute;mica que podr&iacute;a haber llevado a endocarditis de la v&aacute;lvula prot&eacute;sica y trombosis secundaria. La diferenciaci&oacute;n entre pannus y trombosis en ocasiones es dif&iacute;cil y el diagn&oacute;stico acertado es crucial para la terap&eacute;utica ya que el primero no responde a la terapia trombol&iacute;tica. Uno de los par&aacute;metros que sugieren su aparici&oacute;n, es la disfunci&oacute;n en el tiempo, ya que la trombosis ocurre de manera temprana luego del reemplazo valvular, mientras que el pannus es tard&iacute;o, pero en ocasiones, como en los dos casos expuestos, la trombosis puede ocurrir de manera tard&iacute;a.</p>       <p>De otro lado, la ecogenicidad es otro par&aacute;metro que puede ayudar a diferenciar estas dos entidades, dado que la trombosis presenta ecogenicidad menor (9). A pesar de ello, el diagn&oacute;stico diferencial es dif&iacute;cil. </p>       <p>Dentro de las nuevas herramientas disponibles para la evaluaci&oacute;n de la causa de disfunci&oacute;n figura la ecocardiograf&iacute;a tridimensional, la cual que se ha convertido en una importante herramienta en la pr&aacute;ctica cl&iacute;nica gracias al desarrollo de la adquisici&oacute;n en tiempo real que permite evaluar de manera m&aacute;s adecuada las caracter&iacute;sticas f&iacute;sicas de la v&aacute;lvula y su comportamiento durante el ciclo cardiaco (10). La utilidad de esta t&eacute;cnica ya ha sido evaluada en la valoraci&oacute;n de las v&aacute;lvulas prot&eacute;sicas y ha demostrado buena correlaci&oacute;n con los hallazgos quir&uacute;rgicos (11), con lo cual puede diferenciarse la presencia de pannus o de trombo (12-14). La identificaci&oacute;n correcta de la causa de la disfunci&oacute;n permite la implementaci&oacute;n del tratamiento adecuado. En la actualidad, las recomendaciones indican la cirug&iacute;a de reemplazo valvular como tratamiento de elecci&oacute;n en trombosis prot&eacute;sicas izquierdas, especialmente en pacientes en clase funcional III-IV. La indicaci&oacute;n de tromb&oacute;lisis se reserva para aquellos con riesgo quir&uacute;rgico alto o en quienes la cirug&iacute;a est&aacute; contraindicada, y para los casos de trombosis que afectan las v&aacute;lvulas derechas (15). </p>       <p>La mortalidad quir&uacute;rgica, sobre todo en pacientes en clase funcional III-IV, es notoriamente alta (33% al 88%) (16, 17). Existen diferentes series en las que se evalu&oacute; el tratamiento con fibrinol&iacute;ticos y se mostr&oacute; una mortalidad que oscil&oacute; entre 2,8% y 11,8% (7, 8) con una tasa de complicaciones emb&oacute;licas del orden de 2,6% a 19%. En el seguimiento, la recurrencia de trombosis es la complicaci&oacute;n m&aacute;s frecuente, cuya tasa de &eacute;xito mejora al realizarse un nuevo tratamiento con fibrinol&iacute;ticos. </p>       <p>En los pacientes en clase funcional I-II, la indicaci&oacute;n de fibrin&oacute;lisis o cirug&iacute;a depender&aacute; de la decisi&oacute;n individual y de los hallazgos que aporte la ecocardiograf&iacute;a transesof&aacute;gica (18-21). Pese a ello, la terap&eacute;utica de la trombosis de las pr&oacute;tesis valvulares contin&uacute;a siendo motivo de controversia. </p>   <font size="3" face="Verdana"><b>Bibliograf&iacute;a</b></font>     ]]></body>
