We also report the successful management with stenting for superior vena cava syndrome.</p>     <p> <b><i>Keywords</i></b>: catheters, stent, veins.</p>  <hr size="1">  <font size="3" face="Verdana"><b>Introducci&oacute;n</b></font>     <p> La obstrucci&oacute;n de la vena cava superior trae como consecuencia una constelaci&oacute;n de signos y s&iacute;ntomas conocidos en conjunto como s&iacute;ndrome de vena cava superior.</p>       <p>La vena en menci&oacute;n es el principal conducto para el drenaje venoso de la cabeza, el cuello, las extremidades superiores y el t&oacute;rax superior. Se trata de un vaso de pared fina, blando, localizado en el mediastino medio y rodeado de estructuras m&aacute;s r&iacute;gidas, como los ganglios mediast&iacute;nicos y para-traqueales, la tr&aacute;quea y el bronquio principal derecho, la arteria pulmonar y la aorta. Por tanto, la vena cava superior es susceptible de ser comprimida desde el exterior por cualquier lesi&oacute;n ocupante de espacio. Se extiende desde la uni&oacute;n de las venas innominadas derecha e izquierda a la aur&iacute;cula derecha. Su principal vaso auxiliar, la vena &aacute;cigos, entra en la vena cava superior posteriormente, justo por encima de la reflexi&oacute;n peric&aacute;rdica. Otros sistemas colaterales son las venas mamarias internas, las venas para-espinales y la red venosa esof&aacute;gica. Las venas subcut&aacute;neas son v&iacute;as importantes y su ingurgitaci&oacute;n en el cuello y el t&oacute;rax es un hallazgo f&iacute;sico t&iacute;pico del s&iacute;ndrome de vena cava superior. A pesar de estas v&iacute;as colaterales, si hay obstrucci&oacute;n de la vena cava superior, casi siempre est&aacute; elevada la presi&oacute;n venosa en el compartimento superior. La obstrucci&oacute;n del flujo de la vena cava produce hipertensi&oacute;n venosa de la cabeza, el cuello y las extremidades superiores, lo que, a su vez, es responsable de la presentaci&oacute;n cl&iacute;nica caracter&iacute;stica.</p>       <p>Se describe el caso de un reci&eacute;n nacido prematuro con s&iacute;ndrome de vena cava superior que recibi&oacute; manejo con terapia fibrinol&iacute;tica e implante exitoso de stent en la vena cava superior.</p>   <font size="3" face="Verdana"><b>Descripci&oacute;n del caso</b></font>       <p>Reci&eacute;n nacido de 25 semanas (edad gestacional dada por ecograf&iacute;a obst&eacute;trica y Test de Ballard), producto de embarazo m&uacute;ltiple, de madre de 33 a&ntilde;os, sin controles prenatales, quien present&oacute; ruptura prematura de membranas con prolapso de cord&oacute;n umbilical del feto #1, que se encontraba en posici&oacute;n pod&aacute;lica, y al llegar a la instituci&oacute;n prestadora de salud se hall&oacute; sin signos vitales. </p>       <p>Se realiz&oacute; ces&aacute;rea de urgencias para el feto #2, y se obtuvo reci&eacute;n nacido de sexo masculino, con peso: 700 gramos, talla: 35 cent&iacute;metros, y APGAR de 7, 8, 8 durante los minutos 1, 5, 10 respectivamente. Se insert&oacute; cat&eacute;ter venoso central bilumen a los 30 d&iacute;as de nacido, para dar inicio a nutrici&oacute;n parenteral; a sus 44 d&iacute;as de vida present&oacute; sepsis generalizada por Staphylococcus aureus y Enterobacter cloacae, la cual se trat&oacute; con antimicrobianos de amplio espectro. </p>       ]]></body>
<body><![CDATA[<p>En el tercer d&iacute;a de tratamiento antibi&oacute;tico, desarroll&oacute; edema en cara, cuello y zona superior del tronco, el cual fue progresivo (<a href="#figura1">Figura 1</a>), y se acompa&ntilde;&oacute; de deterioro del estado hemodin&aacute;mico. Se realiz&oacute; ecocardiograma encontr&aacute;ndose imagen sugestiva de trombo en vena cava superior. Recibi&oacute; manejo con heparina (75 U/kg dosis inicial, luego a 10 U/kg/hora en infusi&oacute;n continua) y estreptoquinasa (4.000 U/kg dosis inicial, luego a 2.000 U/kg/hora en infusi&oacute;n continua) a trav&eacute;s de cat&eacute;ter venoso central, sin mejor&iacute;a cl&iacute;nica al cuarto d&iacute;a de manejo, y persistencia del trombo en la vena cava superior. </p>       <p>    <center><a name="figura1"></a>    <br>     <img src="img/revistas/rcca/v19n4/v19n4a10f1.jpg"></center></p>     <p>Al quinto d&iacute;a del inicio del cuadro cl&iacute;nico, se realiz&oacute; venograf&iacute;a v&iacute;a vena femoral derecha demostr&aacute;ndose una obstrucci&oacute;n total de la vena cava superior con m&uacute;ltiples colaterales desde la circulaci&oacute;n de la cabeza; se observ&oacute;, adem&aacute;s, obstrucci&oacute;n de la vena innominada (<a href="#figura2">Figura 2</a>). Se pas&oacute; gu&iacute;a a trav&eacute;s del trombo, con implante de dos stents cobalto-cromo (ProKinetic&reg;) con un di&aacute;metro de 5 x 15 mm cada uno (<a href="#figura3">Figura 3</a>); as&iacute; mismo, se verific&oacute; su adecuada posici&oacute;n durante el procedimiento. En la angiograf&iacute;a final desde la vena cava superior se demostr&oacute; normalizaci&oacute;n del flujo (<a href="#figura4">Figura 4</a>).</p>       <p>    <center><a name="figura2"></a>    <br>     <img src="img/revistas/rcca/v19n4/v19n4a10f2.jpg"></center></p>       <p>    <center><a name="figura3"></a>    ]]></body>
<body><![CDATA[<br>     <img src="img/revistas/rcca/v19n4/v19n4a10f3.jpg"></center></p>       <p>    <center><a name="figura4"></a>    <br>     <img src="img/revistas/rcca/v19n4/v19n4a10f4.jpg"></center></p>   <font size="3" face="Verdana"><b>Discusi&oacute;n y conclusiones </b></font>     <p>El s&iacute;ndrome de vena cava superior es una patolog&iacute;a de baja incidencia en ni&ntilde;os. La causa de esta obstrucci&oacute;n var&iacute;a de acuerdo con la edad del paciente. En la poblaci&oacute;n pedi&aacute;trica, la cirug&iacute;a de cardiopat&iacute;as cong&eacute;nitas y los linfomas son la causa m&aacute;s frecuente de s&iacute;ndrome de vena cava superior (1). </p>       <p>La trombosis venosa es una de las mayores complicaciones asociadas al uso prolongado de cat&eacute;ter venoso central en reci&eacute;n nacidos (2-6). El riesgo de formaci&oacute;n de trombos en prematuros y en ni&ntilde;os de bajo peso con cat&eacute;ter venoso central, es elevado ya que el sistema hemost&aacute;tico neonatal se caracteriza por una activaci&oacute;n de la coagulaci&oacute;n fisiol&oacute;gica, causada por la mayor concentraci&oacute;n y actividad de varios factores coagulantes, como aumento de la formaci&oacute;n de trombina y de la activaci&oacute;n endotelial y disminuci&oacute;n de los niveles plasm&aacute;ticos de antitrombina III y de plasmin&oacute;geno, con la siguiente alteraci&oacute;n de la capacidad de lisar trombos (7-9). Adem&aacute;s, hay menor flujo sangu&iacute;neo a trav&eacute;s del cat&eacute;ter en comparaci&oacute;n con ni&ntilde;os m&aacute;s grandes y el cat&eacute;ter ocupa una porci&oacute;n m&aacute;s significativa de la secci&oacute;n transversal del vaso (10).</p>       <p>Otros factores asociados al s&iacute;ndrome de vena cava superior son: edad, g&eacute;nero, historia de un cat&eacute;ter venoso anterior, procesos infecciosos, di&aacute;metro del dispositivo endovenoso utilizado y naturaleza del mismo (silicona o poliuretano), sustancias administradas (nutrici&oacute;n parenteral, antimic&oacute;ticos), t&eacute;cnica de colocaci&oacute;n y tiempo del cat&eacute;ter venoso central en el lugar de inserci&oacute;n; hay estudios que demuestran formaci&oacute;n de trombo al cuarto d&iacute;a de la implantaci&oacute;n de cat&eacute;ter venoso central (11-13, 25-27).</p>       <p>De otra parte, el s&iacute;ndrome de vena cava superior puede producir obstrucci&oacute;n de la v&aacute;lvula tric&uacute;spide con insuficiencias valvular y card&iacute;aca de progreso acelerado. Puede actuar como origen potencial de infecci&oacute;n y dar lugar a la propagaci&oacute;n de los &eacute;mbolos a los pulmones o la circulaci&oacute;n sist&eacute;mica (14-17). </p>       <p>Las opciones de tratamiento &oacute;ptimo para los ni&ntilde;os con s&iacute;ndrome de vena cava superior permanecen en estudio; de hecho la informaci&oacute;n actual se limita a reportes de casos y series de casos peque&ntilde;os. </p>       <p>En este sentido, se han descrito informes anecd&oacute;ticos de casos &eacute;xitosos de extirpaci&oacute;n quir&uacute;rgica de trombos en beb&eacute;s prematuros de peso al nacer inferior a 1.900 gramos (18-23), que a menudo requieren bypass cardiopulmonar, o presentan paro circulatorio total o hipotermia profunda con complicaciones y muerte. </p>       ]]></body>
<body><![CDATA[<p>La terapia trombol&iacute;tica ha sido el tratamiento de elecci&oacute;n para el s&iacute;ndrome de vena cava superior tromb&oacute;tica en adultos. A este respecto los primeros informes se publicaron a mediados de 1980. As&iacute;, por ejemplo, Gray y colaboradores, publicaron su experiencia con 16 pacientes, y concluyeron que la fibrin&oacute;lisis central a trav&eacute;s del cat&eacute;ter venoso central, era m&aacute;s efectiva porque los agentes se entregan directamente al co&aacute;gulo (73% lisis completa). Este estudio favoreci&oacute; la uroquinasa sobre la estreptoquinasa en t&eacute;rminos de eficacia y efectos secundarios (24). </p>       <p>En el caso que aqu&iacute; se expone, se inici&oacute; con terapia trombol&iacute;tica sin mejor&iacute;a cl&iacute;nica, por lo cual se procedi&oacute; al implante de dos stents, el mismo que se llev&oacute; a cabo con &eacute;xito; se resolvieron los s&iacute;ntomas y no hubo complicaciones perioperatorias ni posoperatorias.</p>       <p>Pese a que se han descrito casos de colocaci&oacute;n de stent en adultos en procesos tumorales predominantemente pulmonares y mediast&iacute;nicos donde, con &eacute;xito, se ha logrado la resoluci&oacute;n del s&iacute;ndrome de vena cava superior (25), no existen reportes de casos de implante de stent en pacientes prematuros con s&iacute;ndrome de vena cava superior. </p>       <p>Se han reportado adem&aacute;s complicaciones secundarias a la colocaci&oacute;n de stent hasta en 7% de los casos, de las cuales las m&aacute;s frecuentes son: infecci&oacute;n, trombosis, migraci&oacute;n del stent, tromboembolia pulmonar y perforaci&oacute;n del trayecto venoso; aunque esta &uacute;ltima de baja frecuencia (28, 29).</p>       <p>Los puntos finales para futuras investigaciones acerca del s&iacute;ndrome de vena cava superior deber&iacute;an abarcar, entre otros, los siguientes aspectos: </p>       <p>1.	&Eacute;xito en la prevenci&oacute;n de la trombosis; la heparina profil&aacute;ctica en cat&eacute;ter venoso central (CVC) no ha sido evaluada de forma adecuada en ensayos controlados aleatorios en los reci&eacute;n nacidos y en la actualidad no hay evidencia para apoyar su uso en la pr&aacute;ctica m&eacute;dica (30).     <br>   2.	Costo de los tratamientos profil&aacute;cticos.    <br>   3.	Comparaci&oacute;n de los costos entre tratamiento fibrinol&iacute;tico y tratamiento para prevenci&oacute;n de la trombosis.    <br>   4.
