Objetivo: Evaluar la asociación entre la selección del tratamiento farmacológico según el subtipo de hipertensión y el control de la presión arterial.  Materiales y método: Estudio de cohorte del mundo real, que incluyó sujetos con un diagnóstico de novo de hipertensión arterial que recibieron tratamiento farmacológico antihipertensivo siguiendo las guías actuales. De modo retrospectivo, los pacientes fueron clasificados por subtipos de hipertensión arterial según el patrón de predominio, sistólico o diastólico. Además, se evaluó la selección apropiada del tratamiento farmacológico según el subtipo de hipertensión arterial y la hemodinámica subyacente de cada subtipo. El control de la presión arterial en el seguimiento, dentro de las primeras doce semanas después del diagnóstico, se definió como cifras de presión arterial &lt; 140/90 mmHg.  Resultados: Entre los sujetos incluidos en el estudio (n = 1397), el promedio de edad fue 52.4 ± 13.2. El inicio de un tratamiento farmacológico antihipertensivo adecuado se asoció con mayor control de la presión arterial (OR: 2.17; IC: 1.49-3.15; p &lt; 0.001). Los pacientes que no alcanzaron el control de la presión arterial presentaron mayor frecuencia del subtipo de hipertensión sistólico-diastólica divergente y mayores alteraciones hemodinámicas.  Conclusiones: La selección adecuada del tratamiento farmacológico según el subtipo de hipertensión arterial se asocia con un mejor control de la presión arterial en pacientes con hipertensión arterial de reciente diagnóstico.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Introduction: It is unknown whether the selection of individualized pharmacological treatment according to arterial hypertension (AH) subtype leads to greater blood pressure (BP) control.  Objective: To evaluate the association between the selection of pharmacological treatment based on AH subtype and BP control.  Materials and method: This was a real-world cohort study that included subjects with a de novo diagnosis of AH who received anti-hypertensive pharmacological treatment following current guidelines. Retrospectively, patients were classified by AH subtype according to the pattern of systolic or diastolic predominance of AH. Also, appropriateness of the pharmacological treatment was assessed based on AH subtype and the underlying hemodynamics of each subtype. BP control at the follow-up, within the first 12 weeks after diagnosis, was defined as &lt; 140/90 mmHg.  Results: Among the subjects included in the study (n = 1397), the mean age was 52.4 ± 13.2. Initiation of appropriate anti-hypertensive pharmacological treatment was associated with greater BP control (OR 2.17; CI: 1.49-3.15; p &lt; 0.001). The patients who did not reach BP control presented a higher frequency of divergent systolic-diastolic hypertension subtype and greater hemodynamic alterations.
