Metodología: se realizó una búsqueda bibliográfica en la base de datos Medline y en el buscador Pubmed, usando la palabra clave "endometrial polyps", identificando aquella información relevante relacionada con su fisiopatología, publicada entre los años 2000 - 2011, de artículos publicados tanto en castellano como en inglés.o Resultados: se pueden resumir los fenómenos asociados con la aparición de los pólipos endometriales en factores hormonales, inflamatorios, proliferativos y genéticos. Con respecto a la influencia hormonal se ha demostrado un aumento de los receptores de estrógenos y progestágenos, y una sobreexpresión de la aromatasa en el endometrio de los pólipos endometriales. Con respecto a los factores inflamatorios y proliferativos, la presencia de pólipos se asocia con el aumento de la concentración de mastocitos activados, aumento de los niveles de cicloxigenasa 2 y el incremento de otrassustancias inhibidoras de la apoptosis, tales como, el Bcl2 y el Ki67. Además, se han relacionado anormalidades genéticas en diversos cromosomas, junto con reacomodamientos producidos por proteínas de alta movilidad, con la presencia de los EP. Conclusiones: aunque la etiología de los pólipos endometriales sigue siendo desconocida, se puede pensar que para su aparición se requiere de un proceso proliferativo estimulado por algunos factores hormonales asociados a un fenómeno inflamatorio]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT Objective: To make a review of the available literature about the pathophysiology of endometrial polyps (EP) development. Methods: A search was conducted using Medline database and the browser Pubmed in order to retrieve relevant information related to the pathophysiology of endometrial polyps, using as search terms endometrial polyps, including publications from 2000 through 2011. Results: The factors associated with the endometrial polyps can be summarized within hormonal, inflammatory, proliferative and genetic factors. Related to the hormonal influence, it has been demonstrated by an increase in estrogens and progestagen receptors, and overexpression of aromatase in endometrial tissue of EP. Related with inflammatory and proliferative factors EP are associated with the increase of mastocytes activated concentration, increase of cycloxigenase2 levels and increase of other inhibiting apoptosis substances such as Bcl 2 and Ki67. Furthermore, EP has been associated with genetic abnormalities in various chromosomes, together with rearrangement produced by high mobility proteins. <body><![CDATA[<p>Endometrial Polyps, Review</p><hr></p></font>    <p><b><font face="Verdana" size="3">INTRODUCCI&Oacute;N</font></b></p>    <p><font size="2" face="Verdana">Los p&oacute;lipos endometriales (EP) son proyeccionesformadas por estroma fibroso recubierto deepitelio y gl&aacute;ndulas endometriales. En su mayor&iacute;a,son tumoraciones de tipo benigno, cuyaanormalidad b&aacute;sica es la hiperplasia, sin embargohay datos que indican que 1 a 2 % de losp&oacute;lipos endometriales sufren transformaci&oacute;nmaligna (1-3). Esta entidad es poco frecuenteen menores de 20 a&ntilde;os y su incidencia aumentacon la edad, con mayor prevalencia en la perimenopausiay un declive gradual despu&eacute;s de lamenopausia (4). Los p&oacute;lipos endometriales sonsintom&aacute;ticos en cerca de la mitad de los casos,siendo el principal s&iacute;ntoma el sangrado vaginalanormal. En algunos casos es un hallazgo incidental(4).</font></p>    <p><font size="2" face="Verdana">Hasta la fecha existen diversas teor&iacute;as que tratande explicar la etiopatogenia de los p&oacute;liposendometriales, sin embargo no se ha logradollegar a una conclusi&oacute;n clara sobre del origende los mismos. Por tanto, aunque el tratamientoes relativamente sencillo, a&uacute;n no se sabe con certeza entre aquellas pacientes asintom&aacute;ticascu&aacute;les requieren tratamiento, cu&aacute;l es el riesgode recidiva o cu&aacute;les son sus implicaciones sobrela fertilidad.</font></p>    <p><font size="2" face="Verdana">Esta revisi&oacute;n busca estudiar las diferentes teor&iacute;asacerca de la etiolog&iacute;a y la fisiopatolog&iacute;a delos p&oacute;lipos endometriales, y de esta manera,aportar conocimientos que puedan generar conceptos&uacute;tiles para definir estrategias de manejo.</font></p>    <p><font size="4" face="Verdana"><b>METODOLOG&Iacute;A</b></font></p>    <p><font size="2" face="Verdana">Se realiz&oacute; una b&uacute;squeda bibliogr&aacute;fica en la basede datos Medline y en el buscador Pubmed, usandolas palabras clave "endometrial polyp". Se revisaronlos art&iacute;culos disponibles, publicados entre2000 - 2011, en ingl&eacute;s y castellano, utilizandopara el an&aacute;lisis aquellos que hac&iacute;an &eacute;nfasis eninformaci&oacute;n relacionada con la fisiopatolog&iacute;a delos p&oacute;lipos endometriales. Posteriormente serevisaron art&iacute;culos de a&ntilde;os anteriores, que seconsideraron relevantes para el entendimientode la fisiopatolog&iacute;a de los EP. Inicialmente seidentificaron 86 art&iacute;culos, de ellos 60 cumpl&iacute;anlos criterios de inclusi&oacute;n: 10 casos y controles,19 estudios de cohorte, 10 series de casos y 11art&iacute;culos de revisi&oacute;n y, finalmente, se realiz&oacute; unan&aacute;lisis cualitativo de los datos recolectados.</font> </p>    <p><b><font face="Verdana" size="3">ETIOLOG&Iacute;A YFISIOPATOLOG&Iacute;A</font></b></p>    <p><font size="2" face="Verdana">La etiolog&iacute;a y fisiopatolog&iacute;a de los p&oacute;lipos endometrialesa&uacute;n no se encuentra descrita en sutotalidad. Se han formulado algunas hip&oacute;tesisrelacionadas con su origen, las cuales ser&aacute;n expuestasa continuaci&oacute;n. </font></p>    <p><font size="2" face="Verdana"><b>Factores hormonales</b></font></p>    ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Los estr&oacute;genos y progest&aacute;genos son conocidosmoduladores de la proliferaci&oacute;n y la diferenciaci&oacute;nendometrial a trav&eacute;s de sus receptores. Losreceptores de estr&oacute;genos (ER) se clasifican enreceptores de estr&oacute;geno beta (ER-&szlig;) y receptoresde estr&oacute;geno alfa (ER-&alpha;). Los ER-&alpha; se expresanen &uacute;tero, h&iacute;gado, mama y ri&ntilde;&oacute;n; los ER-&szlig; entejidos no reproductivos como hueso, cerebro,hip&oacute;fisis, tracto urinario, aparato cardiovasculary pr&oacute;stata, y en tejidos reproductivos como ovarioy test&iacute;culo. Ambos receptores se presentanen ovarios, cerebro, hueso, sistema cardiovasculary mamas. Los ER endometriales tienen unpapel esencial en la fisiolog&iacute;a reproductiva, siendoellos determinantes del estado morfol&oacute;gico yfuncional del tracto genital y especialmente delendometrio (5).</font></p>    <p><font size="2" face="Verdana">Algunos estudios han mostrado una relaci&oacute;n entrela presencia de p&oacute;lipos endometriales y laexpresi&oacute;n de receptores hormonales (6,7). En unestudio realizado con pacientes postmenopa&uacute;sicasy sangrado vaginal, se encontr&oacute; una mayorexpresi&oacute;n de receptores, tanto de estr&oacute;genoscomo de progesterona, en las c&eacute;lulas estromalesy glandulares de las muestras de los p&oacute;liposendometriales, comparadas con las del endometriode pacientes sin p&oacute;lipos (6).</font></p>    <p><font size="2" face="Verdana"> A diferencia de estos hallazgos, Liu no encontr&oacute;diferencias en la expresi&oacute;n de ER-beta y ER alfaen el endometrio del p&oacute;lipo y el endometrio distanteal p&oacute;lipo. Pero s&iacute; una mayor expresi&oacute;n dereceptores de progesterona (PR) y del receptorestrog&eacute;nico de membrana ligado a prote&iacute;na G(GPR 30) (8).</font></p>    <p><font size="2" face="Verdana">Los niveles de aromatasa, enzima implicada enla producci&oacute;n local de estr&oacute;genos, tambi&eacute;n sehan asociado a la presencia de p&oacute;lipos endometriales.Maia encontr&oacute; niveles mayores dearomatasas, tanto en el tejido del p&oacute;lipo comoen el endometrio adyacente, lo cual contrastabacon la menor cantidad de esta enzima en el endometriode las pacientes sin p&oacute;lipos endometriales(9). Algunos autores sugieren que la basede la patog&eacute;nesis podr&iacute;a ser la sobre-expresi&oacute;nde la aromatasa endometrial, especialmente enpacientes premenop&aacute;usicas (10).</font></p>    <p><font size="2" face="Verdana">La exposici&oacute;n a hormonas ex&oacute;genas tambi&eacute;n seha asociado con el riesgo de aparici&oacute;n de p&oacute;liposendometriales. Maia elabor&oacute; otro estudiocon el fin de conocer los efectos de la terapia dereemplazo hormonal en los p&oacute;lipos endometrialesdurante la menopausia, para ello incluy&oacute; 44pacientes que consultaron por sangrado anormaly ten&iacute;an un endometrio mayor de 4 mm,de las cuales 16 tomaban terapia de suplenciacombinada con estr&oacute;genos y progest&aacute;genos,mientras que las restante 28 mujeres no ten&iacute;anhistoria de consumo de hormonas en los 12 mesesanteriores. Se encontr&oacute; que el uso continuode progest&aacute;genos, durante la menopausia, seasoci&oacute; a una menor incidencia de p&oacute;lipos endometriales,junto con una disminuci&oacute;n de lastasas de proliferaci&oacute;n y un descenso en la expresi&oacute;nde algunos factores antiapopt&oacute;ticos enel endometrio (bcl-2 y Ki-67) (11).Esta situaci&oacute;n es similar a la que se produce enla fase l&uacute;tea del ciclo menstrual, en la cual existeun aumento en la tasa apopt&oacute;tica de las c&eacute;lulasendometriales (11). </font></p>    <p><font size="2" face="Verdana">Sin embargo, no todos losestudios respaldan la idea de un efecto inhibitoriode la proliferaci&oacute;n endometrial por parte delos progest&aacute;genos: algunos trabajos reportan unefecto antiapopt&oacute;tico con una correlaci&oacute;n positivaentre los receptores de progesterona y losniveles de bcl-2 y Ki67 (6).</font></p>    <p><font size="2" face="Verdana">Las pacientes que usan tibolona pueden tenerun mayor riesgo de presentar EP. El grupo deP&eacute;rez Medina realiz&oacute; un estudio en 485 mujerespostmenopa&uacute;sicas, divididas en dos grupos:249 quienes recib&iacute;an dosis terap&eacute;uticas de tibolonay 244 hormonoterapia con estr&oacute;genos yprogest&aacute;genos combinados (12). La tibolona increment&oacute;el riesgo de p&oacute;lipos endometriales enun factor de tres, en comparaci&oacute;n con la hormonoterapiacombinada, lo cual estar&iacute;a en directarelaci&oacute;n con los datos encontrados acerca dela acci&oacute;n inhibitoria de la progesterona sobre laproliferaci&oacute;n endometrial. La tibolona produceuna hiperplasia del estroma, lo que hace caracter&iacute;sticoen estas pacientes los p&oacute;lipos de tipoatr&oacute;fico (12).</font></p>    <p><font size="2" face="Verdana">Otra raz&oacute;n para pensar en la relaci&oacute;n causalentre los p&oacute;lipos endometriales y el est&iacute;mulohormonal, es la mayor incidencia de p&oacute;lipos observadaen pacientes que toman moduladoresselectivos de los receptores de estr&oacute;genos, talescomo el tamoxifeno o el raloxifen (13-15). Ioannisasoci&oacute; la presencia de p&oacute;lipos endometrialesen pacientes que toman tamoxifeno con losniveles de catepsina D, endoproteasa asociadacon los fen&oacute;menos de proliferaci&oacute;n celular y renovaci&oacute;ntisular, y sugiri&oacute; este mecanismo comosecundario al estimulo hormonal, espec&iacute;ficamenteal efecto estrog&eacute;nico del tamoxifeno (16).Lo anterior es corroborado por los hallazgos delgrupo de la Dra. Dibi, que demostr&oacute; que todaslas pacientes que desarrollaron p&oacute;lipos mientrastomaban tamoxifeno, eran ER positivas (17).Incluso algunos autores sugieren el uso de levonorgestrelen las pacientes que toman tamoxifenopara prevenir la aparici&oacute;n de los EP (18).</font></p>    <p><font size="2" face="Verdana"><b>Factores inflamatorios y proliferativos</b></font></p>    <p><font size="2" face="Verdana"><b>Mastocitos </b></font></p>    ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">Algunos estudios han sugerido que los mastocitosjuegan un rol fundamental en el desarrollode los p&oacute;lipos nasales y los p&oacute;lipos adenomatososintestinales (19). Parece que el papel delos mastocitos en la iniciaci&oacute;n y progresi&oacute;n delos p&oacute;lipos endometriales es igualmente importante.</font></p>    <p><font size="2" face="Verdana">A la fecha, es sabido que los mastocitos cumplenfunciones de gran importancia en variasrespuestas biol&oacute;gicas como son: angiog&eacute;nesis(21), cicatrizaci&oacute;n de heridas, inflamaci&oacute;n y remodelaci&oacute;ntisular (22). Adicionalmente, contienenuna variedad de potentes mediadores comola histamina, heparina, proteasas, leucotrienos,citocinas y factores de crecimiento, que posiblementeintervendr&iacute;an en los diversos cambiosmorfol&oacute;gicos asociados, no solo con el ciclomenstrual, sino tambi&eacute;n con la aparici&oacute;n y progresi&oacute;nde los p&oacute;lipos endometriales (23).</font></p>    <p><font size="2" face="Verdana"> Una sustancia fundamental presente en el mastocitoes la triptasa, un compuesto bioqu&iacute;micoentre cuyas funciones se encuentra el actuarcomo un mit&oacute;geno de las c&eacute;lulas epiteliales(24). Tambi&eacute;n estimula la liberaci&oacute;n de IL-8 y laexpresi&oacute;n de ICAM1, encargados de producir unaumento de la permeabilidad vascular y la adhesi&oacute;nde los leucocitos a la pared endotelial (25).As&iacute; pues, se deduce que el papel de los mastocitoses esencial en la reparaci&oacute;n epitelial.</font></p>    <p><font size="2" face="Verdana"> En un estudio llevado a cabo por Al-Jefout, enel cual se aplicaron t&eacute;cnicas inmuno-histoqu&iacute;micaspara estudiar la presencia de triptasa de losmastocitos activados en muestras de p&oacute;liposendometriales, endometrio adyacente y endometriolejano al p&oacute;lipo, se identificaron densidadeselevadas de mastocitos activados en lostejidos de los p&oacute;lipos en comparaci&oacute;n con elendometrio adyacente (p <0,001) y con el endometriolejano al p&oacute;lipo (23).</font></p>    <p><b><font face="Verdana" size="2">C&eacute;lulas estromales  </font></b></p>    <p><font size="2" face="Verdana">Las c&eacute;lulas estromales at&iacute;picas son c&eacute;lulas primitivasprovenientes de c&eacute;lulas mesenquimalesmultipotenciales, las cuales tienen la capacidadpara diferenciarse a cualquier linaje del estromaendometrial o del musculo liso. Tai y Tavassolidemostraron la presencia de c&eacute;lulas estromalesat&iacute;picas en pacientes con EP, sugiriendo unarelaci&oacute;n entre los p&oacute;lipos y las c&eacute;lulas mesenquimales(26). Sin embargo, a&uacute;n no se aclaranlos mecanismos fisiopatol&oacute;gicos o los factoresasociados al desarrollo de los p&oacute;lipos por estav&iacute;a, debido a que la presencia de estas c&eacute;lulasse relaciona en general, con procesos reactivoso degenerativos.</font></p>    <p><b><font face="Verdana" size="2">Cicloxigenasa-2  </font></b></p>    <p><font size="2" face="Verdana">Las cicloxigenasas son un grupo de enzimas queintervienen en diferentes procesos fisiol&oacute;gicoscomo la coagulaci&oacute;n sangu&iacute;nea, la funci&oacute;n renaly el mantenimiento de la funci&oacute;n gastrointestinal,promueven la proliferaci&oacute;n celular e inhibela apoptosis (27), adem&aacute;s participan de algunosprocesos fisiopatol&oacute;gicos como la inflamaci&oacute;n yel c&aacute;ncer (28). Algunos autores han sugerido unaasociaci&oacute;n de la cicloxigenasa-2 (COX<sub>2</sub>) en el desarrollode los p&oacute;lipos endometriales (9, 29).</font></p>    <p><font size="2" face="Verdana">Se ha mostrado la participaci&oacute;n de la COX<sub>2</sub> enla etiopatogenia de los p&oacute;lipos intestinales. Seinvestig&oacute; acerca de una posible relaci&oacute;n de estaenzima con la patog&eacute;nesis de los p&oacute;lipos endometriales.Se realiz&oacute; un estudio con 52 mujeres,de las cuales 18 no hab&iacute;an recibido est&iacute;mulohormonal, 25 fueron expuestas a estr&oacute;genosconjugados vaginal, cinco fueron tratadas con tibolonay cuatro con tamoxifeno. Independientedel est&iacute;mulo hormonal, los p&oacute;lipos endometrialesexhibieron diversos grados de expresi&oacute;n deCOX<sub>2</sub> en su epitelio glandular. Adicionalmente,llama la atenci&oacute;n la presencia de p&oacute;lipos endometrialessin un incremento en la proliferaci&oacute;nendometrial, tal como se observ&oacute; en las mujerestratadas con tibolona y tamoxifeno (30).</font></p>    <p><font size="2" face="Verdana">Esto refuerza la idea que la COX<sub>2</sub> es un factorimportante en el desarrollo de p&oacute;lipos endometrialesen este grupo de pacientes y que su aparici&oacute;nse asocia m&aacute;s a un proceso inflamatorioque a uno exclusivamente proliferativo (29). Erdemogluyet al. encontraron mayores niveles deCOX<sub>2</sub> en los p&oacute;lipos endometriales de mujerespremenopa&uacute;sicas, comparados con los de mujerespostmenopausicas, sugiriendo una fisiopatolog&iacute;adiferente en estos dos grupos (31). Otrosautores no lograron demostrar esta asociaci&oacute;nde los EP con niveles elevados de COX<sub>2</sub> (32)</font></p>    ]]></body>
<body><![CDATA[<p><b><font face="Verdana" size="2">Apoptosis </font></b></p>    <p><font size="2" face="Verdana">La apoptosis o muerte celular programada, esun proceso gen&eacute;ticamente controlado medianteel cual las c&eacute;lulas inducen su propia muerte (33).Una red de se&ntilde;ales de traducci&oacute;n celular controlaen forma precisa el proceso de apoptosis(34). Este mecanismo se encuentra alterado enlos p&oacute;lipos endometriales. Taylor sugiere que,m&aacute;s una proliferaci&oacute;n sin control, se trata de uncrecimiento anormal del tejido durante las diferentesetapas ciclo endometrial (7).</font></p>    <p><font size="2" face="Verdana">Estudios relacionados someten a estudio inmunohistoqu&iacute;micomuestras de p&oacute;lipos y tejido endometrial,encontrando una sobre expresi&oacute;n delmodulador anti-apopt&oacute;tico Bcl2, especialmentedurante la fase proliferativa, sugiriendo de estamanera una modificaci&oacute;n de la apoptosis en algunosmomentos del ciclo (35-39).</font></p>    <p><font size="2" face="Verdana"> Un estudio realizado por Miranda (40) demostr&oacute;una mayor expresi&oacute;n del Ki<sub>67</sub>, un marcadorde proliferaci&oacute;n celular, en los p&oacute;lipos endometrialesde mujeres expuestas al tamoxifeno, encomparaci&oacute;n con aquellas muestras de mujeresno expuestas a esta hormona. En el grupo depacientes con c&aacute;ncer de endometrio esta expresi&oacute;nfue a&uacute;n mayor. En el mismo estudio se encontr&oacute;una sobre-expresi&oacute;n de CD<sub>31</sub>, marcadorde angiog&eacute;nesis, en las pacientes con p&oacute;liposendometriales, tanto en las que usaban tamoxifeno,como en las que no usaban. Otros autoresdemostraron tambi&eacute;n esta asociaci&oacute;n (41,42,43)</font></p>    <p><b><font face="Verdana" size="2">Factores gen&eacute;ticos</font></b></p>    <p><font size="2" face="Verdana">Existen reportes citogen&eacute;ticos sobre los p&oacute;liposendometriales como el realizado por Paola DalCin, en el que fueron estudiadas 33 muestras dep&oacute;lipos endometriales, de las cuales 19 (57 %)presentaron reacomodamientos cromos&oacute;micosclonales que se distribuyeron entre subgrupos:aquellos con alteraciones en la regi&oacute;n cromos&oacute;mica6p21-p22; las que compromet&iacute;an la regi&oacute;n12q13-15 y por &uacute;ltimo las identificadas enla regi&oacute;n 7q22. Los investigadores reportaronun cuarto subgrupo con un cariotipo normal. Laconclusi&oacute;n de este grupo de trabajo es que dela misma manera que en otros tipos de tumoresmesenquimales benignos, la cantidad de alteracionescitogen&eacute;ticas en los p&oacute;lipos endometrialeses amplia a pesar de su apariencia cl&iacute;nica ymorfol&oacute;gica tan similar; por tanto, se hace necesariobuscar un com&uacute;n denominador de esostumores a nivel molecular (44).</font></p>    <p><font size="2" face="Verdana"> Otro estudio fue llevado a cabo por Tallini, quieninvestig&oacute; la presencia de anormalidades en elgrupo de prote&iacute;nas de alta movilidad (HMG), queson prote&iacute;nas nucleares que juegan un importantepapel en la regulaci&oacute;n de la estructura yfunci&oacute;n de la cromatina (45).</font></p>    <p><font size="2" face="Verdana"> Coincidentemente, las prote&iacute;nas de alta movilidadse encuentran localizadas en las regiones6p21 y 12q15 (46) (discutidas anteriormente), ypor tanto, podr&iacute;an estar relacionados con losreacomodamientos de estas regiones cromos&oacute;micas.Los investigadores analizaron 95 tumoreshumanos de origen mesenquimatoso con tendenciaa presentar alteraciones en las regiones6p21 y 12q15, de los cuales 28 correspond&iacute;an aEP. Los resultados del estudio demostraron quela expresi&oacute;n de las HMG es de com&uacute;n ocurrenciaen los tumores benignos de tipo mesenquimatoso,incluyendo a los p&oacute;lipos endometrialesy que &eacute;sta, se relaciona con alteraciones de lasregiones cromos&oacute;micas estudiadas. Otros estudioshan mostrado resultados similares (47, 48,49, 50).</font></p>    <p><b><font face="Verdana" size="3">CONCLUSIONES</font></b></p>    <p><font size="2" face="Verdana">Aunque la etiolog&iacute;a de los EP sigue siendo desconocida,se puede deducir que para su aparici&oacute;nse requiere de un proceso proliferativo,estimulado por algunos factores hormonales yasociado a un fen&oacute;menos inflamatorios. En loscambios que inducen la proliferaci&oacute;n epitelial seencuentran involucradas sustancias que inhibenla apoptosis. Todo esto requiere probablementeun terreno previamente susceptible. Estos aspectosdeben reforzar la idea que el manejo delos p&oacute;lipos endometriales debe ir m&aacute;s all&aacute; de susimple resecci&oacute;n.</font></p>    ]]></body>
