El propósito de esta revisión es presentar las indicaciones y técnicas actuales utilizadas para ayudar a prevenir, en lo posible, la reabsorción del reborde residual asegurando el éxito de la rehabilitación sobre implantes.]]></p></abstract>

The purpose of this review is to explore the indications and present actual techniques to help prevent residual ridge resorption, as much as possible, guaranteeing the success of Prosthodontic rehabilitation with implants. </P>     <P><b>Key words</b>: Alveolar ridge preservation, Alveolar ridge resorption, Tooth extraction, Bone grafting, Dental implants. </P>  <hr>     <p><font size="3"><b>Resumen</b></font></p>     <p>La reabsorci&oacute;n &oacute;sea es una consecuencia fisiol&oacute;gica de la p&eacute;rdida dental que puede convertirse en un riesgo funcional y est&eacute;tico significativo para la colocaci&oacute;n de implantes dentales. Los cambios dimensionales del reborde alveolar pueden ser manejados con diferentes materiales de injerto y procedimientos quir&uacute;rgicos reportados en la literatura. El prop&oacute;sito de esta revisi&oacute;n es presentar las indicaciones y t&eacute;cnicas actuales utilizadas para ayudar a prevenir, en lo posible, la reabsorci&oacute;n del reborde residual asegurando el &eacute;xito de la rehabilitaci&oacute;n sobre implantes. </P>     <P><b>Palabras clave</b>: Preservaci&oacute;n del reborde alveolar, Reabsorci&oacute;n &oacute;sea, Extracci&oacute;n dental, Injerto &oacute;seo, Implantes dentales. </P>   <hr>     <p><font size="3"><b>Introducci&oacute;n</b></font></P>     <p>Uno de los mayores problemas para el periodoncista, cirujano, y/o rehabilitador a la hora de colocar un implante dental es no tener el volumen &oacute;seo suficiente para lograr estabilidad primaria y una posici&oacute;n adecuada del mismo. </P>     <P>Seg&uacute;n el Consenso de la Tercera Conferencia de ITI (International Team for Implantology), podemos colocar el implante inmediato (mismo acto quir&uacute;rgico de la extracci&oacute;n), mediato temprano (4-8 semanas de cicatrizaci&oacute;n), mediato tard&iacute;o (1216 semanas de cicatrizaci&oacute;n) o tard&iacute;o (m&aacute;s de 6 meses luego de la extracci&oacute;n).<Sup>1 </Sup></P>     ]]></body>
<body><![CDATA[<P>Los procedimientos de preservaci&oacute;n de reborde alveolar se realizan cuando no es posible la colocaci&oacute;n inmediata de implantes, ya sea por p&eacute;rdida &oacute;sea luego de la exodoncia, pacientes j&oacute;venes que est&eacute;n en etapa de crecimiento activo o cuando el paciente no tiene el presupuesto suficiente para una pronta colocaci&oacute;n del implante.<Sup>2 </Sup></P>     <P>Est&aacute; documentado que sin t&eacute;cnicas de preservaci&oacute;n, se puede perder hasta 40 % en altura y 60 % en espesor del reborde<Sup>3</Sup> durante los primeros 6 meses post-extracci&oacute;n,<Sup>4 </Sup>y luego entre 0,5-1,0% anualmente limitando el pron&oacute;stico est&eacute;tico y funcional de una pr&oacute;tesis implanto-retenida.</P>     <P>Los cambios dimensionales de los sitios postextracci&oacute;n<Sup>5,6 </Sup>pueden ser manejados con diferentes materiales de injerto y procedimientos quir&uacute;rgicos de preservaci&oacute;n o regeneraci&oacute;n seg&uacute;n el n&uacute;mero y grosor de paredes &oacute;seas remanentes. </P>     <P>Debemos tomarnos el tiempo de estudiar las condiciones espec&iacute;ficas de cada paciente y el tratamiento prot&eacute;sico a seguir, para poder decidir qu&eacute; t&eacute;cnica y materiales vamos a utilizar. Es importante ser cuidadoso porque los criterios de &eacute;xito actuales, no solo buscan la supervivencia de los implantes, sino adem&aacute;s una est&eacute;tica adecuada, funci&oacute;n y aceptaci&oacute;n por parte del paciente<Sup>1,7,8 </Sup></P>      <p>Hasta la &uacute;ltima parte del siglo 20 hab&iacute;a poca preocupaci&oacute;n acerca de la reabsorci&oacute;n del reborde post-extracci&oacute;n. Los esfuerzos iniciales para prevenirla se enfocaban en t&eacute;cnicas prot&eacute;sicas y diferentes patrones en el uso de dentaduras.<Sup>3,9 </Sup></P>     <P>Un m&eacute;todo propuesto para mantener los contornos &oacute;seos fue la retenci&oacute;n radicular no vital basada en la observaci&oacute;n de que la reabsorci&oacute;n &oacute;sea no ocurr&iacute;a alrededor de dientes retenidos. Esta idea fue abandonada debido a complicaciones en tejidos blandos.<Sup>9,10</Sup> Utilizando un concepto similar en los 80&acute;s, la preservaci&oacute;n de reborde fue hecha utilizando conos de hidroxiapatita (HA) en forma de ra&iacute;ces y part&iacute;culas colocadas en los alv&eacute;olos.<Sup>11 </Sup></P>     <P>Actualmente, la colocaci&oacute;n de implantes debe ser guiada prot&eacute;sicamente con una correcta posici&oacute;n tridimensional que permita un soporte &oacute;ptimo y estabilidad de los tejidos duros y blandos circundantes. Ante esto, el objetivo de este art&iacute;culo de revisi&oacute;n es describir las t&eacute;cnicas y materiales de preservaci&oacute;n de reborde disponibles actualmente para la colocaci&oacute;n exitosa de implantes end&oacute;seos. </P>      <p><font size="3"><b>Cicatrizaci&oacute;n del alv&eacute;olo post-extracci&oacute;n y cambios dimensionales del reborde alveolar </b></font></P>     <P>Se ha reportado que durante los 3 primeros meses post-extracci&oacute;n se producen los mayores cambios dimensionales en sentido horizontal y vertical del reborde alveolar;<Sup>12</Sup> dos tercios de la reabsorci&oacute;n &oacute;sea total.<Sup>13</Sup> En el Consenso de Osteolog&iacute;a de 2011, se describe que el reborde alveolar sufre de una reducci&oacute;n horizontal promedio de 3,8 mm (29-63%) y una reducci&oacute;n vertical de 1,24 mm (11-22%) a los 6 primeros meses post-extracci&oacute;n.<Sup>14 </Sup></P>     <P>Todos estos cambios estructurales en el hueso se producen a trav&eacute;s de procesos osteocl&aacute;sticos y osteobl&aacute;sticos, deposici&oacute;n de col&aacute;geno y posterior mineralizaci&oacute;n de la matriz col&aacute;gena.<Sup>9</Sup> Primero se produce una hemorragia, seguida por la formaci&oacute;n de un co&aacute;gulo sangu&iacute;neo.<Sup>5</Sup> La reacci&oacute;n inflamatoria producida estimula el reclutamiento de c&eacute;lulas para formar tejido de granulaci&oacute;n. </P>     ]]></body>
<body><![CDATA[<P>Durante los primeros 4 d&iacute;as, el epitelio prolifera a trav&eacute;s de la periferia del alv&eacute;olo y se evidencia tejido conectivo inmaduro. Despu&eacute;s de 7 d&iacute;as, el co&aacute;gulo ha sido reemplazado completamente por tejido de granulaci&oacute;n. En esta etapa, se evidencia tejido osteoide en la base del alv&eacute;olo. En las siguientes 2 a 3 semanas luego de la extracci&oacute;n, comienza la mineralizaci&oacute;n desde la base hacia la parte coronal. Este proceso es acompa&ntilde;ado por una continua re-epitelializaci&oacute;n, cubriendo el alv&eacute;olo completamente despu&eacute;s de la sexta semana post-extracci&oacute;n. La m&aacute;xima densidad radiogr&aacute;fica se observa alrededor de 100 d&iacute;as.<Sup>15 </Sup></P>     <P>Una serie de estudios en perros entre el 2003 y 2008 fueron fundamentales para la comprensi&oacute;n de los procesos de cicatrizaci&oacute;n post-extracci&oacute;n. Ara&uacute;jo y Lindhe demostraron que dentro de las primeras 8 semanas hay marcada actividad osteocl&aacute;stica, resultando en reabsorci&oacute;n de las paredes &oacute;seas en la regi&oacute;n crestal, especialmente en la altura de la pared vestibular.<Sup>12 </Sup></P>     <P>Es importante comprender los eventos antes mencionados para as&iacute; prevenir, en lo posible, la reabsorci&oacute;n del reborde residual. Si las dimensiones del alv&eacute;olo son mantenidas, se podr&iacute;a reducir la necesidad de futuros procedimientos quir&uacute;rgicos de aumento de reborde, y as&iacute; simplificar la posterior colocaci&oacute;n de implantes.<Sup>11 </Sup></P>       <p><font size="3"><b>Indicaciones para la preservaci&oacute;n del reborde alveolar </b></font></P>     <P>Entre las indicaciones para procedimientos de preservaci&oacute;n de reborde se tienen:<Sup>12,16 </Sup></P> <ul>    <li>Sitios donde la pared vestibular sea menor a 1,52 mm de espesor, y en sitios donde est&eacute; perdida o hayan sido da&ntilde;adas una o m&aacute;s paredes alveolares.</li>    <br>     <li>Cuando sea crucial mantener el volumen &oacute;seo para as&iacute; disminuir el riesgo de comprometer estructuras anat&oacute;micas (seno maxilar, nervio dentario inferior) luego de que ocurra la reabsorci&oacute;n &oacute;sea. </li>    <br>     <li>En sitios con alta demanda est&eacute;tica para mantener, dentro de lo posible, los contornos de los tejidos duros y blandos. </li>    ]]></body>
<body><![CDATA[<br>     <li>En pacientes donde van a ser extra&iacute;dos m&uacute;ltiples dientes y sea necesaria una futura rehabilitaci&oacute;n ya sea con implantes o con pr&oacute;tesis parcial fija.</li>    </ul>       <p><font size="3"><b>Materiales utilizados para t&eacute;cnicas de preservaci&oacute;n del reborde alveolar </b></font></P>     <P>Los materiales que se han utilizado para intentar preservar el reborde alveolar son los mismos empleados para regeneraci&oacute;n &oacute;sea o tisular guiada.<Sup>17</Sup> Estos incluyen: injertos &oacute;seos aut&oacute;logos, alog&eacute;nicos y xenog&eacute;nicos, membranas, esponjas e implantes dentales como preservadores de reborde.</P>     <P>Estos biomateriales presentan ventajas y desventajas y dependiendo de su estructura y composici&oacute;n bioqu&iacute;mica, pueden ser reabsorbibles y no reabsorbibles. El cl&iacute;nico debe conocer las propiedades biol&oacute;gicas, las caracter&iacute;sticas de los materiales, las t&eacute;cnicas de obtenci&oacute;n y sus aplicaciones cl&iacute;nicas, para as&iacute; poder escoger el m&aacute;s adecuado. </P>       <p><font size="3"><b>Injertos &oacute;seos </b></font></P>     <P>Entre las diferentes opciones de injertos &oacute;seos disponibles se encuentran: </P> <ul>    <li>Injertos aut&oacute;logos o aut&oacute;genos (cortical, esponjoso o c&oacute;rtico-esponjoso)<Sup>18</Sup> En cuanto a sus propiedades biol&oacute;gicas, se considera el Gold Standard ya que es el &uacute;nico que posee las 3 propiedades de osteog&eacute;nesis, osteoinducci&oacute;n y osteoconducci&oacute;n. Posee nula capacidad antig&eacute;nica por obtenerse del mismo individuo. Se pueden obtener de diferentes zonas donantes intraorales (ment&oacute;n, tuberosidad maxilar, rama mandibular, rebordes ed&eacute;ntulos o torus), o extraorales (cresta il&iacute;aca, tibia, calota). Las &uacute;ltimas se utilizan en forma de injertos en bloque en casos de p&eacute;rdidas &oacute;seas avanzadas, pero su inconveniente es que requieren de anestesia general para su obtenci&oacute;n. Otra desventaja de los injertos aut&oacute;logos es su r&aacute;pida tasa de reabsorci&oacute;n, por lo que en ocasiones se combinan con aloinjertos o xenoinjertos para mejorar esta propiedad. Hay diferencias en cuanto a la reabsorci&oacute;n vertical seg&uacute;n los sitios extraorales donantes: cresta ili&iacute;aca 12-60% y calota 0-15%. La reabsorci&oacute;n horizontal de los injertos en bloque aut&oacute;logos se ha reportado entre el 10-50%.<Sup>19,20 </Sup></li>    <br>     ]]></body>
<body><![CDATA[<li> Injertos alog&eacute;nicos o aloinjertos (corticales, esponjosos o c&oacute;rtico-esponjosos). Son procedentes de otro individuo de la misma especie. Hay dos principales: mineralizados congelados-secados (FDBA) y desmineralizados congelados-secados (DFDBA). La desmineralizaci&oacute;n podr&iacute;a exponer las prote&iacute;nas morfogen&eacute;ticas &oacute;seas (BMPs) para estimular la diferenciaci&oacute;n de c&eacute;lulas pluripotenciales indiferenciadas hacia osteoblastos (osteoinducci&oacute;n).<Sup>21</Sup> Los aloinjertos necesitan un procesamiento especial para eliminar su capacidad antig&eacute;nica.<Sup>22,23 </Sup> </li>    <br>     <li> Injertos heter&oacute;logos o xenoinjertos (corticales o esponjosos). Su fuente de procedencia es un animal de otra especie diferente a humanos.<Sup>24</Sup> Dentro de este grupo de injertos tenemos el Bio-Oss&reg;<Sup>25 </Sup>y Bio-Oss Collagen&reg;<Sup>26,27 </Sup>de la casa comercial Geistlich Pharma. Se ha observado que luego de 4 meses de realizada la t&eacute;cnica de preservaci&oacute;n de reborde alveolar con Bio-Oss&reg;, el recambio &oacute;seo es de un 25-38% en defectos infra&oacute;seos de 3 paredes. Por otra parte, en defectos de 1 &oacute; 2 paredes, se recomienda esperar 6 meses antes de la colocaci&oacute;n de implantes (Ackermann 2011). Son osteoconductivos. </li>    <br>     <li>Injertos alopl&aacute;sticos o sint&eacute;ticos. Son materiales biocompatibles, sint&eacute;ticos e inorg&aacute;nicos que funcionan como material de relleno y no de regeneraci&oacute;n para futura colocaci&oacute;n de implantes.<Sup>28,29</Sup> Estos proveen un andamiaje para la osteoconducci&oacute;n de las c&eacute;lulas formadoras de hueso. Su principal ventaja es que obvian la necesidad de un sitio donante del propio sujeto. Los m&aacute;s utilizados son la hidroxiapatita y el fosfato tric&aacute;lcico. </li>    </ul>       <P><font size="3"><b>Membranas </b></font></P>     <P>Un factor importante a considerar en las t&eacute;cnicas de preservaci&oacute;n de reborde, es mantener el injerto &oacute;seo en su posici&oacute;n y evitar que los tejidos blandos interfieran en la cicatrizaci&oacute;n &oacute;sea. Durante las primeras fases de cicatrizaci&oacute;n, se produce una competencia entre el tejido &oacute;seo y el blando para rellenar el alv&eacute;olo, debido a que la formaci&oacute;n de este &uacute;ltimo es m&aacute;s r&aacute;pida. Las membranas han demostrado ser eficaces en proteger el material de injerto del medio ambiente oral mediante un efecto barrera, previniendo el r&aacute;pido crecimiento de los tejidos blandos.<Sup>30 </Sup></P>     <P>Los estudios cl&iacute;nicos muestran que los alv&eacute;olos postextracci&oacute;n tratados con el uso de membranas, con o sin injertos &oacute;seos, tienen mayores dimensiones del reborde comparados con los sitios que no son tratados con estos materiales.<Sup>31 </Sup></P>     <P>Diferentes tipos de membranas han sido probadas para realizar preservaciones de reborde,<Sup>4,32</Sup> pero las m&aacute;s com&uacute;nmente utilizadas y que han dado mejores resultados son las de col&aacute;geno.<Sup>33,34 </Sup></P>     ]]></body>
<body><![CDATA[<P>Algunos problemas documentados con las membranas incluyen: dehiscencias de tejidos blandos, exposiciones, desplazamientos y colapsos de las mismas. Se pueden comportar como cuerpos extra&ntilde;os y pueden dar lugar a infecci&oacute;n y retrasos en la cicatrizaci&oacute;n. Si tuvieran que ser retiradas precozmente, la regeneraci&oacute;n &oacute;sea ser&iacute;a menos predecible.<Sup>35 </Sup></P>     <P>Se debe tener en cuenta que el empleo rutinario de membranas e injertos &oacute;seos no garantiza el &eacute;xito del tratamiento de preservaci&oacute;n de reborde, ya que estos no evitan la reabsorci&oacute;n del <i>bundle bone</i> aunque (hueso dependiente del ligamento periodontal) s&iacute; la pueden llegar a compensar.<Sup>36 </Sup></P>       <P><font size="3"><b>Otros "materiales de relleno" - esponjas </b></font></P>     <P>Otros materiales colocados en alv&eacute;olos postextracci&oacute;n son las esponjas de &aacute;cido polil&aacute;ctico/ poliglic&oacute;lico (PL/PG) o de col&aacute;geno.<Sup>37</Sup> Las m&aacute;s utilizadas son las de col&aacute;geno. Act&uacute;an como un andamiaje para el transporte de diferentes factores como la prote&iacute;na morfogen&eacute;tica recombinante humana-2 (rhBMP-2),<Sup>38,39</Sup> el factor de crecimiento/ diferenciaci&oacute;n recombinante humano-5<Sup>40</Sup> o para el p&eacute;ptido sint&eacute;tico de uni&oacute;n celular P-15 (synthetic cell-binding peptide P-15).<Sup>41</Sup> Adem&aacute;s, ayudan a controlar el sangrado (hemost&aacute;ticas) al mismo tiempo que estabilizan el co&aacute;gulo sangu&iacute;neo, y atraen c&eacute;lulas necesarias para la cicatrizaci&oacute;n (quimiot&aacute;cticas).<Sup>42</Sup> Otra funci&oacute;n de estas esponjas de col&aacute;geno, y que muchas veces no se toma en cuenta, es que pueden proteger al injerto &oacute;seo durante la primera fase de cicatrizaci&oacute;n. </P>       <P><font size="3"><b>Implantes como preservadores de reborde </b></font></P>     <P>Anteriormente se pensaba que la colocaci&oacute;n de implantes inmediatos, era un m&eacute;todo para preservar la tabla &oacute;sea vestibular.<Sup>43,44</Sup> Actualmente estos conceptos han sido reevaluados. </P>     <P>Botticelli et al.<Sup>43 </Sup>reportaron que despu&eacute;s de la colocaci&oacute;n de implantes inmediatos, ocurre una reabsorci&oacute;n &oacute;sea sustancial desde la parte exterior del reborde alveolar. Este hecho fue corroborado por Ara&uacute;jo et al.,<Sup>5 </Sup>quienes encontraron que los implantes no previenen el remodelado del alv&eacute;olo post-extracci&oacute;n. A los 3 meses, la altura &oacute;sea en sitios con implantes fue similar a la de los sitios ed&eacute;ntulos. Ellos sugirieron que esto se puede deber, en parte, a la desaparici&oacute;n temprana del bundle bone y adem&aacute;s, a la interrupci&oacute;n del aporte sangu&iacute;neo en bucal debido a la elevaci&oacute;n del colgajo. El proceso de remodelado del hueso alveolar contin&uacute;a incluso luego de la oseointegraci&oacute;n de implantes. De hecho, parte del hueso que est&aacute; "integrado" sobre la superficie del implante, se pierde a las 8 semanas de cicatrizaci&oacute;n en el aspecto bucal.<Sup>45 </Sup></P>     <P>Debido a esto, se concluye que ni la colocaci&oacute;n ni la carga inmediata de implantes se consideran como t&eacute;cnicas de preservaci&oacute;n de reborde alveolar.<Sup>25 </Sup></P>       <P><font size="3"><b>Procedimientos utilizados para preservaci&oacute;n del reborde alveolar </b></font></P>     <P>La t&eacute;cnica m&aacute;s sencilla de realizar una preservaci&oacute;n de reborde alveolar es a trav&eacute;s de la exodoncia atraum&aacute;tica y lograr el cierre primario del mismo para permitir una cicatrizaci&oacute;n biol&oacute;gica del alv&eacute;olo. Otros m&eacute;todos descritos por Darby et al. com&uacute;nmente utilizados son.<Sup>7</Sup></p> <ol>    ]]></body>
<body><![CDATA[<li>Injerto colocado en el alv&eacute;olo cubierto con membrana, y colgajo desplazado para lograr cierre primario parcial o completo de la herida. </li>     <li>Cubrimiento del injerto por colgajo rotacional o desplazado coronal, pero sin membrana. </li>     <li>Membranas solas sobre el alv&eacute;olo, con cubrimiento parcial o total utilizando tejidos blandos. </li>    </ol>       <p>Actualmente los procedimientos de preservaci&oacute;n de reborde est&aacute;n encaminados a ser menos invasivos con t&eacute;cnicas flapless (sin elevar colgajo). </P>     <P>Los resultados de los estudios son controversiales al comparar estas t&eacute;cnicas con las convencionales. Fickl et al. en 2008 reportan menor remodelado &oacute;seo con t&eacute;cnicas de preservaci&oacute;n sin elevar colgajo.<Sup>17</Sup> Sin embargo, los resultados de la revisi&oacute;n sistem&aacute;tica del 2012<Sup>37</Sup> muestran que la t&eacute;cnica con colgajo demostr&oacute; significativamente menos reabsorci&oacute;n horizontal cuando se comparaba con la flapless (IC: 95% &#91;1,01; 3,51&#93;; p = 0,003). Esto se adjudic&oacute; a la importancia de adquirir cierre primario de la herida, principalmente cuando colocamos injertos &oacute;seos y membranas. </p>     <P>Algunas recomendaciones dadas en el Consenso de Osteolog&iacute;a 2012 acerca de la preservaci&oacute;n de alv&eacute;olo son:<Sup>14 </Sup></P> <ul>    <li>Levantar colgajo y colocar un biomaterial. </li>     <li>Cierre primario de la herida.</li>     <li>Materiales con baja tasa de reabsorci&oacute;n y de reemplazo.</li>    ]]></body>
<body><![CDATA[</ul>      <P><font size="3"><b>Exodoncia atraum&aacute;tica </b></font></P>     <P>Para limitar los da&ntilde;os del sitio de exodoncia debemos tomar en consideraci&oacute;n una t&eacute;cnica m&iacute;nimamente traum&aacute;tica, con instrumentos finos y fuerza limitada.<Sup>46,47</Sup> Debemos utilizar elevadores delgados especialmente dise&ntilde;ados para exodoncias atraum&aacute;ticas. </p>     <P>De esta manera, se desinsertan las fibras coronales del ligamento periodontal y se van separando las tablas &oacute;seas alveolares evitando alguna fractura para finalmente luxar el diente y extraerlo. En dientes multirradiculares podemos realizar t&eacute;cnicas de odontosecci&oacute;n. Adem&aacute;s, se pueden perforar las paredes alveolares por medio de la decorticalizaci&oacute;n para fomentar que el sangrado nutra al injerto &oacute;seo.<Sup>2 </Sup></P>       <P><font size="3"><b>Cubrimiento del alv&eacute;olo con tejido blando </b></font></P>     <P>Se ha propuesto el cubrimiento del alv&eacute;olo postextracci&oacute;n con tejidos blandos para retener, estabilizar y proteger los materiales de injerto. Varias t&eacute;cnicas han sido sugeridas para lograr estos objetivos: colgajos desplazados coronalmente, rotados o el uso de injertos gingivales libres o de tejido conectivo subepitelial. En el procedimiento descrito como Soft Tissue Punch Technique<Sup>48</Sup> se cubren los alv&eacute;olos post-exodoncia con injertos gingivales libres sin la necesidad de elevar colgajos. Esta t&eacute;cnica asegura que el injerto gingival proteja el injerto &oacute;seo y la herida fresca luego de la extracci&oacute;n. Adem&aacute;s ayuda a conseguir una estructura y espesor de los tejidos blandos &oacute;ptimos para la posterior colocaci&oacute;n de implantes. </P>     <P>Por otro lado, hay procedimientos donde no se cubre el alv&eacute;olo post-extracci&oacute;n con tejidos blandos, por el contrario se permite la cicatrizaci&oacute;n espont&aacute;nea. La T&eacute;cnica Bio-Col consiste en la colocaci&oacute;n de xenoinjerto bovino (Bio-Oss&reg;) protegido por una esponja de col&aacute;geno (Collaplug&reg;) para permitir la epitelializaci&oacute;n espont&aacute;nea del alv&eacute;olo debajo de un p&oacute;ntico.<Sup>49 </Sup></P>       <P><font size="3"><b>Resultados y Predictibilidad de los procedimientos de preservaci&oacute;n </b></font></P>     <P>Las t&eacute;cnicas de preservaci&oacute;n, reducen significativamente la p&eacute;rdida en altura y espesor del reborde alveolar, ofreciendo un mantenimiento m&aacute;s predecible de las dimensiones del alv&eacute;olo para posterior colocaci&oacute;n de implantes, sin embargo, independientemente de esto, siempre va a haber una reducci&oacute;n dimensional de la cresta &oacute;sea.<Sup>50 </Sup></p>     <P>Los porcentajes de &eacute;xito de implantes colocados en alv&eacute;olos injertados son iguales a los colocados en alv&eacute;olos no injertados con dimensiones adecuadas, por lo tanto, la t&eacute;cnica de preservaci&oacute;n de reborde no afecta el &eacute;xito de los implantes.<Sup>51 </Sup></P>       ]]></body>
<body><![CDATA[<P><font size="3"><b>Conclusiones </b></font></P>     <P>La preservaci&oacute;n de reborde es una t&eacute;cnica que, seg&uacute;n la literatura, ha demostrado reducir significativamente la reabsorci&oacute;n &oacute;sea dimensional que sufre la cresta alveolar despu&eacute;s de la extracci&oacute;n dental, facilitando as&iacute; la futura colocaci&oacute;n de implantes. Se debe tener claro que estos procedimientos limitan el proceso de reabsorci&oacute;n, pero no preservan el volumen ni la altura &oacute;sea inicial en su totalidad. </p>     <P>Antes de realizar una t&eacute;cnica de preservaci&oacute;n de reborde, se debe analizar muy bien el caso del paciente y las condiciones cl&iacute;nicas observadas despu&eacute;s de la extracci&oacute;n dental. Una de las condiciones m&aacute;s importantes es el n&uacute;mero y grosor de paredes &oacute;seas remanentes. Esto va a determinar si se puede realizar un procedimiento de preservaci&oacute;n o si es necesario una t&eacute;cnica regenerativa. </P>     <P>No hay evidencia cient&iacute;fica que una t&eacute;cnica de preservaci&oacute;n de alv&eacute;olo sea superior a otra. Sin embargo, se debe escoger un procedimiento que sea predecible y que se adec&uacute;e a las condiciones cl&iacute;nicas y econ&oacute;micas del paciente. </P>     <P>Se necesitan m&aacute;s ensayos cl&iacute;nicos controlados aleatorizados sobre cicatrizaci&oacute;n de alv&eacute;olos sin aplicar t&eacute;cnicas de preservaci&oacute;n, y posterior colocaci&oacute;n de implantes que ayude en la toma de decisiones cl&iacute;nicas. </P> <hr>       <P><font size="3"><b>Referencias </b></font></P>     <!-- ref --><P>1.
