Para su diagnóstico se han utilizado diferentes métodos convencionales como la inspección visual y táctil y la radiografía digital y no convencional como la transiluminación y fluorescencia laser. Entre los métodos convencionales se encuentran el Sistema Internacional de Detección y Diagnóstico de Caries (ICDAS) y NYVAD (Sistema creado por el autor NYVAD), los cuales han demostrado una buena reproductibilidad y validez. La literatura reafirma estos hallazgos donde estos sistemas ayudan a la detección de lesiones cariosas en estadio precoz y la estimación de su gravedad. Aunque el sistema ICDAS en comparación con el sistema NYVAD, mejora el rendimiento en términos de sensibilidad y fiabilidad, siendo el método ideal para el diagnóstico de caries. Esta revisión bibliográfica tomó en cuenta información a partir del 2003, incluidos revisiones y estudios comparativos donde se demuestra que el sistema ICDAS es utilizado como un método eficiente para la detección de caries en etapa temprana.]]></p></abstract>

Seg&uacute;n la Organizaci&oacute;n Mundial de la Salud (OMS) se define como un proceso patol&oacute;gico localizado que se inicia tras las erupci&oacute;n dental y puede producir reblandecimiento del tejido duro del diente evolucionando hasta la formaci&oacute;n de una cavidad, de igual forma se produce por los subproductos &aacute;cidos resultantes de la fermentaci&oacute;n bacteriana de los carbohidratos de la dieta ; es multifactorial y se asocia con el consumo de carbohidratos y deficiente salud oral (2), existiendo la intervenci&oacute;n de factores primarios tales como el hu&eacute;sped (saliva y dientes), la microflora (microorganismos) y el sustrato (dieta), as&iacute; como tambi&eacute;n factores moduladores como, edad, estrato socioecon&oacute;mico, nivel educativo, cultura (3-5), h&aacute;bitos alimenticios y percepci&oacute;n y conocimientos de higiene oral (6-8) . Por otro lugar, se ha asociado la caries y la deficiente salud oral con la aparici&oacute;n de enfermedades sist&eacute;micas entre ellas cardiopat&iacute;as, enfermedad respiratoria, diabetes mellitus isqu&eacute;mica, y accidentes cerebrovasculares (9).</p>      <p>La prevalencia de caries es significativa en diferentes pa&iacute;ses del mundo (10,11). En la actualidad existen m&aacute;s de 29 m&eacute;todos para el diagn&oacute;stico de caries a nivel mundial, lo cual dificulta la realizaci&oacute;n de estudios comparativos entre las poblaciones (12). Estos m&eacute;todos se han clasificado como convencionales, entre ellos la inspecci&oacute;n visual realizado con el sistema ICDAS , sistema NYVAD, radiogr&aacute;fico y t&aacute;ctil entre otros y m&eacute;todos no convencionales como la transiluminaci&oacute;n y la fluorescencia, todos ellos difieren en aplicabilidad y precisi&oacute;n de acuerdo a la superficie donde exista la sospecha de lesi&oacute;n, estos han evolucionado a trav&eacute;s del tiempo por la necesidad de mejorar la exactitud, diagn&oacute;stico precoz, y eficacia en el momento de plantear un tratamiento cl&iacute;nico (13).</p>      <p>La caries es considerada como la primera causa de consulta odontol&oacute;gica en todos los grupos de edad (14,15) por lo anterior, los estudios demuestran una gran necesidad de implementaci&oacute;n de acciones de promoci&oacute;n y prevenci&oacute;n y tambi&eacute;n de conducir estudios epidemiol&oacute;gicos para determinar la prevalencia, inicio y severidad de caries con m&eacute;todos eficaces como el sistema ICDAS (16,-18).</p>      <p><b>Comparaci&oacute;n del Sistema ICDAS con m&eacute;todos diagn&oacute;sticos de caries dental.</b></p>      <p>Entre los m&eacute;todos diagn&oacute;sticos se encuentran, el sistema ICDAS (International Caries Detection and Assessment System) y NYVAD (Sistema creado por el autor Nyvad), exploraci&oacute;n cl&iacute;nica, inspecci&oacute;n visual, exploraci&oacute;n t&aacute;ctil con sonda, radiograf&iacute;a digital, transiluminaci&oacute;n (FOTI), m&eacute;todo de conductibilidad el&eacute;ctrica (ECM), y fluorescencia entre otros (19-24). Tambi&eacute;n existen &iacute;ndices para su diagn&oacute;stico como el ceo-d (dientes cariados, extra&iacute;dos y obturados) para dentici&oacute;n temporal, el COP-D (dientes cariados, obturados y perdidos) para dentici&oacute;n permanente los cuales permiten evaluar la prevalencia de caries (25,26).</p>      ]]></body>
<body><![CDATA[<p>El sistema ICDAS, estaba conformado por 5 criterios, este fue consensuado en Baltimore-Maryland, USA en el a&ntilde;o 2005 donde se le da el nombre de ICDAS II y se incluye el criterio 0 correspondiente a diente sano (27), resultando con 6 criterios de diagn&oacute;stico actualmente, este es muy &uacute;til para finalidades en la pr&aacute;ctica cl&iacute;nica, la investigaci&oacute;n (28-30) y el desarrollo de programas de salud p&uacute;blica (31-33). El sistema ICDAS est&aacute; dise&ntilde;ado por un conjunto de criterios y c&oacute;digos unificados, con diagn&oacute;sticos principalmente visuales, basados en las caracter&iacute;sticas de los dientes limpios y secos (34-36). La nomenclatura comprende dos d&iacute;gitos, el primero de 0 a 8 que corresponde al c&oacute;digo de restauraci&oacute;n y sellante, el n&uacute;mero 9 al c&oacute;digo de diente ausente; y el segundo d&iacute;gito de 0 a 6 al c&oacute;digo de caries en esmalte y dentina (3739). El ICDAS presenta un 70 al 85% de sensibilidad y una especificidad de 80 al 90% para detectar caries en dentici&oacute;n temporal y permanente (4042), su fiabilidad ha sido considerada como alta con un coeficiente de kappa de 0,80 (43,44) demostrando su excelente precisi&oacute;n y an&aacute;lisis significativo comparado con otros m&eacute;todos como el radiogr&aacute;fico (45-47). As&iacute; mismo, por la detecci&oacute;n temprana permite reducir la prevalencia de caries en los diferentes grupos de poblaci&oacute;n especialmente en ni&ntilde;os con dentici&oacute;n mixta, donde se afectan los primeros molares permanentes y el nivel interproximal (48-50) (<a href="#t1">Tabla 1</a>).</p>     <p align="center"><a name="t1"></a><img src="img/revistas/ceso/v28n2/v28n2a08t1.jpg"></p>      <p>Por su parte el sistema NYVAD espec&iacute;fica unos criterios donde se diferencia las lesiones de caries activas e inactivas de acuerdo con una combinaci&oacute;n de criterios visuales y t&aacute;ctiles, present&aacute;ndose tres niveles de gravedad dependiendo de la profundidad de las lesiones: superficie intacta, discontinuidad superficial en el esmalte, y cavidad evidentemente en la dentina (51) (<a href="#t2">Tabla. 2</a>).</p>     <p align="center"><a name="t2"></a><img src="img/revistas/ceso/v28n2/v28n2a08t2.jpg"></p>      <p>El sistema NYVAD, presenta una mayor especificidad y exactitud en comparaci&oacute;n con m&eacute;todos como la radiograf&iacute;a interproximal (52). Tambi&eacute;n ha sido evaluado con ICC, con resultados en NYVAD = 0,68,= 0,48, ICDAS ICC = 0,66, k=0,47, los dos m&eacute;todos se basan en el color y la textura de la superficie del diente, el m&eacute;todo NYVAD puede ser llamado descriptivo mientras que el ICDAS es prescriptivo. Es as&iacute; como los criterios de NYVAD e ICDAS-II tienen una buena reproducibilidad y validez comparables en la detecci&oacute;n de lesiones cariosas y estimaci&oacute;n de su gravedad (53). Se han reportado diferencias en el momento de comparar resultados entre los criterios ICDAS y criterios NYVAD correspondiendo a ICDAS criterios 2 y 5 con NYVAD criterio 3. Aunque los dos m&eacute;todos se han considerado como las mejores opciones de diagn&oacute;stico, el sistema ICDAS en comparaci&oacute;n con el sistema NYVAD, mejora el rendimiento en t&eacute;rminos de sensibilidad y fiabilidad (53). En un estudio donde se evaluaron lesiones cavitacionales y no cavitacionales se compar&oacute; el sistema ICDAS y NYVAD , los resultados muestran que fue significativamente mayor para ICDAS II + LAA (Lesion Activity Assessment system) (14,6 &plusmn; 5,4) que para NYVAD (3,90 &plusmn; 3,9) (p &lt;0,001). As&iacute; como tambi&eacute;n se report&oacute; que el riesgo a caries fue significativamente mayor con los criterios ICDAS II + LAA, los cuales pueden concluir a un tratamiento adecuado de la caries (54,55).</p>      <p>El sistema ICDAS en comparaci&oacute;n con el m&eacute;todo radiogr&aacute;fico, se le atribuye la detecci&oacute;n desde los primeros cambios en las propiedades &oacute;pticas del esmalte, demostrando la superioridad de los ex&aacute;menes visuales frente a los radiogr&aacute;ficos (1). En cuanto a lesiones diagnosticadas con el ICDAS presentaron un 67% las cuales hab&iacute;an progresado, a diferencia con el m&eacute;todo radiogr&aacute;fico donde solo fue de un 62%. En contraste con otro estudio donde la inspecci&oacute;n visual y m&eacute;todos radiogr&aacute;ficos presentan especificidades m&aacute;s altas, pero menor sensibilidad que los m&eacute;todos de fluorescencia laser (56). Es importante tener en cuenta que un m&eacute;todo ideal para la detecci&oacute;n de caries debe ofrecer una alta especificidad, sensibilidad, y reproductibilidad (31,57, 58).</p>      <p>Otro m&eacute;todo es la tomograf&iacute;a microcomputalizada que a pesar de su alto rendimiento diagn&oacute;stico e imagen tridimensional, no se puede constituir como una alternativa al examen histol&oacute;gico. Sin embargo, se necesita m&aacute;s investigaciones para la comparaci&oacute;n con otras modalidades diagn&oacute;sticas (45,46). Por otra parte, la transiluminaci&oacute;n y la radiograf&iacute;a han presentado una mayor especificidad y sensibilidad para la detecci&oacute;n de lesiones de caries en esmalte y dentina comparados con m&eacute;todos como la fluorescencia laser (59).</p>      <p>Los resultados demuestran que el Sistema ICDAS mediante la inspecci&oacute;n visual parece ser suficiente para ser utilizado en la pr&aacute;ctica cl&iacute;nica, la detecci&oacute;n y evaluaci&oacute;n de la profundidad de la lesi&oacute;n (33,60). Por otra parte, el formato actual del &Iacute;ndice CPO-D establecido por la OMS en el campo de la epidemiologia de caries desde 1939 proporciona informaci&oacute;n muy escasa y en algunos casos incompleta, porque se orienta principalmente a la prevalencia de caries mas no en la actividad de la lesi&oacute;n y severidad de esta (61) por lo cual se deber&iacute;a integrar con otros sistemas o m&eacute;todos como el sistema ICDAS el cual proporciona 43% m&aacute;s de informaci&oacute;n que el &iacute;ndice COP-D (14,38,59,62), para lograr un an&aacute;lisis detallado y determinar los factores de riesgo que conlleven a la detecci&oacute;n del inicio de la lesi&oacute;n de caries (63,64).</p>      <p>La aparici&oacute;n de la caries dental se asocia con factores socioculturales, econ&oacute;micos, ambientales y del comportamiento logrando afectar entre el 60 % y 90 % de la poblaci&oacute;n escolar seg&uacute;n la OMS. En Colombia a pesar de todos los esfuerzos realizados, las enfermedades dentales siguen ocupando el segundo lugar de la morbilidad general de la poblaci&oacute;n (14). En los resultados del ENSAB IV el &iacute;ndice ceo-d y COP-D se redujeron en comparaci&oacute;n con la ENSAB III pasando a un ceo-d de 3,0 a 2,83 y COP-D de 2,3 a 1,51. A nivel de los j&oacute;venes de 15 a&ntilde;os se increment&oacute; a 2,35 y para los de 18 a&ntilde;os a 3,18, y contin&uacute;a de forma progresiva con la edad hasta llegar a 15,26 en el grupo de 45 a 64 a&ntilde;os y a 20,55 en los mayores de 65 a&ntilde;os (65). Por lo cual su detecci&oacute;n precoz con m&eacute;todos como el sistema ICDAS es fundamental para la aplicaci&oacute;n de medidas terap&eacute;uticas, preventivas, espec&iacute;ficas y oportunas (14).</p>      <p><font size="3"><b>Discusi&oacute;n</b></font></p>      ]]></body>
<body><![CDATA[<p>La caries dental es considerada como una enfermedad localizada, la cual puede evolucionar desde cambios en el esmalte y la dentina hasta</p>     <p>llegar a ser cavitacional y producir compromiso pulpar (33). Los &iacute;ndices como el ceo-d y el COP-D y sistemas como el ICDAS y NYVAD son importantes para establecer un diagn&oacute;stico temprano (28,29). El m&eacute;todo NYVAD puede ser llamado descriptivo mientras que el ICDAS es prescriptivo. Es as&iacute; como los criterios de NYVAD e ICDAS-II tienen una buena reproducibilidad y validez comparables en la detecci&oacute;n de lesiones cariosas y estimaci&oacute;n de su gravedad (53). Aunque el ICDAS ha demostrado que mejora el rendimiento en t&eacute;rminos de sensibilidad y fiabilidad as&iacute; como tambi&eacute;n tiene mayor significancia para concluir un tratamiento adecuado de caries en comparaci&oacute;n con el sistema NYVAD (45,46). Estudios demuestran que el ICDAS presenta una mayor profundidad del diagn&oacute;stico en la comparaci&oacute;n con el m&eacute;todo radiogr&aacute;fico, por lo cual este es considerado m&aacute;s preciso y con una reproductividad excelente (33,60), igualmente a este se le atribuye la detecci&oacute;n desde los primeros cambios en las propiedades &oacute;pticas del esmalte lo cual no es observable en el examen radiogr&aacute;fico (1). En contraste con otros estudios donde la fluorescencia laser tiene una sensibilidad m&aacute;s alta contrario al examen radiogr&aacute;fico y la inspecci&oacute;n visual del sistema ICDAS que presentaron especificidades m&aacute;s altas pero baja sensibilidad (56). Por otra parte la t&eacute;cnica de transiluminaci&oacute;n y la radiogr&aacute;fica tuvieron mayor especificidad y sensibilidad que la fluorescencia laser y la inspecci&oacute;n visual (60). M&eacute;todos como los radiogr&aacute;ficos permiten establecer con &eacute;xito lesiones cuando se tiene m&aacute;s de un 30 a 40% de perdida tejido dental donde permite determinar la cercan&iacute;a con el nervio pulpar (41), en comparaci&oacute;n con la fluorescencia y transiluminaci&oacute;n donde su precisi&oacute;n no es tan exacta (33).</p>      <p>A pesar de las nuevas tecnolog&iacute;as, los m&eacute;todos visuales se siguen considerando como los m&aacute;s precisos, espec&iacute;ficos y sensibles en la detecci&oacute;n de caries proximales (59). Por otro lugar es importante complementar &iacute;ndices y m&eacute;todos para el diagn&oacute;stico de caries (15,21,22), que contribuyan y permitan complementar el diagn&oacute;stico, pron&oacute;stico y tratamiento cl&iacute;nico (8).</p>      <p><font size="3"><b>Conclusiones</b></font></p>      <p>El ICDAS proporciona 43% m&aacute;s de informaci&oacute;n que el &iacute;ndice COP-D y parece ser suficiente para ser utilizado en la pr&aacute;ctica cl&iacute;nica en la detecci&oacute;n y evaluaci&oacute;n de la profundidad de la lesi&oacute;n.</p>      <p>Los m&eacute;todos diagn&oacute;sticos son diversos pero el sistema ICDAS ha demostrado una alta sensibilidad, reproductividad y precisi&oacute;n para la detecci&oacute;n de caries desde su etapa inicial, determinando su actividad y severidad comparado con oros sistemas como el NYVAD y m&eacute;todos como el radiogr&aacute;fico. Es importante complementar &iacute;ndices y m&eacute;todos para el diagn&oacute;stico de caries que contribuyan y permitan complementar el diagn&oacute;stico, pron&oacute;stico y tratamiento cl&iacute;nico.</p> <hr>      <p><font size="3"><b>Referencias</b></font></p>      <!-- ref --><p>1.
