<body><![CDATA[<P><B>Bloating  y distensi&oacute;n abdominal </B></P>     <P>Dentro del variado grupo de los trastornos funcionales digestivos, nos encontramos con unas molestias muy comunes como son el Bloating y la distensi&oacute;n abdominal (B/DA) que, a pesar de su aparente inocuidad, pueden producir una significativa alteraci&oacute;n de la calidad de vida, particularmente cuando acompa&ntilde;an cuadros como el s&iacute;ndrome de intestino irritable (1). Los t&eacute;rminos de distensi&oacute;n abdominal y el Bloating, en espa&ntilde;ol, no tienen una clara diferenciaci&oacute;n como ocurre en la literatura anglosajona, en la cual frecuentemente se utilizan de manera particular: Bloating para la sensaci&oacute;n subjetiva (s&iacute;ntoma) de distensi&oacute;n del abdomen y el t&eacute;rmino &quot;distensi&oacute;n abdominal&quot; para la valoraci&oacute;n objetiva, visible, al examen cl&iacute;nico (signo). La Fundaci&oacute;n Roma incluy&oacute; en la &uacute;ltima edici&oacute;n de Criterios Diagn&oacute;sticos Roma III de los Trastornos Funcionales Digestivos (2, 3), dentro de los Des&oacute;rdenes Intestinales Funcionales, a la condici&oacute;n C2: &quot;Functional Bloating&quot;, cuando no se encuentra asociada a otro desorden intestinal o gastroduodenal, y dado que todos los criterios de este proceso son basados en s&iacute;ntomas, al describir la definici&oacute;n, esta dice que es una sensaci&oacute;n de distensi&oacute;n abdominal que puede estar asociada o no con una distensi&oacute;n medible, lo cual permite inferir que el s&iacute;ntoma y el signo pueden estar presentes bajo esta misma denominaci&oacute;n. Ahora bien, al realizar la traducci&oacute;n al espa&ntilde;ol, con car&aacute;cter oficial y con el apoyo del Comit&eacute; de Traducciones de la Fundaci&oacute;n Roma, la Revista de Gastroenterolog&iacute;a de M&eacute;xico, traduce el t&eacute;rmino &quot;C2: Functional Bloating&quot; como &quot;C2 Inflamaci&oacute;n / Distensi&oacute;n funcional&quot; y en los criterios diagn&oacute;sticos adicionan otra palabra como &quot;Hinchaz&oacute;n&quot;, seg&uacute;n se lee textualmente: &quot;Sensaci&oacute;n recurrente de inflamaci&oacute;n/ hinchaz&oacute;n o distensi&oacute;n visible cuando menos tres d&iacute;as al mes en los &uacute;ltimos tres meses&quot; (4). Lo cual resulta al menos un poco confuso pues las palabras &quot;hinchaz&oacute;n&quot; e &quot;inflamaci&oacute;n&quot; tienen una acepci&oacute;n de car&aacute;cter m&aacute;s org&aacute;nico que funcional.</P>     <P>Mediante el uso de pletismografia de inductancia abdominal y tomograf&iacute;a abdominal computarizada se ha podido demostrar convincentemente que la distensi&oacute;n abdominal es un fen&oacute;meno real en pacientes con SII, y en algunos de estos puede alcanzar un aumento de 12 cms del per&iacute;metro abdominal (5). Aunque la sensaci&oacute;n de Bloating y la distensi&oacute;n abdominal visible a menudo ocurren juntas no siempre es as&iacute;, pues si bien muchos pacientes con SII se quejan de Bloating solo en aproximadamente la mitad se logra demostrar objetivamente distensi&oacute;n abdominal (5, 6).</P>     <P>Ante estas dificultades planteadas, el prop&oacute;sito es revisar las diferentes perspectivas con que se est&aacute; abordando el problema desde el punto de vista fisiopatol&oacute;gico (<a href="#tabla1">tabla 1</a>), sin pretender zanjar las diferencias en los t&eacute;rminos ling&uuml;&iacute;sticos o en las caracter&iacute;sticas cl&iacute;nicas de estos elementos.</P>     <P align="center"><a href="#tabla1">Tabla 1</a>.  Bloating y distensi&oacute;n abdominal.</P>     <div align="center"><img src="img/revistas/rcg/v26n4/a06t1.JPG"><a name="tabla1"></a> </div>     <P><B>EPIDEMIOLOGIA</B></P>     <P>El Bloating / distensi&oacute;n abdominal (B/DA), ocurre frecuentemente dentro de los des&oacute;rdenes funcionales intestinales, caracter&iacute;sticamente se puede exacerbar con las comidas, fluct&uacute;a de intensidad, empeora al final del d&iacute;a y puede aumentar a medianoche. Estudios de poblaci&oacute;n estiman una prevalencia del 16 al 30% (7) y en pacientes con s&iacute;ndrome de intestino irritable (SII) m&aacute;s del 90% de ellos refieren sufrir de B/DA siendo al parecer m&aacute;s com&uacute;n en mujeres con una relaci&oacute;n 2 a 1 (8). Si bien puede estar presente en ambos subtipos de SII su prevalencia var&iacute;a en un 75% en SII con predominio de estre&ntilde;imiento y en 40% en SII con predominio diarrea (9).</P>     <P>Si bien la mujeres sanas informan cambios en el h&aacute;bito intestinal durante la menstruaci&oacute;n, el 40 a 75% de pacientes con SII informan que el B/DA se exacerba perimestrualmente, lo cual sugerir&iacute;a la posibilidad de una influencia hormonal en la variabilidad de los s&iacute;ntomas (8).</P>     <P><B>FISIOPATOGENIA</B></P>     ]]></body>
<body><![CDATA[<P><B>1. Factores psicosociales </B></P>     <P>Es claro que los s&iacute;ntomas de B/DA hacen parte de los trastornos funcionales digestivos y comparte con ellos los mecanismos similares de su fisiopatolog&iacute;a por lo cual no son ajenos a uno de su componentes primordiales en su g&eacute;nesis, como son los factores de car&aacute;cter psicosocial. En los &uacute;ltimos a&ntilde;os, los avances en la comprensi&oacute;n de la fisiopatolog&iacute;a de los TFD destacan interesantes aspectos que oscilan desde factores psicosociales muy significativos en la infancia (Trauma, abuso) o tambi&eacute;n alteraciones neurofuncionales en la corteza anterior de c&iacute;ngulo (10, 11).</P>     <P>Pese a lo anterior, el sustentar los factores psicosociales y su relaci&oacute;n con B/DA resulta controversial pues muchos de los pacientes relacionan el s&iacute;ntoma con estr&eacute;s (8) y en otros no se encuentran asociados a cuadros cl&iacute;nicos de ansiedad o depresi&oacute;n (12).</P>     <P>Sin embargo, lo que s&iacute; es probable es que el manejo de la informaci&oacute;n aferente y su respuesta se caractericen por una disregulaci&oacute;n entre el sistema nervioso central y el sistema nervioso ent&eacute;rico secundario a factores psicosociales (11).</P>     <P><B>2. Papel del gas </B></P>     <P><B>Exceso de gas </B></P>     <P>Muchos pacientes con SII que se quejan de B/DA, est&aacute;n convencidos que esto es debido a aumento del contenido del gas en abdomen, y dicha posibilidad podr&iacute;a f&aacute;cilmente sustentarse a trav&eacute;s de la fermentaci&oacute;n de carbohidratos no digeridos por las bacterias col&oacute;nicas, o del exceso de aire deglutido (13). A pesar de esto, t&eacute;cnicas de lavado de gas y de TAC han permitido precisar con modernos programas de an&aacute;lisis de im&aacute;genes que este no es el principal problema en la mayor&iacute;a de los pacientes sino especialmente los defectos en la distribuci&oacute;n y el tr&aacute;nsito del gas (14).</P>     <P><B>Tr&aacute;nsito del gas </B></P>     <P>En un estudio con infusi&oacute;n de gas en el yeyuno en voluntarios sanos y en pacientes con SII, 18 de 20 pacientes con SII reten&iacute;an gas, desarrollando distensi&oacute;n o s&iacute;ntomas abdominales y en 16 de 20 voluntarios no ocurr&iacute;a lo mismo (15); adem&aacute;s, estos cambios pueden ser aumentados adicionando una infusi&oacute;n enteral de l&iacute;pidos, lo cual puede proveer una explicaci&oacute;n al &quot;Bloating&quot; postprandial (16). Otra experiencia adicional valorando el tr&aacute;nsito orocecal y col&oacute;nico y con la utilizaci&oacute;n de la pletismografia de inductancia abdominal, pudo establecer que en pacientes con SII predominio estre&ntilde;imiento, el tr&aacute;nsito orocecal y col&oacute;nico lento est&aacute; directamente relacionado con la distensi&oacute;n abdominal, e inversamente con la consistencia de las heces (17). Hallazgos similares recientemente se publicaron utilizando t&eacute;cnicas de centelleograf&iacute;a con xen&oacute;n radiactivo donde pacientes con SII y predominio de estre&ntilde;imiento mostraban alteraci&oacute;n del aclaramiento de una carga de gas en colon derecho, acompa&ntilde;ada de mayor sensaci&oacute;n de &quot;Bloating&quot; y de distensi&oacute;n abdominal medida por circunferencia abdominal (18).</P>     <P><B>3. Disfunci&oacute;n de la sensibilidad visceral</B></P>     ]]></body>
<body><![CDATA[<P>Est&aacute; bien establecido que una gran proporci&oacute;n de pacientes con SII tienen hipersensibilidad visceral y se podr&iacute;a plantear que este proceso tambi&eacute;n contribuye a la sensaci&oacute;n de Bloating (7) y en pacientes con estos des&oacute;rdenes funcionales, un est&iacute;mulo normal, o peque&ntilde;as variaciones del contenido de gas dentro del intestino puedan ser percibidos como Bloating (19, 20).</P>     <P>La hipersensibilidad visceral puede ser mediada por factores neuroendocrinos, la permeabilidad intestinal incrementada, la alteraci&oacute;n en los impulsos aferentes gastrointestinales y su procesamiento central (19). Tambi&eacute;n se ha encontrado que el sistema nervioso aut&oacute;nomo modula la sensibilidad visceral y es sabido que un tono simp&aacute;tico incrementado aumenta los niveles de percepci&oacute;n de los est&iacute;mulos intestinales (21) y ya que algunos pacientes con SII tienen una actividad simp&aacute;tica aumentada este mecanismo podr&iacute;a jugar un papel en el &quot;Bloating&quot; (22). En cuanto a los niveles corticales, tambi&eacute;n es reconocido que los pacientes con SII tienen una atenci&oacute;n mental incrementada a los est&iacute;mulos intestinales de ah&iacute; que aquellos que presentan &quot;Bloating&quot; coloquen una mayor atenci&oacute;n a su abdomen, lo cual constituye una especie de hipervigilancia de su abdomen y todo lo que proviene del intestino (22, 23).</P>    <P>Algunas experiencias cl&iacute;nicas muestran que hay una relaci&oacute;n entre el s&iacute;ntoma de &quot;Bloating&quot; sin distensi&oacute;n objetiva y la presencia de hipersensibilidad visceral; en contraste, si hay un aumento del per&iacute;metro abdominal, durante el curso del d&iacute;a, est&aacute; m&aacute;s asociado a hiposensibilidad y a pacientes con SII con predominio de estre&ntilde;imiento (24, 25).</P>     <P><B>4. Disfunci&oacute;n viscerosom&aacute;tica y disinergia abdominofr&eacute;nica </B></P>     <P>Este es uno de los aspectos fisiopatol&oacute;gicos de m&aacute;s reciente desarrollo e inter&eacute;s de los grupos que investigan el B/DA. Se han propuesto mecanismos como la depresi&oacute;n diafragm&aacute;tica, el exceso de lordosis lumbar, la debilidad de la musculatura abdominal y la protrusi&oacute;n del abdomen como posibles eventos de car&aacute;cter mec&aacute;nico e incluso de control voluntario, en relaci&oacute;n con la musculatura abdominal; sin embargo, su verificaci&oacute;n resulta por lo menos controvertida (26).</P>     <P>Podr&iacute;a asumirse que hay alg&uacute;n tipo de reflejo innato de la musculatura de la pared anterior que permite la acomodaci&oacute;n a la ingesti&oacute;n de las comidas o al &uacute;tero gr&aacute;vido o para mantener el tono abdominal apropiado cuando se est&aacute; de pie o sentado. Es posible que dicho reflejo est&eacute; alterado o exagerado en pacientes que sufren B/DA (7, 29). El uso de electromiograf&iacute;a y an&aacute;lisis bastantes sofisticados de TAC, realizados por el grupo de Barcelona, a voluntarios y pacientes con SII y B/DA, ha demostrado que la acomodaci&oacute;n abdominal a la carga de volumen es un proceso activo que envuelve respuestas musculares abdominofr&eacute;nicas (27). Y los pacientes con SII y &quot;Bloating&quot; tienen una alteraci&oacute;n de los reflejos viscerosom&aacute;ticos con diston&iacute;a de la pared abdominal y con una falla de la contracci&oacute;n t&oacute;nica de la musculatura acompa&ntilde;ada de una relajaci&oacute;n parad&oacute;jica de los oblicuos internos en repuesta a una carga de gas (28). En otra experiencia (30) el grupo compara pacientes con &quot;Bloating&quot; como queja principal con pacientes con dismotilidad intestinal severa y a ambos grupos se les practica TAC en condiciones basales y durante los episodios de &quot;Bloating&quot; severo. No se apreciaron diferencias a nivel basal, pero en los episodios de &quot;Bloating&quot; se encontr&oacute; que los pacientes con dismotilidad demostraron un verdadero aumento del volumen total abdominal con un desplazamiento cef&aacute;lico del diafragma, mientras que los pacientes con des&oacute;rdenes funcionales solo mostraban un peque&ntilde;o aumento del volumen intrabdominal. La distensi&oacute;n estaba relacionada con un descenso del diafragma resultado de la distribuci&oacute;n ventrocaudal del contenido (30) Estos hallazgos se han catalogado de gran valor en la demostraci&oacute;n de la presencia de reflejos viscerosom&aacute;ticos anormales en la generaci&oacute;n de la distensi&oacute;n abdominal (31). Y otro elemento de gran valor en estos estudios es que permiten adicionar otros criterios diagn&oacute;sticos, para diferenciar si la distensi&oacute;n abdominal es solo funcional o es org&aacute;nica, pues en este &uacute;ltimo caso hay un aumento total del volumen y del contenido abdominal y hay desplazamiento cef&aacute;lico del diafragma (30).</P>     <P>Sin embargo, se cuestiona el hecho de que no se incluyeron pacientes con SII con predominio de diarrea ni pacientes con sensaci&oacute;n de &quot;Bloating&quot; solamente, lo cual dificultar&iacute;a el poder generalizar a todos los pacientes estos conceptos.</P>     <P>Recientemente, el mismo grupo nos muestra otra experiencia similar (32) en la cual buscan establecer el papel de diafragma en la DA, y compara adultos sanos y pacientes con DA, a quienes se les realiza una carga de gas, valorando la circunferencia y electromiogr&aacute;ficamente la musculatura de la pared abdominal y el diafragma, demostrando que los pacientes desarrollan significativamente m&aacute;s distensi&oacute;n con una contracci&oacute;n parad&oacute;jica del diafragma y una relajaci&oacute;n del m&uacute;sculo oblicuo interno (32). Lo interesante de este concepto es que abre nuevas expectativas de tratamientos que podr&iacute;an estar dirigidas hacia la correcci&oacute;n de la inapropiada contracci&oacute;n del diafragma con tratamiento de bio-feedback para prevenir la protrusi&oacute;n anterior de la pared abdominal como otra forma de manejo del B/DA (32, 33).</P>     <P><B>5. Papel de la microbiota </B></P>     <P>La noci&oacute;n de que la microbiota intestinal puede estar alterada en pacientes con SIII y B/DA, se ha fundamentado en varias observaciones.</P>     ]]></body>
<body><![CDATA[<P>En primera instancia se ha propuesto que la fermentaci&oacute;n bacteriana de carbohidratos no digeridos aumenta la producci&oacute;n de &aacute;cidos grasos de cadena corta y tambi&eacute;n de gases como el di&oacute;xido de carbono, hidr&oacute;geno y metano. Y m&aacute;s recientemente se demostr&oacute; que en pacientes con SII, la microbiota fecal est&aacute; alterada y la producci&oacute;n de &aacute;cido propi&oacute;nico est&aacute; aumentada. Estos pacientes mostraron un significativo aumentos de Lactobacillus y Veillonella y adem&aacute;s que la producci&oacute;n de &aacute;cido ac&eacute;tico y propi&oacute;nico estuvo relacionada con aumento de los s&iacute;ntomas (dolor abdominal, &quot;Bloating&quot; y variaci&oacute;n del h&aacute;bito intestinal) y una disminuci&oacute;n de la calidad de vida (34, 35). Otra serie muestra c&oacute;mo la presencia de Ruminococcus torques est&aacute; asociada a una mayor severidad de los s&iacute;ntomas en SII (36).</P>     <P>Este aumento en la producci&oacute;n de gas en pacientes con SIII y B/DA (5) ha sido demostrado por rayos X donde parece que los pacientes contienen mayores vol&uacute;menes de gas que los controles (37). E igualmente se han hecho evaluaciones en donde la cantidad gas producida por pacientes con SII es superior a los controles (38); esto contrasta con lo mencionado atr&aacute;s seg&uacute;n lo cual lo importante era el trastorno en el tr&aacute;nsito del gas o la alteraci&oacute;n de la sensibilidad del mismo.</P>     <P>Ahora bien, el sobrecrecimiento bacteriano del intestino delgado (SBID, en ingl&eacute;s SIBO), es una condici&oacute;n cl&iacute;nica causada por un excesivo numero de bacterias en el intestino delgado y caracterizado por s&iacute;ntomas como diarrea, dolor abdominal o Bloating, el cual puede estar asociado al exceso de gas debido a una producci&oacute;n aumentada por fermentaci&oacute;n bacteriana en el intestino (39). Uno de los argumentos de mayor inter&eacute;s pero tambi&eacute;n bastante controvertido y debatido es la relaci&oacute;n de la microbiota intestinal, en particular entre el SBID y la g&eacute;nesis del SII (40, 41). La controversia se inicia desde su diagn&oacute;stico, si bien se considera el est&aacute;ndar de oro al conteo bacteriano superior a 10 x 5 unidades formadoras de colonias / mL en cultivo de aspirado yeyunal, sus limitaciones para obtener la muestra, el car&aacute;cter invasivo, evitar su contaminaci&oacute;n y mantener condiciones de anaerobiosis, lo han cuestionado considerablemente (42). Se han propuesto test de aliento para evaluar el SBID utilizando el metabolismo de carbohidratos por enzimas bacterianas como la prueba aliento de hidr&oacute;geno lactulosa (PAHL).</P>     <P>El grupo del doctor Pimentel ha insistido en la relaci&oacute;n entre SBID y SII, y con su muy interesante editorial (43) apoyado en evidencia propia y de otros estudios propone una modificaci&oacute;n del los postulados de Koch para establecer la relaci&oacute;n y la g&eacute;nesis entre estas 2 condiciones. Ellos encontraron que un 78% de 202 pacientes con SII ten&iacute;an test positivo para PAHL, lo cual era sugestivo para SBID y encontraron una significativa mejor&iacute;a de los s&iacute;ntomas incluido el dolor abdominal, el &quot;Bloating&quot; y la diarrea tratando el SBID con antibi&oacute;ticos y convirtieron los criterios de Roma de positivos a negativos (44) y experiencias similares especialmente a nivel de tratamiento sustentan esta asociaci&oacute;n (45-47).</P>     <P>Sin embargo, se insiste en que la controversia no est&aacute; totalmente saldada pues prevalecen dudas sobre la efectividad de los m&eacute;todos diagn&oacute;sticos para SBID (48, 49); la prevalencia de SBID en pacientes con SII es variable (49) y en general aspectos conceptuales, epidemiol&oacute;gicos y terap&eacute;uticos que requerir&aacute;n ser clarificados (41, 49).</P>     <P><B>6. Papel de los alimentos</B></P>     <P>Se han reconocido al menos 2 mecanismos mediante los cuales algunos alimentos generan B/DA. De una parte la intolerancia, en caso de alimentos grasos, carbohidratos de baja digestibilidad (trigo, ma&iacute;z), az&uacute;cares no absorbibles (bebidas gaseosas), fibra insoluble, lactosa (productos l&aacute;cteos) y vegetales (repollo, coliflor). Estos alimentos pueden influir en la generaci&oacute;n de los s&iacute;ntomas en el tracto gastrointestinal a trav&eacute;s de muchas v&iacute;as que incluyen la activaci&oacute;n de los mecanorreceptores por el volumen y sus propiedades f&iacute;sicas y la activaci&oacute;n de los quimiorreceptores, as&iacute; como componentes no absorbibles pueden influir a trav&eacute;s de acciones osm&oacute;ticas y de la fermentaci&oacute;n en el colon (50-52).</P>     <P>De otro lado, el mecanismo es la hipersensibilidad, en la cual la activaci&oacute;n del sistema inmune, a trav&eacute;s de fen&oacute;menos al&eacute;rgicos e incluso mecanismos de inflamaci&oacute;n m&iacute;nima de la mucosa intestinal se encuentran involucrados en la aparici&oacute;n de estos s&iacute;ntomas (53-55).</P>     <P>Como una consecuencia pr&aacute;ctica, no sobrar&iacute;a indicar aqu&iacute; la importancia de la restricci&oacute;n de los alimentos mencionados en el manejo de estos pacientes. </P>     <P><B>7. Algunas alteraciones motoras </B></P>     ]]></body>
<body><![CDATA[<P><B>Papel de la motilidad gastrointestinal</B></P>     <P>Si bien ya se ha aludido al importante papel del tr&aacute;nsito intestinal en el manejo del gas, ahora la atenci&oacute;n se dirige a la presencia de alteraciones espec&iacute;ficas de la funci&oacute;n motora g&aacute;strica y su relaci&oacute;n con B/DA y es el caso de la gastroparesia en la cual es observada muy com&uacute;nmente este s&iacute;ntoma y produce un afecci&oacute;n de la calidad de vida de los pacientes, lo cual requiere atenci&oacute;n terap&eacute;utica en este subgrupo de pacientes (56).</P>     <P><B>Papel de la funci&oacute;n anorrectal </B></P>     <P>Este aspecto de la fisiopatolog&iacute;a del B/DA ha tenido poca relevancia, pero genera inter&eacute;s dado que se observa alteraci&oacute;n de la funci&oacute;n anorrectal especialmente en pacientes con estre&ntilde;imiento. En esta observaci&oacute;n se encontr&oacute; que estos pacientes muestran un tiempo de expulsi&oacute;n del bal&oacute;n rectal prolongado y es un predictor de distensi&oacute;n abdominal, lo cual sugiere que la inefectiva evacuaci&oacute;n del gas y las heces con la prolongada expulsi&oacute;n del bal&oacute;n rectal puede ser uno de los mecanismo del B/DA (57).</P>     <P>Estas observaciones no pueden ser miradas fuera del contexto de los pacientes con trastornos funcionales digestivos en los cuales las alteraciones motoras son parte important&iacute;sima de su g&eacute;nesis. </P>     <P><B>TRATAMIENTO</B></P>     <P>Solo mencionaremos aqu&iacute; una visi&oacute;n general del tratamiento actual del B/DA, en la cual podemos observar una gran diversidad de opciones. Una revisi&oacute;n reciente muestra detalladamente posibilidades que oscilan entre el manejo de la dieta, y los probi&oacute;ticos, as&iacute; como tambi&eacute;n otras alternativas: Surfactantes como la simeticona, antiespasm&oacute;dicos: trimebutina, bromuro de otilonio; laxantes osm&oacute;ticos o incrementadotes del volumen, estimulantes de la secreci&oacute;n de fluidos: lubiprostone, procin&eacute;ticos, antibi&oacute;ticos, prebi&oacute;ticos, probi&oacute;ticos etc. (58) (<a href="#tabla2">tabla 2</a>).</P>     <P align="center"><a href="#tabla2">Tabla 2</a>. <body><![CDATA[<P>Es importante recordar que el manejo de esta queja hace parte del contexto general de un paciente con un trastorno funcional digestivo y que requiere contemplar al paciente en su entorno e incluso considerar el uso de gu&iacute;as de manejo estandarizadas, para el adecuado enfoque (59).</P>     <P><B>CONCLUSIONES</B></P>     <P>Se han revisado los conceptos fisiopatol&oacute;gicos que est&aacute;n en relaci&oacute;n con unas quejas muy comunes de los pacientes con trastornos funcionales digestivos como son el &quot;Bloating&quot; y la distensi&oacute;n abdominal. Se insiste en mantener la diferencia ling&uuml;&iacute;stica entre los dos t&eacute;rminos, el primero para el s&iacute;ntoma y el segundo para el signo. Se revisan los distintos mecanismos en relaci&oacute;n a la producci&oacute;n y manejo del gas intestinal, las alteraciones del tr&aacute;nsito y motoras, la respuesta sensorial y viscerosom&aacute;tica, en particular la disfunci&oacute;n o disinergia abdominofr&eacute;nica. Tambi&eacute;n se analiz&oacute; la participaci&oacute;n de la microbiota intestinal en la fisiopatolog&iacute;a del B/DA haciendo especial consideraci&oacute;n de la controvertida participaci&oacute;n del SBID. Finalmente, se hicieron algunas consideraciones en relaci&oacute;n a los alimentos y una visi&oacute;n panor&aacute;mica del manejo del B/DA.</P>     <P><B>REFERENCIAS</B></P>     <!-- ref --><P>1.
