Fuente: la búsqueda se realiza en PubMed, Science Direct e ISI Web of Knowledge usando palabras clave: estimulación eléctrica del colon, estreñimiento. Se consideraron los estudios en humanos y los estudios experimentales cuyo objetivo era intervenir los patrones de motilidad del colon en niños y adultos con estas técnicas. Principales conclusiones: el estreñimiento es parte de los trastornos de la motilidad gastrointestinal. Es una patología de etiología multifactorial, con grave impacto sobre la calidad de vida en los casos resistentes a la terapia convencional. La cirugía, que se utiliza en los casos más graves, no está exenta de complicaciones y los resultados están lejos de curar a los enfermos. Algunos medicamentos han sido eficaces en la enfermedad leve, aunque en los casos graves no se ha demostrado su utilidad. Otras estrategias han sido desarrolladas e incluyen estimulación eléctrica del colon con diferentes metodologías, con el objetivo de aumentar la motilidad del órgano. Se hace una extensa revisión de la literatura disponible como punto de partida para enfocar los esfuerzos por realizar. Conclusión: la estimulación eléctrica en pacientes con estreñimiento severo es una terapia prometedora para lograr, desde el punto de vista fisiológico, el restablecimiento de la función del colon, en busca de la menor invasión posible en este grupo de pacientes.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Objective: The objective of this study is to present an overview of current treatments for constipation based on techniques to directly or indirectly stimulate the colon electrically. Sources: This study is based on searches of PubMed, ScienceDirect.com, and ISI Web of Knowledge using the keywords electrical stimulation of the colon and constipation. We considered studies of these techniques in humans and experimental studies aimed at intervening in colon motility patterns in children and adults. Principal Conclusions: Constipation is a disorder of gastrointestinal motility. Its etiology is multifactorial etiology, it has severe impacts on patient quality of life, and it is resistant to conventional therapy in many cases. Surgery, used in the most severe cases, has the risk of complications and the results are far from what is needed to cure the patients. Some drugs have been effective for treating milder cases, but have not been proven useful for more severe cases has. Other strategies have been developed to increase motility including various methods of electrical stimulation of the colon. This extensive review of the available literature is a starting point for focusing additional efforts in this area. Conclusion: Electrical stimulation of patients with severe constipation is a promising therapy to achieve restoration of colon function from the physiological point of view in the least invasive manner possible.]]></p></abstract>

<body><![CDATA[<p><b>Palabras clave</b></p>     <p>Constipaci&oacute;n, inercia col&oacute;nica, estimulaci&oacute;n el&eacute;ctrica,   colon, motilidad gastrointestinal, tr&aacute;nsito gastrointestinal.</p>     <p>La constipaci&oacute;n cr&oacute;nica o estre&ntilde;imiento cr&oacute;nico es una   patolog&iacute;a que en diferentes evaluaciones poblacionales tiene una prevalencia   del 2%, con otros resultados que muestran una prevalencia del 21% en mujeres y   8% en hombres (1), con una incidencia general entre el 2% y el 30% en la   poblaci&oacute;n general (2). En Latinoam&eacute;rica se reporta que el estre&ntilde;imiento cr&oacute;nico   tiene una prevalencia estimada del 5%-21% en la regi&oacute;n, con una relaci&oacute;n   mujer:var&oacute;n de 3:1 (3), con alta afectaci&oacute;n en la calidad de vida de los   pacientes y sus familias (4). Debido a la gran cantidad de interpretaciones que   se le dan a la constipaci&oacute;n y a la evacuaci&oacute;n (5), se han tratado de clasificar   los s&iacute;ntomas y las caracter&iacute;sticas de la constipaci&oacute;n, de la evacuaci&oacute;n y de   las heces utilizando los criterios de Roma III (6). Hay diferentes espectros de   la enfermedad, que esta puede clasificarse desde el punto de vista cl&iacute;nico o   anat&oacute;mico teniendo en cuenta si el compromiso es difuso del colon o si hay un   trastorno de la evacuaci&oacute;n. En algunos casos, los ex&aacute;menes diagn&oacute;sticos son   normales pero el paciente tiene sintomatolog&iacute;a importante. En t&eacute;rminos   generales, entonces, el estre&ntilde;imiento cr&oacute;nico se clasifica en estre&ntilde;imiento de   tr&aacute;nsito lento, que se divide en inercia col&oacute;nica e hiperreactividad col&oacute;nica, trastorno   de la evacuaci&oacute;n y s&iacute;ndrome de intestino irritable, con predominio del   estre&ntilde;imiento (constipaci&oacute;n funcional) (7, 8). La estrategia terap&eacute;utica se   basa entonces en la determinaci&oacute;n mediante ex&aacute;menes paracl&iacute;nicos del compromiso   del colon que diagnosticar&aacute; cualquiera de las variables mencionadas (9). El   grupo m&aacute;s complejo de pacientes a tratar son aquellos que cursan con inercia   col&oacute;nica o constipaci&oacute;n de tr&aacute;nsito lento, que se describe como la forma de   constipaci&oacute;n con evacuaciones con frecuencias tan bajas de 2 a 4 semanas, que   no est&aacute; asociada con dilataci&oacute;n demostrada del colon o el recto (10). Se han   propuesto m&uacute;ltiples tratamientos para el manejo de la constipaci&oacute;n, que van   desde el tratamiento diet&eacute;tico, que puede fracasar en la mitad de los casos   aproximadamente (11), hasta la colectom&iacute;a total (12). Se han desarrollado   terapias con dispositivos m&eacute;dicos, todas ellas basadas en la estimulaci&oacute;n   el&eacute;ctrica o magn&eacute;tica del colon o de los troncos nerviosos, desde su origen en   el sacro, e incluso de los troncos nerviosos perif&eacute;ricos, que pueden servir de   est&iacute;mulos para los nervios propios del colon (13); sin embargo, los   dispositivos de estimulaci&oacute;n el&eacute;ctrica han sido dise&ntilde;ados para el m&uacute;sculo   card&iacute;aco o el tejido nervioso, que responden r&aacute;pidamente a la estimulaci&oacute;n   el&eacute;ctrica, mientras que el m&uacute;sculo liso gastrointestinal es lento en su   respuesta y, por tanto, son necesarios pulsos largos para alterar su funci&oacute;n (14).</p>     <p>La constipaci&oacute;n cr&oacute;nica es una patolog&iacute;a del aparato   gastrointestinal que puede ir desde una simple molestia hasta una enfermedad   grave que presenta alteraciones en la calidad de vida de las personas y sus   familias. Los ni&ntilde;os con constipaci&oacute;n pueden parecer quietos, retra&iacute;dos,   apenados y malgeniados en las evaluaciones m&eacute;dicas (4) y esto incluye a sus   cuidadores (8) con igual impacto en la calidad de vida de los adultos (15). La   fisiopatolog&iacute;a de la constipaci&oacute;n cr&oacute;nica primaria es multifactorial e incluye   dieta, absorci&oacute;n y motilidad col&oacute;nica, funci&oacute;n motora y sensorial y factores   psicol&oacute;gicos y conductuales (16). Por tanto, muchas veces es dif&iacute;cil guiar el   tratamiento de estos enfermos, m&aacute;s cuando se decide hacer tratamientos   invasivos. El diagn&oacute;stico de una condici&oacute;n con tantas variables no es f&aacute;cil,   por eso son de utilidad las diferentes escalas que orienten el diagn&oacute;stico,   eval&uacute;an la severidad de los s&iacute;ntomas y su impacto en la calidad de vida, as&iacute;   como la respuesta a la terapia y los cambios de los s&iacute;ntomas a trav&eacute;s del   tiempo (17). Basados en los diferentes tipos y etiolog&iacute;as de la constipaci&oacute;n,   se debe orientar el tratamiento utilizando diferentes m&eacute;todos diagn&oacute;sticos:   tr&aacute;nsito col&oacute;nico con marcadores radiopacos o is&oacute;topos, pruebas de funci&oacute;n   anorrectal: prueba de expulsi&oacute;n del bal&oacute;n, defecograf&iacute;a, resonancia magn&eacute;tica   pelviana, gammagraf&iacute;a de evacuaci&oacute;n, manometr&iacute;a anorrectal, electromiograf&iacute;a   (18) y c&aacute;psula inal&aacute;mbrica para evaluar el tr&aacute;nsito col&oacute;nico y de todo el   intestino (19), que orienten el tratamiento. Varios niveles de terapia han sido   desarrollados: aumento de la ingesta de fibra, cambios en el estilo de vida   (7), laxantes (20), que muestran resultados variables en el manejo de grupos   especiales de pacientes (21), polietilenglicol (1, 22), irrigaci&oacute;n transanal   (23, 24), nuevos medicamentos como prucaloprida, lubiprostona y linaclotida,   que con diferentes mecanismos de acci&oacute;n demuestran ser eficaces en el tratamiento de la constipaci&oacute;n (25, 26).</p>     <p>Se han propuesto tambi&eacute;n varios procedimientos quir&uacute;rgicos   para el tratamiento de la inercia col&oacute;nica o de los trastornos de la   evacuaci&oacute;n, con el inconveniente del riesgo de complicaciones quir&uacute;rgicas. La   t&eacute;cnica de Malone consiste en la tunelizaci&oacute;n del ap&eacute;ndice cecal a la piel para   colocar enemas anter&oacute;grados y favorecer la evacuaci&oacute;n (27, 28); sin embargo, el   procedimiento no est&aacute; libre de complicaciones que obligan a hacer revisiones   quir&uacute;rgicas durante el seguimiento (29). La colectom&iacute;a total es un   procedimiento quir&uacute;rgico mayor que se utiliza en pacientes con inercia col&oacute;nica   que se resiste al tratamiento convencional (30); no obstante, una proporci&oacute;n   significante de los pacientes tiene tambi&eacute;n anormalidades en la motilidad del   tracto digestivo superior (6) o del intestino delgado, lo que permite perpetuar   la sintomatolog&iacute;a abdominal. Se ha utilizado la manometr&iacute;a antroduodenal para   tratar de diferenciar este grupo de pacientes, en los cuales, de ser anormal,   se continuar&aacute; con sintomatolog&iacute;a tipo distensi&oacute;n y dolor abdominal, lo que   llevar&aacute; a un resultado menos satisfactorio (31). Respecto a los trastornos de   la evacuaci&oacute;n u obstrucci&oacute;n a la evacuaci&oacute;n, manifestaci&oacute;n de la disfunci&oacute;n del   piso p&eacute;lvico, en estos la constipaci&oacute;n se refiere como el esfuerzo prolongado   para la evacuaci&oacute;n, el dolor con la evacuaci&oacute;n, la necesidad de soporte   perineal o la sensaci&oacute;n de evacuaci&oacute;n incompleta (32). No es f&aacute;cil categorizar   a este tipo de pacientes debido a la interposici&oacute;n de etiolog&iacute;as, al s&iacute;ndrome   del intestino irritable, a la neuropat&iacute;a del pudendo o a la disfunci&oacute;n del piso   p&eacute;lvico, que dificultan la delimitaci&oacute;n de la real causa de los s&iacute;ntomas (33).   En este tipo de pacientes se utilizan varias t&eacute;cnicas quir&uacute;rgicas que corrigen   el factor etiol&oacute;gico que causa la obstrucci&oacute;n, el prolapso rectal o la   intusucepci&oacute;n interna, el enterocele o el sigmoidocele, la resecci&oacute;n rectal   transanal con grapas (34) y finalmente los estomas como &uacute;ltimo recurso (35). La   resecci&oacute;n transanal con grapas incrementa la sensaci&oacute;n rectal disminuyendo los   s&iacute;ntomas y mejorando la calidad de vida de los pacientes (36). Este es   considerado el procedimiento de elecci&oacute;n para el rectocele y la intusucepci&oacute;n   interna; sin embargo, hay persistencia de los s&iacute;ntomas hasta en el 11% de los   pacientes despu&eacute;s de 11 meses de seguimiento (37).</p>     <p>La constipaci&oacute;n, entonces, tiene un amplio rango de causas.   Una vez que las causas secundarias, que incluyen medicamentos, trastornos   metab&oacute;licos o neurol&oacute;gicos y c&aacute;ncer colorrectal obstructivo, sean descartadas,   las alteraciones funcionales del colon y del anorrecto deben evaluarse (6).   Todav&iacute;a se requiere de m&aacute;s investigaci&oacute;n b&aacute;sica y cl&iacute;nica sobre la motilidad   col&oacute;nica para identificar qu&eacute; tipo o subtipo de estre&ntilde;imiento no receptivo al   tratamiento m&eacute;dico podr&iacute;a responder con &eacute;xito a la electroestimulaci&oacute;n y cu&aacute;l   es la mejor manera de lograrlo (38).</p>     <p>La estimulaci&oacute;n el&eacute;ctrica es utilizada cl&iacute;nicamente para   tratar un amplio rango de condiciones. Se ha demostrado su efectividad cl&iacute;nica   para el control del dolor, fortalecimiento muscular, movilizaci&oacute;n de tejidos   blandos y cicatrizaci&oacute;n de las heridas. La terapia interferencial es una forma   de estimulaci&oacute;n el&eacute;ctrica que usa corrientes de frecuencia mediana, usualmente con   una onda sinusoidal (39). El tratamiento con corriente interferencial altera la   concentraci&oacute;n intracelular de enzimas y otras mol&eacute;culas que son importantes en   muchos procesos metab&oacute;licos y pueden ayudar a explicar los efectos de esta   terapia (40). El control de la motilidad del tracto gastrointestinal depende de   la inervaci&oacute;n extr&iacute;nseca simp&aacute;tica y parasimp&aacute;tica y de la inervaci&oacute;n   intr&iacute;nseca originada en el sistema nervioso ent&eacute;rico (41), por lo que esta se   puede ver alterada por m&uacute;ltiples motivos. En consecuencia, han sido   desarrolladas diferentes estrategias para lograr la estimulaci&oacute;n del colon   mediante est&iacute;mulo transabdominal (42, 43), est&iacute;mulo nervioso directo a los   troncos aferentes o est&iacute;mulo directo en la pared col&oacute;nica (38, 44), e incluso   estudios en animales con estimulaci&oacute;n magn&eacute;tica funcional a nivel cervical con   estimulaci&oacute;n a nivel del nervio vago (45).</p>     <p>Se han desarrollado varios m&eacute;todos de estimulaci&oacute;n el&eacute;ctrica   gastrointestinal (14):</p>     <p>1. Electrodos intramusculares. Usualmente colocados en la   muscular propia del colon. Tienen como ventaja que se garantiza el contacto   directo del &oacute;rgano a intervenir. Su desventaja es que constituye un   procedimiento quir&uacute;rgico invasivo.</p>     <p>2. Electrodos intraluminales o mucosos. La principal   desventaja es que no se puede garantizar el contacto entre el electrodo y la   mucosa.</p>     ]]></body>

Considerada como estimulaci&oacute;n   el&eacute;ctrica con electrodos (agujas).</p>     <p>El inter&eacute;s en la estimulaci&oacute;n el&eacute;ctrica del colon se ha   derivado, entre otros estudios, de los efectos colaterales, diarrea, que   presentan los pacientes en los cuales se utiliza estimulaci&oacute;n el&eacute;ctrica para el   tratamiento de la incontinencia urinaria (46). Al discutir acerca de la   estimulaci&oacute;n el&eacute;ctrica de los &oacute;rganos gastrointestinales, tenemos en mente el   concepto de marcapaso cuando se estimula a una frecuencia similar o un poco   mayor que la frecuencia intr&iacute;nseca del &oacute;rgano (47) y se busca la real presencia   de un sitio marcapaso en el colon y en el recto con respuestas electrofisiol&oacute;gicas   que los identifican (48, 49). La estimulaci&oacute;n el&eacute;ctrica de tiras de colon   induce liberaci&oacute;n de acetilcolina de las neuronas ent&eacute;ricas, lo cual a su vez   inicia contracciones que pueden comenzar y generar contracciones que soportan   la propulsi&oacute;n col&oacute;nica (50). De igual manera, en estudios animales, cuando se   colocaron electrodos en forma directa se observ&oacute; un aumento en la velocidad del   tr&aacute;nsito col&oacute;nico (51). Se han utilizado diferentes t&eacute;cnicas para lograr esto   con resultados variables: estimulaci&oacute;n el&eacute;ctrica transabdominal, en donde es   probable que se estimulen localmente las fibras nerviosas cut&aacute;neas, las fibras   simp&aacute;ticas y transcut&aacute;neas que van al intestino o a nervios en el intestino   (39); sin embargo, se desconoce el mecanismo de funcionamiento de la corriente   (52, 53), aunque se habla de neuromodulaci&oacute;n (54, 55). Se ha encontrado que   esta es una t&eacute;cnica promisoria y que debe utilizarse antes de llegar a la   cirug&iacute;a (56); adem&aacute;s disminuye la necesidad de procedimientos quir&uacute;rgicos para   el tratamiento de pacientes con constipaci&oacute;n de tr&aacute;nsito lento (44). Con esta   t&eacute;cnica de estimulaci&oacute;n transcut&aacute;nea en pacientes adultos se ha observado   mejor&iacute;a en el n&uacute;mero de evacuaciones por semana y de las escalas de medici&oacute;n de   constipaci&oacute;n (57). Ahora se investiga cu&aacute;l es la mejor forma de entregar dicha   electricidad: si en pulsos largos o en tren de pulsos. Es as&iacute; como se ha   encontrado una mejor respuesta en animales con los trenes de pulsos (16). Se   han reportado estudios aislados de utilizaci&oacute;n de estimulaci&oacute;n el&eacute;ctrica   transanal en pacientes con trastorno de la evacuaci&oacute;n dado b&aacute;sicamente por la   ausencia o la disminuci&oacute;n de la sensaci&oacute;n evacuatoria. La estimulaci&oacute;n   el&eacute;ctrica en pacientes constipados puede tener alg&uacute;n efecto en la reactivaci&oacute;n   del marcapaso rectal, la regulaci&oacute;n en la coordinaci&oacute;n rectoanal, el incremento   de la motilidad col&oacute;nica o el mejoramiento en la sensaci&oacute;n rectal (58, 59). La   t&eacute;cnica para estimular la motilidad col&oacute;nica a trav&eacute;s de la suplencia nerviosa   del colon, que produce un efecto fisiol&oacute;gico, se hace actualmente a trav&eacute;s de   los electrodos en los for&aacute;menes sacros, con base en la t&eacute;cnica utilizada para   el manejo de la irritabilidad del detrusor y la retenci&oacute;n urinaria (60). Los   estudios de estimulaci&oacute;n magn&eacute;tica espinal tambi&eacute;n muestran un aumento del   tr&aacute;nsito col&oacute;nico en trabajos realizados en ancianos (61). Estas t&eacute;cnicas no   invasivas adem&aacute;s mejoran la calidad de vida de los pacientes, que es el fin   &uacute;ltimo de esta tecnolog&iacute;a (57, 62).</p>     <p>Por tanto, la tendencia actual se divide en varias   posibilidades para lograr la estimulaci&oacute;n del colon tratando de no llegar a   procedimientos invasivos. La estimulaci&oacute;n que utiliza corriente de baja   frecuencia a trav&eacute;s de 4 electrodos colocados en los dermatomas de S2-S3 ha   demostrado que aumenta en una forma significativa los movimientos intestinales   al d&iacute;a y a la semana en pacientes con constipaci&oacute;n idiop&aacute;tica (63). Un grupo   especial de pacientes es aquellos que presentan disfunci&oacute;n evacuatoria   relacionada con hiposensibilidad rectal, disinergia funcional del piso p&eacute;lvico   u obstrucci&oacute;n mec&aacute;nica del tracto de salida, en los cuales la cirug&iacute;a abdominal   o perineal no es una opci&oacute;n. Por ende, la estimulaci&oacute;n neural sacra permite un   ensayo m&iacute;nimamente invasivo con baja morbilidad y deber&aacute; ser la primera opci&oacute;n   en aquellos con constipaci&oacute;n de tr&aacute;nsito lento y o hiposensibilidad rectal,   siendo la estimulaci&oacute;n neural sacra un procedimiento efectivo en el tratamiento   de la constipaci&oacute;n cr&oacute;nica (64). La investigaci&oacute;n tambi&eacute;n se ha orientado hacia   la opci&oacute;n de la estimulaci&oacute;n directa del colon mediante electrodos insertados   en la capa muscular a nivel de la uni&oacute;n rectosigmoidea v&iacute;a laparosc&oacute;pica,   conectados a un estimulador colocado en un bolsillo inguinal subcut&aacute;neo, lo que   produce un aumento en el n&uacute;mero total de movimientos intestinales (65). Se han   utilizado tambi&eacute;n m&eacute;todos de estimulaci&oacute;n perif&eacute;rica, que estimulan los   m&uacute;sculos de la pared abdominal mediante estimulaci&oacute;n el&eacute;ctrica funcional a   nivel del oblicuo externo y transverso abdominal, lo que produce tambi&eacute;n un   aumento del patr&oacute;n evacuatorio y disminuci&oacute;n de la toma de laxantes (66). Otro   m&eacute;todo es la estimulaci&oacute;n del nervio tibial posterior, que genera un aumento en   la frecuencia de las evacuaciones espont&aacute;neas despu&eacute;s de 2 semanas de   tratamiento; incluso disminuye el tiempo en el evento evacuatorio, con la   subsecuente mejor&iacute;a en la calidad de vida (67).</p>     <p>Es claro, entonces, que para el grupo de pacientes con   constipaci&oacute;n de tr&aacute;nsito lento hay diferentes opciones antes de someter al   paciente a procedimientos con morbilidad importante y en los que, en la mayor&iacute;a   de los casos, se generan complicaciones sin llegar a cumplir con el objetivo de   curar al enfermo.
