Existen diversos factores que son considerados de mal pronóstico para pacientes tratados por la vía artroscópica, uno de estos factores es la deformidad angular en varo, para la cual se necesita de la realización de osteotomía correctora. Objetivo: Analizar las ventajas y desventajas de las técnicas de intervención de artroscopia y osteotomía en pacientes con artrosis del compartimento tibiofemoral medial y deformidad angular en varo. Materiales y metodología de búsqueda: Se realizó una búsqueda en las bases de datos Pubmed, Hinari, Scielo y Medline mediante el gestor de búsqueda y administrador de referencias EndNote utilizando los términos "arthroscopy of the knee", "arthroscopy and osteotomy of the knee" y "osteotomy of the knee", obteniendo un total de 350 artículos de los cuales se utilizaron 59 citas seleccionadas para realizar la revisión, 49 de ellas de los últimos cinco años donde se incluyeron tres libros y ocho citas del propio autor. Resultados: Se encontró una relación existente entre lesiones y deformidad angular, especialmente en el compartimento tibiofemoral medial. La deformidad en varo como factor de mal pronóstico en pacientes tratados por artroscopia quedó demostrada en un estudio realizado por el autor con anterioridad. Conclusiones: La artroscopia y osteotomía tibial alta abierta simultánea con la artroscopia, ofrecen más ventajas para el paciente respecto a otras intervenciones, en el tratamiento de pacientes con artrosis del compartimento tibiofemoral medial y deformidad angular en varo entre las que se encuentran un solo tiempo anestésico, la corrección de la deformidad, el funcionamiento adecuado de las articulaciones y el alivio del dolor. Objective: to analyze the advantages of simultaneous arthroscopy and osteotomy in patients suffering from medial tibiofemoral osteoarthritis of the knee associated with varus deformity. Methodology: a search in the databases Pubmed, Hinari, Scielo and Medline was done through the information locator EndNote by using the words arthroscopy of the knee, arthroscopy and osteotomy of the knee, y osteotomy of the knee, resulting in a total of 350 articles which 59 of them selected for review, 49 of them in the last five years, including three books and eight own references. Results: the relationship between intrarticular lesions and angular deformity was found, especially those located at the medial tibiofemoral compartment. The author stated that varus deformity is associated to poor prognosis, taken into account a previous investigation. Conclusions: arthroscopy and high tibial osteotomy combined offers more advantages over other interventions in the treatment of patients with knee osteoarthritis of the medial tibiofemoral compartment and varus angular deformity, such as deformity correction, proper joint function and pain relief. M&Eacute;D.UIS. 2016;29(1):45-51.</p> <hr>     <p align="center"><font size="3"><b><u>INTRODUCCI&Oacute;N</u></b></font></p>      <p align="justify">La artrosis de la rodilla tambi&eacute;n conocida por Gonartrosis Primaria (GP) es una enfermedad articular degenerativa no inflamatoria que se caracteriza por la degeneraci&oacute;n del cart&iacute;lago articular, esclerosis subcondral, formaci&oacute;n de osteofitos y alteraci&oacute;n de las partes blandas como: membrana sinovial, c&aacute;psula articular, ligamentos y m&uacute;sculos<sup>1,2</sup>. La GP es de causa idiop&aacute;tica y se asocia por lo general a trastornos circulatorios de la extremidad<sup>3</sup>.</p>      <p align="justify">La artrosis degenerativa es una entidad muy frecuente en la actualidad. Los factores responsables en el incremento del n&uacute;mero pacientes son: el aumento de la expectativa de vida de la poblaci&oacute;n, la necesidad de una mejor funci&oacute;n articular de este grupo de enfermos y la presencia de esta afecci&oacute;n en edades m&aacute;s tempranas que lo usual; esta entidad comienza por lo general a los 40 a&ntilde;os o m&aacute;s<sup>1-3</sup>. Seg&uacute;n datos de la oficina nacional de estad&iacute;sticas de Cuba<sup>4</sup> la poblaci&oacute;n de la provincia de Camag&uuml;ey en el a&ntilde;o 2013 fue de 782 592 y las personas con 40 a&ntilde;os de edad o m&aacute;s fueron 372 812, lo que representa un grupo significativo de personas que pueden sufrir esta enfermedad.</p>      <p align="justify">Por otro lado se encuentra una prevalencia significativa de GP; un estudio nacional realizado por Reyes Llerena <i>et al.</i><sup>5</sup>, mostr&oacute; que la incidencia de esta enfermedad es del 20,4&#37; en una poblaci&oacute;n estudiada, as&iacute; mismo en una investigaci&oacute;n realizada por Sharma L y Kapoor D<sup>6</sup> en Rotterdam se encontr&oacute; que solo 135 personas de 1040 entre 55 a 65 a&ntilde;os de edad no presentaron signos radiogr&aacute;ficos de artrosis. De igual forma, la Organizaci&oacute;n Mundial de la Salud estima que esta enfermedad afecta al menos un 10&#37; de la poblaci&oacute;n por encima de 60 a&ntilde;os, y de ellos m&aacute;s de 20 millones habitan en las Am&eacute;ricas.</p>      <p align="justify">Por otra parte, se estima el aumento en la prevalencia de GP, en un estudio realizado por Hunter DJ <i>et al</i>.<sup>7</sup> se plante&oacute; que la incidencia de artrosis es del 25&#37; al 30&#37; en pacientes entre 45 y 64 a&ntilde;os de edad y se incrementa a un 85&#37; en personas por encima de 65 a&ntilde;os. En Estados Unidos, se estima que el n&uacute;mero de personas con GP va en incremento de un 15&#37; de su poblaci&oacute;n (40 millones) en el a&ntilde;o 1995 a un 18&#37; (59 millones) para el a&ntilde;o 2020<sup>8</sup>.</p>      <p align="justify">Las deformidades angulares de la rodilla pueden ser en varo o valgo, debido a causas primarias o secundarias. En especial, la deformidad angular en varo es un factor de mal pron&oacute;stico y es un signo certero de progresi&oacute;n de la enfermedad, enti&eacute;ndase por varo cuando la pierna del enfermo se acerca a la l&iacute;nea medial del cuerpo en el plano axial<sup>9-11</sup>.</p>      <p align="justify">Para el tratamiento quir&uacute;rgico de pacientes con GP y deformidad angular se emplean varios m&eacute;todos como: la artroscopia, osteotom&iacute;as correctoras y la artroplastia unicompartimental. La selecci&oacute;n del m&eacute;todo quir&uacute;rgico depende de factores relacionados con el enfermo (edad, actividad f&iacute;sica, peso corporal), el cirujano (entrenamiento) y la instituci&oacute;n (disponibilidad de recursos)<sup>12, 13</sup>.</p>      <p align="justify">El objetivo de esta investigaci&oacute;n es analizar las ventajas y desventajas de las t&eacute;cnicas de intervenci&oacute;n de artroscopia y osteotom&iacute;a en pacientes con artrosis del compartimento tibiofemoral medial y deformidad angular en varo.</p>      <p align="center"><font size="3"><b><u>METODOLOG&Iacute;A DE B&Uacute;SQUEDA</u></b></font></p>      ]]></body>
<body><![CDATA[<p align="justify">La presente revisi&oacute;n se efect&uacute;o desde 1 de agosto de 2014 hasta el 3 de mayo de 2015; la b&uacute;squeda de la informaci&oacute;n se realiz&oacute; en un periodo de dos meses y se emplearon las siguientes palabras: &quot;<i>arthroscopy of the knee</i>&quot;, &quot;<i>arthroscopy and osteotomy of the knee</i>&quot; y &quot;osteotomy of the knee&quot;, a partir de la informaci&oacute;n obtenida se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un total de 350 art&iacute;culos publicados en las bases de datos Pubmed, Hinari, SciELO y Medline mediante el gestor de b&uacute;squeda y administrador de referencias EndNote, de ellos se utilizaron 59 citas seleccionadas para realizar la revisi&oacute;n, 49 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se incluyeron tres libros y ocho citas del propio autor (Ver <a href="#f01">Figura 1</a>).</p>      <p align="center"><a name="f01"></a><img src="img/revistas/muis/v29n1/v29n1a06f1.jpg"></p>      <p align="justify">Se consideraron estudios de pacientes con deformidad angular de la rodilla en varo tratados por v&iacute;a artrosc&oacute;pica, osteotom&iacute;a o ambos procedimientos simult&aacute;neamente. Se excluyeron los pacientes con deformidad en valgo de la rodilla.</p>      <p align="center"><font size="3"><b><u>PRESENTACI&Oacute;N DE LA LESI&Oacute;N Y MECANISMO</u></b></font></p>      <p align="justify">Las lesiones intrarticulares son frecuentes en pacientes con GP y deformidad angular en varo de la rodilla<sup>14,15</sup>. Seg&uacute;n Ilahi OA <i>et al</i>.,<sup>16</sup> en su investigaci&oacute;n 29 de 32 enfermos presentaron lesi&oacute;n del menisco interno para un 91&#37;. Adem&aacute;s el menisco lateral estuvo afectado en el 50&#37; de los pacientes, en este mismo compartimento se detect&oacute; lesi&oacute;n condral inestable en el 13&#37; y cuerpos libres articulares en el 9&#37;. Por otra parte en el compartimento anterior, la presencia de lesi&oacute;n condral inestable fue del 31&#37;. Este estudio demostr&oacute; la estrecha relaci&oacute;n entre deformidad angular en varo y afecciones intrarticulares<sup>16</sup>.</p>      <p align="justify">El menisco y el cart&iacute;lago articular sufren el mismo proceso degenerativo caracter&iacute;stico de los pacientes que sufren de GP afectando adem&aacute;s los ligamentos y tejido sinovial<sup>17-19</sup>. El desgarro de menisco en pacientes con GP est&aacute; asociado a lesiones pre-existentes en la estructura y composici&oacute;n del menisco lesionado, las fuerzas de estr&eacute;s y los cambios degenerativos en la matriz de col&aacute;geno del mismo disminuyen considerablemente su capacidad tensil, lo que predispone al menisco a traumatismos a&uacute;n de baja energ&iacute;a o de forma espont&aacute;nea al realizar alguna actividad f&iacute;sica que requiera poca demanda mec&aacute;nica<sup>20-22</sup>. Una vez que se presenta la lesi&oacute;n de menisco asociada a cambios degenerativos de la articulaci&oacute;n es necesario la resecci&oacute;n del tejido meniscal lesionado, por lo que aumentan las cargas biomec&aacute;nicas de la articulaci&oacute;n y favorece el avance de la enfermedad degenerativa articular<sup>23-25</sup>.</p>      <p align="justify">Otra situaci&oacute;n muy com&uacute;n en pacientes con GP es el desplazamiento del menisco, su presencia demuestra da&ntilde;o severo del tejido meniscal y es un signo de mal pron&oacute;stico relacionado con el progreso de la enfermedad degenerativa articular, ya que aumenta el estrechamiento del espacio articular, hecho que se evidencia en la radiograf&iacute;a simple<sup>26-28</sup>.</p>      <p align="center"><font size="3"><b><u>TRATAMIENTO</u></b></font></p>      <p align="justify">La artroscopia de la rodilla seg&uacute;n Burks RT<sup>29</sup> es indicada en pacientes con gonartrosis debido a su utilidad en definir el estado del cart&iacute;lago articular, permite el tratamiento de lesiones focales y prolonga el uso de la articulaci&oacute;n. Dentro de los procedimientos que pueden ser realizados por la v&iacute;a artrosc&oacute;pica en pacientes con GP son variados entre ellos el lavado articular, desbridamientos, meniscectom&iacute;as, microfracturas, adem&aacute;s de la extracci&oacute;n de cuerpos libres articulares y osteofitos<sup>30-32</sup>.</p>      <p align="justify">El lavado articular y el desbridamiento tienen un efecto limitado en pacientes con GP, si el enfermo presenta deformidad angular de la extremidad tanto en varo como en valgo, de all&iacute; la necesidad de combinar procedimientos de realineaci&oacute;n de la extremidad<sup>33-35</sup>.</p>      ]]></body>
<body><![CDATA[<p align="justify">Las microfracturas son realizadas con un instrumental quir&uacute;rgico puntiagudo que permite la salida de sangre, provocando la formaci&oacute;n de un co&aacute;gulo, que permite la reconstituci&oacute;n de la zona da&ntilde;ada del cart&iacute;lago articular. Esta t&eacute;cnica puede ser empleada en pacientes con &aacute;reas afectadas de m&aacute;s de 3 cm<sup>2</sup>, a excepci&oacute;n del paciente que tenga una actividad f&iacute;sica alta. Despu&eacute;s de llevar a cabo este procedimiento, se debe suspender el apoyo del peso corporal por 6 semanas. Las microfracturas no deben ser realizadas en pacientes que presenten da&ntilde;o o ruptura de la integridad en la placa subcondral<sup>36,37</sup>.</p>      <p align="justify">Al revisar la literatura internacional autores como Sharma L <i>et al</i>.<sup>38</sup> afirman que la deformidad angular en varo es un factor de mal pron&oacute;stico y un indicador de progresi&oacute;n de la enfermedad. En un estudio realizado por &Aacute;lvarez L&oacute;pez A<sup>39</sup>, en pacientes con GP tratados por v&iacute;a artrosc&oacute;pica, la deformidad angular en varo estuvo presente en el 95,6&#37; de los enfermos con resultados desfavorables y se calcul&oacute; el <i>Odds Ratio</i>, lo que signific&oacute; que un paciente con esta deformidad angular es un 11,6&#37; m&aacute;s susceptible de obtener un resultado desfavorable. De los factores relacionados con los resultados desfavorables este fue el que aport&oacute; una mayor cantidad de pacientes. Mediante esta investigaci&oacute;n el autor demostr&oacute; que los pacientes con GP y deformidad angular en varo, necesitan de un procedimiento quir&uacute;rgico adicional a la artroscopia, ya que por s&iacute; misma los resultados no son satisfactorios.</p>      <p align="center"><font size="3"><b><u>CONSIDERACIONES DEL TRATAMIENTO</u></b></font></p>      <p align="justify">La osteotom&iacute;a tiene como objetivos: corregir la deformidad angular, provocar la descompresi&oacute;n de la circulaci&oacute;n venosa, retrasar la evoluci&oacute;n natural de la enfermedad y demorar otros procedimientos mayores como la artroplastia total de rodilla<sup>40-42</sup>. Para el tratamiento de la deformidad angular en varo de la rodilla en pacientes con GP se pueden emplear osteotom&iacute;as de 2 tipos: tibial alta medial abierta o tibial alta lateral cerrada. Entre las ventajas principales de la primera se destacan la menor dificultad t&eacute;cnica y la alta precisi&oacute;n, no obstante necesitan de injerto &oacute;seo, mayor periodo de consolidaci&oacute;n, y existe la posibilidad de da&ntilde;o al nervio tibial<sup>43-45</sup>. La segunda no necesita de injerto &oacute;seo y el tiempo de consolidaci&oacute;n es m&aacute;s corto, pero tiene como desventaja de ser un procedimiento t&eacute;cnicamente m&aacute;s demandante, menor precisi&oacute;n y gran posibilidad de da&ntilde;o del nervio ci&aacute;tico popl&iacute;teo externo<sup>46-48</sup>.</p>      <p align="justify">La deformidad angular en varo que afecta el compartimento tibiofemoral medial, es considerada por algunos autores como el sitio de inicio m&aacute;s com&uacute;n de la GP con evoluci&oacute;n r&aacute;pida de la enfermedad y es un factor a considerar en relaci&oacute;n a la respuesta limitada de varias formas de tratamiento<sup>49-51</sup>.</p>      <p align="center"><font size="3"><b><u>COMPARACI&Oacute;N DEL TRATAMIENTO</u></b></font></p>      <p align="justify">La artroscopia en relaci&oacute;n a la osteotom&iacute;a puede ser realizada antes, simult&aacute;neamente o despu&eacute;s, existiendo en cada caso ventajas y desventajas. La realizaci&oacute;n simult&aacute;nea de los dos procedimientos ofrece las mayores ventajas, para pacientes con deformidad angular en varo y GP (Ver <a href="#t01">Tabla 1</a>).</p>      <p align="center"><a name="t01"></a><img src="img/revistas/muis/v29n1/v29n1a06t1.jpg"></p>      <p align="justify">Para llevar a cabo esta t&eacute;cnica, se realiza primero la artroscopia de la articulaci&oacute;n, que permite solucionar las afecciones intrarticulares, en los pacientes con deformidad angular en varo existe una gran asociaci&oacute;n de lesi&oacute;n en el menisco interno y lesi&oacute;n osteocondral de tipos III y IV seg&uacute;n la clasificaci&oacute;n de <i>Outerbridge</i> RE<sup>52</sup> del mismo lado.</p>      <p align="justify">Posteriormente, se lleva a cabo la osteotom&iacute;a a trav&eacute;s de un abordaje medial, ya que si el paciente en un futuro necesita de otros procedimientos como una artroplastia total de rodilla, no existir&aacute;n dificultades con la v&iacute;a de acceso. En ocasiones, se puede realizar un abordaje lateral, pero se necesita de una longitud de la incisi&oacute;n quir&uacute;rgica mucho mayor<sup>53-55</sup>.</p>      ]]></body>
<body><![CDATA[<p align="justify">Para la protecci&oacute;n del paquete vasculonervioso, se coloca un separador curvo en la regi&oacute;n posterior de la tibia proximal, una vez realizado el corte &oacute;seo, se procede a abrir la l&iacute;nea de osteotom&iacute;a mediante maniobras manuales y a colocar la l&aacute;mina semitubular modificada para mantener la apertura &oacute;sea (Ver <a href="#f02">Figura 2</a>). El uso de estas l&aacute;minas, como las dise&ntilde;adas y comercializadas por Tomofix<sup>&reg;56</sup>, no necesita de inmovilizaci&oacute;n enyesada y el apoyo del peso corporal del paciente es precoz. Sin embargo, al emplear esta l&aacute;mina modificada el apoyo debe ser permitido de 8 a 12 semanas, seg&uacute;n el control radiogr&aacute;fico, una vez retirada la inmovilizaci&oacute;n enyesada a las cuatro semanas. Por otra parte, para el relleno del espacio &oacute;seo se pueden emplear injert&oacute; &oacute;seo aut&oacute;logo de cresta iliaca, clavija de peron&eacute; o hidroxiapatita c&aacute;lcica. El procedimiento es controlado bajo magnificaci&oacute;n radiogr&aacute;fica mediante un arco en C. Con el empleo de esta t&eacute;cnica quir&uacute;rgica se logra la consolidaci&oacute;n &oacute;sea y la apertura del espacio tibiofemoral medial (Ver <a href="#f03">Figura 3</a>)<sup>57-59</sup>.</p>      <p align="center"><a name="f02"></a><img src="img/revistas/muis/v29n1/v29n1a06f2.jpg"></p>      <p align="center"><a name="f03"></a><img src="img/revistas/muis/v29n1/v29n1a06f3.jpg"></p>      <p align="justify">Esta t&eacute;cnica corrige la deformidad angular y permite el tratamiento de las lesiones intrarticulares (Ver <a href="#f04">Figura 4</a>). Se recomienda en enfermos con afecci&oacute;n predominante del compartimento tibiofemoral medial que tengan buena calidad &oacute;sea y buen estado de las partes blandas y tiene como principales limitaciones la afecci&oacute;n tricompartimental sim&eacute;trica de la rodilla y el uso de injerto &oacute;seo. Se destaca que la t&eacute;cnica es de bajo costo, al emplear placas AO semitubulares, es segura tomando las medidas descritas y ofrece beneficio al enfermo por mejorar su funci&oacute;n articular y prolongar la necesidad de la artroplastia total de rodilla<sup>9, 55, 57</sup>.</p>      <p align="center"><a name="f04"></a><img src="img/revistas/muis/v29n1/v29n1a06f4.jpg"></p>      <p align="center"><font size="3"><b><u>CONCLUSIONES</u></b></font></p>      <p align="justify">La artroscopia y osteotom&iacute;a tibial alta abierta combinadas en un solo tiempo quir&uacute;rgico ofrecen m&aacute;s ventajas para el paciente, respecto a otras intervenciones en el tratamiento de enfermos con gonartrosis del compartimento tibiofemoral medial y deformidad en varo, algunas de estas ventajas son la realizaci&oacute;n de un solo tiempo anest&eacute;sico, la suspensi&oacute;n del apoyo del peso corporal una sola vez, la evaluaci&oacute;n del cart&iacute;lago articular que asume la carga de peso despu&eacute;s de la osteotom&iacute;a, la correcci&oacute;n de la deformidad, el alivio del dolor y un inicio pronto de la rehabilitaci&oacute;n mejorando el funcionamiento y equilibrio biomec&aacute;nico de la articulaci&oacute;n.
