<body><![CDATA[<p><font size="3"><b>INTRODUCCI&Oacute;N</b></font></p>     <p>El t&eacute;rmino endoftalmitis hace referencia a la inflamaci&oacute;n   de tejidos y l&iacute;quidos intraoculares, usualmente   de etiolog&iacute;a infecciosa; se la clasifica como ex&oacute;gena   cuando el microorganismo es inoculado al globo   ocular desde el exterior &#40;ejemplo: por trauma o cirug&iacute;a&#41;,   o end&oacute;gena, cuando el microorganismo invade el globo   ocular desde el torrente sangu&iacute;neo; esta &uacute;ltima representa   entre 2 &#37; y 5 &#37; del total de las endoftalmitis &#40;1-3&#41;.   En el presente art&iacute;culo se describen tres pacientes con   endoftalmitis end&oacute;gena asociada a absceso hep&aacute;tico   por <i>Klebsiella pneumoniae</i> y se hace una revisi&oacute;n   completa de la literatura.</p>       <p>&nbsp;</p>     <p><font size="3"><b>DESCRIPCI&Oacute;N DE LOS CASOS</b></font></p>     <p><b>Caso 1</b></p>     <p>Mujer de 69 a&ntilde;os, con antecedente personal de diabetes   mellitus tipo 2 para la que requer&iacute;a insulina. Ingres&oacute; a la instituci&oacute;n el 4 de septiembre del 2013   remitida de otro hospital donde se encontraba desde   el 30 de agosto por crisis hiperglic&eacute;mica hiperosmolar   desencadenada por una infecci&oacute;n. All&iacute; le   hicieron hemocultivos en los que creci&oacute; <i>K. pneumoniae</i>  multisensible. Durante la hospitalizaci&oacute;n   present&oacute; dolor en el ojo derecho, ojo rojo, edema   bipalpebral, proptosis y disminuci&oacute;n de la agudeza   visual, posteriormente dolor en el hipocondrio   derecho. Al ingreso el ojo derecho presentaba agudeza   visual &#40;AV&#41; de no percepci&oacute;n de la luz &#40;NPL&#41;,   quemosis, hiperton&iacute;a p&eacute;trea, secreci&oacute;n purulenta   abundante en los fondos de saco, restricci&oacute;n total de   los movimientos oculares, c&oacute;rnea opaca e hipopi&oacute;n   del 100 &#37;. Los resultados de laboratorio fueron: leucocitos   21.500/<i>&#956;</i>L con neutr&oacute;filos 92,4 &#37;; plaquetas   158.000/<i>&#956;</i>L, hemoglobina 10,9 g/dL, bilirrubina total   1,27 mg/dL, bilirrubina directa 0,74 mg/dL, AST   76 UI/L, ALT 118 UI/L, GGT 198 UI/L, FA: 267 UI/L,   PCR 14,72 mg/L, VSG: 115 mm en una hora, HbA1C:   mayor de 20.  </p>     <p>En la tomograf&iacute;a toracoabdominal se encontraron   una colecci&oacute;n hep&aacute;tica &#40;<a href="#f1">figura 1</a>&#41;, derrame pleural   y zonas necr&oacute;ticas en el par&eacute;nquima pulmonar   &#40;<a href="#f2">figura 2</a>&#41;. En los cultivos del material del absceso   hep&aacute;tico &#40;obtenido por drenaje&#41;, los hemocultivos   y el material del ojo derecho &#40;despu&eacute;s de la evisceraci&oacute;n&#41;   se obtuvo crecimiento de <i>K. pneumoniae</i>  sensible a amikacina, ciprofloxacina, ertapenem,   cefepime, cefoxitina, gentamicina, imipenem, meropenem,   ceftazidima y piperacilina/tazobactam,   no productora de betalactamasas de espectro extendido   &#40;BLEE negativa&#41;. Recibi&oacute; tratamiento intravenoso   con antibi&oacute;ticos, inicialmente piperacilina +   tazobactam y posteriormente se redujo el espectro   seg&uacute;n la sensibilidad a cefotaxime + metronidazol   durante 28 d&iacute;as y adicionalmente 5 d&iacute;as de amikacina.   El tratamiento t&oacute;pico ocular se hizo con moxifloxacina,   oxitetraciclina, prednisolona y atropina.   Se decidi&oacute; hacer la evisceraci&oacute;n del ojo derecho   por el compromiso grave y porque en la resonancia   magn&eacute;tica &#40;RM&#41; de la &oacute;rbita se hall&oacute; perforaci&oacute;n   ocular. </p>        <p align="center"><a name="f1"></a><img src="/img/revistas/iat/v29n2/v29n2a11f1.jpg"></p>        <p align="center"><a name="f2"></a><img src="/img/revistas/iat/v29n2/v29n2a11f2.jpg"></p>     <p><b>Caso 2  </b></p>     ]]></body>
<body><![CDATA[<p>Mujer de 50 a&ntilde;os, sin antecedentes personales de importancia,   que ingres&oacute; a la instituci&oacute;n el 29 de julio   del 2014 por fiebre y dolor intenso en el ojo derecho.   Hab&iacute;a consultado 7 d&iacute;as antes por visi&oacute;n borrosa en   dicho ojo sin otros s&iacute;ntomas y se la hab&iacute;a estudiado   ambulatoriamente como posible panuve&iacute;tis; recibi&oacute;   tratamiento oral con trimetoprim-sulfametoxazol y   clindamicina, y tratamiento t&oacute;pico con prednisolona   y atropina. Dos d&iacute;as despu&eacute;s del ingreso inform&oacute; dolor   en el hipocondrio derecho. En el examen de ingreso   presentaba AV de movimiento de manos &#40;MM&#41; en el   ojo derecho, que se deterior&oacute; d&iacute;as despu&eacute;s a percepci&oacute;n   luminosa &#40;PL&#41;, quemosis y vitre&iacute;tis grave &#40;<a href="#f3">figura   3</a>&#41;. Los resultados de laboratorio fueron los siguientes:   leucocitos 30.300/<i>&#956;</i>L con neutr&oacute;filos 87,5 &#37;, plaquetas   746.000/<i>&#956;</i>L, VSG 120 mm en una hora, PCR 5,76 mg/L,   hemocultivos negativos. En la tomograf&iacute;a abdominal   se observ&oacute; una colecci&oacute;n hep&aacute;tica &#40;<a href="#f4">figura 4</a>&#41;. En el cultivo   del material del absceso hep&aacute;tico, obtenido por   drenaje, se aisl&oacute;<i> K. pneumoniae</i> sensible a amikacina,   ciprofloxacina, ertapenem, cefepime, cefoxitina, gentamicina,   imipenem, meropenem, ceftazidima y piperacilina/   tazobactam, BLEE negativa; en muestra tomada   del v&iacute;treo se observaron bacilos gramnegativos y   leucocitos, pero no hubo crecimiento en el cultivo. La   paciente recibi&oacute; tratamiento antibi&oacute;tico intravenoso   inicialmente con metronidazol + ertapenem y posteriormente   con ceftriaxona; se le hicieron adem&aacute;s tres   aplicaciones intrav&iacute;treas de antibi&oacute;tico separadas por   48 horas &#40;la primera con vancomicina y ceftazidima   y las siguientes con ceftazidima&#41; y se le hizo vitrectom&iacute;a   posterior. El tratamiento t&oacute;pico ocular se hizo con   moxifloxacina, prednisolona y atropina. Posterior al   alta se llev&oacute; a cabo retinopexia con silic&oacute;n m&aacute;s lensectom&iacute;a.   En la &uacute;ltima evaluaci&oacute;n se encontraron AV de   NPL y desprendimiento de retina en embudo.</p>       <p align="center"><a name="f3"></a><img src="/img/revistas/iat/v29n2/v29n2a11f3.jpg"></p>       <p align="center"><a name="f4"></a><img src="/img/revistas/iat/v29n2/v29n2a11f4.jpg"></p>     <p> <b>Caso 3</b></p>     <p> Hombre de 75 a&ntilde;os, sin antecedentes personales de   importancia, admitido a la instituci&oacute;n el 31 de diciembre   del 2014 por fiebre y escalofr&iacute;os de 9 d&iacute;as de   evoluci&oacute;n con posterior aparici&oacute;n de miodesopsias y   p&eacute;rdida progresiva de la visi&oacute;n por el ojo izquierdo;   adem&aacute;s, ojo rojo, edema bipalpebral y dolor intenso.   Al examen de ingreso se encontr&oacute; el ojo izquierdo   con AV PL, secreci&oacute;n purulenta en fondos de saco,   hipopi&oacute;n y opacidad de medios. Los resultados de   laboratorio fueron como sigue: leucocitos 17.100/<i>&#956;</i>L   con neutr&oacute;filos 84,4 &#37;, Hb 15,5 g/dL, plaquetas   451.000/<i>&#956;</i>L, VSG 114 mm en una hora; uroan&aacute;lisis:   piuria y bacterias abundantes; hemocultivos   negativos; AST 45/UI/L, ALT 58/UI/L, GGT 243/UI/L,   FA 154/UI/L, bilirrubina directa 0,63 mg/dL. En la tomograf&iacute;a   abdominal se encontr&oacute; una colecci&oacute;n hep&aacute;tica   &#40;<a href="#f5">figura 5</a>&#41;. En los cultivos del material del absceso   hep&aacute;tico, obtenido por drenaje, la orina y el v&iacute;treo   se obtuvo crecimiento de <i>K. pneumoniae</i> sensible a amikacina, ciprofloxacina, ertapenem, cefepime,   cefoxitina, gentamicina, imipenem, meropenem,   ceftazidima y piperacilina/tazobactam, BLEE negativa.   Recibi&oacute; tratamiento intravenoso con antibi&oacute;ticos   inicialmente piperacilina + tazobactam, luego   se redujo el espectro seg&uacute;n la sensibilidad a ceftriaxona.   Se hizo vitrectom&iacute;a y recibi&oacute; dos aplicaciones   intrav&iacute;treas de vancomicina y ceftazidima y una   dosis de ceftazidima intracameral; el tratamiento   t&oacute;pico ocular se efectu&oacute; con moxifloxacina, vancomicina,   ceftazidima, oxitetraciclina, prednisolona y   atropina; sin embargo, por falta de respuesta y progresi&oacute;n   del compromiso ocular &#40;<a href="#f6">figura 6</a>&#41; se eviscer&oacute;   el ojo izquierdo. </p>       <p align="center"><a name="f5"></a><img src="/img/revistas/iat/v29n2/v29n2a11f5.jpg"></p>       <p align="center"><a name="f6"></a><img src="/img/revistas/iat/v29n2/v29n2a11f6.jpg"></p>       <p>&nbsp;</p>     <p><font size="3"><b>REVISI&Oacute;N DE LA LITERATURA Y DISCUSI&Oacute;N</b></font></p>     <p> Las infecciones por<i> K. pneumoniae</i> son frecuentes   en la pr&aacute;ctica cl&iacute;nica y pueden ser nosocomiales o   adquiridas en la comunidad &#40;4&#41;; estas &uacute;ltimas han   aumentado en a&ntilde;os recientes y representan el 41 &#37;   de todas las causadas por este germen en los Estados   Unidos y el 68 &#37; en Taiw&aacute;n &#40;5&#41;.</p>     ]]></body>
<body><![CDATA[<p> Entre las infecciones en que se ha implicado a esta   bacteria se encuentra el absceso hep&aacute;tico, que puede   ser polimicrobiano o monomicrobiano &#40;6&#41;. La prevalencia   de <i>K. pneumoniae</i> como agente causal de   absceso hep&aacute;tico pi&oacute;geno ha venido en aumento en   los &uacute;ltimos a&ntilde;os, primero en pa&iacute;ses orientales &#40;7-10&#41;   y m&aacute;s recientemente en los occidentales &#40;11-14&#41;. Es   el microorganismo aislado con mayor frecuencia en   esta enfermedad.  </p>     <p>Desde 1986 se ha descrito el s&iacute;ndrome de absceso hep&aacute;tico   criptog&eacute;nico invasor por <i>Klebsiella pneumoniae</i>  &#40;CIKPLA, por la sigla en ingl&eacute;s de <i>cryptogenic   invasive Klebsiella pneumoniae liver abscess</i>&#41; &#40;15-17&#41;;   consiste en la presencia de un absceso hep&aacute;tico primario/   criptog&eacute;nico &#40;sin enfermedad hepatobiliar subyacente&#41;   &#40;18&#41; con siembras infecciosas secundarias;   suele ser una infecci&oacute;n adquirida en la comunidad   que se presenta con mayor frecuencia en personas   diab&eacute;ticas &#40;19-21&#41; o con otros trastornos asociados   con inmunosupresi&oacute;n como insuficiencia renal y malignidad   &#40;22&#41;. Este cuadro cl&iacute;nico se ha informado principalmente   en pa&iacute;ses asi&aacute;ticos &#40;4&#41; como Taiw&aacute;n &#40;9,23&#41;,   Singapur &#40;24,25&#41;, Corea &#40;26&#41; y Jap&oacute;n &#40;27,28&#41;, con una   gran diferencia en cuanto a la prevalencia respecto a   los pa&iacute;ses occidentales, en los que se cuenta exclusivamente   con reportes de casos aislados en Espa&ntilde;a &#40;29&#41;,   Suecia &#40;30&#41;, Estados Unidos &#40;2,31&#41;, islas del Caribe &#40;32&#41;,   B&eacute;lgica &#40;33&#41; e Italia &#40;34&#41;.</p>     <p> El principal factor de riesgo para el desarrollo de este   s&iacute;ndrome es la infecci&oacute;n por cepas de <i>K. pneumoniae</i>  m&aacute;s virulentas, que presentan mayor resistencia a la   fagocitosis y a la muerte intracelular mediada por   complemento, neutr&oacute;filos y macr&oacute;fagos &#40;35-37&#41;, y   que se caracterizan por tener fenotipo hipermucoviscoso   y serotipo capsular K1 y K2 &#40;38-40&#41;. El 63 &#37; de los   aislamientos hechos en material de abscesos hep&aacute;ticos   por <i>K. pneumoniae</i> corresponden al serotipo K1 y   esta proporci&oacute;n aumenta al 85 &#37; en los casos asociados   con endoftalmitis &#40;41&#41;. Se han descrito dos genes   en relaci&oacute;n con las cepas de<i> K. pneumoniae</i> que producen   enfermedad invasiva: el <i>rmpA</i> &#40;regulador del   fenotipo mucoide A&#41;, que regula la s&iacute;ntesis del polisac&aacute;rido   extracapsular &#40;8,42&#41;, y el<i> mag A</i> &#40;asociado a la   mucoviscosidad A&#41;, que codifica para una prote&iacute;na de   membrana, esencial para la formaci&oacute;n de un exopolisac&aacute;rido   protector, y que se ha detectado en 24 &#37; de   las cepas con fenotipo hipermucoviscoso &#40;4,5,43-45&#41;.   El gen <i>magA</i> se encuentra &uacute;nicamente en las cepas   del serotipo K1 y corresponde a la variante genot&iacute;pica   espec&iacute;fica para esta cepa del gen <i>Wzy</i>, que codifica   para la polimerasa capsular &#40;2,41,46&#41;. Se han reportado   tambi&eacute;n algunos serotipos no K1 ni K2 que tienen   virulencia similar, y que se caracterizan por la expresi&oacute;n   de un sider&oacute;foro, la aerobactina &#40;4,47&#41;. Las cepas   de <i>K. pneumoniae</i> aisladas en este s&iacute;ndrome suelen   tener alta sensibilidad a antibi&oacute;ticos, con frecuencia   son resistentes &uacute;nicamente a la ampicilina &#40;5,20,48&#41;.  </p>     <p>Se ha propuesto que la siembra de la bacteria en el h&iacute;gado   se hace a partir del tracto gastrointestinal &#40;4&#41;, por   lo que la colonizaci&oacute;n intestinal por <i>K. pneumoniae</i>,   sobre todo del fenotipo hipermucoviscoso, es un factor   de riesgo para el desarrollo de absceso hep&aacute;tico &#40;49&#41;; la   frecuencia de colonizaci&oacute;n intestinal por este microorganismo   se ha reportado entre el 19 &#37; y el 38 &#37; en pa&iacute;ses   europeos y el 75 &#37; en Taiw&aacute;n, lo que podr&iacute;a explicar   al menos en parte la mayor prevalencia del s&iacute;ndrome   en pa&iacute;ses asi&aacute;ticos &#40;50&#41;.</p>     <p> Las complicaciones infecciosas secundarias descritas   incluyen: endoftalmitis, que es la m&aacute;s frecuente, se   presenta hasta en 60 &#37; de los casos; bacteriemia &#40;4&#41;,   absceso pulmonar &#40;40 &#37;&#41;, meningitis &#40;25 &#37;&#41;, fascitis   necrosante &#40;4 &#37;&#41;, y otros de menor frecuencia como   osteomielitis, prostatitis, absceso muscular y artritis   s&eacute;ptica. La mortalidad por este s&iacute;ndrome se estima en   11 &#37; &#40;5,51&#41;.</p>     <p> Los siguientes son los principales s&iacute;ntomas: fiebre, escalofr&iacute;os   y dolor abdominal; usualmente hay leucocitosis   y elevaci&oacute;n de la PCR y puede haber adem&aacute;s   trombocitopenia y alteraci&oacute;n en las pruebas de funci&oacute;n   hep&aacute;tica. En los casos asociados con endoftalmitis   los s&iacute;ntomas oculares suelen aparecer antes del   diagn&oacute;stico de absceso hep&aacute;tico &#40;52&#41;, pero tambi&eacute;n   pueden manifestarse varios d&iacute;as despu&eacute;s; son t&iacute;picos   el dolor ocular, el ojo rojo, el edema palpebral, la visi&oacute;n   borrosa, la inflamaci&oacute;n de la c&aacute;mara anterior y   el hipopi&oacute;n, la elevaci&oacute;n de la presi&oacute;n intraocular, el edema corneal, la disminuci&oacute;n o p&eacute;rdida del reflejo   rojo y la dificultad para visualizar el fondo de ojo por   la opacidad de los medios &#40;53&#41;.  </p>     <p>El pron&oacute;stico visual es malo en los casos de endoftalmitis   &#40;53&#41;: m&aacute;s del 85 &#37; de los pacientes quedan   con d&eacute;ficit visual grave a pesar de recibir tratamiento   intensivo con antibi&oacute;ticos sist&eacute;micos e intrav&iacute;treos   &#40;20,50,54,55&#41; y se describen tasas de evisceraci&oacute;n y   enucleaci&oacute;n entre 20 &#37; y 100 &#37; &#40;56,57&#41;; la evoluci&oacute;n   es mejor si se hacen tempranamente el diagn&oacute;stico y   el tratamiento con antibi&oacute;ticos &#40;4&#41; y depende tambi&eacute;n   de la AV inicial &#40;58&#41;.  </p>     <p>El tratamiento recomendado es el drenaje de las colecciones   y la administraci&oacute;n sist&eacute;mica de antibi&oacute;ticos   elegidos seg&uacute;n el antibiograma; en los casos de   endoftalmitis se recomienda particularmente el uso   de ceftriaxona ya que tiene buena penetraci&oacute;n demostrada   a los humores acuoso y v&iacute;treo &#40;20,59&#41;; en   estos casos se requieren adem&aacute;s la aplicaci&oacute;n intrav&iacute;trea   de antibi&oacute;ticos &#40;generalmente una cefalosporina   de tercera generaci&oacute;n como cefotaxime o ceftazidima&#41;   y la vitrectom&iacute;a, seg&uacute;n el criterio del m&eacute;dico tratante.   Cuando se a&iacute;slen microorganismos productores   de betalactamasas de espectro extendido, se debe   recurrir a un carbapen&eacute;mico, pero esta situaci&oacute;n es   muy rara. La duraci&oacute;n del tratamiento depende de la   respuesta cl&iacute;nica y de la resoluci&oacute;n del absceso documentada   por im&aacute;genes.  </p>     <p>Algunos autores recomiendan serotipificar todas las   cepas de <i>K. pneumoniae</i> aisladas de abscesos hep&aacute;ticos   para predecir el riesgo de s&iacute;ndrome invasor, y   buscar la presencia de absceso hep&aacute;tico en los casos   de meningitis y bacteriemia por <i>K. pneumoniae</i> &#40;20&#41;.</p>     <p> Nuestros casos, similares a los reportados en la literatura   internacional en cuanto al aislamiento de <i>K.   pneumoniae</i> multisensible que produce absceso hep&aacute;tico   primario y siembras infecciosas secundarias,   apoyan la tendencia descrita de aumento de esta entidad   en pa&iacute;ses occidentales. No se hizo en nuestros   casos serotipificaci&oacute;n del microorganismo ni detecci&oacute;n   de los genes <i>magA</i> o<i> rmpA</i>. Entre las infecciones   secundarias una paciente present&oacute; afectaci&oacute;n pulmonar   y bacteriemia; en los otros dos casos hubo &uacute;nicamente   endoftalmitis. El pron&oacute;stico del s&iacute;ndrome es   bueno en cuanto a la resoluci&oacute;n de la infecci&oacute;n sist&eacute;mica,   pero es malo cuando hay compromiso ocular, a   pesar de la sensibilidad del microorganismo a los antibi&oacute;ticos.   En la mayor&iacute;a de los pacientes se presenta   deterioro visual grave a pesar de recibir tratamiento   antibi&oacute;tico apropiado sist&eacute;mico e intrav&iacute;treo como   en el caso de nuestros pacientes, en dos de los cuales   hubo que eviscerar el ojo y el otro termin&oacute; sin percepci&oacute;n   luminosa, a pesar de conservar el globo ocular.   Es importante considerar que en los tres casos la AV   al ingreso era muy mala &#40;NPL, PL y MM&#41; lo cual es   un factor determinante para el desenlace visual, por   lo que es de suma importancia que estos pacientes   sean evaluados por el servicio de oftalmolog&iacute;a desde   el momento en que se hace el diagn&oacute;stico de absceso   hep&aacute;tico por <i>K. pneumoniae</i>. Las series m&aacute;s grandes   de casos de endoftalmitis end&oacute;gena por <i>K. pneumoniae</i>  se han reportado en pa&iacute;ses orientales: en Singapur,   en una serie de 10 pacientes, la AV fue de NPL en   el 50 &#37;, en ninguno fue mejor de MM y 20 &#37; requirieron   evisceraci&oacute;n &#40;56&#41;. En una serie de 27 pacientes en   China 89 &#37; tuvieron AV final PL o peor y 41 &#37; requirieron   evisceraci&oacute;n o enucleaci&oacute;n &#40;60&#41;. En una serie   de 53 casos en Taiw&aacute;n solo el 35,8 &#37; lograron AV final   cuentadedos o mejor &#40;58&#41;.</p>     ]]></body>
