MÉTODOS: se utilizaron 11 radiografías digitales directas tomadas a estudiantes de ortodoncia, las cuales se introdujeron en el programa Cephapoint. Se hizo la medición de 9 ángulos, cada uno medido en la radiográfia digital de manera manual y en el programa Cephapoint. Todas las medidas fueron hechas por 3 operadores con intervalo de 1 semana. Se halló el promedio de error interobservador para medir la reproducibilidad de cada medida angular, y el promedio de error intraobservador para determinar la precisión de cada observador. RESULTADOS: : el ángulo FH/N/Pg tuvo la menor diferencia en el error interobservador (0,10&deg;) en ambos métodos, favoreciendo el trazado manual. Mientras que los ángulos con menor diferencia de error interobservador en el trazado computarizado fueron: II-NB (0,11&deg;) y N-A/Pg (0,11&deg;). La reproducibilidad intraobservador mostró un Coeficiente de Correlación Intraclase (CCI), excelente para ambos métodos. CONCLUSIONES: la reproducibilidad de las medidas angulares con el trazado manual y computarizado no presentó diferencias significativas. De acuerdo a los hallazgos de este estudio, los métodos evaluados brindan igual validez diagnóstica.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[INTRODUCTION: digital cephalometry allows handling errors produced during manual tracing; the purpose of this study was therefore to evaluate the reproducibility and precision of angle measures between manual tracing and that obtained with Cephapoint in digital radiography. METHODS: 11 direct digital radiographs taken to orthodontics students were introduced in the Cephapoint computer program. 9 angles were measured in both hand-tracing digital radiography and Cephapoint. All measurements were made by 3 operators with 1-week interval. We calculated the average interobserver error to find the reproducibility of each angle measure, and the average intra-observer error to determine the accuracy of each observer. RESULTS: : the FH/N/Pg angle showed the smallest interobserver error difference (0.10&deg;) in both methods, favoring manual tracing. On the other hand, the angles with the smallest inter-observer error difference in computerized tracing were LI-NB (0.11&deg;) and N-A/Pg (0.11&deg;). Intraobserver reproducibility showed excellent Intraclass Correlation Coefficient (ICC) in both methods. CONCLUSIONS: reproducibility of angular measurements did not show significant differences between manual and computerized tracing. According to the findings of this study, the methods under evaluation offer equal diagnostic validity.]]></p></abstract>

<body><![CDATA[<p><b>Palabras clave:</b> cefalometr&iacute;a, reproducibilidad de resultados, radiograf&iacute;a, imagen radiogr&aacute;fica por emisi&oacute;n de doble fot&oacute;n. </p>  <hr noshade>             <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana"><b>INTRODUCCI&Oacute;N </b></font></p>       <p>En ortodoncia, gran parte del &eacute;xito del tratamiento y la satisfacci&oacute;n del paciente se basan en varios aspectos, como el diagn&oacute;stico, las diferentes opciones terap&eacute;uticas y la habilidad del operador. El diagn&oacute;stico oportuno y adecuado, es fundamental para establecer los objetivos del tratamiento, siendo necesarios varios medios y herramientas, dentro de las cuales est&aacute; el trazado cefalom&eacute;trico, utilizado desde 1930, para la evaluaci&oacute;n de datos antropom&eacute;tricos.<SUP>1</SUP> Este se basa en la toma de medidas, usando una radiograf&iacute;a del cr&aacute;neo del paciente, en donde se ubican puntos, se trazan planos y se miden &aacute;ngulos. Los resultados son comparados con unas medidas preestablecidas por diferentes estudios.<sup>2-6</sup> De acuerdo con estas investigaciones se han normalizado los m&eacute;todos de an&aacute;lisis que han hecho posible que el desarrollo de la radiograf&iacute;a cefalom&eacute;trica se considere como un medio diagn&oacute;stico confiable.<sup>7</sup></p>      <P>Los trazos pueden ser hechos de manera manual o digital, con software como el Quick Ceph,<sup>8</sup> Dolphin Imaging,<sup>9-12</sup> VistaDent,<sup>13</sup> entre otros, los cuales brindan eficacia en la obtenci&oacute;n del diagn&oacute;stico, adem&aacute;s de ayudar en el almacenamiento de im&aacute;genes de los pacientes.<sup>14</sup></P>      <p>El trazado manual hecho adecuadamente, ofrece resultados comparables a los obtenidos con el an&aacute;lisis cefalom&eacute;trico hecho con el m&eacute;todo computarizado, por lo que se considera un m&eacute;todo confiable y de alta validez cl&iacute;nica.<sup>15, 16</sup></p>      <p>Los errores del an&aacute;lisis cefalom&eacute;trico pueden ser sistem&aacute;ticos y aleatorios, dentro de estos &uacute;ltimos se encuentran los de localizaci&oacute;n e identificaci&oacute;n de puntos de referencia y los de medici&oacute;n.<sup>14</sup> El uso de cefalometr&iacute;as digitales puede eliminar los errores sistem&aacute;ticos que se generan en el trazado manual, durante la realizaci&oacute;n de l&iacute;neas entre puntos de referencia y la medici&oacute;n con un transportador.<sup>14</sup></p>      <p>Recientemente, Bonilla y colaboradores,<sup>17</sup> hicieron un estudio para determinar la reproducibilidad de 14 puntos cefalom&eacute;tricos en tejidos duros. En ese estudio crearon y usaron el software Cephapoint, que permite la ubicaci&oacute;n de puntos en un monitor de computador directamente con el cursor. Usaron una muestra de 22 radiograf&iacute;as, once radiograf&iacute;as convencionales y once digitales, cada par tomadas en un mismo paciente y en el mismo equipo, en el cual, primero se hizo la toma de la radiograf&iacute;a digital y despu&eacute;s se hizo la toma de la radiograf&iacute;a convencional. Encontraron que todos los puntos presentan similar reproducibilidad en las dos radiograf&iacute;as, con menor error interobservador en la imagen digital directa. Los autores recomendaron continuar la investigaci&oacute;n para evaluar medidas angulares en la radiograf&iacute;a digital directa y compararla con otros m&eacute;todos.</p>      <p>El objetivo de este estudio fue evaluar la reproducibilidad de las medidas angulares entre un trazo manual y el obtenido con el programa Cephapoint en una radiograf&iacute;a digital.</p>      ]]></body>
<body><![CDATA[<p>&nbsp;</p>      <p><font size="3" face="Verdana"><b>M&Eacute;TODOS</b></font></p>     <p>Se dise&ntilde;&oacute; un estudio de concordancia en el que se utilizaron 11 radiograf&iacute;as de perfil digitales directas, tomadas a estudiantes de ortodoncia, utilizadas en el estudio de Bonilla y colaboradores.<sup>17</sup></p>      <p>Las radiograf&iacute;as fueron tomadas en la posici&oacute;n natural de la cabeza, por un operador entrenado. Para cada participante se obtuvo una imagen radiogr&aacute;fica digital directa fosfoactivada, procesada en el equipo FCR CAPSULA X&reg;, la cual transfiere la imagen inmediatamente del equipo al monitor del computador. La imagen radiogr&aacute;fica de cada participante fue importada al programa Cephapoint, dise&ntilde;ado en un estudio previo.<sup>17</sup></p>      <p>Se hizo una selecci&oacute;n de las medidas angulares, definiendo claramente su ubicaci&oacute;n por tres observadores: posici&oacute;n del maxilar con respecto a la base del cr&aacute;neo, se mide con el &aacute;ngulo formado por los planos que van desde Silla Turca a Nasion y Nasion a punto A (SNA); posici&oacute;n de la mand&iacute;bula con respecto a la base del cr&aacute;neo, &aacute;ngulo tomado en la intersecci&oacute;n de los planos Silla Turca a Nasion y Nasion a punto B (SNB); inclinaci&oacute;n incisivo superior, &aacute;ngulo formado por eje longitudinal del  incisivo superior m&aacute;s vestibular y el plano palatino (U1-PP); &aacute;ngulo de Lande, formado por la intersecci&oacute;n del plano de Frankfort y la l&iacute;nea que va del punto Nasion al punto A (FH/NA); profundidad facial, &aacute;ngulo formado por el plano de Frankfort y la l&iacute;nea del punto A al punto Pogonion (FH/N/Pg); &aacute;ngulo de la convexidad de tejidos duros, formado por la intersecci&oacute;n de los planos que van del punto Nasion al punto A y del punto A al punto Pogonion (N-A/Pg); inclinaci&oacute;n del incisivo inferior con NB &aacute;ngulo formado por eje longitudinal del Incisivo Inferior m&aacute;s vestibular y el plano que va de Nasion al punto B (II/NB); &aacute;ngulo nasolabial formado por la intersecci&oacute;n de los planos que van de Columnela a Subnasal y de Subnasal a Labral superior (Cm/Sn/Ls); y &aacute;ngulo de la convexidad de tejidos blandos tomado en la intersecci&oacute;n del plano entre Glabela de tejidos blandos y Subnasal y del plano que va desde Subnasal al punto Pogonion de tejidos blandos(G'/Sn-Pg').</p>      <p>Todos los trazos manuales fueron hechos utilizando la impresi&oacute;n de la radiograf&iacute;a digital en papel radiogr&aacute;fico, con la impresora DRY PIX 2000 (FUJI FILM USA&reg;), en una escala corregida de 1:1, con ayuda de un negatoscopio bajo condiciones de luz artificial. Un mismo operador hizo el perfilograma para cada radiograf&iacute;a digital, el cual se traz&oacute; 3 veces, de manera que no existiera una diferencia mayor de 1 mm entre las estructuras anat&oacute;micas trazadas. Se utiliz&oacute; igualmente papel cefalom&eacute;trico (Ortho Organizers&reg;) de 0,76 mm de grosor (0,03 pulgadas), y mina HB 0,5 mm (Faber Castell&reg;). Para el trazo de los planos, se us&oacute; una regla milimetrada (Faber Castell&reg;) y para las medidas se us&oacute; un correl&oacute;metro de Bimler. Los tres observadores hicieron el trazado manual de los &aacute;ngulos para cada radiograf&iacute;a digital.</p>      <p>La posici&oacute;n promedio para cada una de las medidas angulares identificadas por tres observadores, fue definida como el "par&aacute;metro de referencia" en este estudio. Este par&aacute;metro fue utilizado para determinar los errores interobservador en el trazado manual y digital. Las diferencias promedio en grados, entre el par&aacute;metro de referencia y las mediciones hechas por los observadores, fue definido como el error interobservador, y este, a su vez, fue utilizado como la variable que determina la reproducibilidad para cada medida angular. Consecuentemente, la reproducibilidad en la identificaci&oacute;n de las medidas angulares para cada uno de los m&eacute;todos (manual y digital), podr&iacute;a ser comparada como las diferencias en la magnitud de estas distancias desde el promedio, entre los dos tipos de trazado.</p>      <p>La precisi&oacute;n o el error intraobservador se define como el nivel de acuerdo con las mediciones de cada observador, indicando qu&eacute; tan cerca se encuentran las medidas angulares obtenidas en una primera y segunda observaci&oacute;n, con intervalos semanales. Se obtiene comparando las medidas de cada operador y se calcula con el m&eacute;todo estad&iacute;stico Coeficiente de Correlaci&oacute;n Intraclase (CCI). <body><![CDATA[<p><font size="3" face="Verdana"><b>AN&Aacute;LISIS ESTAD&Iacute;STICO</b></font></p>      <p>Los datos fueron recogidos en Microsoft Excel versi&oacute;n 2007, y se analizaron en el programa estad&iacute;stico para Ciencias Sociales SPSS versi&oacute;n 15.0. Las variables cuantitativas se analizaron por medio de promedios y desviaciones est&aacute;ndar, para la comparaci&oacute;n entre el trazado manual y computarizado, se utiliz&oacute; la prueba t-student para grupos independientes y la prueba de Levene para igualdad de varianzas. En todas las pruebas se utiliz&oacute; el nivel de significancia &Alpha; = 0,05 y los intervalos de confianza se utilizaron con 95% de confiabilidad.</p>       <p>&nbsp;</p>      <p><font size="3" face="Verdana"><b>RESULTADOS</b></font></p>      <p>Tanto en las medidas tomadas en el trazo manual de la radiograf&iacute;a digital como en la imagen de la radiograf&iacute;a digital importada en el programa Cephapoint, el error interobservador y la desviaci&oacute;n est&aacute;ndar que se presentan en la <a href="#t1">tabla 1</a>, muestran niveles superiores a 7,9&deg; en los &aacute;ngulos Cm/Sn/Ls y U1-PP en los dos m&eacute;todos de trazado manual y digital.</p>     <p align="center"><a name="t1"><img src=/img/revistas/rfoua/v25n2/v25n2a05t1.jpg></a> </p>      <p>En el trazado manual, los &aacute;ngulos que presentaron mayor diferencia en el promedio de error interobservador, comparado con el trazo digital, fueron: G'/Sn-Pg' (0,32&deg;), II-NB (0,11&deg;) y N-A/Pg (0,11&deg;). En el trazado computarizado, los &aacute;ngulos que presentaron mayor diferencia en el error interobservador, fueron SNA (0,52&deg;), U1-PP (0,52&deg;), Cm/Sn/Ls (0,47&deg;), SNB (0,21&deg;), FH/NA (0,20&deg;) y FH/N/Pg (0,10&deg;). Al comparar el promedio del error interobservador entre el trazo manual y el computarizado, no se encontraron diferencias significativas. (p &ge; 0,05). </p>      <p>En la mayor&iacute;a de los &aacute;ngulos, el nivel de dispersi&oacute;n del error interobservador fue menor en el trazado manual, como lo indican los datos de desviaci&oacute;n est&aacute;ndar (<a href="#t1">tabla 1</a>). </p>      <p>Al comparar los dos trazados, con respecto a sus desviaciones est&aacute;ndar, se encontraron mayores diferencias en el m&eacute;todo manual para los &aacute;ngulos FH/NA, G'/Sn-Pg', N-A/Pg y SNA, y para el m&eacute;todo computarizado en Cm/Sn/Ls, FH/N/Pg, II-NB, SNB y U1-PP, sin diferencias estad&iacute;sticamente significativas.</p>       <p>El &aacute;ngulo FH/N/Pg tuvo la menor diferencia en el error interobservador (0,10&deg;) en ambos m&eacute;todos, favoreciendo el trazado manual, igualmente los &aacute;ngulos con menor diferencia de error interobservador en el trazado computarizado fueron: II-NB (0,11&deg;) y N-A/Pg (0,11&deg;). Mientras que los &aacute;ngulos SNA (0,52&deg;) y U1-PP (0,52&deg;), fueron los que presentaron mayor diferencia de error interobservador para el m&eacute;todo manual, y el &aacute;ngulo G'/Sn-Pg' (0,32&deg;) para el m&eacute;todo computarizado.</p>      ]]></body>
<body><![CDATA[<p>En cuanto a la evaluaci&oacute;n de la precisi&oacute;n o error intraobservador, para el m&eacute;todo manual, mostr&oacute; un Coeficiente de Correlaci&oacute;n Intraclase (CCI) excelente, siendo este mayor de 0,9 para todas las medidas, excepto para FH/NA, el cual fue de 0,847 (observador 2) (<a href="#t2">tabla 2</a>).</p>     <p align="center"><a name="t2"><img src=/img/revistas/rfoua/v25n2/v25n2a05t2.jpg></a> </p>      <p>En el m&eacute;todo computarizado, el CCI estuvo por encima de 0,844, con excepci&oacute;n del &aacute;ngulo FH/N/Pg, el cual fue de 0,784 (observador 2), y N-A/Pg, el cual fue de 0,793 (observador 1) (<a href="#t3">tabla 3</a>).</p>      <p align="center"><a name="t3"><img src=/img/revistas/rfoua/v25n2/v25n2a05t3.jpg></a> </p>        <p>&nbsp;</p>      <p><font size="3" face="Verdana"><b>DISCUSI&Oacute;N</b></font></p>        <p>En el presente estudio se encontr&oacute; que los promedios de error interobservador entre el trazado manual y el computarizado no presentaron diferencias significativas entre los m&eacute;todos; sin embargo, hubo valores m&aacute;s altos para los &aacute;ngulos SNA y U1-PP en el trazado computarizado y G'/Sn-Pg' en el trazado manual.</p>      <p>Con relaci&oacute;n al &aacute;ngulo SNA, algunos autores<sup>1, 12, 18</sup> refieren que con el m&eacute;todo computarizado se observa una disminuci&oacute;n de las diferencias entre las mediciones cefalom&eacute;tricas, convirti&eacute;ndose en un m&eacute;todo m&aacute;s preciso, debido a diferentes caracter&iacute;sticas del <i>software</i>, como la medici&oacute;n en pixeles, el contraste y el brillo. Estos factores lo convierten en un m&eacute;todo m&aacute;s fiable, especialmente cuando la localizaci&oacute;n de puntos de referencia debe hacerse en un contorno con profundidad en el hueso, como A, B y N. La ubicaci&oacute;n de estos puntos de referencia es importante al  momento de determinar la magnitud de una discrepancia horizontal en el maxilar, en una medida angular como SNA, por lo que el error de los puntos de referencia a lo largo del eje horizontal ser&iacute;a m&aacute;s significativo que el error a lo largo del eje vertical.<sup>1</sup> </p>      <p>En este sentido, cualquier cambio en la posici&oacute;n horizontal del punto A, se traduce en un cambio significativo en el resultado de SNA. La importancia de seleccionar un punto de referencia para uso en un an&aacute;lisis cefalom&eacute;trico, se refleja en un acertado diagn&oacute;stico y planificaci&oacute;n del tratamiento.<sup>1</sup> En concordancia, Lim y colaboradores<sup>18</sup> encontraron que los puntos de referencia anat&oacute;micos con radiodensidad baja, como el punto A, tienden a ser menos fiables en su identificaci&oacute;n en la radiograf&iacute;a computarizada.</p>      <p>Los resultados obtenidos en este estudio para el &aacute;ngulo U1-PP, son similares a los obtenidos por Collins y colaboradores,<sup>19</sup> quienes indican que los planos maxilar y mandibular construidos en una radiograf&iacute;a son hechos entre dos puntos que son dif&iacute;ciles de localizar. Adem&aacute;s, afirman que el aumento en la variabilidad de las medidas se debe a que es necesario digitalizar cuatro puntos para medir ciertos &aacute;ngulos (U1-PP),  y 3 puntos para otros &aacute;ngulos (SNA y SNB). Otras razones de esta variaci&oacute;n, son la superposici&oacute;n de las ra&iacute;ces que hace dif&iacute;cil la ubicaci&oacute;n exacta de los &aacute;pices,<sup>12, 20</sup> y la falta de contraste en esta zona,<sup>12</sup> lo que hace que las medidas que se relacionan con los &aacute;pices radiculares de los incisivos se encuentren dentro de los puntos de referencia menos reproducibles.<sup>21, 22</sup> Igualmente, Bonilla y colaboradores,<sup>17</sup> encontraron diferencias en los ejes X y Y para ENA y ENP, tanto en la imagen convencional como en la digital, alterando la reproducibilidad de estos puntos para el correcto trazado del plano palatino.</p>      ]]></body>
<body><![CDATA[<p>El &aacute;ngulo G'/Sn-Pg' present&oacute; mayor diferencia de error interobservador para el trazado manual, ya que el punto Subnasal presenta mayor promedio de error en el eje X, adem&aacute;s, el punto Pogonion es de dif&iacute;cil ubicaci&oacute;n en un &aacute;rea curva, similar a lo reportado por Chen.<sup>20</sup></p>      <p>En este estudio, el &aacute;ngulo FH/N/Pg tuvo el menor error interobservador, es decir, alta reproducibilidad en ambos m&eacute;todos, a diferencia de lo reportado por Sayinsu y colaboradores,<sup>11</sup> donde todos los par&aacute;metros con correlaciones m&aacute;s bajas, fueron medidas relacionadas con el plano de Frankfort, el cual pasa a trav&eacute;s de Porion y Orbitario. En este sentido, otros autores,<sup>12, 20, 24</sup> observaron que Porion est&aacute; situado en complejas estructuras radiopacas, las cuales se superponen y que el punto Orbital es m&aacute;s impreciso, probablemente por el alineamiento estrecho verticalmente del lado derecho e izquierdo de las &oacute;rbitas.<sup>9</sup> Asimismo, Bonilla y colaboradores,<sup>17</sup> encontraron que el punto Porion present&oacute; mayor desviaci&oacute;n est&aacute;ndar en el eje Y y el punto Infraorbitario en el eje X en la imagen convencional, en concordancia con Geelen y colaboradores,<sup>21</sup> quienes encontraron amplia dispersi&oacute;n en la distribuci&oacute;n del error en los dos ejes, indicando ser un punto impreciso. De igual manera, McClure y colaboradores,<sup>9</sup> reportaron que Pogonion se encuentra situado en la curva del contorno del ment&oacute;n, por lo tanto, este punto puede ser dif&iacute;cil de identificar.</p>      <p>En este estudio, el &aacute;ngulo Cm/Sn/Ls estuvo dentro de los mayores promedios de error interobservador en el trazado computarizado. En cuanto a esta medida, algunos autores<sup>24</sup> reportan que el &aacute;ngulo Nasolabial es una medida con gran relevancia cl&iacute;nica en el an&aacute;lisis de los tejidos blandos, requiriendo para su medici&oacute;n la construcci&oacute;n de dos l&iacute;neas a lo largo del contorno inferior de la nariz y el labio. Sin embargo, se presenta alta variaci&oacute;n en el m&eacute;todo de trazado, como lo indican otros estudios.<sup>3, 25-27</sup> Por su parte, Hwang y colaboradores,<sup>24</sup> encontraron diferencias significativas para el &aacute;ngulo Nasolabial y una baja reproducibilidad cuando se us&oacute; el m&eacute;todo de trazado de la l&iacute;nea tangente, resultado que se atribuy&oacute; a la forma anat&oacute;mica (forma de "S") que tiene la parte inferior de la nariz y no a la falta de coherencia en la construcci&oacute;n de la tangente. Asimismo, concluyeron que la variabilidad en la elaboraci&oacute;n de una tangente en el labio superior, contribuye a la baja reproducibilidad del &aacute;ngulo Nasolabial. En otro estudio, Swennen y colaboradores,<sup>12</sup> no encontraron diferencias estad&iacute;sticamente significativas entre los m&eacute;todos de an&aacute;lisis y el formato de imagen, con excepci&oacute;n del &aacute;ngulo Nasolabial, el cual excedi&oacute; la significancia cl&iacute;nica. Por lo tanto, se evidencia la dificultad en la construcci&oacute;n de este &aacute;ngulo.</p>      <p>Los valores que se observan en la <a href="#t1">tabla 1</a>, con respecto a los promedios de error interobservador, corresponden al promedio de los &aacute;ngulos obtenidos para el trazado manual y computarizado. El tipo de maloclusi&oacute;n no se determin&oacute; como criterio de inclusi&oacute;n de la muestra y, por tanto, los valores de la tabla reflejan esta variabilidad entre los &aacute;ngulos medidos, raz&oacute;n por la cual el an&aacute;lisis de resultados se hizo con base en las diferencias entre los m&eacute;todos.</p>      <p>Todos los Coeficientes de Correlaci&oacute;n Intraclase (CCI) indican altos niveles de precisi&oacute;n para ambos m&eacute;todos, sin embargo, se report&oacute; menor correlaci&oacute;n para el &aacute;ngulo FH/NA, en el trazado manual, y FH/N/Pg, N-A/Pg, en el m&eacute;todo computarizado.</p>      <p>De acuerdo con los resultados de este estudio, las diferencias en estos m&eacute;todos no son cl&iacute;nicamente relevantes, por lo que la aplicaci&oacute;n de uno u otro an&aacute;lisis no afecta el diagn&oacute;stico. Independientemente del m&eacute;todo aplicado, el cl&iacute;nico debe estar entrenado y calibrado para la ejecuci&oacute;n del mismo.<sup>11</sup> Por lo tanto, la elecci&oacute;n del m&eacute;todo de an&aacute;lisis depende del criterio del ortodoncista, en cuanto a la valoraci&oacute;n de las ventajas, desventajas, costos, tiempo, accesibilidad y comodidad.</p>       <p>&nbsp;</p>      <p><font size="3" face="Verdana"><b>CONCLUSIONES</b></font></p>        <p>La reproducibilidad de las medidas angulares entre el trazado manual y el trazado computarizado, no present&oacute; diferencias significativas, lo que indica que los dos m&eacute;todos brindan igual validez diagn&oacute;stica.</p>      <p>&nbsp;</p>       ]]></body>
