Though not well-known, his mechanisms of action include the regulation of vasoactives molecules, inhibition of the platelet aggregation, reduction of the inflammation and the reduction of cardiovascular risk factors such as the C-Reactive Protein (CRP), Homocistein, triglycerides and cholesterol.      <p><b>Key words</b>: mediterranean diet, fatty acids omega-3, endothelium, cardiovascular disease.  <hr>     <p><b>Introducci&oacute;n</b></p>      <p>El efecto beneficioso para la salud cardiovascular de la dieta mediterr&aacute;nea es algo bien conocido desde los hist&oacute;ricos estudios de Ancel Keys y su "Seven Countries Study", en los que despej&oacute; muchas dudas sobre la diferente distribuci&oacute;n de la enfermedad coronaria y su relaci&oacute;n con el modelo diet&eacute;tico y con el contenido de l&iacute;pidos, mostrando claramente que el tipo de grasa de la dieta, m&aacute;s que la cantidad de la misma, estaba relacionada con el riesgo cardiovascular. Comprob&oacute; que en los pa&iacute;ses de la cuenca mediterr&aacute;nea, a pesar de los altos niveles de colesterol plasm&aacute;tico, de la diabetes, de la hipertensi&oacute;n, del alto consumo de grasas saturadas y del porcentaje de fumadores, similar a los de los pa&iacute;ses n&oacute;rdicos, la tasa de mortalidad cardiovascular era aproximadamente un 36% menor que en los Estados Unidos y un 39% inferior a las tasas de Inglaterra (1-3).     <p>Si bien Keys describi&oacute; la dieta mediterr&aacute;nea como basada en el aceite de oliva, la fruta y el pescado, en los diferentes pa&iacute;ses mediterr&aacute;neos (Espa&ntilde;a, Portugal, Francia, Italia, Croacia, Bosnia, Albania, Grecia, Chipre, Turqu&iacute;a, Siria, L&iacute;bano, Egipto, Libia, Malta, T&uacute;nez, Argelia y Marruecos) se encuentran variaciones significativas y no todos tienen el  mismo tipo de dieta. En la dieta del sur de Italia  por ejemplo,  el contenido de grasas es del 30%, frente al 40% de la de Creta, existiendo las mismas variaciones en los distintos pa&iacute;ses en cuanto al consumo de fibra o carbohidratos (4).     <p>Estudios sobre los diversos elementos de la dieta mediterr&aacute;nea avalan los beneficios cardiovasculares de cada uno de ellos, tales como el aceite de oliva, del que se conocen sus efectos y mecanismos de protecci&oacute;n cardiovasculares (5-7), los frutos secos, que combinados con el aceite de oliva y como suplemento de la dieta mediterr&aacute;nea han presentado un efecto reductor de factores de riesgo cardiovascular frente a una dieta baja en grasa (7), el jam&oacute;n ib&eacute;rico, que reduce los niveles lip&iacute;dicos plasm&aacute;ticos (8) y el vino tinto, que junto con las frutas y los vegetales, son ricos en folatos y reducen los niveles de homociste&iacute;na, que incluso a nivel no muy elevado, es un importante factor de riesgo de arteriosclerosis en cualquier territorio: coronario, cerebral, o perif&eacute;rico (9). En el caso del vino sus propiedades antioxidantes mostraron una reducci&oacute;n de la mortalidad cardiovascular en un 30% y por todas las causas de un 20%, aunque las tasas de mortalidad por c&aacute;ncer y por muerte violenta fueran m&aacute;s elevadas que en los abstemios (10-11).      <p><b>Beneficios cardiovasculares del consumo de pescado</b></p>     <p>En el caso del pescado, el componente de la dieta mediterr&aacute;nea que nos interesa en esta revisi&oacute;n, las primeras observaciones sobre los beneficios cardiovasculares de su consumo no proceden de la dieta mediterr&aacute;nea, sino de las observaciones de Hugh Sinclair quien, analizando la dieta de los esquimales Inuit en la d&eacute;cada de 1940, encontr&oacute; que aunque estos consum&iacute;an enormes cantidades de grasa, apenas presentaban problemas cardiacos. Sus observaciones le llevaron a concluir que la causa de esta ausencia de problemas cardiacos estribaba en que, as&iacute; consumieran mucha grasa de foca y pescado, estas grasas deb&iacute;an de tener efectos protectores frente a las cardiopat&iacute;as (12). Posteriormente, &eacute;l mismo se someti&oacute; a 100 d&iacute;as de dieta esquimal a base de pescado y grasa de foca, con la que no s&oacute;lo perdi&oacute; peso, sino que su sangre se volvi&oacute; m&aacute;s fluida (13), siendo estas observaciones las primeras en indicar la reducci&oacute;n en la coagulaci&oacute;n y trombosis asociadas al consumo de pescado.      ]]></body>
<body><![CDATA[<p><b>Omega- 3 y mecanismos de acci&oacute;n</b></p>     <p>Hoy se sabe que la causa de los beneficios cardiovasculares del consumo de pescado se encuentra en su alto contenido en &aacute;cidos grasos insaturados, aquellos que presentan enlaces dobles en su cadena, con lo que sus mol&eacute;culas presentan cambios de direcci&oacute;n en los lugares d&oacute;nde aparecen esos dobles enlaces. Los &aacute;cidos grasos son las m&aacute;s simples de las grasas: independiente de la longitud de su cadena, contiene un grupo &aacute;cido (COOH) al comienzo de la misma, o extremo alfa y en el extremo opuesto, un grupo metilo, el llamado omega, en el que el &aacute;tomo de carbono est&aacute; saturado por la uni&oacute;n al carbono anterior y a tres &aacute;tomos de hidr&oacute;geno (CH<sub>3</sub>). Seg&uacute;n d&oacute;nde se encuentre el primero de los dobles enlaces (<a href="#fig1">Figura 1</a>), a partir del extremo omega, encontramos diversas familias: omega-3 (en el carbono 3), omega-6 (en el carbono 6) y omega-9 (en el carbono 9), siendo las membranas celulares m&aacute;s fluidas, cuanto mayor sea su concentraci&oacute;n en &aacute;cidos grasos insaturados.      <p>    <center><a name= "fig1"><img src="/img/revistas/med/v15n2/v15n2a09F01.gif" border= "0"></a></center></p>  Entre los omega-3, los mas importantes a nivel cardioprotector son el &aacute;cido docosahexaen&oacute;ico o docosa-4,7,10,13,16,19-hexaen&oacute;ico (DHA), de 22 carbonos y seis dobles enlaces y el eicosapentaen&oacute;ico  o eicosa-5,8,11,14,17-pentaen&oacute;ico (EPA), de 20 carbonos y cinco dobles enlaces (<a href="#fig2">Figura 2</a>). Su mecanismo cardioprotector no es bien conocido, pero existen indicios de que se incorporan a fosfol&iacute;pidos de las membranas celulares (14,15), lo que se asocia con un descenso del estr&eacute;s oxidativo y con un aumento de los productos finales del &oacute;xido n&iacute;trico (NO) (16), siendo esta la causa por la que mejora la funci&oacute;n endotelial.      <p>    <center><a name= "fig2"><img src="/img/revistas/med/v15n2/v15n2a09F02.gif" border= "0"></a></center></p>      <p><b>Beneficios cardiovasculares de los &aacute;cidos grasos omega-3</b></p>     <p>Existen evidencias de que el consumo de pescado una o dos veces a la semana y por lo tanto de este tipo de &aacute;cidos grasos, reduce la muertes cardiovasculares, principalmente la muerte s&uacute;bita, en casos de infarto agudo de miocardio (17-19),  asoci&aacute;ndose tambi&eacute;n los omega-3 con menos muerte s&uacute;bitas primarias (20). El riesgo de fallecer por enfermedad coronaria es un 25% inferior en los pacientes que comen pescado, frente a los que no lo incluyen en su dieta. En el estudio Dart Trial con 2033 pacientes que se recuperaron de un infarto de miocardio se observ&oacute; que los varones que com&iacute;an una vez a la semana pescado, reduc&iacute;an en un 39% la mortalidad, as&iacute; no se afectara el riesgo de infarto (21).     <p>Los &aacute;cidos grasos omega-3 tambi&eacute;n presentan efectos beneficiosos en el control de las arritmias (22), aparentemente mediante el control de los canales de calcio (23). En  cultivos celulares de coraz&oacute;n de rata se ha comprobado que los omega-3 disminuyen la actividad el&eacute;ctrica, haci&eacute;ndolas menos propensas a las arritmias (24,25), al mantener m&aacute;s estables el&eacute;ctricamente las c&eacute;lulas del m&uacute;sculo del coraz&oacute;n (26). En un estudio cl&iacute;nico con adultos mayores que de manera regular consum&iacute;an pescado azul (asado o cocido) se comprob&oacute; que presentaban menos incidencias de fibrilaci&oacute;n auricular que aquellos adultos que raramente com&iacute;an este tipo de pescado, mientras que el consumo de pescado frito o de emparedados de pescado no estaba ligado a la reducci&oacute;n del riesgo de fibrilaci&oacute;n auricular (27). En otros estudios cl&iacute;nicos tambi&eacute;n se demostr&oacute; que los omega-3  reduc&iacute;an el riesgo de desencadenar arritmias (28-30).      <p><b>Mecanismos moleculares de los &aacute;cidos grasos omega-3 en las acciones cardiovasculares</b></p>     ]]></body>
<body><![CDATA[<p>Complementar la alimentaci&oacute;n con aceite de pescado o con suplementos de &aacute;cidos grasos omega-3 ha aportado evidencia de que pueden modular favorablemente diferentes enfermedades (31,32). En pacientes obesos e hipertensos, la combinaci&oacute;n de p&eacute;rdida de peso y el consumo diario de aceite de pescado reduce el riesgo de padecer enfermedades cardiovasculares (33) y su uso, como suplemento de la dieta, inhibe tambi&eacute;n  la producci&oacute;n de citoquinas proinflamatorias, mejora la actividad antioxidante celular, disminuye el da&ntilde;o provocado por las especies reactivas del ox&iacute;geno y reduce la activaci&oacute;n del factor nuclear &kappa;&beta; (34,35). Otros par&aacute;metros de riesgo cardiovascular que tambi&eacute;n se ven beneficiados con el consumo de una dieta enriquecida con aceite de pescado son los niveles de apolipoprote&iacute;nas A1 y B y de  tromboxano B2, as&iacute; como los de fibrin&oacute;geno, que se reducen m&aacute;s de un 10% en tres semanas de tratamiento (36), hechos que explican la prevenci&oacute;n cl&iacute;nica de fen&oacute;menos tromb&oacute;ticos que ocurren con este tipo de dieta (37).      <p>La disminuci&oacute;n de los fen&oacute;menos tromb&oacute;ticos por una dieta rica en omega-3 (38), ya sea en forma de pescado, de aceite de pescado, o de suplementos de omega-3, no es la &uacute;nica y se observa tambi&eacute;n la reducci&oacute;n de algunos factores de riesgo cardiovascular como la prote&iacute;na C reactiva (39,40), la homociste&iacute;na (41) y la concentraci&oacute;n plasm&aacute;tica de triglic&eacute;ridos y de colesterol total (41-44), mediante la ingesta combinada de cuatro gramos de EPA + DHA, m&aacute;s dos gramos de &aacute;cido gamma-linol&eacute;nico (45), que se acompa&ntilde;a de un aumento del colesterol HDL (14,42,46-48). Adem&aacute;s, se regula la inflamaci&oacute;n mediante la inhibici&oacute;n de la agregaci&oacute;n plaquetaria (49), reduciendo la actividad del factor de von Willebrand (50), regulando la acci&oacute;n de diversas mol&eacute;culas de adhesi&oacute;n como  ICAM-1 (41,51-54), o VCAM-1 (41,50,55,56) y reduciendo la actividad de mol&eacute;culas vasoconstrictoras como la endotelina (57). Sin embargo,  existen estudios que demuestran que este efecto de los &aacute;cidos grasos omega-3 sobre la endotelina no se produce en individuos sanos, a menos que se les aplique concentraciones elevadas de cuatro gramos al d&iacute;a (58). Tambi&eacute;n se atribuye a los omega-3 una activaci&oacute;n de mol&eacute;culas vasodilatadoras como el &oacute;xido n&iacute;trico (NO) (55,59,60) y bloqueo,  a nivel de la placa arterioscler&oacute;tica, de factores de crecimiento como el factor de crecimiento derivado de las plaquetas (PDGF), un potente factor mitog&eacute;nico relacionado con el crecimiento de estas  placas (61).      <p><b>&Aacute;cidos grasos omega-3 y funci&oacute;n endotelial</b></p>     <p>Se ha comprobado que bajo un tratamiento con aceite de pescado hay mejor&iacute;as en la vasodilataci&oacute;n endotelio dependiente, debido a que se estimula la liberaci&oacute;n de NO por el endotelio y en pacientes con diabetes mellitus, este puede ser un mecanismo  de protecci&oacute;n frente al vasoespasmo y la trombosis (62). Sin embargo, en todos los estudios endotelio independientes en los que la vasodilataci&oacute;n se lograba con nitroprusiato s&oacute;dico, con o sin la dieta enriquecida, no se observaron diferencias. No obstante, en un estudio en el que se emple&oacute; s&oacute;lo EPA (1800 mg/d&iacute;a), mejor&oacute; tanto la vasodilataci&oacute;n endotelio dependiente como la independiente, lo que indica que mejora tambi&eacute;n los sistemas vasodilatadores que no dependen del NO (63). En pacientes con hipercolesterolemia, lo mismo que en sujetos sin esta patolog&iacute;a, se produce mejor&iacute;a de la vasodilataci&oacute;n NO dependiente, lo que puede explicar un posible mecanismo de protecci&oacute;n cardiovascular del aceite de pescado en estos pacientes (64).     <p>Respecto al comportamiento del enzima &oacute;xido n&iacute;trico sintasa inducible (&iota;NOS), el &aacute;cido graso poliinsaturado docosahexan&oacute;ico inhibe la producci&oacute;n del NO derivado de macr&oacute;fago, en respuesta al IFN-gamma y a TNF-alfa. Este puede ser tambi&eacute;n el mecanismo que explique, al menos en parte, la mejor&iacute;a de los procesos autoinmunes bajo tratamiento diet&eacute;tico con aceite de pescado, rico en DHA (65-66). Un mecanismo de acci&oacute;n similar que puede tener el aceite de pescado, tambi&eacute;n a trav&eacute;s de la v&iacute;a del NO, se sugiere para proteger contra la demencia del Alzheimer o para hacer m&aacute;s lento su desarrollo (67).      <p><b>Balance entre omega-3 y omega-6</b></p>     <p>Teniendo en cuenta que los &aacute;cidos grasos omega-3 reducen la inflamaci&oacute;n y los omega-6 la promueven, es comprensible que una dieta no equilibrada, en la que en la que predomine el mayor consumo de productos c&aacute;rnicos frente a productos de pescado (y como consecuencia el predominio de omega-6), promueva el desarrollo de enfermedad. Pero si la dieta es saludable, como ocurre con la dieta mediterr&aacute;nea, en que la relaci&oacute;n entre omega-3 y omega-6 es 1 a 6, contrario a la dieta de pa&iacute;ses como Estados Unidos, en que esta  relaci&oacute;n es 1 a 12, se promueve por el contrario el mantenimiento de la salud, porque disminuyen los procesos inflamatorios antes se&ntilde;alados. Por todo ello, la American Heart Association recomienda que la dieta de una persona sana incluya pescado al menos dos veces a la semana, mientras que un paciente con enfermedad cardiovascular lo deber&iacute;a hacer diariamente, de manera tal que logre ingerir un gramo por d&iacute;a de EPA + DHA (ya sea en pescado o en aceite de pescado) (68). Aunque haya reportes de la literatura que no muestren un efecto claro en la mortalidad y las complicaciones cardiovasculares asociadas al consumo de suplementos de &aacute;cidos grasos (69), del an&aacute;lisis de varios  estudios se ha deducido que los pacientes con niveles elevados de triglic&eacute;ridos deber&iacute;an recibir de dos a cuatro gramos diarios de EPA + DHA, de aceites de pescado purificados (70), habi&eacute;ndose fijado que la cantidad necesaria de pescado a consumir para reducir el riesgo cardiovascular es de 200 gramos a la semana (71). Tambi&eacute;n existen datos que sugieren que en personas mayores una peque&ntilde;a cantidad puede ser ya cardioprotectora, especialmente porque se reduce el riesgo de arritmia, de fibrilaci&oacute;n ventricular y tambi&eacute;n de artritis reumatoide (72-73).     <p>En el metan&aacute;lisis de Hooper (69) se indica, como comunicaci&oacute;n personal de Marchioli, que en el Gissi Prevenzione Trial se concluy&oacute; que para personas que han tenido un infarto resulta m&aacute;s beneficioso el consumo de suplementos de omega-3, as&iacute; en otros estudios se indique que como prevenci&oacute;n de enfermedades cardiovasculares,  el consumo de pescado dentro de una dieta equilibrada es m&aacute;s beneficioso (incluso con la existencia del riesgo de acumulaci&oacute;n de metil-mercurio) que el consumo de suplementos de &aacute;cidos grasos (74), aconsejando el consumo modesto del mismo de una a dos veces a la semana (75).
