<body><![CDATA[<p><b>Introducci&oacute;n</b></p>      <p>La toxoplasmosis es una zoonosis producida por el par&aacute;sito intracelular obligado Toxoplasma gondii (1). La infecci&oacute;n aguda adquirida despu&eacute;s del nacimiento en el paciente inmunocompetente suele ser asintom&aacute;tica en el 90% de los casos (2), pero en estados de inmunosupresi&oacute;n se facilita la reactivaci&oacute;n de la enfermedad, debido a que se activan las formas latentes del par&aacute;sito que persisten dentro de quistes tisulares (3-4). Tanto en infecciones agudas como cr&oacute;nicas el par&aacute;sito es responsable del desarrollo de manifestaciones cl&iacute;nicas que incluyen linfadenopat&iacute;as, encefalitis, miocarditis y neumonitis (5). La toxoplasmosis aguda se ha relacionado en la mayor&iacute;a de los casos con alg&uacute;n estado de inmunosupresi&oacute;n y las descripciones en la literatura de pacientes sin d&eacute;ficit inmune con afecci&oacute;n pulmonar por Toxoplasma gondii son muy escasas (6-9). Sin embargo, un n&uacute;mero creciente de reportes recientes han motivado el inter&eacute;s de la comunidad cient&iacute;fica en este tema (9-10). </p>     <p>Se trata de una infecci&oacute;n que en humanos se relaciona estrechamente con los h&aacute;bitos alimenticios y los cambios ambientales (12), ya sea por la ingesta de carnes de mam&iacute;feros (principalmente cerdo y cordero) inadecuadamente cocinadas e infectadas con quistes tisulares, o por la ingesta de aguas o alimentos contaminados con los ooquistes que expulsan los gatos en sus heces. Tambi&eacute;n es posible el contagio por transfusiones de hemoderivados y por trasplante de &oacute;rganos s&oacute;lidos (13). La seroprevalencia se correlaciona con los h&aacute;bitos de higiene y alimentarios de la poblaci&oacute;n siendo en los cerdos en Estados Unidos de entre el 0,6% al 93% (14). La seroprevalencia humana en algunos pa&iacute;ses de Latinoam&eacute;rica est&aacute; en alrededor del 90%, sin que en la mayor&iacute;a de casos se documente enfermedad cl&iacute;nica manifiesta (15). </p>     <p>La presentaci&oacute;n cl&iacute;nica de la toxoplasmosis est&aacute; dada por inflamaci&oacute;n de los ganglios en cabeza y cuello, cefalea, enfermedad leve con fiebre semejante a la mononucleosis, mialgias, odinofagia y en ocasiones s&iacute;ntomas gastrointestinales. No es raro encontrar hepatomegalia y hepatitis, con moderada elevaci&oacute;n de niveles de transaminasas (elevaci&oacute;n de 5-10). El hallazgo del laboratorio cl&iacute;nico reporta linfocitosis y linfocitos at&iacute;picos y a menudo el diagn&oacute;stico se hace con la confirmaci&oacute;n serol&oacute;gica, as&iacute; la regla de oro para el diagnostico sea la observaci&oacute;n del par&aacute;sito en la muestra (16-18).</p>      <p><b>Reporte de caso</b></p>      <p>Paciente masculino de 26 a&ntilde;os natural de Gachantiv&aacute; (Boyac&aacute;, Colombia), soldado profesional remitido del Hospital de Tolemaida (Tolima, Colombia), que ingres&oacute; al servicio de urgencias del Hospital Militar Central con cuadro cl&iacute;nico de tres semanas de evoluci&oacute;n caracterizado por fiebre no cuantificada, astenia, adinamia, hiporexia, malestar general, dolor en regi&oacute;n inguinal y cervical, s&iacute;ntomas que se instauraron durante patrullaje en zona selv&aacute;tica. Inicialmente fue tratado con acetaminof&eacute;n y dipirona, pero por la persistencia de los s&iacute;ntomas decidieron evacuarlo de la zona once d&iacute;as despu&eacute;s. Continu&oacute; el tratamiento analg&eacute;sico en Tolemaida, en donde le realizaron estudios de laboratorio (no se tienen los reportes) y ante la no mejor&iacute;a decidieron remitirlo a Bogot&aacute;. Como antecedentes personales, leishmaniasis tratada en dos oportunidades, el &uacute;ltimo episodio hace un a&ntilde;o. Niega procedimientos quir&uacute;rgicos, hospitalizaciones y alergias y dice consumir tres cigarrillos al d&iacute;a durante nueve a&ntilde;os. Conductas sexuales con uso de preservativo de manera ocasional, relaciones heterosexuales y pareja estable. Como antecedentes epidemiol&oacute;gicos refiere malas condiciones sanitarias y hacinamiento, ingesta de agua de r&iacute;os, pozos y aguas estancadas, as&iacute; como consumo de carne de animales salvajes mal cocidos, como la danta y acu&aacute;ticos como cangrejos y caracoles de r&iacute;o. Niega antecedentes familiares y en revisi&oacute;n por sistemas niega cambios en el h&aacute;bito intestinal o s&iacute;ntomas respiratorios, pero si hay p&eacute;rdida subjetiva de peso. Ingres&oacute; en buen estado general con signos vitales de presi&oacute;n arterial de 120/80 mmHg, pulso de 76, respiraciones 20, temperatura de 37oC, un examen f&iacute;sico que revel&oacute; adenopat&iacute;as cervicales, retroauriculares y occipitales de mas o menos dos cm y dolorosas a la palpaci&oacute;n. En regi&oacute;n axilar izquierda se palparon dos adenopat&iacute;as dolorosas de mas o menos dos cm, ruidos cardiacos sin alteraci&oacute;n, disminuci&oacute;n de la trasmisi&oacute;n de ruidos ventilatorios sin agregados, abdomen sin dolor a la palpaci&oacute;n, no viceromegalias, pero a nivel inguinal se palp&oacute; una cadena ganglionar bilateral de tres cm y dolorosas. El resto del examen f&iacute;sico sin alteraciones. Los resultados de ex&aacute;menes de laboratorio se resumen en la <a href="img/revistas/med/v17n2/v17n2a11t01.jpg" target="_blank">tabla 1</a>.</p>      <p>Se inicia tratamiento con TMT- sulfametoxazol 160/800 mg cada doce horas v&iacute;a oral y se solicita valoraci&oacute;n por cirug&iacute;a general para la toma de biopsia de ganglio con el siguiente reporte: Bx de ganglio inguinal izquierdo: linfadenitis, componente mixto, hiperplasia folicular que sugiere etiolog&iacute;a viral, tinci&oacute;n PAS negativa para microorganismos, naturaleza benigna. La radiograf&iacute;a de t&oacute;rax (<a href="#fig1">figura 1</a>) mostr&oacute; infiltrado reticulonodular en ambos campos y la tomograf&iacute;a report&oacute; patr&oacute;n de vidrio esmerilado con zona de consolidaci&oacute;n en base pulmonar izquierda (<a href="#fig2">figura 2</a>).</p>      <p>    <center><a name= "fig1"><img src="img/revistas/med/v17n2/v17n2a11f01.jpg"></a></center></p>     <p>    ]]></body>
<body><![CDATA[<center><a name= "fig2"><img src="img/revistas/med/v17n2/v17n2a11f02.jpg"></a></center></p>      <p>Se realiz&oacute; fibrobroncoscop&iacute;a que se encontr&oacute; dentro de los l&iacute;mites normales (<a href="#fig3">figura 3</a>) y se tomaron muestras bacteriol&oacute;gicas que reportaron un Gram negativo para microorganismos, KOH negativo, BX Transbronquial negativo para microorganismos; no granuloma. A los tres d&iacute;as de iniciado el tratamiento antibi&oacute;tico se document&oacute; resoluci&oacute;n de la fiebre con disminuci&oacute;n del tama&ntilde;o de las adenomegalias y pasados dias se tom&oacute; una nueva radiograf&iacute;a de t&oacute;rax reportada como normal (<a href="#fig4">figura 4</a>).</p>      <p>    <center><a name= "fig3"><img src="img/revistas/med/v17n2/v17n2a11f03.jpg"></a></center></p>     <p>    <center><a name= "fig4"><img src="img/revistas/med/v17n2/v17n2a11f04.jpg"></a></center></p>      <p>El diagn&oacute;stico de toxoplasmosis se confirm&oacute; con el reporte positivo de inmunoglobulinas espec&iacute;ficas para el par&aacute;sito (<a href="#tab2">tabla 2</a>).</p>      <p>    <center><a name= "tab2"><img src="img/revistas/med/v17n2/v17n2a11t02.jpg"></a></center></p>      <p><b>Discusi&oacute;n</b></p>      ]]></body>
<body><![CDATA[<p>T. gondii es un par&aacute;sito con capacidad de invadir todas las c&eacute;lulas nucleadas del organismo, teniendo mayor preferencia por n&oacute;dulos linf&aacute;ticos, cerebro, pulm&oacute;n y coraz&oacute;n. Alrededor del 10% al 20% de los pacientes cursan con s&iacute;ntomas, pero los individuos imunocompetentes son usualmente asintom&aacute;ticos (12). Este protozoo pertenece a la familia Sarcocystidae, que incluye los g&eacute;neros Sarcocystis y Toxoplasma, caracterizada por la proliferaci&oacute;n del estadio activo del par&aacute;sito llamado taquizo&iacute;to, capaz de producir necrosis por activaci&oacute;n de una respuesta inflamatoria exacerbada en la que predominan c&eacute;lulas mononucleares. Despu&eacute;s de un periodo de activaci&oacute;n celular suele desencadenarse una respuesta inmune espec&iacute;fica que generalmente controla la infecci&oacute;n. Es bien conocido el papel del INF-&gamma; en la diferenciaci&oacute;n de los macr&oacute;fagos (M&phi;) y de la respuesta inmune espec&iacute;fica que se monta para el control de los agentes infecciosos intracelulares, as&iacute; como el de otras citoquinas como la interleuquina 12 (Il12) y el factor de necrosis tumoral alfa (TNF-&alpha;) en el control de la parasitemia. La activaci&oacute;n de los linfocitos CD40, CD154 y la generaci&oacute;n de &oacute;xido n&iacute;trico (ON), mecanismos correspondientes a una respuesta inmune celular de tipo Th1, termina finalmente estimulando la producci&oacute;n de anticuerpos espec&iacute;ficos que controlan la proliferaci&oacute;n del par&aacute;sito. Reciente informaci&oacute;n in vivo confirma la importancia de productos intermediaros del metabolismo del nitr&oacute;geno como pieza clave para el control de infecci&oacute;n intracelular, as&iacute; como que la &oacute;xido n&iacute;trico sintetasa 2 es esencial para el control de la fase cr&oacute;nica de la infecci&oacute;n por Toxoplasma gondii (19-20).</p>      <p>Con respecto a la toxoplasmosis y el embarazo se calcula que en Colombia entre dos y diez de cada mil nacidos vivos sufren toxoplasmosis cong&eacute;nita. Por tanto, anualmente nacen m&aacute;s de 3000 ni&ntilde;os con la enfermedad, de los cuales entre el 85% y el 90% son asintom&aacute;ticos. Por otro lado, 47% de la poblaci&oacute;n femenina colombiana posee anticuerpos contra T. gondii, seg&uacute;n el Estudio Nacional de Salud- MSN, de 1983), lo que demuestra, a pesar de no existir un estudio nacional m&aacute;s reciente, la alta prevalencia del par&aacute;sito en nuestro medio, similar al de otros pa&iacute;ses tropicales en v&iacute;as de desarrollo. Uno de los estudios m&aacute;s importantes sobre toxoplasmosis cong&eacute;nita en Latinoam&eacute;rica fue realizado por G&oacute;mez-Mar&iacute;n et al en el departamento del Quind&iacute;o (Colombia), quienes informaron una frecuencia de seroconversi&oacute;n en maternas de 1,9% (IC 95%: 1,2-2,8%). En Europa, en donde la principal fuente de contagio son las carnes mal cocidas, se observan altos porcentajes de seropositividad y en B&eacute;lgica, en donde la seroprevalencia ha llegado a superar el 50%, el Programa Nacional para la Prevenci&oacute;n de Toxoplasmosis en el Embarazo report&oacute; en 1998 una reducci&oacute;n del 63% en la frecuencia de seroconversi&oacute;n en mujeres gr&aacute;vidas, con tan s&oacute;lo promover adecuados h&aacute;bitos higi&eacute;nicos espec&iacute;ficos para evitar la enfermedad (21).</p>      <p>En los paciente con sida es alta la tasa de neumon&iacute;a por Toxoplasma, comparados con los individuos imunocompetentes en quienes como ya se dijo, la presentaci&oacute;n cl&iacute;nica predominante es la ganglionar, con adenomegalias (22). El compromiso pulmonar en el contexto de infecci&oacute;n del inmunosuprimido con afecci&oacute;n sist&eacute;mica ocupa el tercer lugar despu&eacute;s de la encefalitis y la miocarditis (22-25), siendo los valores de INF-&gamma; y la funci&oacute;n de los M&phi; alveolares factores determinantes para el desarrollo de la enfermedad y el grado de compromiso local. Estudios experimentales han documentado alteraciones en las concentraciones pulmonares de interleucinas sintetizadas por linfocitos Th1 de pacientes infectado por Toxoplasma gondii(26).</p>      <p>Los s&iacute;ntomas m&aacute;s comunes de la toxoplasmosis pulmonar son tos, fiebre, disnea, y malestar general (27,28), la presentaci&oacute;n usual es benigna, pudiendo existir progresi&oacute;n hacia hipoxemia severa y falla respiratoria. Los reportes de caso disponibles en la literatura documentaron una presentaci&oacute;n m&aacute;s frecuente en hombres j&oacute;venes (80%) con una edad media de 34 a&ntilde;os (29). Todos los pacientes descritos presentaron IgM e IgG positivos al momento del estudio, as&iacute; sea la prueba de oro para el diagn&oacute;stico, la visualizaci&oacute;n de los par&aacute;sitos con una tinci&oacute;n permanente, como lo de Giemsa. El patr&oacute;n radiol&oacute;gico cl&aacute;sico es el infiltrado reticular y nodular bilateral, pero el primero de ellos obliga al diagn&oacute;stico diferencial de neumon&iacute;a por Pneumocystisis jirovecii (30). Otros hallazgos radiol&oacute;gicos son la consolidaci&oacute;n pulmonar y el derrame pleura, observados en menor proporci&oacute;n. El an&aacute;lisis histopatol&oacute;gico usualmente muestra neumonitis con infiltrado intersticial y exudado fibrinoide alveolar (31). El r&eacute;gimen terap&eacute;utico m&aacute;s efectivo para el tratamiento de esta entidad infecciosa sigue siendo la combinaci&oacute;n de sulfadiazina y pirimetamina, pero conclusiones extrapoladas de estudios recientes en pacientes con inmunocompromiso y encefalitis, mostraron similar efectividad para el trimetropin sulfametoxazol con tasas de &eacute;xito, fallo terap&eacute;utico y recurrencia que permiten el uso de este &uacute;ltimo medicamento en la pr&aacute;ctica cl&iacute;nica en forma segura (32). En el caso que se reporta no se realiz&oacute; diagn&oacute;stico microbiol&oacute;gico, sin embargo las presentaciones cl&iacute;nica y radiol&oacute;gica, junto con la resoluci&oacute;n de la sintomatolog&iacute;a y la normalizaci&oacute;n de la radiograf&iacute;a de t&oacute;rax constituyeron los elementos diagn&oacute;sticos positivos para el paciente.</p>  <hr>      <p><b>Referencias</b>      <!-- ref --><p>1.
