<body><![CDATA[<center><font face='verdana' size='2'><b>Palavras chave</b></font></center>     <p><font face='verdana' size='2'> S&iacute;ndrome de morte s&uacute;bita do lactante, s&uacute;bita do lactante, SMSL, Metabolismo, Fatores de risco. </font></p> <hr>    <br>     <center><font face='verdana' size='3'><b>INTRODUCCI&Oacute;N</b></font></center>     <p><font face='verdana' size='2'> El s&iacute;ndrome de muerte s&uacute;bita del lactante (SMSL), puede definirse como la muerte inesperada de un ni&ntilde;o menor de un a&ntilde;o de edad, que permanece sin explicaci&oacute;n despu&eacute;s de una investigaci&oacute;n minuciosa, incluyendo el examen del lugar de la muerte, la revisi&oacute;n de la historia cl&iacute;nica y una necropsia rigurosa (1), aunque existen controversias relacionadas con esta definici&oacute;n (2,3,4). </font></p>     <p><font face='verdana' size='2'> Hasta la d&eacute;cada de los 90, su incidencia oscilaba entre un 1 y 3 por mil reci&eacute;n nacidos vivos, en pa&iacute;ses occidentales y hasta un 7 por mil en Australia y Nueva Zelanda (5,6). Siendo los factores epidemiol&oacute;gicos m&aacute;s relacionados con este s&iacute;ndrome, los siguientes: posici&oacute;n prona al dormir, el calor excesivo de la cuna y la habitaci&oacute;n del ni&ntilde;o y las superficies blandas, y el consumo de tabaco durante el embarazo (7,8). En t&eacute;rminos de porcentaje alrededor del 80 % de las muertes ocurre entre el primer y sexto mes. Entre los 6 y 12 meses ocurre el 20 % restante. La implementaci&oacute;n de ciertas medidas, como el cambio de la postura de los ni&ntilde;os al dormir, de prono a supino, han disminuido esa incidencia hasta en un 50% (9,10,11,12). Actualmente y como consecuencia de las indicaciones de hacer dormir al lactante boca arriba, la incidencia se redujo a menos de 1 por cada 1.000 reci&eacute;n nacidos vivos, siendo alrededor del 60 % varones y un 40 % mujeres (13). </font></p>     <p><font face='verdana' size='2'> Se dan explicaciones fisiopatol&oacute;gicas a los factores relacionados con este s&iacute;ndrome, que van desde la postulaci&oacute;n de la hipercarbia e hipoxia, generadas al dormir con la cara apoyada en la almohada, hasta las alteraciones en los quimiorreflejos lar&iacute;ngeos (14,15), pasando las m&aacute;s antiguamente postuladas, como son las infecciones respiratorias agudas y el reflujo gastroesof&aacute;gico (16). </font></p>     <p><font face='verdana' size='2'> Entre los principales mecanismos fisiol&oacute;gicos que tratan de explicar el SMSL se encuentran la hipoxia s&uacute;bita (por ejemplo causada por una infecci&oacute;n respiratoria) (17), convulsiones epil&eacute;pticas inducidas por hipoxemia (18), hipoglicemia a consecuencia de una enfermedad metab&oacute;lica (19), arritmia maligna causada por el s&iacute;ndrome Q-T largo (20), y sofocaci&oacute;n accidental o intencional (21). </font></p>     <p><font face='verdana' size='2'> <b>Modelo del triple riesgo</b>    <br>    ]]></body>
<body><![CDATA[<br> Algunos estudios postulan que la epidemiolog&iacute;a del SMSL se basa en la presentaci&oacute;n de un lactante vulnerable, definido como un lactante que posee un defecto o anormalidad subyacente e inaparente, tal cual ser&iacute;a una anormalidad en zonas del sistema nervioso responsables de la respiraci&oacute;n y la frecuencia cardiaca, pero perteneciente al denominado “Modelo de Triple riesgo”, el cual tiene en cuenta otros dos factores como son: estar en la etapa cr&iacute;tica del desarrollo, es decir, en los primeros seis meses de vida con un crecimiento r&aacute;pido y un cambio en el control del equilibrio fisiol&oacute;gico. Sobre todo en los modelos del sue&ntilde;o y del despertar o variaciones en la respiraci&oacute;n, en la frecuencia cardiaca, en la presi&oacute;n arterial y en la temperatura del organismo, as&iacute; como factores externos de estr&eacute;s tales como la posici&oacute;n boca abajo para dormir, la exposici&oacute;n al humo del cigarrillo, o una infecci&oacute;n de las v&iacute;as a&eacute;reas superiores que no pueda superar el lactante vulnerable y puedan causarle la muerte (22). </font></p>     <p><font face='verdana' size='2'> De acuerdo con este modelo, debe haber interacci&oacute;n entre los tres elementos, para que se produzca la muerte s&uacute;bita e inesperada. La vulnerabilidad entonces no podr&iacute;a ser detectada hasta que el ni&ntilde;o ingresa en esta etapa cr&iacute;tica del desarrollo y se ve expuesto a los factores externos de estr&eacute;s, sin embargo existen num&eacute;ricamente muchas m&aacute;s razones (factores de riesgo) que tratan de explicar este s&iacute;ndrome, como puede apreciarse en los apartes siguientes. </font></p>     <p><font face='verdana' size='2'> <b>Implicaciones fisiopatol&oacute;gicas relacionadas con el sistema respiratorio</b>    <br>    <br> Por ser el sistema respiratorio uno de los m&aacute;s se&ntilde;alados como implicados en el SMSL, dentro de los m&uacute;ltiples estudios disponibles, se cuenta con el de Scout y colaboradores (23), que en 1978 postularon que algunas muertes en la cuna podr&iacute;an ser el resultado del desarrollo de bronquiolitis, lo que llevaba a la muerte antes de la aparici&oacute;n de s&iacute;ntomas relacionados con enfermedad severa. Con frecuencia, los ni&ntilde;os afectados de SMSL, presentan evidencias de inflamaci&oacute;n de las v&iacute;as a&eacute;reas (24,25,26,26). Algunos marcadores sugieren en este mismo grupo, una respuesta aguda a infecci&oacute;n pulmonar, tales como infiltrados inflamatorios peribronquiales, incremento de inmunoglobulinas en traquea y pulmones, altos niveles de citoquinas en pulmones (IL-4.IL-5, IL-10), as&iacute; como gran activaci&oacute;n de c&eacute;lulas T (27,28,29). Hasta un 80% de los ex&aacute;menes postmortem en v&iacute;ctimas del SMLM muestran hemorragias petequiales en pleura visceral (30), lo que sugiere una combinaci&oacute;n de infecci&oacute;n e hipoxia (31). Dentro de los agentes etiol&oacute;gicos que afectan el sistema respiratorio, asociados con SIDS han sido encontrados en su gran mayor&iacute;a, en diferente proporci&oacute;n diferentes virus (Sincitial respiratorio, Influenza, Rinovirus, Adenovirus, Citomegalovirus), reportados en estudios de tipo casos y controles desde 1966 hasta 1993 (32,33,34,35,36,37). </font></p>     <p><font face='verdana' size='2'> Con base en esta relaci&oacute;n, se sugieren varias hip&oacute;tesis patol&oacute;gicas e inmunol&oacute;gicas como son: La liberaci&oacute;n de citokinas desde los eosin&oacute;filos, lo que conlleva a una respuesta anormal de las v&iacute;as a&eacute;reas y a edema pulmonar (38), Liberaci&oacute;n de interleukina I y alfa interfer&oacute;n, lo que conlleva a depresi&oacute;n de las neuronas del centro respiratorio en el tallo cerebral (39,40), provisi&oacute;n de una v&iacute;a de entrada para toxinas bacterianas lo que genera endotoxemia, como resultado de la inflamaci&oacute;n de las v&iacute;as a&eacute;reas, destacando el <i>Staphylococus aureus</i>, como el ejemplo m&aacute;s probable de germen que toma ventaja en tejidos previamente inflamados (41,42), estimulaci&oacute;n de quimiorreceptores lar&iacute;ngeos (43), y disfunci&oacute;n de quimiorreceptores perif&eacute;ricos, tales como los cuerpos neuroepiteliales, involucrados en el control autom&aacute;tico de la respiraci&oacute;n (44). </font></p>     <p><font face='verdana' size='2'> Debe tenerse en cuenta que estas hip&oacute;tesis est&aacute;n directamente relacionadas con los denominados cambios de temperatura (per&iacute;odos fr&iacute;os) (45,46) y la presencia de infecciones respiratorias previas (47,48), as&iacute; como otros factores de riesgo comunes a un alto porcentaje de v&iacute;ctimas de este s&iacute;ndrome (consumo de cigarrillo por parte de los padres, grupo socioecon&oacute;mico, origen &eacute;tnico, bajo peso al nacer, nacimientos pret&eacute;rmino, madres muy j&oacute;venes, embarazos en madres solteras, g&eacute;nero y lactancia materna (49). </font></p>     <p><font face='verdana' size='2'> <b>Factores ambientales asociados con riesgo incrementado de SMSL</b>    <br>    <br> Dentro de los factores ambientales relacionados con SIDS, cabe tener en cuenta dos grandes clasificaciones. En la primera, los factores de riesgo se conocen como antenatales o relacionados con el embarazo y entre ellos destacan el h&aacute;bito de fumar en las madres (50), consumo de alcohol (especialmente periconcepcionalmente y durante el primer trimestre del embarazo) (51,52,53), consumo de drogas ilegales (especialmente opi&aacute;ceos (54,55), control prenatal inadecuado (56,57), bajo nivel socioecon&oacute;mico (58),madres muy j&oacute;venes, (59) embarazos en madres solteras (60), bajo nivel educativo de los padres (61), alto n&uacute;mero de partos (62), intervalos cortos entre embarazos (63), hipoxia intrauterina (64), retardo en el crecimiento fetal (65). En la segunda, los factores est&aacute;n relacionados directamente con el ni&ntilde;o como son: corta edad (entre 2 y 4 meses de edad promedio de m&aacute;xima mortalidad) (66), sexo masculino (67), raza/grupo &eacute;tnico (68,69), no uso del “chupete” para dormir (70,71), bebes prematuros (72), posici&oacute;n prona al dormir (73,74,75,76,77), reciente enfermedad febril (78), exposici&oacute;n al humo del cigarrillo (79), colchones o superficies para dormir demasiado blandas (80), estr&eacute;s t&eacute;rmico, sobrecalentamiento del ambiente donde el ni&ntilde;o duerme (81), cara cubierta por ropa de cama (82), compartir la cama con padres o hermanos u otro tipo de personas (83,84,85), dormir en un cuarto aparte de el de los padres (86) y ambientes muy fr&iacute;os (invierno), as&iacute; como la falta de calefacci&oacute;n en estos ambientes (87). </font></p>     ]]></body>
<body><![CDATA[<p><font face='verdana' size='2'> <b>Factores de riesgo gen&eacute;tico y metab&oacute;lico</b>    <br>    <br> M&uacute;ltiples investigaciones tratan de explicar el SMSL con base en disfunciones org&aacute;nicas tales como las alteraciones del ritmo card&iacute;aco y el s&iacute;ndrome del QT largo, describiendo mutaciones en varios genes codificadores de las sub-unidades estructurales de los canales del sodio y potasio del coraz&oacute;n en familias con el s&iacute;ndrome (88), alteraciones en la regi&oacute;n promotora del gen transportador de serotonina (89,90,91,92), problemas del desarrollo del sistema nervioso aut&oacute;nomo (93), problemas en el gen del complemento C4 y de la interleukina 10, con incremento en el riesgo de infecci&oacute;n e inflamaci&oacute;n, as&iacute; como alteraciones neurol&oacute;gicas, procesos hemorr&aacute;gicos debidos a malformaciones anat&oacute;micas y trastornos de la coagulaci&oacute;n (94). </font></p>     <p><font face='verdana' size='2'> Actualmente, se responsabiliza tambi&eacute;n a los trastornos metab&oacute;licos como posible causa fundamental del SMSL, siendo reportadas algunas entidades como: alteraciones en la cadena respiratoria mitocondrial, aciduria dicarbox&iacute;lica, aciduria etilmal&oacute;nica, aciduria metilmal&oacute;nica, aciduria propi&oacute;nica, aciduria isoval&eacute;rica, glucogenosis tipo I, galactosemia, fructosemia, enfermedad de Wolman, homocistinuria, trastornos del ciclo de la urea, acidosis l&aacute;cticas (biotinidasa, piruvato deshidrogenasa) y trastornos de la &beta;-oxidaci&oacute;n mitocondrial de los &aacute;cidos grasos entre otras (95,95,97,98). </font></p>     <p><font face='verdana' size='2'> De las entidades metab&oacute;licas anteriormente mencionadas, los trastornos de la &beta;-oxidaci&oacute;n mitocondrial de los &aacute;cidos grasos, son los m&aacute;s implicados, estando relacionadas con el SMSL las siguientes deficiencias enzim&aacute;ticas: transportador de carnitine (OCTN2) carnitina/acilcarnitina translocasa, carnitina palmitoiltransferasa II, varias acil-CoA deshidrogenasas , 3-hidroxiacil-CoA deshidrogenasa de cadena larga y la prote&iacute;na trifuncional, as&iacute; como las m&uacute;ltiple acil-CoA deshidrogenasas (99,100). </font></p>     <center><font face='verdana' size='3'><b>CONCLUSI&Oacute;N</b></font></center>     <p><font face='verdana' size='2'> La multifactorialidad del SMSL hace que cada vez se dise&ntilde;en m&aacute;s estudios tratando de explicar este s&iacute;ndrome, pero debe tenerse en cuenta ante todo que la interacci&oacute;n entre los factores gen&eacute;ticos y los factores ambientales, genera una regulaci&oacute;n auton&oacute;mica insuficiente que desencadena el SMSL. Cabe anotar adem&aacute;s que existe un grupo especial, denominado “ni&ntilde;os con riesgo incrementado de sufrir SMSL”, el cual incluye todos aquellos ni&ntilde;os que han sufrido alg&uacute;n evento aparentemente amenazador para su vida, dentro de los cuales, la observaci&oacute;n m&aacute;s com&uacute;n es la presentaci&oacute;n de cianosis o cambios s&uacute;bitos en el color, adem&aacute;s ni&ntilde;os que nazcan en familias con antecedentes de SMSL nacidos pret&eacute;rmino (101,102). </font></p>     <p><font face='verdana' size='2'> En nuestras poblaciones comenzamos a conocer casos aislados de posibles familias que presentan este s&iacute;ndrome posiblemente debido a alteraciones en la &beta;-oxidaci&oacute;n mitocondrial de los &aacute;cidos grasos, por eso es de capital importancia apoyar la investigaci&oacute;n en ciencias b&aacute;sicas y b&aacute;sico-cl&iacute;nicas con miras a generar aportes tendientes al planteamiento de hip&oacute;tesis de fisiopatolog&iacute;a, para que se tenga en el futuro una explicaci&oacute;n de este s&iacute;ndrome, cambiando radicalmente su definici&oacute;n e incrementando su prevenci&oacute;n. </font></p> <hr>    <br>     <center><font face='verdana' size='3'><b>REFERENCIAS BIBLIOGR&Aacute;FICAS</b></font></center>     ]]></body>
