Por estas razones se realizó un estudio para determinar la situación nutricional en menores de 10 años en el municipio Pereira, 2011. En este artículo se presenta la segunda parte de sus resultados para desnutrición crónica. Materiales y Métodos: estudio de corte transversal, con una muestra probabilística, estratificada, evaluando el indicador talla para la edad en niños y niñas menores de 10 años del municipio Pereira (2011). La muestra fue recogida en colegios y en hogares familiares del municipio. Los resultados son comparados con los indicadores de los estudios de 2001 y 2008, con metodologías similares. Resultados: Para 2011 se aprecia una mejoría considerable en los niveles de desnutrición crónica, comparado con los resultados de 2001, con una reducción de 8,4 puntos porcentuales (de 41,1% a 32,7%) (20,4% de reducción). En los menores de 2 años, la prevalencia de desnutrición crónica fue de 35,4%, en tanto que los de 2 a 9,999 años fue de 32,09%. Discusión: Los presentes resultados deben orientar al diseño de políticas públicas que impulsen el avance del desarrollo social, el mejoramiento de las condiciones de vida, así como incidir en forma directa en la disponibilidad de alimentos y la educación nutricional en el Municipio a los fines de mejorar en forma integral las condiciones nutricionales que tiene la población infantil en Pereira. Para evaluar el impacto de dichas intervenciones especialmente en desnutrición crónica, deben mantenerse estudios periódicos de este tipo a mediano plazo para ver sus efectos y la tendencia en su reducción.]]></p></abstract>

Discussion: Present results should orient the design of public policies that promote the advance in social development, improvement of life conditions as well ways to impact in the availability of food and nutritional education in the municipality in order to integrally improve the nutritional conditions of the childhood population in Pereira. in order to assess the impact of those interventions, especially in chronic malnutrition, this kind of periodical studies should be done to see their effects and trends in its reduction.]]></p></abstract>

M&aacute;s aun en el caso de las alteraciones del mismo a mediano y largo plazo, como ocurre con la desnutrici&oacute;n cr&oacute;nica, lo cual amerita evaluar peri&oacute;dicamente dicha condici&oacute;n en la poblaci&oacute;n infantil (1-22). M&aacute;s all&aacute; de eso, ser&iacute;a ideal adicionalmente que los pa&iacute;ses pudiesen contar con sus propios patrones de referencia nacional y no s&oacute;lo usar el internacional, generado a partir del estudio multic&eacute;ntrico de referencia de la Organizaci&oacute;n Mundial de la Salud (WHO Multicentre Growth Reference Study, WHO-MGRS) (23-29).</p>     <p align="justify">En el estudio del a&ntilde;o 2001 de evaluaci&oacute;n del estado nutricional en la poblaci&oacute;n infantil del municipio Pereira, departamento Risaralda, Colombia (<a href="#f1">Figura 1</a>), se encontr&oacute; que en menores de 10 a&ntilde;os la desnutrici&oacute;n cr&oacute;nica alcanz&oacute; un valor de 41,1%. Posteriormente, en el a&ntilde;o 2008, dicho valor fue de 27,4% (30,31).</p>     ]]></body>
<body><![CDATA[<p align="justify">En el presente estudio, basado en metodolog&iacute;as probabil&iacute;sticas, que permiten estimar mejor la realidad del estado nutricional de toda la poblaci&oacute;n menor de 10 a&ntilde;os del municipio para el presente momento (diciembre 2011), se presentan los resultados concernientes al indicador talla para la edad (desnutrici&oacute;n cr&oacute;nica) en la mencionada poblaci&oacute;n y los detalles metodol&oacute;gicos y muestrales del estudio. Ya que en la primera parte, se describieron los aspectos metodol&oacute;gicos generales y de la muestra, as&iacute; como los resultados para el indicador de desnutrici&oacute;n global (32).</p>     <p align="justify">En la siguiente parte, tercera, se presentaran los resultados relativos al indicador peso para la talla (desnutrici&oacute;n aguda).</p>     <p align="justify">La metodolog&iacute;a empleada en el presente estudio fue similar a la de los estudios publicados en 2001 y 2008 (30,31), con el fin de poder hacer comparaciones con relaci&oacute;n a sus resultados con la actual evaluaci&oacute;n (32).</p>    <p align="center"><a name="f1"></a><a href="img/revistas/rmri/v19n1/v19n1a12-1.jpg" target="_blank">Figura 1</a></p>     <p><font face="verdana" size="3"><b><a name="bookmark2"></a>Materiales y m&eacute;todos</b></font></p>     <p><em>Tipo de Estudio</em></p>     <p align="justify">Este fue un estudio probabil&iacute;stico, estratificado, biet&aacute;pico y sistem&aacute;tico, en el cual se midi&oacute; la variable talla en menores de 2 a&ntilde;os con un Infant&oacute;metro Port&aacute;til Marca Kramer &reg;; rango de Medici&oacute;n: 50 a 102 cms; apreciaci&oacute;n: 0,5 cms (5 mm) y en &gt;2 a&ntilde;os con un Tall&iacute;metro Port&aacute;til Marca Kramer &reg;; rango de Medici&oacute;n: 0 a 200 cms; apreciaci&oacute;n: 0,5 cms (5 mm).</p>     <p align="justify">Todas las mediciones hechas por los evaluadores (nutricionistas) fueron realizadas con los mismos equipos que fueron adquiridos ad hoc para la presente investigaci&oacute;n, tal como fue descrito previamente (32).</p>     <p><em>Poblaci&oacute;n de Estudio y Tama&ntilde;o de la Muestra</em></p>     <p align="justify">La poblaci&oacute;n de estudio estuvo conformada por los menores de 10 a&ntilde;os de edad (0 a 9,999 a&ntilde;os) que residen en el Municipio Pereira (<a href="#f1">Figura 1</a>), zonas urbanas y zonas rurales y que asist&iacute;an a instituciones educativas y hogares familiares donde se realiz&oacute; la evaluaci&oacute;n, como fue ya descrito (32). De dicha poblaci&oacute;n se obtuvo una muestra final de 1627 sujetos como se ha mencionado (32).</p>     ]]></body>
<body><![CDATA[<p><em>An&aacute;lisis Estad&iacute;stico</em></p>     <p align="justify">El procedimiento general fue descrito en el primer reporte de este estudio (32). Se tomaron como criterios para la clasificaci&oacute;n los valores de la OMS (World Health Organization Multicentre Growth Reference Study, WHO-MGRS) para menores de 10 a&ntilde;os (Peso/Edad, P/E; Talla/Edad, T/E; Peso/Talla, P/T; &Iacute;ndice de Masa Corporal/Edad, IMC/E).</p>     <p align="justify">Las variables cuantitativas fueron resumidas como medias con sus desviaciones est&aacute;ndar (&plusmn;DE). Las variables cualitativas fueron resumidas como proporciones (%) o prevalencias (%) con sus correspondientes intervalos de confianza (IC95%). La comparaci&oacute;n de las proporciones se realiz&oacute; con la prueba de chi cuadrado (c<sup>2</sup>), en tanto que las medias con la prueba t de Student, verificando los supuestos, en caso contrario con la U de Mann-Whitney. Todos los an&aacute;lisis estad&iacute;sticos se realizaron con un nivel de confianza de 95%, consider&aacute;ndose p significativa &lt;0,05.</p>     <p><font face="verdana" size="3"><b><a name="bookmark3"></a>Resultados</b></font></p>     <p><em>Talla para la Edad - Desnutrici&oacute;n Cr&oacute;nica</em></p>     <p align="justify">Al caracterizar el estado nutricional en los ni&ntilde;os y ni&ntilde;as de 0 a 9,999 a&ntilde;os de acuerdo al indicador Talla para la Edad (T/E desnutrici&oacute;n cr&oacute;nica) (talla estandarizada y clasificada por la referencia internacional, OMS, 2005), se encontr&oacute; que 59,0% de ellos se clasifican como Talla Normal para la edad, 8,3% talla alta para la edad y 32,7% baja para la edad (<a href="#c1">Cuadro 1</a>).</p>    <p align="center"><a name="c1"></a><a href="img/revistas/rmri/v19n1/v19n1a12-2.jpg" target="_blank">Cuadro 1</a></p>     <p align="justify">En el grupo de edad de 0 a 1,999 a&ntilde;os, se encontr&oacute; que 43,1% de ellos se clasifican como Normal para la edad, 21,5% alta para la edad y 35,4% baja para la edad. Al analizar dicho estado nutricional de acuerdo al sexo se encontr&oacute; (<a href="#c2">Cuadro 2</a>) que el d&eacute;ficit de talla fue mayor en el sexo masculino (36,0%) que en el sexo femenino (34,7%), en tanto que la talla alta fue m&aacute;s frecuente en el sexo femenino (22,3%) que en el sexo masculino (20,9), de forma tal que de los ni&ntilde;os el 43,2% fueron clasificados con una talla normal para su edad, en tanto que de las ni&ntilde;as el 43,0% de ellas fueron clasificadas con una talla normal para su edad (<a href="#c2">Cuadro 2</a>).</p>    <p align="center"><a name="c2"></a><a href="img/revistas/rmri/v19n1/v19n1a12-3.jpg" target="_blank">Cuadro 2</a></p>     <p align="justify">Para el estado nutricional en menores de 2 a&ntilde;os seg&uacute;n la procedencia geogr&aacute;fica (urbana o rural), se encontr&oacute; en la zona rural mayor frecuencia de d&eacute;ficit de talla para la edad (70,0%) en comparaci&oacute;n con la zona urbana (34,0%). Por otra parte se encontr&oacute; en la zona urbana mayor frecuencia de talla alta para la edad (22,0%) en comparaci&oacute;n con la zona rural (10,0%). Finalmente la talla normal para la edad fue m&aacute;s prevalente en la zona urbana (44,0%) que en la zona rural (20,0%).</p>     ]]></body>
<body><![CDATA[<p align="justify">Al analizar el estado nutricional por subgrupos de edad se observ&oacute; que en el subgrupo de edad donde se encontr&oacute; mayor d&eacute;ficit de talla para la edad fue el de 1,000 a 1,999 a&ntilde;os (39,7%), sin embargo dentro del d&eacute;ficit, donde se encontr&oacute; la mayor frecuencia de d&eacute;ficit severo fue en el grupo de 0,000 a 1,999 a&ntilde;os (35,0%). Para la talla alta para la edad, &eacute;sta se observ&oacute; en mayor frecuencia en el grupo de 0,000 a 0,999 a&ntilde;os (25,6%). El subgrupo de edad que present&oacute; mayor proporci&oacute;n de casos con talla normal para la edad fue el de 0,000 a 0,999 a&ntilde;os (43,4%).</p>     <p align="justify">La situaci&oacute;n nutricional en los ni&ntilde;os y ni&ntilde;as de 2 a 9,999 a&ntilde;os de acuerdo al indicador talla para la edad (T/E desnutrici&oacute;n cr&oacute;nica) (talla estandarizada y clasificada por la referencia internacional, OMs, 2005), se encontr&oacute; que 62,34% de ellos se clasifican como Normal para la edad, 5,57% alta para la edad y 32,09% baja para la edad. Al analizar dicho estado nutricional de acuerdo al sexo se encontr&oacute; (<a href="#c3">Cuadro 3</a>) que el d&eacute;ficit de talla fue mayor en el sexo masculino (34,4%) que en el sexo femenino (29,6%), de igual forma la talla alta fue m&aacute;s frecuente en el sexo masculino (6,0%) que en el sexo femenino (5,1%), de forma tal que de los ni&ntilde;os el 59,7% fueron clasificados con una talla normal para su edad, en tanto que de las ni&ntilde;as el 65,3% de ellas fueron clasificadas con una talla normal para su edad (<a href="#c2">Cuadro 3</a>).</p></font>      <p align="center"><font size="2" face="verdana"><a name="c3"><img src="img/revistas/rmri/v19n1a12-4.jpg"/></a></font></p> <font face="verdana" size="2">     <p align="justify">Al analizar dicho estado nutricional seg&uacute;n la procedencia geogr&aacute;fica (urbana o rural), se encontr&oacute; en la zona rural mayor frecuencia de d&eacute;ficit de talla para la edad (43,5%) en comparaci&oacute;n con la zona urbana (30,3%). Por otra parte se encontr&oacute; en la zona urbana mayor frecuencia de talla alta para la edad (6,3%) en comparaci&oacute;n con la zona rural (0,6%). Finalmente la talla normal para la edad fue m&aacute;s prevalente en la zona urbana (63,3%) que en la zona rural (56,0%).</p>     <p align="justify">Al analizar el estado nutricional por subgrupos de edad se observ&oacute; que en el grupo de edad donde se encontr&oacute; mayor d&eacute;ficit de talla para la edad fue el de 2,000 a 2,999 a&ntilde;os (39,3%), sin embargo dentro del d&eacute;ficit, donde se encontr&oacute; la mayor frecuencia de d&eacute;ficit severo fue en el grupo de 9,000 a 9,999 a&ntilde;os (11,4%) (<a href="#c4">Cuadro 4</a>). Para la talla alta para la edad, &eacute;sta se observ&oacute; en mayor frecuencia en el grupo de 5,000 a 5,999 a&ntilde;os (7,4%) (<a href="#c4">Cuadro 4</a>). El grupo de edad que present&oacute; mayor proporci&oacute;n de casos con talla normal para la edad fue el de 8,000 a 8,999 a&ntilde;os (68,2%) (<a href="#c4">Cuadro 4</a>).</p>     <p align="center"><a name="c4"></a><a href="img/revistas/rmri/v19n1/v19n1a12-5.jpg" target="_blank">Cuadro 4</a></p>     <p align="justify">Sin embargo, como se aprecia en el <a href="#c4">Cuadro 4</a>, el grado de variaci&oacute;n para cada indicador no fue mayor de 25,5% entre el m&iacute;nimo y m&aacute;ximo de cada grupo de edad, siendo menor de 11% en las principales categor&iacute;as (d&eacute;ficit global, 10,8%, normal, 10,7% y exceso, 6,8%).</p>     <p><font face="verdana" size="3"><b><a name="bookmark4"></a>Discusi&oacute;n</b></font></p>     <p align="justify">La evaluaci&oacute;n a mediano y largo plazo del estado nutricional representa una importante valoraci&oacute;n de indicadores que reflejan no solamente la condici&oacute;n de riesgo hacia anormalidades nutricionales como la malnutrici&oacute;n por d&eacute;ficit o exceso sostenidas en el tiempo (33,34), sino tambi&eacute;n diversas consecuencias que se ven reflejadas en dichos indicadores a partir de variaciones en aspectos como ingesta de alimentos y nutrientes, insuficiente o excesiva, la pr&aacute;ctica de la actividad f&iacute;sica y distintas enfermedades que pueden influenciarle (35), adem&aacute;s permite evaluar el rendimiento, la salud e incluso la supervivencia (36-39). En forma indirecta puede emplearse como un marcador de condiciones de vida favorables o desfavorables que se asocian en el tiempo con las condiciones de desnutrici&oacute;n aguda y cr&oacute;nica (13,14,40-43).</p>     <p align="justify">Los resultados observados en el presente estudio son considerablemente consistentes con lo esperado tanto desde el punto de vista de la tendencia del comportamiento de los indicadores, como de la situaci&oacute;n actual de los mismos. Hay una considerable reducci&oacute;n as&iacute; como un comportamiento consistente en los diferentes grupos de edad y sexo, con diferencias moderadas entre ellos. El mejoramiento en la proporci&oacute;n de ni&ntilde;os y ni&ntilde;as con desnutrici&oacute;n cr&oacute;nica es particularmente importante cuando se le compara con el estudio realizado en el a&ntilde;o 2001 (24-31,33-44). Sin embargo debe acotarse que sigue siendo un importante problema de salud p&uacute;blica en el Municipio Pereira, donde de acuerdo a esta estimaci&oacute;n, que procede de un estudio con una muestra probabil&iacute;stica se permite inferir el comportamiento en la poblaci&oacute;n infantil, casi 1 de cada 3 menores de 10 a&ntilde;os present&oacute; desnutrici&oacute;n cr&oacute;nica.</p>     ]]></body>
<body><![CDATA[<p align="justify">Por ello, a pesar de que se observa mejor&iacute;a en el tiempo hay que seguir perfeccionando las intervenciones. La proporci&oacute;n de ni&ntilde;os menores de 10 a&ntilde;os de edad con desnutrici&oacute;n cr&oacute;nica viene disminuyendo desde el a&ntilde;o 2001, pasando por el a&ntilde;o 2008, hasta los resultados del actual estudio (44-49). Las intervenciones que se han hecho debe intensificarse para mejorar aun m&aacute;s las condiciones nutricionales y de salud de la poblaci&oacute;n menor de 10 a&ntilde;os en Pereira durante los pr&oacute;ximos a&ntilde;os y que la desnutrici&oacute;n cr&oacute;nica siga disminuyendo. Para evidenciarlo, es decir para evaluar el impacto de dichas intervenciones especialmente en desnutrici&oacute;n cr&oacute;nica, deben mantenerse estudios peri&oacute;dicos de este tipo a mediano plazo para ver sus efectos y la tendencia en su reducci&oacute;n.</p>     <p align="justify">La desnutrici&oacute;n global encontrada fue mayor en el &aacute;rea rural (70,0%) que en la urbana (34,0%) en el grupo de 0 a 1,999 a&ntilde;os y similarmente, pero con una diferencia menos marcada en el grupo de 2 y m&aacute;s a&ntilde;os (43,5% vs 30,3%, respectivamente). De acuerdo a ello, se podr&iacute;a pensar que las intervenciones que han permitido mejorar las condiciones de mediano a largo plazo en desnutrici&oacute;n cr&oacute;nica en el medio rural del municipio han sido de menor impacto, especialmente en el grupo de menores de 2 a&ntilde;os, durante los &uacute;ltimos 2 a&ntilde;os previos a la realizaci&oacute;n del presente estudio.</p>     <p align="justify">Los presentes resultados deben orientar a un mejor dise&ntilde;o de pol&iacute;ticas p&uacute;blicas con efectos previsibles a mediano y largo plazo, que impulsen el avance del desarrollo social (46-56), el mejoramiento de las condiciones de vida, as&iacute; como incidir en forma directa en la disponibilidad de alimentos y la educaci&oacute;n nutricional en el Municipio con el fin de mejorar en forma integral las condiciones nutricionales que tiene la poblaci&oacute;n infantil en Pereira.
