En ese sentido, se hace necesario determinar cuáles son los instrumentos de medición más empleados para la evaluación de la condición física, capacidad funcional y niveles de actividad física de esta población. A través de una revisión sistemática de publicaciones científicas, se logró establecer que, para la medición de la condición física, la batería más utilizada es la SPPB (Short Physical Performance Battery); respecto al nivel de actividad física, la acelerometría es uno de los métodos directos más empleados, junto a los cuestionarios de autoreporte y para identificar la capacidad funcional, el índice de katz es el reportado con mayor frecuencia.]]></p></abstract>

En Colombia,  seg&uacute;n  la Encuesta  Nacional  de  Demograf&iacute;a y  Salud,  adelantada por Ojeda <i>et al</i>. (2011),  indica que  la proporci&oacute;n  de poblaci&oacute;n  de 65 a&ntilde;os y m&aacute;s aument&oacute; de 5 al  7%, entre 1990 y 2010. El envejecimiento presenta  niveles de dependencia y de discapacidad elevados,  que oscilan entre el 20 y 53,5% (Yoshida <i>et al. </i>2012).</p>     <p>   El proceso  de envejecimiento,  generalmente, viene seguido de  una  p&eacute;rdida  global de  la salud,  debido  a un  incremento del riesgo de padecer enfermedades de car&aacute;cter  cr&oacute;nico degenerativo:  cardiovasculares, diabetes,  c&aacute;ncer,  enferme- dades  respiratorias,  trastornos relacionados con  el sistema  m&uacute;sculo  esquel&eacute;tico, como  la sarcopenia y la osteoporosis. Lo anterior, se ve reflejado en una p&eacute;rdida de la movilidad y un aumento en la dependencia, generando un mayor riesgo de ca&iacute;das, de fracturas y de discapacidad.</p>     ]]></body>
<body><![CDATA[<p>   La  condici&oacute;n   f&iacute;sica  es,  entonces,  un  factor  clave  en  la preservaci&oacute;n de la movilidad y la independencia, que permite a  los  adultos  mayores   realizar  las  actividades  cotidianas, como  subir escaleras, levantar y transportar objetos, entrar y salir de las sillas o de un veh&iacute;culo y caminar  alrededor  de las tiendas,  edificios o parques  (Paterson  &amp; Warburton, 2010).</p>     <p>   Un mayor  nivel de  actividad  f&iacute;sica est&aacute;  relacionado  con  la participaci&oacute;n   en   las  actividades   f&iacute;sicas  moderadas,  que pueden  retrasar  el declive funcional  y reducir  el riesgo  de padecer enfermedades, por  tanto,  mantener un  estilo  de vida  activo garantizar&aacute;  una  mayor  independencia funcional (Landinez <i>et al. </i>2012).</p>     <p>   La capacidad funcional tambi&eacute;n se puede ver afectada por las enfermedades cr&oacute;nicas o a las ca&iacute;das, que son los accidentes m&aacute;s  comunes en  esta  edad  y que  est&aacute;n  asociados a  la p&eacute;rdida de control corporal,  causado por la disminuci&oacute;n  de la fuerza muscular,  que es uno de los efectos m&aacute;s conocidos del envejecimiento  (Curcio <i>et al. </i>2009).</p>     <p>   Es as&iacute;, que  el prop&oacute;sito  de este  estudio  fue determinar los instrumentos con mayor grado de confiabilidad y de validez, para la evaluaci&oacute;n de la condici&oacute;n f&iacute;sica, capacidad funcional y niveles de actividad f&iacute;sica, en el adulto mayor.</p>     <p><b>MATERIALES Y M&Eacute;TODOS</b></p>     <p><b>Estrategia    de   b&uacute;squeda</b>. La   revisi&oacute;n   sistem&aacute;tica,   se centr&oacute;   en  recopilar  la  evidencia  cient&iacute;fica  acerca   de  los instrumentos m&aacute;s  utilizados para evaluar la condici&oacute;n  f&iacute;sica y los niveles de actividad f&iacute;sica en el adulto  mayor,  para  lo cual, se establecieron, como  estrategia  inicial de b&uacute;squeda, los descriptores, seg&uacute;n  los t&eacute;rminos  DeCS (Descriptores  en ciencias  de  la salud)  y Mesh (Pubmed),  obteniendo como  resultado,    las   siguientes   palabras   clave:   adulto   mayor, condici&oacute;n   f&iacute;sica,  actividad  f&iacute;sica  y rendimiento   f&iacute;sico.  Se combinaron las  palabras  clave  aplicando  los  descriptores booleanos AND/OR.</h3>     <p>   Se utilizaron varias estrategias de indagaci&oacute;n, se realizaron b&uacute;squedas en revistas especializadas,  en el registro especia- lizado de  ensayos  controlados del grupo  Cochrane, en  las bases  de datos  electr&oacute;nicas  de Medline, PubMed, Proquest, Hinari, SportDiscus, EMBASE, CINAHL, PsychINFO, PEDro y Ovid.</p>     <p>   La  estrategia   de  b&uacute;squeda en  las  bases   electr&oacute;nicas,  se realiz&oacute; acorde  a la metodolog&iacute;a de b&uacute;squeda sensible  para ensayos controlados y aleatorios.</p>     <p>   Las listas obtenidas fueron combinadas usando el software bibliogr&aacute;fico EndNote  y los duplicados  fueron  suprimidos.  Se  estableci&oacute;,  como  espectro   de  b&uacute;squeda, a&ntilde;o  2005  al  2015,  sin restricci&oacute;n de idioma.</p>     <p><b>Criterios  de  inclusi&oacute;n   y  proceso   de  selecci&oacute;n.</b> Para  la selecci&oacute;n   primaria  de  los  art&iacute;culos,   se  establecieron  los siguientes  criterios  de  inclusi&oacute;n:  Estudios  que  describieran  los  instrumentos  de  evaluaci&oacute;n   para   la  medici&oacute;n   de  la condici&oacute;n  f&iacute;sica funcional  y los niveles de  actividad  f&iacute;sica; art&iacute;culos  publicados  en revistas, que  se encuentren dentro del  Q1,  que  establece   SCIMAGO (International  Scientific Journal   &amp;  Country  Ranking)  y aprobaci&oacute;n  por  pares  de evaluaci&oacute;n independientes.</p>     ]]></body>
<body><![CDATA[<p>Los art&iacute;culos,  se revisaron  y se evaluaron,  a trav&eacute;s de  una matriz de an&aacute;lisis, para ponderar  su calidad cient&iacute;fica, seg&uacute;n  el  protocolo  de  Cochrane  Review, quedando incluidos  los estudios que tuvieron una ponderaci&oacute;n superior a 7,0 puntos.</p>     <p><b>Extracci&oacute;n  de  datos.</b> La  informaci&oacute;n  que  se  extrajo  de cada  uno  de  los  estudios  estaba   dividida en  tres  t&oacute;picos principales:</p>     <p><u>Datos   generales   del  art&iacute;culo:</u>  Base   de   datos   a  la  cual pertenece, t&iacute;tulo, autores,  revista, a&ntilde;o  de publicaci&oacute;n,  pa&iacute;s del estudio y palabras  clave.</p>     <p><u>Poblaci&oacute;n   de   referencia:</u>   Rangos   de   edad,   m&eacute;todo    de selecci&oacute;n  de  la  muestra, tama&ntilde;o de  la  muestra, tipo  de estudio y criterios de inclusi&oacute;n.</p>     <p><u>Variables    e    instrumentos   utilizados:</u>   Condici&oacute;n    f&iacute;sica, capacidad funcional, nivel de actividad f&iacute;sica.</p>     <p><b>RESULTADOS Y DISCUSI&Oacute;N</b></p>     <p>Respecto    a   los   instrumentos   m&aacute;s    utilizados   para    la cuantificaci&oacute;n  de la condici&oacute;n  f&iacute;sica, capacidad funcional y niveles de actividad f&iacute;sica del adulto  mayor,  se identificaron un  total  de  1.916  documentos,  posiblemente, pertinentes seg&uacute;n  el t&iacute;tulo y el resumen; de &eacute;stos,  se excluyeron 1.320,  recuper&aacute;ndose 548  para  evaluaci&oacute;n  en  profundidad   y se seleccionaron 56, como  adecuados para  su inclusi&oacute;n en el estudio.  El flujo de los estudios  seleccionados, se puede  ver en la <a href="#f1">figura 1</a>.</p>      <p><a name="f1"></a></p>    <p align="center"><img src="img/revistas/rudca/v20n2/v20n2a04f1.jpg"></p>     <p>   En   el   proceso    de   extracci&oacute;n    de   datos,    30   estudios  corresponden a la medici&oacute;n  del nivel de actividad f&iacute;sica, 20 sobre la condici&oacute;n  f&iacute;sica y 1 valor&oacute; la capacidad funcional del adulto  mayor;  en  este  mismo  proceso, cinco  estudios relacionaron   dos  variables:  uno  nivel de  actividad  f&iacute;sica y capacidad funcional y los cuatro restantes, condici&oacute;n f&iacute;sica y nivel de actividad f&iacute;sica.</p>     ]]></body>
<body><![CDATA[<p>   El <a href="#c1">cuadro 1</a> evidencia los estudios seleccionados y evaluados  en  cuanto  a las variables,  los m&eacute;todos o instrumentos, el tipo de estudio  y la poblaci&oacute;n,  con referencia a la edad.  En la caracterizaci&oacute;n, se evidencian estudios  de tipo descriptivo,  76,8%; experimental, 16,1% y, de cohorte,  7,1%. De acuerdo  con  las  variables  capacidad  funcional,  condici&oacute;n   f&iacute;sica  y nivel  de  actividad  f&iacute;sica, se  establecieron los instrumentos de  medici&oacute;n,  empleados en los 56  art&iacute;culos  analizados.  El cuadro  2  recopila  la frecuencia  en  que  son  utilizados  los instrumentos o las pruebas, para la medici&oacute;n de las variables estudiadas.</p>         <p><a name="c1"></a></p>    <p align="center"><img src="img/revistas/rudca/v20n2/v20n2a04c1.jpg"></p>          <p>   Los  resultados   de  este  estudio   indican  la  short  physical performance battery  (SPPB), como  la bater&iacute;a  de  test  m&aacute;s utilizada (48%), para  la valoraci&oacute;n de la condici&oacute;n  f&iacute;sica en adultos  mayores.  Esta bater&iacute;a consta  de tres test: equilibrio, velocidad de la marcha  y levantarse  y sentarse en una  silla cinco veces. Esta bater&iacute;a fue creada por Guralnik <i>et al. </i>(1994) y validada con cinco mil adultos  mayores  de 71 a&ntilde;os;  cada prueba  y el puntaje  total presentaron una fuerte correlaci&oacute;n con  un  autoreporte de  discapacidad. Tanto  los &iacute;ndices  de incapacidad autoinformados como  los  puntajes   de  SPPB fueron predictores de mortalidad  a corto plazo y de admisi&oacute;n en un hogar geri&aacute;trico.</p>     <p>   Desde  el  desarrollo  de  la  SPPB,  tanto  su  fiabilidad y se sensibilidad, han sido confirmados, para valorar la condici&oacute;n  f&iacute;sica en adultos  mayores.  Varios  estudios  epidemiol&oacute;gicos internacionales   y  nacionales    demostraron  su   validez  y confiabilidad (Soler <i>et al</i>. 2012).  Tambien  posee  valores de referencia para la poblaci&oacute;n  estadounidense y espa&ntilde;ola.</p>     <p>   El 52% de los estudios  reflejan el uso  de pruebas  aisladas, que  combinan  ciertas  capacidades  f&iacute;sicas,  como   fuerza, resistencia   aer&oacute;bica,   resistencia   muscular,    flexibilidad  y habilidades   motrices,   equilibrio,  coordinaci&oacute;n,  velocidad y  agilidad,  que  son  seleccionadas por  el  investigador  en funci&oacute;n  de  las  necesidades del  estudio;  sin  embargo, es importante resaltar  que  la gran  mayor&iacute;a  de  estas  pruebas  corresponden a la bater&iacute;a, senior fitness test (SFT) dise&ntilde;ada por Rikli &amp; Jones, en 2001,  para evaluar la condici&oacute;n  f&iacute;sica en  el  adulto  mayor.  Los  par&aacute;metros  de  condici&oacute;n   f&iacute;sica que  incluye  esta  bater&iacute;a  son:  fuerza  muscular,   miembros superiores   e  inferiores,  resistencia   aer&oacute;bica,   flexibilidad y agilidad y cuenta  con valores de referencia por sexo y edad para  la poblaci&oacute;n  norteamericana, establecidos a trav&eacute;s de un estudio realizado a 7.000  adultos  mayores.  Su validez y su confiabilidad han sido ampliamente probadas, no solo en poblaci&oacute;n norteamericana sino tambi&eacute;n  en adultos mayores  en Colombia (Gomez <i>et al. </i>2013).</p>     <p>   Por su parte, Rikli &amp; Jones (2001) plantean  que existe poca informaci&oacute;n  disponible  que determine  el nivel de condici&oacute;n f&iacute;sica necesario para que el adulto mayor pueda mantener su independencia. Los pocos  estudios  publicados  al respecto  han  involucrado  valoraciones   en  laboratorio,   tales  como  consumo  m&aacute;ximo   de   ox&iacute;geno,   consumo  pico   m&aacute;ximo de  ox&iacute;geno  o fuerza muscular,  medidas  que  proporcionan informaci&oacute;n  importante para  la comunidad cient&iacute;fica, pero que  no son  bien entendidas o f&aacute;cilmente  interpretadas por la mayor&iacute;a  de los profesionales  de la salud,  por los l&iacute;deres de  programas  o  por  los  propios   adultos   mayores.   Uno de  los argumentos que  podr&iacute;an  explicar el amplio  uso  de esta bater&iacute;a  es que los est&aacute;ndares que se eval&uacute;an con esta bater&iacute;a:  fuerza,  resistencia,   agilidad  y  balance   din&aacute;mico,  variables fisiol&oacute;gicas claves, est&aacute;n &iacute;ntimamente ligadas con el mantenimiento de la independencia f&iacute;sica en la edad adulta.</p>     <p>   La diferencia entre aplicar la SFT o SPPB y pruebas  aisladas es  que  estas   definieron  est&aacute;ndares  de  relaci&oacute;n  entre   la condici&oacute;n   f&iacute;sica  y la  independencia  f&iacute;sica  o  predictor  de riesgo  de  discapacidad en  el adulto  mayor,  que  permiten ubicar al adulto mayor, de acuerdo  a la edad y al g&eacute;nero,  en rangos  de eficiencia.</p>     <p>  En  t&eacute;rminos   generales, se  podr&iacute;a  decir  que  los  estudios  que no aplican estas  bater&iacute;as  de forma completa, s&iacute; utilizan algunas de sus pruebas. Por otra parte, es importante resaltar que no todas las pruebas que contienen estas bater&iacute;as fueron dise&ntilde;adas por los autores;  por ejemplo,  las pruebas  Timed Up-and-Go test utilizada en la SPPB y la 8 Foot up and go de  la SFT son derivadas  de la Get-Up-and-Go  test, creada  por  Mathias <i>et al. </i>(1986).</p>     <p>   En   cuanto    a   los   est&aacute;ndares   el   SPPB,   por   ejemplo, utiliza  un   sistema   ordinal   de   clasificaci&oacute;n   en   lugar   de puntuaci&oacute;n a escala  continua,  que limita su capacidad para detectar   cambios   graduales en  el  rendimiento   individual. Adicionalmente, se ha identificado que la prueba de equilibrio es demasiado f&aacute;cil y la 5-times  chair stand  muy dif&iacute;cil, para ser discriminadores efectivos del rendimiento  (Rikli &amp; Jones,  2001). La SFT emplea  siete pruebas, que abarcan variables fisiol&oacute;gicas fundamentales de  la condici&oacute;n  f&iacute;sica, mientras  que la SPPB, tres.</p>     ]]></body>
<body><![CDATA[<p>   La valoraci&oacute;n de los niveles de actividad f&iacute;sica que realiza una persona  durante  el d&iacute;a ha  sido  abordada desde  diferentes perspectivas   t&eacute;cnicas   y  metodol&oacute;gicas.  En  los  estudios  revisados,  la acelerometria fue uno de los m&eacute;todos directos m&aacute;s utilizados; 47,4% de los trabajos emplearon este m&eacute;todo, que tiene una gran fiabilidad para valorar el nivel de actividad f&iacute;sica en personas de edad  avanzada,  como  lo demostr&oacute; el estudio de Bueno <i>et al. </i>(2016), quien identific&oacute; una estrecha correlaci&oacute;n  (r=0.83) con  el Gold est&aacute;ndar, que  es  el agua doblemente; sin embargo, esta gran correlaci&oacute;n se pude ver afectada  por el tipo de aceler&oacute;metro que se utilice.</p>     <p>   En  cuanto   a  los  m&eacute;todos  indirectos,   se  encuentran  los cuestionarios,  de  los  cuales   se  han   reportado  16  para la  poblaci&oacute;n  adulta.  A  trav&eacute;s  de  ellos,  se  puede  medir  el tipo de  actividad en los dominios  de  tiempo  de  ocio o de trabajo, frecuencia media de sesiones  por unidad de tiempo, duraci&oacute;n  por minutos  e intensidad,  gasto  metab&oacute;lico de la actividad; son utilizados en grandes  poblaciones, debido a su bajo costo y f&aacute;cil aplicabilidad. Dentro de estos cuestionarios el m&aacute;s empleado fue el IPAQ, 18,6%.</p>     <p>   Para    identificar   la   confiabilidad    y   la   validez   de    los cuestionarios, se  desarroll&oacute;  un  estudio  en  el  2009,  cuyo objetivo  fue  evaluar  las  propiedades psicom&eacute;tricas de  los cuestionarios que  miden  la actividad f&iacute;sica, a partir de tres criterios: la consistencia interna, la estabilidad  del test-retest y la fiabilidad interobservadores e identificaron diversidad en la  valoraci&oacute;n  de  los tipos  de  actividad  (Guirao-Goris <i>et al.</i> 2009).  En cuanto  a las propiedades psicom&eacute;tricas, para  el IPAQ largo, se ha demostrado una fiabilidad alrededor de 0,8 (r = 0,81; IC 95 %: 0,79-0,82)  y para la versi&oacute;n corta, de 0,65 (r = 0,76;  IC 95 %: 0,73-0,77). Los coeficientes  de validez observados  entre   las  formas   IPAQ  sugieren   que   ambas versiones,  larga y corta,  tienen una concordancia razonable (r =  0,67;  IC 95  %: 0,64-0,70)  (Toloza &amp; G&oacute;mez-Conesa,  2007); sin embargo, presenta  una limitaci&oacute;n para el trabajo con  adulto  mayor,  ya que  fue dise&ntilde;ado  para  la poblaci&oacute;n  de  18  a  65  a&ntilde;os  y acude  a  preguntas de  las  actividades realizadas  en  los &uacute;ltimos  7 d&iacute;as,  que  pueden  aumentar el sesgo  de memoria  por parte de esta poblaci&oacute;n,  es decir, que se puede  maximizar el error de medici&oacute;n.</p>     <p>   Por ello, en Suecia, Hurtig-Wennl&ouml;f <i>et al. </i>(2010) validaron el cuestionario  en poblaci&oacute;n  mayor de 65 a&ntilde;os  y demostraron que el IPAQ-E tiene una adecuada validez y fiabilidad, pero la versi&oacute;n solo  est&aacute;  en  sueco  e ingl&eacute;s.  Finalmente, en  un reciente  estudio   realizado  por  Rubio <i>et  al</i>.  (2017),  cuyo prop&oacute;sito   era  validar  en  t&eacute;rminos   psicom&eacute;tricos  el  IPAQ en  poblaci&oacute;n  mayor  de  65  a&ntilde;os,  se  observ&oacute;  que,  en  una poblaci&oacute;n  de 139 adultos,  con edades  entre 65-93  a&ntilde;os,  el cuestionario  present&oacute; buenas caracter&iacute;sticas de  validez, de fiabilidad y de consistencia.</p>     <p>   En   lo  relacionado   con   la  validez  de   los  cuestionarios para  determinar con  precisi&oacute;n  el nivel de  actividad  f&iacute;sica desarrollada, el m&eacute;todo  m&aacute;s  apropiado  es la acelerometr&iacute;a; para  el  caso   del  IPAQs,  present&oacute;  buenos   resultados   de fiabilidad y de validez, con  diversos tipos de aceler&oacute;metros Actigraph, Actireg, Acti-trainer, Actical y Caltrac (Helmerhorst <i>et al. </i>2012).</p>     <p>   En  la  presente   revisi&oacute;n,  se   identifican   diversas   escalas  para  medir la capacidad funcional en adultos  mayores;  sin embargo, el &iacute;ndice de katz fue el instrumento m&aacute;s  utilizado, creado   en  1958,   por  un  equipo   multidisciplinar  dirigido por S. Katz y formado  por enfermeras, m&eacute;dicos,  asistentes sociales, terapeutas ocupacionales y fisioterapeutas, del The Benjam&iacute;n Rose Hospital (un hospital geri&aacute;trico y de enfermos  cr&oacute;nicos  de Cleveland, Ohio), para delimitar la dependencia en fracturas de cadera.  Este instrumento permite determinar la independencia del adulto  mayor  en cuanto  el desarrollo de  las  actividades  de  la vida diaria: ba&ntilde;arse, vestirse,  usar el  ba&ntilde;o,   movilidad,   continencia   y  alimentaci&oacute;n.  Como ventaja,  el  &iacute;ndice  de  katz logra,  a trav&eacute;s  de  su  valoraci&oacute;n, identificar  en  qu&eacute;  actividades  espec&iacute;ficas  el adulto  mayor presenta   independencia  y  dependencia  (Trig&aacute;s-Ferr&iacute;n <i>et al. </i>2011).  La escala  en  menci&oacute;n  puede  ser  empleada en estudios  epidemiol&oacute;gicos, al ser un  m&eacute;todo  de  bajo  costo y gran  reproductibilidad.  Su  fiabilidad y su  validez ha  sido valorada  en  m&uacute;ltiples  estudios,   presentando  coeficientes  de  correlaci&oacute;n   altos   &gt;0.70  y  test-retest &gt;0.90  (Trig&aacute;s- Ferr&iacute;n <i>et al. </i>2011). El segundo instrumento m&aacute;s  empleado fue  la Escala  de  Barthel,  cuya  valoraci&oacute;n  arroja  el grado de dependencia  general,  clasificada  en: dependencia total, grave, moderada o leve.</p>     <p>   No se  pude  afirmar  que  exista  una  escala  dominante, se puede afirmar que las principales escalas  se caracterizan  por su  brevedad  y sencillez a la hora  de ser utilizadas, adem&aacute;s presentan una alta fiabilidad y reproducibilidad.  La mayor&iacute;a de estos autores  hacen  referencia  a estas  3 escalas:  &Iacute;ndice de  Katz,  escala  de  Barthel  y Cuestionario   AVD  (Montero Mendoza &amp; Pelegr&iacute;n Molina, 2010).</p>     <p><b>CONCLUSIONES</b></p>     <p>Existen diversos instrumentos para evaluar la condici&oacute;n f&iacute;sica, la capacidad funcional y los niveles de actividad f&iacute;sica para la poblaci&oacute;n de edad avanzada, que demuestran tener un apoyo emp&iacute;rico  y racional,  suficientemente fuerte,  para  justificar su  uso,  tanto  por  investigadores   como  por  profesionales,  con  estimaciones  razonables.   Todas  ellas  fueron  creadas  antes  del 2000  y validadas  a trav&eacute;s  del tiempo  y hasta  la fecha, por estudios  experimentales, epidemiol&oacute;gicos y meta an&aacute;lisis.  El  uso  en  diversos  casos  depende de  si ha  sido traducida  y validada al Espa&ntilde;ol  de Colombia.  En el caso  de los  aceler&oacute;metros, su  utilizaci&oacute;n es  un  poco  m&aacute;s  limitada por los costos  y su nivel de  fiabilidad depende del tipo de aceler&oacute;metros que se utilice.</p>     <p><u>Conflicto  de   intereses:</u>   El  manuscrito  fue  preparado  y revisado  con  la participaci&oacute;n  de todos  los autores,  quienes declaramos que no existe conflicto de intereses  que ponga  en riesgo la validez de los resultados  presentados.</p>     ]]></body>
