Métodos Participaram do estudo 400 crianças e adolescentes, sendo que 300 sujeitos (75 %) tiveram seus resultados utilizados para o desenvolvimento das equações e 100 sujeitos (25 %) para a validação das mesmas. Aplicou-se o método paramétrico de regressão Stepwise Correlation, sendo as variáveis independentes constituídas por medidas antropométricas e tomando-se como base decisória o EPE (Erro Padrão de Estimativa) combinado ao índice de regressão (R) e de determinação (R2). Resultados O modelo preditor mais significativo, correspondeu a:Idade Óssea = 0,062*Estatura(cm) + 0,426*Id.Cronol. + 0,041*MassaCorp.(Kg) - 0,390* Diâmetro Fêmur. No segundo momento, tomou-se como variável dependente a Maturação Sexual segundo os critérios de Tanner (IMT), e nesse contexto adicionou-se como variável independente a Idade Óssea (IO), calculando-se o segundo Modelo preditor nas mesmas condições de aceitabilidade anteriormente mencionadas: Índice Maturação Tuner (IMT) = 0,5423*Idade Óssea estimada - 0,0374*Estatura (cm) + 0,0388*MassaCorp (Kg). Conclusões A obtenção de resultados por métodos não invasivos é um avanço no diagnóstico médico para prevenções de doenças, como também equações com esses objetivos, um facilitador na orientação e seleção esportiva.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Objective This study had objective develop and validate prediction equations bone age and an index of sexual maturity for children 9 to 14 years old male from the city of Rio de Janeiro. Methods The study included 400 children and adolescents and 300 subjects (75 %) had their results used to develop the equations and 100 subjects (25 %) for the validation of the same. Applied the method parametric regression Stepwise Correlation, being the independent variables consist of anthropometric measurements and taking decisions based on the SEE (Standard Error of Estimate) combined with regression rate (R) and determination (R2). Results The most significant predictor model, accounted for: Age Bone= 0,062 * Height(cm) + 0,426 * Id.Cronol. + 0,041 * MassaCorp.(Kg) - 0,390 * Diameter Femur. Secondly, it was taken as the dependent variable sexual maturation according to the criteria of Tanner (IMT), and in this context was added as an independent variable the bone age (IO), calculating the second predictor model under the same conditions of acceptability previously mentioned: Maturation Index Tuner (IMT) = 0,5423 * age estimated bone - 0,0374 * Height(cm) + 0,0388 * MassaCorp.(Kg). Conclusions The results obtained by non-invasive methods are a breakthrough in medical diagnostics for disease prevention, as well as equations with these objectives, a facilitator in guiding the selection and sports.]]></p></abstract>

Métodos Participaron 400 niños y adolescentes. Los resultados de 300 sujetos (75 %) se utilizaron para desarrollar las ecuaciones y los de 100 sujetos (25 %) para validarlos. Se utilizó método paramétrico de correlación por medio de regresión por pasos. Las variables independientes fueron las medidas antropométricas y las decisiones se tomaron con base en el error estándar estimado, combinado con el índice de regresión (R) y la determinación (R2). Resultados El modelo predictor más significativo es: Edad ósea = 0,062 * Altura (cm) + 0.426 * Id.Cronol. + 0,041 * MassaCorp (Kg.) - 0.390 * Diámetro fémur. La segunda vez, se convirtió en la maduración sexual como variable dependiente en función de criterios de Tanner (IMT), y en ese contexto se agregó como una variable independiente el Medio Bone (IO). Un segundo modelo predictor, obtenido en las mismas condiciones de aceptabilidad mencionadas anteriormente, fue: Índice maduración Tuner (IMT) = 0.5423 * Edad ósea estimada - 0,0374 * Altura (cm) + 0.0388 * MassaCorp (Kg). Conclusión Los resultados obtenidos mediante métodos no invasivos son un avance en el diagnóstico médico para la prevención de enfermedades; las ecuaciones obtenidas con estos objetivos, son un facilitador en la orientación y selección deportiva.]]></p></abstract>

Embora a antropometria durante o crescimento apresentem altera&ccedil;&otilde;es na estatura, massa corporal, di&acirc;metros &oacute;sseos, per&iacute;metros, circunfer&ecirc;ncias corporais, comprimentos &oacute;sseos e proporcionalidade corporal, s&atilde;o indicadores fundamentais em estudos de sa&uacute;de com crian&ccedil;as e adolescentes (1,2). </p>      <p>Oliveira et al (3) relatam que no Brasil foi observado uma maior preval&ecirc;ncia de sobrepeso em adolescentes com renda mais alta. No combate a obesidade, a atividade f&iacute;sica programada &eacute; uma vari&aacute;vel indispens&aacute;vel, sendo poss&iacute;vel observar no estudo de Feferbaum et al (4). </p>      ]]></body>
<body><![CDATA[<p>O IMC (&Iacute;ndice de Massa Corporal) (5,6), torna-se vari&aacute;vel fundamental na avalia&ccedil;&atilde;o do crescimento, por&eacute;m a mesma tem sido analisada em rela&ccedil;&atilde;o &agrave; idade cronol&oacute;gica, gerando uma perda de seu poder explicativo, j&aacute; que nesta etapa da vida a idade biol&oacute;gica torna-se um par&acirc;metro mais fidedigno (7,8). </p>      <p>Para se avaliar a idade biol&oacute;gica h&aacute; v&aacute;rios m&eacute;todos, sendo a radiografia de m&atilde;o e punho esquerdo, o qual observa a presen&ccedil;a ou aus&ecirc;ncia de centros de ossifica&ccedil;&atilde;o, o m&eacute;todo mais aceito (9). Esta metodologia utiliza a radiografia da crian&ccedil;a e/ou adolescente avaliado com outras radiografias-padr&atilde;o de diversas faixas et&aacute;rias e sexo, para que a idade &oacute;ssea correspondente possa ser encontrada(10,11). </p>      <p>Logo, o acompanhamento da sa&uacute;de como um todo, e em especial a sa&uacute;de &oacute;ssea nos jovens tornou-se um caso de sa&uacute;de p&uacute;blica, pois atrav&eacute;s de estudos populacionais o n&atilde;o ganho de massa &oacute;ssea. Segundo Silva et al (12), a quantidade de massa &oacute;ssea ganha at&eacute; o final da adolesc&ecirc;ncia, &eacute; um dos fatores determinantes para a prote&ccedil;&atilde;o da osteopenia e posteriormente da osteoporose, na vida adulta. Ainda Silva et al (13), prop&otilde;e equa&ccedil;&otilde;es preditivas para a avalia&ccedil;&atilde;o da densidade mineral &oacute;ssea em adolescentes do sexo masculino. </p>      <p>Em conson&acirc;ncia com o avan&ccedil;o da idade ou matura&ccedil;&atilde;o &oacute;ssea, encontra-se a matura&ccedil;&atilde;o sexual que com a puberdade &eacute; respons&aacute;vel por diversas mudan&ccedil;as na composi&ccedil;&atilde;o corporal, na massa muscular e em v&aacute;rios componentes da aptid&atilde;o f&iacute;sica (14). No estudo de Mac&ecirc;do e Fernandes Filho (15), onde o desenvolvimento puberal foi avaliado por meio das caracter&iacute;sticas sexuais secund&aacute;rias segundo Tanner, encontrou-se rela&ccedil;&atilde;o entre o desempenho atl&eacute;tico dos adolescentes e os diversos est&aacute;gios de matura&ccedil;&atilde;o sexual. </p>      <p>Logo, fica claro que a predi&ccedil;&atilde;o da idade &oacute;ssea e da matura&ccedil;&atilde;o sexual por um instrumento n&atilde;o invasivo &eacute; de grande valia aos profissionais da &aacute;rea da sa&uacute;de e esporte que poderiam atrav&eacute;s de um simples exame cl&iacute;nico identificar a idade &oacute;ssea e o n&iacute;vel de matura&ccedil;&atilde;o sexual da crian&ccedil;a/paciente. Com isto, o objetivo do presente estudo &eacute; desenvolver e validar equa&ccedil;&otilde;es de predi&ccedil;&atilde;o da idade &oacute;ssea e da matura&ccedil;&atilde;o sexual em crian&ccedil;as de 9 a 14 anos do g&ecirc;nero masculino da cidade do Rio de Janeiro. </p>      <p align="center"><b>MATERIAIS E M&Eacute;TODOS</b></p>      <p><b>Amostra</b></p>     <p>Participaram do estudo 400 crian&ccedil;as e adolescentes de um total de 536 escolares do g&ecirc;nero masculino com idades entre 9 e 14 anos. Todos os participantes estavam matriculados no ensino fundamental e em um dos 40 n&uacute;cleos do Projeto de Inicia&ccedil;&atilde;o Esportiva SUDERJ EM FORMA do munic&iacute;pio do Rio de Janeiro nos anos de 2007 e 2008, sendo 300 sujeitos (75 %) para o desenvolvimento das equa&ccedil;&otilde;es e 100 sujeitos (25 %) para a valida&ccedil;&atilde;o das mesmas. </p>      <p>Dos 536 alunos, foram avaliados por faixa et&aacute;ria: 78 alunos de 9 anos, 101 alunos de 10 anos, 94 alunos de 11 anos, 72 alunos com 12 anos, 93 alunos com 13 anos e 98 alunos com 14 anos. (estes n&uacute;meros n&atilde;o batem com os 400) </p>      <p>O presente trabalho atende as normas para a realiza&ccedil;&atilde;o de pesquisa em seres humanos, Resolu&ccedil;&atilde;o 196/96, do Conselho Nacional de Sa&uacute;de de 10/10/96 (16). </p>      ]]></body>
<body><![CDATA[<p><B>Procedimentos e Instrumentos</B></p>     <p>Inicialmente foi aferida a massa corporal total em quilogramas (kg) utilizando-se uma balan&ccedil;a com capacidade m&aacute;xima 180kg, do tipo digital com plataforma em vidro temperado e visor digital da marca G-TECH - Ind&uacute;stria Brasileira, com alunos descal&ccedil;os, usando roupas leves e sem portar objetos pesados. </p>     <p>A estatura foi mensurada em cent&iacute;metros (cm), em um estadi&ocirc;metro port&aacute;til da marca Sanny, com precis&atilde;o de 0.1cm, de acordo com protocolo de Fernandes Filho (17). </p>      <p>Utilizou-se um paqu&iacute;metro de marca Sanny, com precis&atilde;o de 1 mm, para mensura&ccedil;&atilde;o do di&acirc;metro &oacute;sseo (&uacute;mero e f&ecirc;mur) e uma trena antropom&eacute;trica Sanny para aferi&ccedil;&atilde;o da perimetria (bra&ccedil;o e perna). </p>      <p>A obten&ccedil;&atilde;o do &Iacute;ndice de Massa Corporal&ndash;IMC realizou-se atrav&eacute;s da rela&ccedil;&atilde;o entre a massa corporal total e a estatura ao quadrado dos participantes (IMC= Massa Corporal Total/Estatura<sup>2</sup>). </p>       <p>A idade esquel&eacute;tica foi estimada segundo Greulich e Pyle (18), atrav&eacute;s de radiografia de punho e m&atilde;o esquerda, enquanto que a matura&ccedil;&atilde;o sexual foi analisada atrav&eacute;s das pranchas de Tanner em rela&ccedil;&atilde;o aos pelos pubianos e tamanho da genit&aacute;lia de acordo com os crit&eacute;rios de Marshall e Tanner (19). </p>      <p>An&aacute;lise Estat&iacute;stica </p>     <p>No sentido de compormos um Modelo Preditor de Idade &Oacute;ssea a partir das informa&ccedil;&otilde;es derivadas do Protocolo de Tanner, separou-se 25% da Amostra para valida&ccedil;&atilde;o do modelo, tendo antes testado a normalidade das distribui&ccedil;&otilde;es das vari&aacute;veis observadas segundo o protocolo Komogorov-Smirnov, no qual as mesmas se mostraram bem comportadas abaixo da curva Normal Gaussiana. </p>      <p>Para composi&ccedil;&atilde;o do Modelo Preditor, tomamos inicialmente a vari&aacute;vel IO (Idade &Oacute;ssea) e &iacute;ndice de Matura&ccedil;&atilde;o Sexual de Tanner (IMT) como as vari&aacute;veis dependentes do estudo. No primeiro momento, aplicamos o m&eacute;todo param&eacute;trico de regress&atilde;o Stepwise Correlation tendo as vari&aacute;veis independentes constitu&iacute;das pelo corpo de medidas antropom&eacute;tricas, qualidades f&iacute;sicas e de perimetria. Dessa abordagem um conjunto significativo de vari&aacute;veis foram descartadas pelo modelo Stepwise que observou redund&acirc;ncias e ou aus&ecirc;ncia de correla&ccedil;&atilde;o. Tal modelo, foi reduzido ao melhor, tomando-se como base decis&oacute;ria o EPE (Erro Padr&atilde;o de Estimativa) e combinado ao &iacute;ndice de regress&atilde;o (R) e de determina&ccedil;&atilde;o (R<sup>2</sup>), onde quanto menor for o EPE (erro padr&atilde;o estimativa) e quanto mais pr&oacute;xima da unidade forem os valores de R e R<sup>2</sup>, mais representativo &eacute; o modelo preditor. </p>      <p>No Cross Validation (Valida&ccedil;&atilde;o) aplicamos o Modelo Preditor na parcela destacada da Amostra inicialmente (25%) que fora escolhida randomicamente. Os resultados denotaram um coeficiente de correla&ccedil;&atilde;o entre as Idades &Oacute;sseas Observadas e Preditas igual a 0,99, denotando alta correla&ccedil;&atilde;o entre os mesmos e confirmando a fidedignidade do Modelo Preditor da Idade &Oacute;ssea. </p>      ]]></body>
<body><![CDATA[<p>No segundo momento tomamos como vari&aacute;vel dependente a Matura&ccedil;&atilde;o Sexual de Tanner (IMT) e nesse contexto, adicionamos como vari&aacute;vel independente Idade &Oacute;ssea (IO) e calculou-se o segundo Modelo preditor, em combina&ccedil;&atilde;o com as demais vari&aacute;veis independentes mencionadas anteriormente e observando o mesmo m&eacute;todo param&eacute;trico stepwise. </p>      <p>O resultado do &iacute;ndice de matura&ccedil;&atilde;o sexual acrescido do erro padr&atilde;o deve ser analisado de acordo com a idade &oacute;ssea, pois se a IO for abaixo da Idade cronol&oacute;gica poder&aacute; se classificar a matura&ccedil;&atilde;o pelo menor &iacute;ndice do intervalo e vice-versa. </p>      <p align="center"><b>RESULTADOS</b></p>      <p>A partir da an&aacute;lise estat&iacute;stica mencionada na &aacute;rea de materiais e m&eacute;todos chegou-se ao modelo preditor mais significativo, o qual correspondeu ao que se segue abaixo: </p>     <p>Idade &Oacute;ssea = 0,062*Estatura (cm) + 0,426*Id.Cronol.+ 0,041*MassaCorp. (Kg) - 0,390* Di&acirc;metro F&ecirc;mur (cm) </p>     <p>Com os seguintes par&acirc;metros estat&iacute;sticos: R=0,998; R ajustado=0,997; R<sup>2</sup>=0,997 e EPE (erro padr&atilde;o da estimativa)=0,729 anos. </p>     <p>Em seguida buscou-se um modelo preditor com as mesmas condi&ccedil;&otilde;es de aceitabilidade mencionada na an&aacute;lise estat&iacute;stica que gerou a equa&ccedil;&atilde;o que segue abaixo: </p>      <p>&Iacute;ndice Matura&ccedil;&atilde;o Tanner (IMT) = 0,5423*Idade &Oacute;ssea estimada - 0,0374*Estatura (cm) + 0,0388*MassaCorp (Kg) </p>     <p>Com os seguintes par&acirc;metros estat&iacute;sticos: R=0,945; R ajustado=0,894; R<sup>2</sup>=0,892 e EPE (erro padr&atilde;o de estimativa)=0,925. </p>      <p align="center"><b>DISCUSS&Atilde;O</b></p>      ]]></body>
<body><![CDATA[<p>A an&aacute;lise f&iacute;sica e funcional na inf&acirc;ncia e adolesc&ecirc;ncia a partir da idade cronol&oacute;gica n&atilde;o &eacute; o melhor par&acirc;metro, pois nestas fases da vida ocorrem varia&ccedil;&otilde;es no ritmo, na velocidade e na dura&ccedil;&atilde;o do crescimento e do desenvolvimento entre indiv&iacute;duos de mesma idade cronol&oacute;gica (20-22). Estas diverg&ecirc;ncias s&atilde;o geradas por diferen&ccedil;as biol&oacute;gicas ou maturacionais, a qual &eacute; considerada fator primordial, j&aacute; que a mesma interfere diretamente na composi&ccedil;&atilde;o corporal e nas qualidades f&iacute;sicas b&aacute;sicas (7,23-25), principalmente em meninos (14). </p>      <p>Outro fator importante na observa&ccedil;&atilde;o maturacional est&aacute; relacionado &agrave; orienta&ccedil;&atilde;o e sele&ccedil;&atilde;o esportiva, pois a an&aacute;lise individualizada de um aluno/atleta facilita a compreens&atilde;o do estado e consequentemente o progn&oacute;stico de desenvolvimento em rela&ccedil;&atilde;o a um esporte (26). Ou seja, conhecer o momento em que se encontra a crian&ccedil;a e o adolescente facilita na adequa&ccedil;&atilde;o do treinamento (27,28). </p>      <p>Em pesquisas com crian&ccedil;as e adolescentes, encontra-se o raio-X da m&atilde;o e punho (matura&ccedil;&atilde;o esquel&eacute;tica), o qual &eacute; considerado o mais efetivo e fidedigno (29,30), a matura&ccedil;&atilde;o sexual, o desenvolvimento de pelos axilares e a avalia&ccedil;&atilde;o som&aacute;tica. A matura&ccedil;&atilde;o esquel&eacute;tica possibilita encontrar a idade &oacute;ssea e apesar de ser a mais recomendada, necessita de equipamento espec&iacute;fico, o qual &eacute; manipulado por profissionais treinados. Outro ponto negativo s&atilde;o as discrep&acirc;ncias na an&aacute;lise, j&aacute; que a idade &oacute;ssea pode ser avaliada por v&aacute;rios m&eacute;todos (18,31,32) que podem gerar resultados distintos, pois Van Lenthe et al (33) encontrou diferen&ccedil;as significativas na idade &oacute;ssea ao analisar pelos m&eacute;todos de Fels (31) e Tanner &amp; Whitehouse (32). Outro ponto negativo est&aacute; relacionado &agrave;s radiografias padr&atilde;o, pois as mesmas s&atilde;o desenvolvidas para o pa&iacute;s de origem que podem n&atilde;o ter o mesmo ritmo de crescimento (34). </p>      <p>Em rela&ccedil;&atilde;o &agrave; matura&ccedil;&atilde;o sexual os problemas est&atilde;o relacionados ao constrangimento (35). Outro protocolo &eacute; o de Siret et al (36), para meninas, o qual possui similaridades com o m&eacute;todo empregado neste estudo, por&eacute;m o resultado gerado &eacute; a idade morfol&oacute;gica ou &iacute;ndice de desenvolvimento corporal e n&atilde;o a idade &oacute;ssea como neste estudo. Outro ponto negativo &eacute; necessidade de um n&uacute;mero maior de vari&aacute;veis (di&acirc;metros biacromial e bicristal, circunfer&ecirc;ncias de antebra&ccedil;o direito e esquerdo em meninos e estatura) que n&atilde;o s&atilde;o de comum utiliza&ccedil;&atilde;o em estudos epidemiol&oacute;gicos ou mesmo nos consult&oacute;rios m&eacute;dicos. </p>      <p>Ao se estabelecer o desenvolvimento e a valida&ccedil;&atilde;o de uma equa&ccedil;&atilde;o para a determina&ccedil;&atilde;o da idade &oacute;ssea, e, por conseguinte, a equa&ccedil;&atilde;o de um &iacute;ndice de matura&ccedil;&atilde;o sexual, vai-se ao encontro da facilita&ccedil;&atilde;o da avalia&ccedil;&atilde;o destes par&acirc;metros quando comparada aos m&eacute;todos invasivos, como o raio&ndash;X e o exame cl&iacute;nico de observa&ccedil;&atilde;o do crescimento e desenvolvimento da pilosidade pubiana e genit&aacute;lia externa. </p>      <p>Com isto, as equa&ccedil;&otilde;es sugeridas neste estudo podem ser de grande utiliza&ccedil;&atilde;o, j&aacute; que emprega par&acirc;metro de f&aacute;cil avalia&ccedil;&atilde;o e an&aacute;lise, baixo custo, e &eacute; um m&eacute;todo n&atilde;o invasivo, o que facilita a sua realiza&ccedil;&atilde;o e uma aceita&ccedil;&atilde;o maior pelos comit&ecirc;s de &eacute;tica para a realiza&ccedil;&atilde;o de outros estudos.
