The high rate of success of antimicrobial therapy in most infectious diseases has failed to achieve in bone and joint infections due to physiological and anatomical characteristics of bones; that is why the various types of osteomyelitis require different medical, surgical and therapeutic strategies for proper management. Besides that, the key to successful management of osteomyelitis in general is early diagnosis, including bone sampling for microbiological and pathological analysis to allow for specific and longterm lasting antibiotic therapy. Therefore, it is necessary to have the basics in handling this issue clear and updated. It was carried out a review of pathophysiology, diagnosis and management of osteomyelitis in order to provide basic tools in the management of osteomyelitis for the doctor.]]></p></abstract>

It was carried out a review of pathophysiology, diagnosis and management of osteomyelitis in order to provide basic tools in the management of osteomyelitis for the doctor.</p>      <p><b>Key Words</b>: Osteomyelitis, infection, bone diseases.</p>  <hr size="1">    <br>      <p align="center"><font size="3"><b>INTRODUCCI&Oacute;N</b></font></p>      <p>La osteomielitis es la infecci&oacute;n de huesos causada por organismos piog&eacute;nicos.<sub>1-3</sub> Su incidencia no es alta debido a que el hueso normal es altamente resistente a la infecci&oacute;n, ocurre en pacientes con factores de riesgo: diabetes, las &uacute;lceras de dec&uacute;bito, la cirug&iacute;a, el trauma y el consumo de drogas por v&iacute;a intravenosa. La infecci&oacute;n se produce cuando hay una alta inoculaci&oacute;n de microorganismos, cuando hay un trauma que conduce a da&ntilde;o en los huesos o bien por la presencia de cuerpos extra&ntilde;os. Despu&eacute;s de una fractura abierta, la incidencia de la osteomielitis pode ir del 2% al 16%, dependiendo del grado de traumatismo y el tipo de tratamiento administrado.<sub>4,5</sub></p>      <p>En una revisi&oacute;n hecha por Waldvogel colaboradores, el 19% fue osteomielitis de origen hemat&oacute;geno, el 47% fue secundarios a infecci&oacute;n en un foco continuo y el 34% estaba asociado con la insuficiencia vascular.<sub>6</sub> Osteomielitis hemat&oacute;gena ocurre principalmente en ni&ntilde;os, posterior al cierre de las ep&iacute;fisis de los huesos largos, estos son menos propensos a la siembra microbiana. Por lo tanto, la osteomielitis hemat&oacute;gena es rara en adultos excepto en hu&eacute;spedes inmunocomprometidos.<sub>7</sub> El mayor riesgo relativo se encuentra durante la ni&ntilde;ez, con otro pico en la adolescencia, y se encuentra una proporci&oacute;n baja constante en los adultos.<sub>8</sub></p>      ]]></body>
<body><![CDATA[<p>Una enfermedad que aumenta de manera significativa la incidencia de osteomielitis es la diabetes, a cerca de un tercio de los pacientes diab&eacute;ticos en los que se presentan infecciones en los pies, se les encuentra osteomielitis,<sub>9</sub> y teniendo en cuenta que la prevalencia de la diabetes ha aumentado dram&aacute;ticamente en los &uacute;ltimos a&ntilde;os, actualmente afecta a 246 millones de personas en todo el mundo y se estima que esta prevalencia de diabetes en el mundo sea superior a 360 millones en el a&ntilde;o 2030. Se calcula que el 45% al 60% de las amputaciones no traum&aacute;ticas de las extremidades inferiores se producen en los pacientes diab&eacute;ticos con evidencia de osteomielitis.</p>      <p>Todo lo anterior, hace necesario que los m&eacute;dicos de atenci&oacute;n primaria tengan conceptos claros sobre el diagn&oacute;stico y manejo de la osteomielitis de una manera clara con el fin de agilizar el estudio y manejo de esta patolog&iacute;a. Se realiz&oacute; una b&uacute;squeda en la base de datos de MEDLINE con las palabras claves: osteomielitis, infecci&oacute;n, enfermedades &oacute;seas, fisiopatolog&iacute;a, con las respectivas traducciones en ingl&eacute;s seg&uacute;n los <i>descriptores en Ciencias de la Salud de BIREMEOPS, de la cual se escogieron los art&iacute;culos que los autores consideraron aportaban los conceptos actuales sobre el diagn&oacute;stico y manejo de esta patolog&iacute;a. En esta revisi&oacute;n se exponen los conceptos b&aacute;sicos para realizar una aproximaci&oacute;n diagn&oacute;stica y terap&eacute;utica adecuada de la osteomielitis en general</i>; no se exponen los casos especiales de osteomielitis como son: infecci&oacute;n por &uacute;lceras de presi&oacute;n, osteomielitis vertebral, osteomielitis en paciente con abuso de drogas endovenosas, osteomielitis secundaria a fracturas abiertas, ni osteomielitis en pacientes con infecci&oacute;n por VIH.</p>      <p><b>Fisiopatolog&iacute;a de la osteomielitis</b></p>      <p>La patog&eacute;nesis de la osteomielitis ha sido estudiada en diferentes modelos animales<sub>10,11</sub> el principal agente de infecci&oacute;n es el estafilococo aureus,<sub>12,13</sub> el cual expresa adhesinas para fijarse a diferentes componentes de la matriz &oacute;sea (fibronectina, laminina, col&aacute;geno, y sialoglicoproteina &oacute;sea).<sub>14,15</sub></p>      <p>La expresi&oacute;n de adhesinas une el pat&oacute;geno al col&aacute;geno y a su vez al cart&iacute;lago;<sub>16</sub> algunos estudios han encontrado que el estafilococo aureus puede sobrevivir de manera intracelular en los osteoblastos<sub>17,18</sub> y que la adhesina y la fibronectina juegan un papel importante en su uni&oacute;n a materiales quir&uacute;rgicos, lo cual es un motivo por el cual las terapias cortas presentan altos &iacute;ndices de fracaso.<sub>19,20</sub></p>      <p>El proceso de remodelaci&oacute;n tisular, requiere la coordinaci&oacute;n de osteoblastos, osteoclastos<sub>21</sub> y citoquinas (interleukinas 1, 6 y 11 y factor de necrosis tumoral) que al finalizar, genera una reacci&oacute;n inflamatoria local y un gran factor osteol&iacute;tico, explicando, c&oacute;mo durante la infecci&oacute;n, los fagocitos en el intento de detener los pat&oacute;genos, liberan radicales libres de ox&iacute;geno y enzimas proteol&iacute;ticas, que lisan el tejido &oacute;seo circundante.<sub>22,23</sub></p>      <p>La presencia de metabolitos del &aacute;cido araquid&oacute;nico como la prostaglandina E2, que es un fuerte agonista de los osteoclastos generados en respuesta a la fractura, disminuye la cantidad de inoculo bacteriano necesario para producir la infecci&oacute;n;<sub>24,25</sub> sin embargo esto facilita que la pus se propague a trav&eacute;s de los canales vasculares y aumente la presi&oacute;n intra&oacute;sea, causando alteraci&oacute;n en el flujo sangu&iacute;neo normal, isquemia y necrosis, que al final puede generar la separaci&oacute;n de los fragmentos desvascularizados produciendo el llamado secuestro &oacute;seo.<sub>26,27</sub></p>      <p>La osteomielitis aguda se caracteriza por la presencia de microorganismos, la infiltraci&oacute;n de neutr&oacute;filos y vasos congestionados o trasvasados, as&iacute; que una de las diferencias con una osteomielitis cr&oacute;nica es la presencia de necrosis &oacute;sea.<sub>28,29</sub></p>      <p><b>Sistemas de clasificaci&oacute;n de la osteomielitis</b></p>      <p>Debido a la complejidad de los estadios, la osteomielitis ha presentado m&uacute;ltiples clasificaciones<sub>30,31</sub> y una de las mas populares es la de Waldvogel<sub>32</sub> quien la divide en: osteomielitis aguda o hemat&oacute;gena, osteomielitis cr&oacute;nica (necrosis &oacute;sea) y osteomielitis secundaria a infecci&oacute;n de un foco continuo, la cual puede ser con o sin infecci&oacute;n vascular generalizada. Esta clasificaci&oacute;n se asocia a presencia de trauma y se clasifica como aguda si se desarrolla dos semanas despu&eacute;s del trauma; subaguda si se desarrolla posterior a un mes del trauma y cr&oacute;nica despu&eacute;s de varios meses.</p>      ]]></body>
<body><![CDATA[<p>Otra clasificaci&oacute;n de la osteomielitis es el sistema de Cierny-Mader<sub>33,34</sub> el cual es din&aacute;mico y se basa en los cambios de la condici&oacute;n m&eacute;dica general, comorbilidades y respuesta del paciente al tratamiento (<a href="#t01">Tabla 1</a>); orienta la necesidad o no de cirug&iacute;a y no tiene en cuenta los t&eacute;rminos agudo o cr&oacute;nico y no es aplicable en circunstancias especiales como presencia de infecci&oacute;n en pr&oacute;tesis, materiales implantados e infecci&oacute;n en peque&ntilde;os huesos y tampoco se puede utilizar en casos de tipos especiales de osteomielitis como la osteomielitis vertebral. En este sistema de clasificaci&oacute;n se tienen en cuenta los factores que afectan la respuesta inmune, la vascularizaci&oacute;n local y el metabolismo. Dichos factores pueden ser sist&eacute;micos para el estadio BS: desnutrici&oacute;n, insuficiencia renal o hep&aacute;tica, diabetes mellitus, hipoxia cr&oacute;nica (ejemplo la EPOC), enfermedades inmunes, edades extremas, inmunosupresi&oacute;n o deficiencias inmunes; o tambi&eacute;n pueden ser locales para el estadio Bl: linfedema cr&oacute;nico, &eacute;xtasis venoso, compromiso arterial. fibrosis radioactiva, enfermedad de peque&ntilde;os vasos, neuropat&iacute;a y abuso de tabaco.</p>    <br>      <p align="center"><a name="t01"></a><img src="img/revistas/reus/v12n1/v12n1a15t01.gif"></p>    <br>      <p><b>Estudio diagn&oacute;stico de la osteomielitis</b></p>      <p>El diagn&oacute;stico de osteomielitis se basa en la historia cl&iacute;nica, examen f&iacute;sico y hallazgos de laboratorio, las manifestaciones cl&iacute;nicas var&iacute;an seg&uacute;n la edad del paciente, la localizaci&oacute;n y el tipo de hueso implicado.<sub>35</sub> El dolor constante y los signos inflamatorios locales como el aumento local de la temperatura y el edema de tejidos blandos, afecta con mayor frecuencia las extremidades inferiores y las zonas metafisarias. En la fase bacteri&eacute;mica puede haber desde febr&iacute;cula y debilidad, hasta fiebre de 40ºC y s&iacute;ndrome constitucional importante, sin que se correlacione con la gravedad de la osteomielitis.<sub>36</sub> Tambi&eacute;n es importante el interrogar sobre la existencia de antecedentes de traumatismo.</p>      <p>En el examen f&iacute;sico del paciente con osteomielitis se encuentra dolor a la palpaci&oacute;n de la regi&oacute;n infectada, en algunos casos se encuentran signos de infecci&oacute;n de tejidos blandos que se encuentran sobre el &aacute;rea de infecci&oacute;n &oacute;sea; en los pacientes con diabetes mellitus, la presencia de &uacute;lceras en miembros inferiores est&aacute; fuertemente relacionada con presencia de osteomielitis (sensibilidad del 66% y especificidad del 85% con un valor predictivo positivo del 89% y un valor predictivo negativo del 56%).<sub>37,38</sub> Esto hace que al palpar el hueso a trav&eacute;s de una &uacute;lcera en miembros inferiores, en un paciente diab&eacute;tico se correlaciona de manera muy fuerte con la presencia de osteomielitis y no se recomienda solicitar ayudas diagn&oacute;sticas, si no la toma de una muestra directa, para realizar cultivo y determinar el diagn&oacute;stico etiol&oacute;gico exacto, que facilitar&aacute; el tratamiento.<sub>39,40</sub></p>      <p>Entre los hallazgos de las pruebas de laboratorio se puede encontrar, elevaci&oacute;n en la velocidad de sedimentaci&oacute;n glomerular (VSG). La VSG es muy sensible pero poco espec&iacute;fica, pues se normaliza a las 3-4 semanas en casos de osteomielitis no complicada y en un 25% de los casos, no presenta incremento en fases in&iacute;ciales. La prote&iacute;na C  reactiva se eleva en las primeras 8 horas, alcanza el valor m&aacute;ximo a los 2 d&iacute;as y se normaliza a la semana de haber iniciado el tratamiento; es &uacute;til para el seguimiento y para diferenciar formas complicadas. Estas dos ayudas diagn&oacute;sticas se utilizan tanto para diagn&oacute;stico como para seguimiento al tratamiento. Otros ex&aacute;menes de laboratorio que se utilizan, son el conteo de gl&oacute;bulos blancos, dado que se encuentra aumento en los leucocitos hasta en el 60% de los casos y los hemocultivos que son positivos entre el 20 y el 50% de los casos de osteomielitis aguda.<sub>41,42</sub></p>      <p>En el caso de infecciones en extremidades superiores o inferiores, las radiograf&iacute;as convencionales y la gammagraf&iacute;a son los principales m&eacute;todos diagn&oacute;sticos<sub>43,44</sub> y las sensibilidades y especificidades var&iacute;an de una a otra.</p>      <p>En la radiograf&iacute;a simple se pueden observar signos indirectos como el aumento de partes blandas y/o atenuaci&oacute;n de las l&iacute;neas grasas situadas entre los m&uacute;sculos. Los signos espec&iacute;ficos de reacci&oacute;n peri&oacute;stica (elevaci&oacute;n del periostio, formaci&oacute;n de hueso nuevo) o destrucci&oacute;n &oacute;sea (rarefacci&oacute;n, lisis) tardan en ser visibles (De 7 a 14 d&iacute;as desde el inicio de la cl&iacute;nica).<sub>45,46</sub></p>      ]]></body>
<body><![CDATA[<p>La Gammagraf&iacute;a &oacute;sea con tecnecio 99 (Tc99m) es la t&eacute;cnica diagn&oacute;stica de elecci&oacute;n; en ella se muestra un aumento focal de la captaci&oacute;n del marcador en &aacute;reas con mayor vascularizaci&oacute;n y actividad osteobl&aacute;stica del hueso.<sub>47</sub> Sin embargo esta puede ser negativa en las primeras 48 horas (por la existencia de zonas de infarto y/o isquemia) y no es &uacute;til en el diagn&oacute;stico de osteomielitis neonatal. Si la gammagraf&iacute;a con Tc99 no es definitiva, se recomienda realizar gammagraf&iacute;a con galio67 o con leucocitos marcados In111, muy sensibles como marcadores de inflamaci&oacute;n aguda (es mejor en osteomielitis de huesos de la pelvis).<sub>48</sub></p>      <p>La ultrasonograf&iacute;a, es una t&eacute;cnica r&aacute;pida e inocua, en la que se puede evidenciar el engrosamiento y elevaci&oacute;n del periostio con zonas hipoecoicas profundas y superficiales. Su gran ventaja es que puede detectar alteraciones en tejidos blandos tan s&oacute;lo 24 horas despu&eacute;s del comienzo de la cl&iacute;nica.<sub>49,50</sub></p>      <p>La Tomograf&iacute;a Computarizada (TAC), proporciona una buena definici&oacute;n de la destrucci&oacute;n del hueso cortical, reacci&oacute;n peri&oacute;stica y cambios en los tejidos blandos; es el mejor m&eacute;todo para detectar peque&ntilde;os focos de gas dentro del canal medular, &aacute;reas de erosi&oacute;n o destrucci&oacute;n cortical, peque&ntilde;os cuerpos extra&ntilde;os que act&uacute;an como un nido de la infecci&oacute;n, la extensi&oacute;n de la f&iacute;stula, y la formaci&oacute;n de secuestro.<sub>51,52</sub> La sensibilidad y especificidad no est&aacute;n claramente establecidas, aunque se sabe, que la sensibilidad es menor que en la resonancia magn&eacute;tica.<sub>53,54</sub> La TAC no se recomienda de manera rutinaria en el diagn&oacute;stico de osteomielitis, pero es la imagen de elecci&oacute;n cuando la RMN no est&aacute; disponible.<sub>55</sub> La Resonancia Magn&eacute;tica Nuclear (RMN), proporciona una excelente delimitaci&oacute;n entre el  hueso y los tejidos blandos, as&iacute; como entre tejido normal y anormal y de m&eacute;dula &oacute;sea normal o alterada y detecta osteomielitis entre los 3-5 d&iacute;as de infecci&oacute;n;<sub>56,57</sub> obtiene datos anat&oacute;micos exactos, sirve para descartar lesi&oacute;n medular y resulta &uacute;til en osteomielitis vertebral.<sub>58,59</sub> Sin embargo, posee poca especificidad, pues no diferencia infecci&oacute;n de neoplasia. En un metan&aacute;lisis, Dinh y colaboradores encontraron que la RMN fue la prueba de imagen m&aacute;s precisa para el diagn&oacute;stico de osteomielitis y que adem&aacute;s es capaz de detectar un absceso epidural o flem&oacute;n y compresi&oacute;n de la m&eacute;dula espinal en los casos de osteomielitis vertebral.<sub>60</sub> En los casos en que una resonancia magn&eacute;tica est&aacute; contraindicada, la t&eacute;cnica de elecci&oacute;n es la tomograf&iacute;a computarizada.<sub>61,62</sub></p>      <p>En cuanto a la determinaci&oacute;n de las etiolog&iacute;as, los microorganismos espec&iacute;ficos aislados en los pacientes con osteomielitis bacteriana, var&iacute;an seg&uacute;n la edad del paciente y el contexto en el cual se presenta la infecci&oacute;n (<a href="img/revistas/reus/v12n1/v12n1a15t02.gif"target="_blank">Tablas 2</a> y <a href="#t03">3</a>).<sub>63,64</sub></p>    <br>      <p align="center"><a name="t03"></a><img src="img/revistas/reus/v12n1/v12n1a15t03.gif"></p>    <br>      <p>El <i>Staphylococcus aureus</i> es el m&aacute;s frecuente en los casos de osteomielitis hemat&oacute;gena y corresponde al 90% de los casos.<sub>65,66</sub> <i>Staphylococcus epidermidis</i>, <i>Staphylococcus aureus</i>, <i>Pseudomonas aeruginosa</i>, <i>Serratia marcescens</i> y <i>la Escherichia coli</i> son los agentes com&uacute;nmente encontrados en osteomielitis cr&oacute;nica.<sub>67-69</sub></p>      <p><b>Tratamiento de la osteomielitis</b></p>      <p>Despu&eacute;s de realizar la evaluaci&oacute;n inicial y de establecer la etiolog&iacute;a y la susceptibilidad seg&uacute;n la edad y los grupos de riesgo para determinados pat&oacute;genos, se inicia el tratamiento con antibioticoterapia seg&uacute;n se recomienda en la <a href="img/revistas/reus/v12n1/v12n1a15t04.gif"target="_blank">Tabla 4</a> y se procede a realizar desbridamiento del tejido muerto y la estabilizaci&oacute;n del hueso seg&uacute;n el caso.<sub>70,71</sub> En la mayor&iacute;a de los pacientes la antibioticoterapia inicial produce buenos resultados y su administraci&oacute;n m&iacute;nima debe ser por cuatro semanas (idealmente seis semanas).<sub>72,73</sub></p>      ]]></body>
<body><![CDATA[<p>La delimitaci&oacute;n de tratamiento ha sido dif&iacute;cil por varias razones: El desbridamiento desestima el impacto de los antibi&oacute;ticos; los agentes pat&oacute;genos y las situaciones cl&iacute;nicas son muy heterog&eacute;neas, y se necesitan a&ntilde;os de seguimiento para demostrar la remisi&oacute;n sostenida. Muchos estudios no presentan buena calidad o incluyen un peque&ntilde;o n&uacute;mero de pacientes. Por lo tanto, la mayor&iacute;a de las recomendaciones para el tratamiento de la osteomielitis se basa en la opini&oacute;n de los expertos.<sub>74,75</sub></p>      <p>En los casos en que el diagn&oacute;stico de osteomielitis hemat&oacute;gena es hecho de manera inicial no es necesario realizar el desbridamiento quir&uacute;rgico; sin embargo en caso de fallar el tratamiento antibi&oacute;tico inicial se debe realizar el desbridamiento o repetirlo en caso de que se halla hecho y dar un nuevo ciclo de antibioticoterapia parenteral.<sub>76,77</sub></p>      <p>Ante la sospecha de osteomielitis, la antibioticoterapia emp&iacute;rica parenteral inicial, incluye la nafcillina con cefotaxima o ceftriaxona y en el momento de conocer el resultado de los cultivos se debe ajustar el tratamiento.<sub>78,79</sub> Los ni&ntilde;os con osteomielitis aguda deben recibir inicialmente dos semanas de antibi&oacute;tico parenteral, para luego continuar con manejo por v&iacute;a oral.<sub>80-82</sub></p>      <p>La osteomielitis cr&oacute;nica en adultos es mas refractaria y generalmente es tratada con antibi&oacute;tico y desbridamiento quir&uacute;rgico.<sub>83</sub> No se recomienda la antibioticoterapia emp&iacute;rica dado que el manejo depende del tipo de osteomielitis y se requiere administraci&oacute;n parenteral de cuatro a seis semanas. Sin un desbridamiento adecuado, no se logra una adecuada respuesta a los antibi&oacute;ticos.<sub>84,85</sub> Las fluoroquinolonas son antibi&oacute;ticos utilizados v&iacute;a oral, contra bacterias gram negativas, aunque no sean &oacute;ptimas para el tratamiento de anaerobios.<sub>86</sub> Ninguna de las fluoroquinolonas disponibles en la actualidad, proporciona una cobertura &oacute;ptima contra el estafilococo, lo cual es una desventaja  importante, en vista de la creciente incidencia de infecci&oacute;n por estafilococo nosocomial.<sub>87,88</sub></p>      <p><b>Manejo quir&uacute;rgico</b></p>      <p>Los principios de cirug&iacute;a en la osteomielitis incluyen, un drenaje adecuado y desbridamiento amplio del tejido infectado, la gesti&oacute;n del espacio muerto, cierre completo de la herida, y la estabilizaci&oacute;n de la fractura. M&uacute;ltiples estudios manifiestan, que es m&aacute;s probable el fracaso del tratamiento, si no se realiza el desbridamiento quir&uacute;rgico.<sub>89,90</sub></p>      <p>El desbridamiento quir&uacute;rgico en pacientes con osteomielitis cr&oacute;nica puede ser t&eacute;cnicamente dif&iacute;cil<sub>91</sub> y la calidad de este procedimiento es un factor crucial para el manejo. Despu&eacute;s de la escisi&oacute;n con el desbridamiento de hueso, es necesario borrar el espacio muerto creado por la extracci&oacute;n de tejidos. El manejo del espacio vac&iacute;o incluye mioplastia, transferencia de tejido libre y la utilizaci&oacute;n de vendas impregnadas de antibi&oacute;tico.<sub>92,93</sub> Posterior al desbridamiento, se debe continuar el tratamiento con antibi&oacute;ticos para la infecci&oacute;n de los tejidos blandos cuando &eacute;sta se presente.<sub>94</sub></p>      <p><b>Pron&oacute;stico de los pacientes con osteomielitis</b></p>      <p>En la era preantibi&oacute;tica, la osteomielitis aguda por estafilococo, ten&iacute;a una tasa de mortalidad del 50%.<sub>95</sub> En la actualidad el pron&oacute;stico depende de los factores del hu&eacute;sped, las modalidades de tratamiento y el agente pat&oacute;geno. La mayor tasa de recurrencia se presenta en pacientes con diabetes y enfermedad vascular perif&eacute;rica.<sub>96</sub> En la osteomielitis por S. aureus, el riesgo de recurrencia es m&aacute;s del doble, cuando es necesario el tratamiento con vancomicina, comparado a cuando se utilizan &szlig;-lact&aacute;micos, mientras que no hay diferencia entre estos &uacute;ltimos (ceftriaxona, cefazolina y penicilinas resistentes a penicilinasa).<sub>97</sub> En un estudio de Dombrowski y  colaboradores, el fracaso terap&eacute;utico en  osteomielitis por <i>estafilococcus aureus</i> resistente a metilcilina, fue del 46% (37/81), y la falla en el tratamiento se asoci&oacute; significativamente al uso de solo vancomicina.<sub>98</sub> La <i>P. aeruginosa</i> es un pat&oacute;geno conocido por tener alta tasa de fracaso, en su tratamiento, Tice y col. encontraron que tiene una tasa significativamente mayor de recurrencia en comparaci&oacute;n con <i>S. aureus</i> y mayor correlaci&oacute;n con la amputaci&oacute;n.<sub>99</sub></p>    <br>      ]]></body>
