Os resultados mostraram escassez de trabalhos sobre o tema na literatura internacional e ausência de trabalhos na literatura nacional (brasileira), sendo incluídos no estudo 19 artigos, os quais relataram em sua maioria, reabilitação da linguagem, sendo explorados os ambientes virtuais para a reabilitação com metodologia breve e frequente, assim como, os programas mostraram ser eficazes às tarefas da vida diária, sugerindo sua validade ecológica. Conclui-se que o processo de reabilitação precisa ser mais bem explorado, sistematizado e que as técnicas devem ser divulgadas e publicadas pela literatura científica.]]></p></abstract>

The results showed a paucity of publications on this subject in the international and Brazilian national literatures. We identified only 19 articles, and these reported mostly language rehabilitation exploring virtual environments for rehabilitation with brief and infrequent description of the methodology. The programs showed to be effective in improving tasks of daily living, suggesting their ecologic validity. We concluded that the rehabilitation process needs to be further explored and systematically analyzed and that the effective techniques should be disclosed and published in detail.]]></p></abstract>

Los resultados mostraron escasez de trabajos sobre el tema en la literatura internacional y ausencia de trabajos en la literatura brasileña. En el estudio se incluyeron 19 artículos, los cuales relacionaron en su mayoría rehabilitación del lenguaje, siendo explorados los ambientes virtuales para la rehabilitación con metodología breve y frecuente. Los programas mostraron ser eficaces para las tareas de la vida cotidiana, lo que sugiere su validez ecológica. Se concluye que el proceso de rehabilitaci&oacute;n debe ser mejor explorado, sistematizado y que las t&eacute;cnicas deben ser divulgadas y publicadas por la literatura cient&iacute;fica.</p>      <p align="justify"><i><b>Palabras clave: </b></i>rehabilitaci&oacute;n cognitiva, accidente vascular cerebral, procesos b&aacute;sicos</p>  <hr>      <br>      <p align="justify">O Acidente Vascular Cerebral (AVC) pode causar preju&iacute;zos cognitivos e emocionais na mem&oacute;ria e linguagem (Corbett, Jeferries, &amp; Ralph, 2009), nas fun&ccedil;&otilde;es visuoespaciais, (Lima &amp; Kaihami, 2001), nas fun&ccedil;&otilde;es executivas (Park, Yoon, &amp; Rhee, 2011; Pohjasvaara et al., 2002), aten&ccedil;&atilde;o (Rengachary, He, Shulman, &amp; Corbetta, 2011) e, ainda altera&ccedil;&otilde;es de humor (Terroni, Mattos, Sobreiro, Guajardo, &amp; Fr&aacute;guas, 2009). Ap&oacute;s tr&ecirc;s meses do AVC, utilizando-se uma extensa bateria neuropsicol&oacute;gica em popula&ccedil;&atilde;o cl&iacute;nica foi encontrado que 55% dos pacientes tinham comprometimento em pelo menos um dom&iacute;nio cognitivo, 27% apresentaram d&eacute;ficits cognitivos sem preju&iacute;zo da mem&oacute;ria, 7% com d&eacute;ficits somente na mem&oacute;ria e 9% preju&iacute;zos na mem&oacute;ria e outros preju&iacute;zos cognitivos (Madureira, Guerreiro, &amp; Ferro, 2001). Em rela&ccedil;&atilde;o aos aspectos emocionais a depress&atilde;o parece ser o transtorno mais prevalente, dependendo da localiza&ccedil;&atilde;o e lateralidade da les&atilde;o (Terroni, Leite, Tinone, &amp; Fr&aacute;guas, 2003) e as altera&ccedil;&otilde;es comportamentais mais evidentes ocorrem no comportamento compulsivo (Hashimoto, Hasegawa, Maki, &amp; Tanaka, 1998), na falta de controle inibit&oacute;rio e no comportamento obsessivo-compulsivo (Oya, Sakurai, Takeda, Iwata, &amp; Kanazawa, 1997).</p>      <p align="justify">A gravidade da depress&atilde;o est&aacute; associada tamb&eacute;m, com o preju&iacute;zo funcional e cognitivo em todos os dom&iacute;nios. Geralmente, dependendo do tamanho da les&atilde;o, o perfil neuropsicol&oacute;gico dos pacientes com sintomas depressivos moderados e graves podem ser afetados, causando danos nos dom&iacute;nios de mem&oacute;ria, visuopercep&ccedil;&atilde;o e linguagem (Terroni et al., 2009). Dessa forma, a Neuropsicologia proporciona t&eacute;cnicas que possibilitam tratar as devidas altera&ccedil;&otilde;es, que s&atilde;o de extrema import&acirc;ncia e estabelece quais dentre as ferramentas dispon&iacute;veis s&atilde;o as mais adequadas para o objetivo que se deseja alcan&ccedil;ar no &acirc;mbito da reabilita&ccedil;&atilde;o cognitiva e emocional (Nore&ntilde;a et al., 2010).</p>      <p align="justify">Preju&iacute;zos na cogni&ccedil;&atilde;o podem estar relacionados &agrave; m&aacute; qualidade de vida a longo prazo em acidentes vasculares isqu&ecirc;micos, entre esses, afetando as fun&ccedil;&otilde;es executivas (Oksala et al., 2009), ent&atilde;o a reabilita&ccedil;&atilde;o neuropsicol&oacute;gica se faz necess&aacute;ria. Indiv&iacute;duos avaliados atrav&eacute;s de uma bateria de testes neuropsicol&oacute;gicos de mem&oacute;ria, mem&oacute;ria de trabalho, fun&ccedil;&otilde;es instrumentais e executivas, seis meses ap&oacute;s o primeiro AVE apresentaram correla&ccedil;&atilde;o significativa entre os preju&iacute;zos cognitivos, o funcionamento social e a qualidade de vida (Hommel, Miguel, Naegele, Gonnet, &amp; Jaillard, 2009).</p>      <p align="justify">A reabilita&ccedil;&atilde;o cognitiva &eacute; considerada um dos componentes da reabilita&ccedil;&atilde;o neuropsicol&oacute;gica (&Aacute;vila &amp; Miotto, 2002). Dentre suas fun&ccedil;&otilde;es, a reabilita&ccedil;&atilde;o neuropsicol&oacute;gica abrange uma combina&ccedil;&atilde;o de psicoterapia, participa&ccedil;&atilde;o da fam&iacute;lia atrav&eacute;s de grupos, instru&ccedil;&otilde;es terap&ecirc;uticas aos pacientes, sendo sempre realizada dentro de um contexto multidisciplinar (Manzine &amp; Pavarini, 2009). Avan&ccedil;os na &aacute;rea de reabilita&ccedil;&atilde;o come&ccedil;aram a ocorrer somente ap&oacute;s a Primeira e a Segunda Guerra Mundial, per&iacute;odo no qual os pesquisadores e especialistas passaram a empregar esfor&ccedil;os para compreender como os diferentes tipos de les&otilde;es influenciavam o comportamento humano e como se poderia remedi&aacute;-los (Pontes &amp; H&uuml;bner, 2008).</p>      <p align="justify">A Reabilita&ccedil;&atilde;o Cognitiva identifica e guia necessidades e objetivos individuais, no qual esse processo relaciona estrat&eacute;gias para obter novas informa&ccedil;&otilde;es ou mecanismos compensat&oacute;rios, como o uso da mem&oacute;ria (Clare &amp; Woods, 2008). Com rela&ccedil;&atilde;o aos pacientes acometidos pelo AVC, os objetivos de reabilita&ccedil;&atilde;o s&atilde;o: prevenir complica&ccedil;&otilde;es; recuperar ao m&aacute;ximo as fun&ccedil;&otilde;es cerebrais comprometidas pelo AVC, que podem ser tempor&aacute;rias ou permanentes; devolver o paciente ao conv&iacute;vio social, tanto na fam&iacute;lia quanto no trabalho, reintegrando-o com a melhor qualidade de vida poss&iacute;vel (Damiani &amp; Yokoo, 2002). Ap&oacute;s est&aacute;gio agudo do evento neurol&oacute;gico, ou seja, quando as medidas terap&ecirc;uticas s&atilde;o focadas em &quot;prolongar vidas&quot;, um n&uacute;mero significativo de pacientes s&atilde;o v&iacute;timas de conseq&uuml;&ecirc;ncias do dano cerebral a longo prazo (Korzetkowska et al., 2010). Esses preju&iacute;zos funcionais exigem aten&ccedil;&atilde;o de um especialista, entretanto, o n&uacute;mero de pacientes com informa&ccedil;&otilde;es suficientes a respeito de seus recursos e possibilidade de tratamento &eacute; escasso. Aproximadamente 10% dos indiv&iacute;duos que sofreram um AVC ficam totalmente incapazes; somente em 30% &eacute; recuperada a fun&ccedil;&atilde;o neurol&oacute;gica anterior apresente um risco de recidiva de 20% (Azeredo &amp; Matos, 2003).</p>      ]]></body>
<body><![CDATA[<p align="justify">A partir do exposto acima, este estudo teve como objetivo realizar uma revis&atilde;o exaustiva da literatura a qual permita abranger os diferentes programas de reabilita&ccedil;&atilde;o neuropsicol&oacute;gica, bem como, as ferramentas utilizadas em tais programas. Para tanto, foi feita uma revis&atilde;o sistem&aacute;tica da literatura cient&iacute;fica, a respeito de programas de reabilita&ccedil;&atilde;o cognitiva realizados em pacientes adultos que sofreram les&atilde;o vascular como o AVC e as devidas t&eacute;cnicas utilizadas na reabilita&ccedil;&atilde;o de tais fun&ccedil;&otilde;es.</p>  <font size="3">     <br>    <p align="justify"><b>M&eacute;todo</b></p></font>      <p align="justify">Foi realizada uma busca de artigos cient&iacute;ficos nas bases de dados com publica&ccedil;&otilde;es nacionais e internacionais na &aacute;rea da sa&uacute;de: Bireme, <i>Web of Science </i>e <i>Pubmed, </i>abrangendo o per&iacute;odo de 2011 a 2000. Para tanto, utilizaram-se os descritores conexos atrav&eacute;s de &quot;e&quot; <i>(and): </i>reabilita&ccedil;&atilde;o cognitiva <i>(cognitive rehabilitation); </i>acidente vascular cerebral <i>(stroke). </i>Palavras relacionadas aos termos de busca n&atilde;o foram inclu&iacute;das.</p>      <p align="justify">O material selecionado seguiu os seguintes crit&eacute;rios de inclus&atilde;o: (a) estudos com dados emp&iacute;ricos como modalidade de produ&ccedil;&atilde;o cient&iacute;fica; (b) popula&ccedil;&atilde;o adulta como participantes; (c) dados com humanos; (d) ano de publica&ccedil;&atilde;o entre 2000 e 2011. A partir destes crit&eacute;rios, foi realizada uma leitura preliminar dos resumos encontrados, sendo que os trabalhos originais dos resumos selecionados foram buscados, quando dispon&iacute;veis. Foi feita uma leitura anal&iacute;tica dos artigos identificando a amostra estudada, o m&eacute;todo e os resultados.</p>  <font size="3">     <br>    <p align="justify"><b>Resultados</b></p></font>      <p align="justify">A pesquisa ocorreu em tr&ecirc;s bases de dados de estudos cient&iacute;ficos, uma nacional e duas internacionais. Foram inclu&iacute;dos artigos de metodologia cient&iacute;fica e dados emp&iacute;ricos, em ingl&ecirc;s. Ressalta-se que um total de 19 artigos na &iacute;ntegra foram inclu&iacute;dos na presente revis&atilde;o. Observou-se que seis artigos foram encontrados em ambas as bases de dados internacionais. Na <a href="#f1">Figura 1</a>, h&aacute; detalhes sobre os passos da busca e na <a href="img/revistas/apl/v31n1/v31n1a04t01.jpg" target="_blank">Tabela 1</a> est&atilde;o expostos os resultados da pesquisa.</p>      <p align="center"><a name="f1"></a><img src="img/revistas/apl/v31n1/v31n1a04f01.jpg"></p>      <p align="justify">Observou-se que as fun&ccedil;&otilde;es cognitivas trabalhadas com maior frequ&ecirc;ncia em programas de reabilita&ccedil;&atilde;o foram mem&oacute;ria, fun&ccedil;&otilde;es executivas, compreens&atilde;o e linguagem escrita. Tais dom&iacute;nios cognitivos foram relacionados ao progresso no desempenho em atividades da vida di&aacute;ria, como &eacute; poss&iacute;vel verificar na maioria dos estudos que se propuseram a avaliar tais vari&aacute;veis como Pyun et al. (2009), Sinotte e Coelho (2007), Vallat et al. (2005), Schindler, Kerkhoff, Karnath, Keller, e Goldenber (2002), e van Heugten, Dekker, Deelman, Stehmann-Saris, e Kinebanian (2000).</p>      ]]></body>
<body><![CDATA[<p align="justify">Em rela&ccedil;&atilde;o ao m&eacute;todo utilizado, observou-se o uso da realidade virtual em alguns estudos atrav&eacute;s da explora&ccedil;&atilde;o de ambientes virtuais. Tais ambientes se mostraram satisfat&oacute;rios quanto ao desempenho em atividades da vida di&aacute;ria (AVD), entretanto, interven&ccedil;&otilde;es baseadas na terapia ocupacional multidisciplinar tamb&eacute;m se mostrou eficaz para as AVDs. Esta &uacute;ltima utilizou-se do computador sem intera&ccedil;&atilde;o direta e do treinamento, armazenamento e processamento de fun&ccedil;&otilde;es como mem&oacute;ria de trabalho. Foi poss&iacute;vel observar ainda, algumas t&eacute;cnicas tais como: treino cognitivo e estrat&eacute;gias compensat&oacute;rias, sendo que alguns estudos n&atilde;o definiram suas interven&ccedil;&otilde;es.</p>      <p align="justify">Alguns estudos falharam quanto &agrave; descri&ccedil;&atilde;o mais detalhada das t&eacute;cnicas utilizadas durante a reabilita&ccedil;&atilde;o, entretanto, percebeu-se o uso de per&iacute;odos curtos e de alta frequ&ecirc;ncia na maioria deles. O tempo variou de 10 dias a 15 semanas, com n&uacute;mero de sess&otilde;es que variaram de um a sete vezes por semana, com dura&ccedil;&atilde;o m&iacute;nima de 40 minutos e m&aacute;xima de duas horas, sendo que a maior parte dos estudos relatou resultados satisfat&oacute;rios, com uma exce&ccedil;&atilde;o que n&atilde;o mostrou benef&iacute;cios em longo prazo. Um estudo apenas (Gourlay, Lun, &amp; Liya, 2000) n&atilde;o descreveu o tempo e n&uacute;mero de sess&otilde;es da reabilita&ccedil;&atilde;o.</p>      <p align="justify">Caracter&iacute;sticas de validade ecol&oacute;gica foram verificadas em interven&ccedil;&otilde;es adaptadas &agrave; casa do paciente submetido &agrave; reabilita&ccedil;&atilde;o, em intera&ccedil;&atilde;o direta com os familiares, assim como, em interven&ccedil;&otilde;es onde o pr&oacute;prio paciente optava pelo material a ser utilizado com base em sua prefer&ecirc;ncia, familiaridade e necessidade e, de atividades voltadas para o &acirc;mbito social como nos estudos de Pyun et al. (2009), Rand et al. (2010), Rand, Weiss, e Katz (2009), O'Connor, Cassidy, e Delargy (2005), Gourlay et al. (2000) e van Heugten et al. (2000). A satisfa&ccedil;&atilde;o do paciente quanto ao m&eacute;todo utilizado na reabilita&ccedil;&atilde;o foi considerada apenas no estudo de Georgiadis, Brennan, Barker, e Baron (2004).</p>      <p align="justify">Em rela&ccedil;&atilde;o &agrave;s emo&ccedil;&otilde;es, foi observado um n&uacute;mero reduzido de artigos que verificaram tais aspectos durante o trabalho de reabilita&ccedil;&atilde;o, sendo apenas o estudo de S&aacute;rk&aacute;mo et al. (2008). Este estudo apresentou melhoras no humor deprimido dos pacientes concomitante &agrave; melhora na mem&oacute;ria verbal e aten&ccedil;&atilde;o concentrada. Importante relatar que a maioria dos estudos reabilitou pacientes em per&iacute;odo cr&ocirc;nico ap&oacute;s o AVC, sendo que pouco deles a amostra foi constitu&iacute;da por pacientes na fase inicial da les&atilde;o vascular.</p>  <font size="3">     <br>    <p align="justify"><b>Discuss&atilde;o</b></p></font>      <p align="justify">Atrav&eacute;s da busca nas bases de dados, observou-se que n&atilde;o h&aacute;, na literatura brasileira, trabalhos sobre a reabilita&ccedil;&atilde;o cognitiva e AVC. Tal fato pode ser devido a limita&ccedil;&otilde;es quanto ao conhecimento dos preju&iacute;zos causados pelo AVC a n&iacute;vel social, cognitivo e emocional (Hommel et al., 2009), somado ao crescente interesse na reabilita&ccedil;&atilde;o de indiv&iacute;duos com doen&ccedil;as degenerativas (Manzine &amp; Pavarini, 2009). Ainda h&aacute; necessidade de encontrar o tipo e intensidade de reabilita&ccedil;&atilde;o que traga resultados e benef&iacute;cios ideais &agrave; essa popula&ccedil;&atilde;o neurol&oacute;gica (Fuhrer, 2003).</p>      <p align="justify">Na literatura internacional, os trabalhos s&atilde;o escassos e 11 de 19 descreveram as t&eacute;cnicas utilizadas. Os sobreviventes do AVC, geralmente, se deparam com incapacidades residuais tais como: dificuldades motoras, sensoriais, na linguagem e cognitivas como mem&oacute;ria, por exemplo. A curva que representa a trajet&oacute;ria da recupera&ccedil;&atilde;o das fun&ccedil;&otilde;es f&iacute;sicas e cognitivas afetadas pelo AVC atinge um plat&ocirc; aproximadamente seis meses ap&oacute;s o epis&oacute;dio, per&iacute;odo no qual o AVC &eacute; considerado cr&ocirc;nico. Num per&iacute;odo que varia de um m&ecirc;s a dois anos ap&oacute;s o AVC, os sobreviventes podem sofrer deteriora&ccedil;&atilde;o da funcionalidade, melhorar ou permanecer estabilizados na condi&ccedil;&atilde;o inicial (Skilbeck, 1996). Os achados do presente estudo corroboram com tais dados, os quais foram poss&iacute;veis verificar estudos que realizaram interven&ccedil;&otilde;es no per&iacute;odo agudo da les&atilde;o, e tamb&eacute;m, relataram que o n&iacute;vel de comprometimento na admiss&atilde;o de programas de reabilita&ccedil;&atilde;o est&aacute; relacionado &agrave; melhora no desempenho cognitivo, sendo que apenas um n&atilde;o mostrou tal rela&ccedil;&atilde;o.</p>      <p align="justify">Entre os estudos encontrados, foram observados programas de reabilita&ccedil;&atilde;o atrav&eacute;s da explora&ccedil;&atilde;o de ambientes virtuais, sendo alcan&ccedil;ado um n&iacute;vel de desempenho satisfat&oacute;rio nas atividades da vida di&aacute;ria (AVD) por meio dos recursos virtuais, dados estes corroborados pelo estudo de Laver, George, Thomas, Deutsch, e Crotty (2011). Esses resultados indicam que a realidade virtual lida com tarefas de validade ecol&oacute;gica, as quais s&atilde;o de grande efetividade para o dia-a-dia dos pacientes e proporciona o reaprendizado de dom&iacute;nios funcionais espec&iacute;ficos (Zhang et al., 2002). Os ganhos adquiridos nos programas de reabilita&ccedil;&atilde;o, transferidos, em sua maioria &agrave; vida do paciente, de fato, proporcionam vantagens de uso, pois fornecem maior motiva&ccedil;&atilde;o ao usu&aacute;rio (Walker, Leonardi-Bee, &amp; Bath, 2004). Recursos que auxiliam na compreens&atilde;o de objetos ou situa&ccedil;&otilde;es abstratas, permitem a observa&ccedil;&atilde;o de cenas em diferentes dist&acirc;ncias encorajando assim, a participa&ccedil;&atilde;o ativa de quem o manipula (Cardoso et al., 2004).</p>      <p align="justify">Progressos no desempenho das AVDs da mesma forma foram observados em estudos que utilizaram o m&eacute;todo da terapia ocupacional, de origem multidisciplinar, com tarefas utilizando a tela do computador, por&eacute;m, sem intera&ccedil;&atilde;o direta, e com treinamento, armazenamento e processamento de fun&ccedil;&otilde;es como mem&oacute;ria de trabalho verbal. Tarefas voltadas ao cotidiano do paciente s&atilde;o de extrema import&acirc;ncia considerando que o objetivo &eacute; alcan&ccedil;ar o retorno a vida independente, as atividades di&aacute;rias como retorno &agrave; atividade laboral, aos estudos e a administra&ccedil;&atilde;o de recursos (Spooner &amp; Pachana, 2006). Tais resultados poder&atilde;o ser conquistados com a participa&ccedil;&atilde;o ativa da fam&iacute;lia (Manzine &amp; Pavarini, 2009).</p>      ]]></body>
<body><![CDATA[<p align="justify">A reabilita&ccedil;&atilde;o cognitiva deve ser funcional e ecologicamente v&aacute;lida baseada, principalmente, na combina&ccedil;&atilde;o de estrat&eacute;gias compensat&oacute;rias e treinamentos (Bennett, 2001). Na presente revis&atilde;o, a maioria dos estudos que descreveu em maiores detalhes o m&eacute;todo e t&eacute;cnicas empregadas na reabilita&ccedil;&atilde;o trabalhou utilizando-se mais do treino cognitivo do que de estrat&eacute;gia compensat&oacute;ria. O treino cognitivo pode produzir efeitos importantes e de prote&ccedil;&atilde;o no desempenho neuropsicol&oacute;gico em longo prazo (Valenzuela &amp; Sachdev, 2009). Entretanto, &eacute; importante ressaltar que a finalidade de um estudo cient&iacute;fico de dados emp&iacute;ricos requer uma metodologia breve, sendo que na cl&iacute;nica, os per&iacute;odos tendem a estender-se, o que pode ter influ&ecirc;ncia na maior utiliza&ccedil;&atilde;o de alguns m&eacute;todos e n&atilde;o de outros.</p>      <p align="justify">O objetivo da reabilita&ccedil;&atilde;o cognitiva &eacute; a recupera&ccedil;&atilde;o do paciente ao mais alto n&iacute;vel psicol&oacute;gico, f&iacute;sico e social poss&iacute;vel, que ocorre atrav&eacute;s da estimula&ccedil;&atilde;o cognitiva e da melhora na aprendizagem devido a repeti&ccedil;&otilde;es frequentes, uma vez que, as tarefas de repeti&ccedil;&atilde;o produzem mudan&ccedil;as neuronais alterando as sinapses e o n&uacute;mero de conex&otilde;es (Abrisqueta-Gomez &amp; Santos, 2006). No &uacute;nico estudo que investigou correlatos neurobiol&oacute;gicos a partir do treino cognitivo (Vitali et al., 2007) foi mostrado efeitos positivos e uma poss&iacute;vel indu&ccedil;&atilde;o a reorganiza&ccedil;&atilde;o cerebral. Tal resultado sugere que paciente poder&atilde;o se beneficiar de mudan&ccedil;as a n&iacute;vel cerebral mesmo no per&iacute;odo cr&ocirc;nico da les&atilde;o. Em casos de maior gravidade, torna-se necess&aacute;rio o apoio de familiares at&eacute; que o aprendizado seja unificado a rotina do paciente de maneira adequada (Santos, Figueiredo, &amp; Teixeira, 2011). Preju&iacute;zos cognitivos, principalmente de linguagem e de fun&ccedil;&otilde;es executivas parecem estar estreitamente relacionados com o bom desempenho em tarefas da vida di&aacute;ria e participa&ccedil;&atilde;o em atividades sociais (Viscogliosi et al., 2011), tamb&eacute;m em fases cr&ocirc;nicas da les&atilde;o (Novakovic-Agopian et al., 2011).</p>      <p align="justify">A utiliza&ccedil;&atilde;o de baterias neuropsicol&oacute;gicas possibilita uma an&aacute;lise mais apurada dos preju&iacute;zos cognitivos em pacientes ap&oacute;s AVC. Na revis&atilde;o realizada, foi poss&iacute;vel observar programas de reabilita&ccedil;&atilde;o voltados principalmente para a linguagem, fun&ccedil;&otilde;es executivas e mem&oacute;ria, incluindo mem&oacute;ria de trabalho. Estudo de Passier et al. (2010) mostrou que de 111 indiv&iacute;duos ap&oacute;s AVC hemorr&aacute;gico, 94.6% deles relataram pelo menos uma queixa em seu desempenho cognitivo, como tamb&eacute;m, emocional as quais prejudicavam as AVDs. As queixas mais frequentes foram lentid&atilde;o mental, mem&oacute;ria de trabalho e aten&ccedil;&atilde;o, estando &agrave;s quest&otilde;es emocionais relacionadas com o funcionamento cognitivo de tais fun&ccedil;&otilde;es. Este estudo est&aacute; de acordo com algumas das fun&ccedil;&otilde;es cognitivas trabalhadas nos programas de reabilita&ccedil;&atilde;o na presente revis&atilde;o, assim como, refor&ccedil;a a influ&ecirc;ncia das altera&ccedil;&otilde;es emocionais e cognitivas nas atividades di&aacute;rias, frequentemente estimuladas nos estudos de reabilita&ccedil;&atilde;o.</p>      <p align="justify">Considerando os aspectos da linguagem, como por exemplo, a afasia, esta &eacute; considerada como o mais complexo dist&uacute;rbio da fala ap&oacute;s doen&ccedil;as c&eacute;rebro vascular. Na presente revis&atilde;o, apesar de ter sido uma das fun&ccedil;&otilde;es que apareceu com maior frequ&ecirc;ncia, ainda, pouca aten&ccedil;&atilde;o tem sido dada a paciente com dist&uacute;rbios de linguagem, tanto de express&atilde;o como de compreens&atilde;o, sendo que a maioria realizou estudos de caso. Parece que h&aacute; uma escassez de estudos se propondo a reabilitar pacientes com dificuldades severas de linguagem, limitando-se apenas ao trabalho com dist&uacute;rbios de n&iacute;vel leva a moderado utilizando-se do treino cognitivo e de altas frequ&ecirc;ncias de exposi&ccedil;&atilde;o ao mesmo est&iacute;mulo. Estudo com 52 pacientes diagnosticados af&aacute;sicos (afasia global, afasia de Broca e anomia), ap&oacute;s primeiro epis&oacute;dio de AVC apenas 11 receberam algum tratamento fonoaudi&oacute;logo ap&oacute;s deixar o hospital, sendo que destes, ap&oacute;s um ano, a afasia evoluiu para uma forma mais leve em um n&uacute;mero significativo de pacientes (Klebic, Salihovic, Softic, &amp; Salihovic, 2011). Tal dado indica que dist&uacute;rbios de linguagem, como as afasias, podem ser tratados com interven&ccedil;&otilde;es adequadas e assim alcan&ccedil;ar um resultado satisfat&oacute;rio e est&aacute;vel ao longo do tempo.</p>      <p align="justify">Os estudos revisados tem mostrado o trabalho individual de tal fun&ccedil;&atilde;o sendo que progressos poder&atilde;o ser melhor alcan&ccedil;ados com o trabalho integrado. Dessa forma, resultados mais significativos no desempenho cognitivo dos pacientes poder&atilde;o ser percebidos quanto comparados a resultados de interven&ccedil;&otilde;es com foco mais individualizado, a partir de tarefas que abrangem de maneira mais especifica determinadas fun&ccedil;&otilde;es (Hatfield et al., 2005). Estudo de Lambon, Snell, Fillingham, Conroy, e Sage (2010) mostrou que o efeito combinado de habilidades cognitivas e lingu&iacute;sticas resulta em melhores resultados na reabilita&ccedil;&atilde;o da anomia, sendo que o estudo de Vitali et al. (2007) encontrado na presente revis&atilde;o utilizou t&eacute;cnicas semelhante ao estudo acima citado, tamb&eacute;m, com resultados satisfat&oacute;rios. Os efeitos da m&uacute;sica na linguagem no estudo de S&aacute;rk&aacute;mo et al. (2008) foram positivos, o que est&aacute; de acordo com o estudo de Kim e Tomaino (2008) que mostrou progresso em indiv&iacute;duos com afasia n&atilde;o-fluente atrav&eacute;s da terapia musical.</p>      <p align="justify">Outro aspecto importante &eacute; a intensidade do tratamento. Pesquisa tem mostrado que paciente com dist&uacute;rbios de linguagem se beneficiam de tratamentos intensos, em torno de 5-10 horas por semana (Breitenstein et al., 2009). No presente estudo, foi poss&iacute;vel observar que a reabilita&ccedil;&atilde;o dos pacientes foi feita atrav&eacute;s de sess&otilde;es di&aacute;rias com dura&ccedil;&atilde;o de uma hora cada dia o que &eacute; considerado adequado ao estudo acima citado.</p>      <p align="justify">No in&iacute;cio da reabilita&ccedil;&atilde;o, o paciente deve enfrentar o trabalho integrado de multicomponentes, incluindo aspectos mentais e de tarefas de manipula&ccedil;&atilde;o que parecem ter efeitos duradouros, podendo ser percebidos em longo prazo ap&oacute;s t&eacute;rmino das interven&ccedil;&otilde;es. O uso de tarefas envolvendo, por exemplo, fun&ccedil;&otilde;es executivas como resolu&ccedil;&atilde;o de problemas, pode estar associadas a melhores resultados em express&atilde;o verbal e compreens&atilde;o auditiva. Tal fato ocorre devido ao envolvimento do pensamento cr&iacute;tico, flexibilidade mental, integra&ccedil;&atilde;o de diversos componentes da informa&ccedil;&atilde;o, e manipula&ccedil;&atilde;o mental na capacidade para resolver problemas (Roca et al., 2010). O sucesso na reabilita&ccedil;&atilde;o das fun&ccedil;&otilde;es executivas, bem como, da mem&oacute;ria de trabalho est&aacute; relacionada ao envolvimento com as atividades di&aacute;rias dos pacientes fazendo com que os mesmos se deparem com quest&otilde;es que demandem resolu&ccedil;&atilde;o e planejamento adaptados aos objetivos reais conforme evidenciado por Levine et al. (2011). Nos estudos encontrados na presente revis&atilde;o, as interven&ccedil;&otilde;es objetivando reabilitar fun&ccedil;&otilde;es executivas e mem&oacute;ria de trabalho n&atilde;o foram voltadas a tarefas rotineiras e familiares ao indiv&iacute;duo como nos estudos de Westerberg et al. (2007) e Vallat et al. (2005), entretanto, os resultados foram satisfat&oacute;rios.</p>      <p align="justify">Outra fun&ccedil;&atilde;o cognitiva reabilitada foi a mem&oacute;ria, principalmente verbal, que apareceu com frequ&ecirc;ncia entre as fun&ccedil;&otilde;es reabilitadas na presente revis&atilde;o atrav&eacute;s, principalmente, do treino cognitivo e do aprendizado de novas habilidades. Tal fun&ccedil;&atilde;o geralmente &eacute; trabalhada com a utiliza&ccedil;&atilde;o de estrat&eacute;gias compensat&oacute;rias, sendo que os benef&iacute;cios tendem a se manter ao longo do tempo. Estas estrat&eacute;gias geralmente est&atilde;o relacionadas com a capacidade de o paciente funcionar de maneira independente (Alladi, Meena, &amp; Kaul, 2002; Gross &amp; Rebok, 2011). Complementando os dados acima, sendo o AVC a principal causa de incapacidade a longo prazo, o objetivo da reabilita&ccedil;&atilde;o &eacute; promover melhoras ao longo do tempo, minimizando defici&ecirc;ncias que se mantenham est&aacute;veis (Ifejika-Jones &amp; Barrett, 2011).</p>      <p align="justify">Considerando a import&acirc;ncia do n&uacute;mero de epis&oacute;dios, alguns estudos relataram reabilitar pacientes ap&oacute;s o primeiro epis&oacute;dio de AVC, sendo que a maioria n&atilde;o descreveu tal dado. Esta vari&aacute;vel se torna importante devido ao fato de que, m&uacute;ltiplos infartos cerebrais podem ocasionar quadros de dem&ecirc;ncia vascular, principalmente, les&otilde;es na regi&atilde;o cortical, como tamb&eacute;m, diminui&ccedil;&atilde;o do desempenho cognitivo, como na mem&oacute;ria e velocidade de percep&ccedil;&atilde;o (Arvanitakis, Leurgans, Barnes, Bennett, &amp; Schneider, 2011). Estes dados corroboram com a import&acirc;ncia de se considerar as m&uacute;ltiplas ocorr&ecirc;ncias de AVC, assim como, sinais que indiquem prov&aacute;vel presen&ccedil;a de quadro demencial para o delineamento adequado de um programa de reabilita&ccedil;&atilde;o para essa popula&ccedil;&atilde;o, como treino de mem&oacute;ria, de atividades funcionais e facilita&ccedil;&atilde;o do uso de estrat&eacute;gias compensat&oacute;rias atrav&eacute;s de apoio externo paralelamente ao trabalho dos profissionais (Abrisqueta-Gomez &amp; Ponce, 2006).</p>      <p align="justify">Apenas o estudo de Georgiadis et al. (2004) relatou a satisfa&ccedil;&atilde;o dos pacientes quanto ao m&eacute;todo utilizado, considerando tal vari&aacute;vel fator importante para o sucesso das interven&ccedil;&otilde;es e resultado final do programa de reabilita&ccedil;&atilde;o cognitiva, sendo que isolamento social, ansiedade e depress&atilde;o s&atilde;o frequentes em sobreviventes de dano cerebral (Gainotti, 1993).</p>  <font size="3">     ]]></body>
<body><![CDATA[<br>    <p align="justify"><b>Considera&ccedil;&otilde;es finais</b></p></font>      <p align="justify">O presente estudo refor&ccedil;a a import&acirc;ncia em ganhos funcionais da reabilita&ccedil;&atilde;o cognitiva ap&oacute;s AVC. A velocidade da recupera&ccedil;&atilde;o e o grau de adapta&ccedil;&atilde;o variam de indiv&iacute;duo para indiv&iacute;duo e dependem da gravidade das les&otilde;es e do engajamento em processos de reabilita&ccedil;&atilde;o eficientes. Em um programa de reabilita&ccedil;&atilde;o, &eacute; preciso considerar o tipo de patologia, limita&ccedil;&otilde;es funcionais relacionadas ao meio ambiente e aos aspectos pessoais apresentadas pelo paciente atrav&eacute;s de uma vis&atilde;o mais hol&iacute;stica.</p>      <p align="justify">A expans&atilde;o de estudos sobre tal tema torna-se essencial para o aprimoramento dos programas e divulga&ccedil;&atilde;o das metodologias utilizadas para popula&ccedil;&otilde;es espec&iacute;ficas, como por exemplo, indiv&iacute;duos v&iacute;timas de les&otilde;es neurol&oacute;gicas de origem vascular. Foram observados preju&iacute;zos, principalmente, relacionados &agrave; linguagem ap&oacute;s les&atilde;o por AVC e uma crescente investiga&ccedil;&atilde;o de m&eacute;todos virtuais no processo de reabilita&ccedil;&atilde;o com o objetivo de transferir os ganhos do processo terap&ecirc;uticos &agrave; vida di&aacute;ria. Grande parte dos estudos relatou progresso e resultados positivos ap&oacute;s as interven&ccedil;&otilde;es, entretanto, a maioria dos pacientes teve suas limita&ccedil;&otilde;es detectadas e tratadas na fase aguda, sendo que muitos dos pacientes foram estimulados em per&iacute;odo agudo, ap&oacute;s primeiro epis&oacute;dio de AVC. A reabilita&ccedil;&atilde;o cognitiva exige o envolvimento ativo do paciente, bem como de sua fam&iacute;lia ou indiv&iacute;duos pr&oacute;ximos, objetivando maior sucesso no desenvolvimento do programa de reabilita&ccedil;&atilde;o voltado a rotina e espa&ccedil;os frequentados pelo paciente.</p>      <p align="justify">O presente estudo limitou-se as descri&ccedil;&otilde;es &quot;reabilita&ccedil;&atilde;o cognitiva e acidente vascular cerebral&quot;, ou seja, buscou estudos bastante espec&iacute;ficos e limitados a popula&ccedil;&atilde;o ap&oacute;s AVC.Estudos ainda s&atilde;o necess&aacute;rios para investigar modelos de reabilita&ccedil;&atilde;o, com diferentes intensidades e metodologias, a fim de melhor abordar as t&eacute;cnicas mais utilizadas e de melhor resultados.
