El sangrado por várices esofágicas representa una urgencia quirúrgica que demanda intervención inmediata. El tratamiento de las várices esofágicas por abordaje endoscópico o por TIPS y, por otra parte, el advenimiento del trasplante hepático como un procedimiento estándar, ha disminuido la frecuencia con que se realizan tales derivaciones. Sin embargo, estos procedimientos tienen indicaciones precisas, tanto como intervenciones de urgencia en algunos casos de hemorragia esofágica, como en forma electiva en determinados pacientes con hipertensión portal. Hemos hecho una revisión de la literatura y de los conocimientos actuales de las derivaciones portocava a raíz de un caso tratado en forma exitosa en el Hospital Occidente de Kennedy en Bogotá, Colombia.]]></p></abstract>

However, these procedures have precise indications, both as emergency operations in some cases of esophageal varicose hemorrhage as well as elective procedures in selected patients with portal hypertension. We hereby present a literature review of the &ldquo;state of the art&rdquo; of portocaval shunts motivated by a patient that was successfully treated at Hospital Occidente de Kennedy in Bogot&aacute;, Colombia. </p>     <p>Key words: portal hypertension; esophageal and gastric varices; gastyroiestinal hemorrhage; surgical portocaval shunt.</p> <hr size=1> <h4>Introducci&oacute;n</h4>     <p> Cuando el manejo endosc&oacute;pico para controlar un sangrado de las v&aacute;rices falla, y los TIPS (transjugular intrahepatic portosystemic shunt), no controlan el sangrado o no est&aacute;n disponibles, un procedimiento de descompresi&oacute;n puede salvar la vida del paciente. Determinadas derivaciones portocava son procedimientos quir&uacute;rgicos seguros y eficaces en el control de la hemorragia y los s&iacute;ntomas relacionados con el hiperesplenismo <sup>1</sup>, aun en ni&ntilde;os <sup>2</sup>, exceptuando las derivaciones portocava latero-laterales que raramente son eficaces en el manejo del sangrado agudo del es&oacute;fago. En casos de cirrosis es preferible realizar en forma electiva t&eacute;cnicas que preserven el flujo portal, tales como el shunt espleno-renal distal selectivo de Warren o una pr&oacute;tesis de peque&ntilde;o calibre de interposici&oacute;n, el procedimiento de Sarfeh. Generalmente, se acepta que pacientes con buen pron&oacute;stico, como los Child-Pugh A y B, se consideren buenos candidatos para la derivaci&oacute;n portocava, y en pacientes con trombosis extrahep&aacute;tica de la porta estas derivaciones controlan sangrados recurrentes y evitan la necesidad de endoscopias repetidas sin causar deterioro de la funci&oacute;n hep&aacute;tica. Las desvascularizaciones y otros procedimientos de ablaci&oacute;n de las v&aacute;rices se reservan para aquellos pacientes que no poseen vasos tributarios de la porta que permitan realizar una derivaci&oacute;n <sup>3</sup>. </p>     <p>El sangrado digestivo alto por v&aacute;rices esof&aacute;gicas es la principal causa de mortalidad en pacientes con hipertensi&oacute;n portal <sup>4</sup>. Se calcula que la mortalidad inmediata por un sangrado varicoso no controlado en pacientes cirr&oacute;ticos oscila entre 5 y 8 %, y la mortalidad global es de 20 a 50 % en las primeras seis semanas <sup>4,5</sup>. Se estima que, aun en manos expertas, la hemostasia por v&iacute;a endosc&oacute;pica puede fallar hasta en 20 % de los casos y es en estas emergencias cuando los m&eacute;todos descompresores cobran vigencia y son la siguiente l&iacute;nea de tratamiento. La mortalidad reportada oscila entre 20 y 60 % con la derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular (TIPS) y con las derivaciones quir&uacute;rgicas <sup>6-8</sup>. </p>     <p>La baja mortalidad (20 %) reportada por algunos grupos de expertos en el manejo de derivaciones quir&uacute;rgicas en pacientes cirr&oacute;ticos, puede explicarse porque con una r&aacute;pida intervenci&oacute;n quir&uacute;rgica y un riguroso seguimiento se evitan episodios de shock <sup>9,10</sup>. </p> <h4>Revisi&oacute;n de la literatura </h4>     <p>El prop&oacute;sito de las derivaciones, o shunts, portocava o portosist&eacute;micos es reducir la presi&oacute;n en el sistema venoso porta, pero manteniendo la perfusi&oacute;n del h&iacute;gado y tratando de evitar la encefalopat&iacute;a y el deterioro de la funci&oacute;n hep&aacute;tica. </p>     <p>La historia del manejo quir&uacute;rgico para el tratamiento de la hipertensi&oacute;n portal se inici&oacute; en San Petersburgo (Rusia) en 1877, con Nicolai V. Eck, un joven cirujano interesado en el problema de la ascitis y su relaci&oacute;n con la fibrosis hep&aacute;tica. &Eacute;l concibi&oacute; la idea de una f&iacute;stula veno-venosa para derivar el flujo sangu&iacute;neo y mejorar la congesti&oacute;n venosa intestinal. Public&oacute; la descripci&oacute;n de su t&eacute;cnica de f&iacute;stula veno-venosa en un perro, en un corto art&iacute;culo en el Military Medical Journal <sup>11</sup>, y concluy&oacute; que era seguro y factible practicarla en humanos. </p>     <p>Los estudios publicados en 1983 por Ivan Petrovich Pavlov, m&eacute;dico del Instituto de Medicina Experimental de San Petersburgo, dieron nuevas luces al entendimiento de la fisiolog&iacute;a hep&aacute;tica <sup>11</sup>. Se populariz&oacute; la f&iacute;stula de Eck para el estudio de la fisiolog&iacute;a hep&aacute;tica y, debido a la encefalopat&iacute;a secundaria las alteraciones nutricionales producidas por la derivaci&oacute;n, tom&oacute; fuerza la &ldquo;epiplopexia&rdquo; en humanos, cirug&iacute;a descrita por Drumond y Morrison. </p>     ]]></body>
<body><![CDATA[<p>Posteriormente, surgi&oacute; la llamada com&uacute;nmente cirug&iacute;a de Talma-Morrison, basada en el principio de incrementar la &ldquo;colaterizaci&oacute;n&rdquo; porto-sist&eacute;mica <sup>12</sup>, adhiriendo el epipl&oacute;n al peritoneo parietal. Este procedimiento con el tiempo fue perdiendo adeptos, por sus pobres resultados en el control del sangrado y la ascitis.</p>     <p> La era moderna de las derivaciones porto-sist&eacute;micas se inici&oacute; con Allen O. Whipple, quien en 1945 public&oacute; 10 casos exitosos en pacientes cirr&oacute;ticos, cinco derivaciones espleno-renales y cinco derivaciones porto-cava <sup>11</sup>.</p>     <p> Posteriormente, el Hospital General de Massachusetts en cabeza de Robert R. Linton public&oacute; su experiencia con 125 pacientes con derivaci&oacute;n quir&uacute;rgica realizados entre 1945 y 1954, demostrando en todos ellos hipertensi&oacute;n portal por presencia de varices esof&aacute;gicas en esofagograma (m&eacute;todo diagnostico est&aacute;ndar para la fecha). Los procedimientos realizados fueron derivaciones esplenorrenales (23,8 %), derivaci&oacute;n portocava (40,5 %) y derivaci&oacute;n tipo makeshift, que se refiere a derivaciones entre tributarias peque&ntilde;as de la circulaci&oacute;n portal, como la vena ov&aacute;rica izquierda, la suprarrenal izquierda y la vena cava (20 %) en pacientes que hab&iacute;an sido esplenectomizados previamente o en quienes no fue posible usar la vena porta para la derivaci&oacute;n, informando una incidencia de sangrado postoperatorio de 12 % en el grupo de pacientes con cirrosis comprobada por histopatolog&iacute;a. Fue ligeramente mayor la tasa de resangrado en quienes se realiz&oacute; derivaci&oacute;n esplenorenal versus portocava. Otro punto interesante de este estudio fue la medici&oacute;n de la funci&oacute;n hep&aacute;tica previa al procedimiento y su evoluci&oacute;n, sin una tendencia clara a la mejor&iacute;a bioqu&iacute;mica. En el seguimiento a largo plazo de estos pacientes, los autores concluyeron que se hab&iacute;a demostrado mejor&iacute;a cl&iacute;nica, con disminuci&oacute;n de resangrado, recuperaci&oacute;n de masa muscular, disminuci&oacute;n de ascitis, retorno a sus actividades e incremento en su expectativa de vida <sup>13</sup>. </p>     <p>Las derivaciones porto-sist&eacute;micas para el tratamiento del sangrado varicoso por hipertensi&oacute;n portal tuvieron su auge a comienzos de los a&ntilde;os 70 y 80. Las derivaciones totales fueron remplazadas por las parciales, dada su menor incidencia de encefalopat&iacute;a hep&aacute;tica y el adecuado control de sangrado <sup>14</sup>, y entre las derivaciones parciales, las selectivas (esplenorrenal distal) presentaron inicialmente mejores resultados que las no selectivas (interposici&oacute;n de injerto en H) en el control del sangrado, la permeabilidad de la derivaci&oacute;n y la mejor&iacute;a de funci&oacute;n hep&aacute;tica. Cabe aclarar que, para la fecha, los injertos en H se realizaban con injerto de dacr&oacute;n y por ese entonces, no hab&iacute;a est&aacute;ndares claros sobre el di&aacute;metro que se deb&iacute;a usar en estos injertos <sup>15</sup>, punto que se fue aclarando con los resultados obtenidos por Sarfeh, quien demostr&oacute; en los a&ntilde;os 80 que la encefalopat&iacute;a y la trombosis del injerto se mejoraban al utilizar injertos de politetrafluoretileno (PTFE) en H de peque&ntilde;o di&aacute;metro, recomendando su uso de 8-10 mm como una atractiva alternativa a la derivaci&oacute;n est&aacute;ndar de la &eacute;poca <sup>16</sup>. </p>     <p>Prueba de ello fueron los numerosos estudios prospectivos publicados sobre derivaci&oacute;n espleno-renal distal (Warren), con supervivencia al a&ntilde;o de 86 a 88 % y a los cinco a&ntilde;os de 60 a 85 %, y las tasas de repetici&oacute;n del sangrado entre 6 y 11 % <sup>17-20</sup>. Las derivaciones porto-cava reportadas tambi&eacute;n mostraron unas buenas tasas de supervivencia al a&ntilde;o (82 a 92%) y a los 5 a&ntilde;os (65 %) <sup>21,22</sup>. Adem&aacute;s, reportaron una permeabilidad de los injertos de 97 % y 95 %, a los cuatro a&ntilde;os y siete a&ntilde;os, respectivamente. </p>     <p>El tratamiento de la hipertensi&oacute;n portal no fue ajeno a la evoluci&oacute;n de tecnolog&iacute;as menos invasivas en la atenci&oacute;n m&eacute;dica y quir&uacute;rgica en las &uacute;ltimas d&eacute;cadas. Una molestia equivalente al paso de un cat&eacute;ter venoso central y el no requerir una incisi&oacute;n abdominal ni anestesia general, hicieron r&aacute;pidamente popular el TIPS, cuya aplicaci&oacute;n es diez veces m&aacute;s frecuente que la derivaci&oacute;n quir&uacute;rgica para la prevenci&oacute;n del sangrado varicoso por hipertensi&oacute;n portal <sup>23</sup>. Sin embargo, este procedimiento no est&aacute; exento de complicaciones, como estenosis, trombosis o migraci&oacute;n de la endopr&oacute;tesis <sup>24</sup>. Se calcula que la mitad de los pacientes a quienes se les practica la derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular experimentan alg&uacute;n tipo de falla durante el primer a&ntilde;o, con el consiguiente riesgo de nuevo sangrado, hospitalizaci&oacute;n y utilizaci&oacute;n de recursos del sistema de salud <sup>25</sup>. </p>     <p>La derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular es altamente efectiva para el control del sangrado de las v&aacute;rices, logrando control hasta en el 90 % de los pacientes <sup>26</sup>, por eso es recomendada ampliamente como terapia de rescate cuando el manejo endosc&oacute;pico y farmacol&oacute;gico no logran controlar el sangrado y se deber&iacute;a realizar antes de que se instaure la falla multiorg&aacute;nica; en algunos casos se puede adicionar embolizaci&oacute;n yugular de las varices esof&aacute;gicas para incrementar el efecto hemost&aacute;tico <sup>27</sup>. Entre las contraindicaciones para su uso se incluye la insuficiencia cardiaca congestiva grave, la incompetencia tric&uacute;spidea grave, la hipertensi&oacute;n pulmonar grave (presi&oacute;n media pulmonar &gt;45 mm Hg) y algunas contraindicaciones relativas como son la obstrucci&oacute;n venosa portal y venosa hep&aacute;tica, la presencia de tumores extensos o la enfermedad hep&aacute;tica poliqu&iacute;stica extensa <sup>27, 28</sup>. </p>     <p>En el tratamiento del sangrado varicoso por hipertensi&oacute;n portal, el objetivo inicial es la reanimaci&oacute;n del paciente; se usan medicamentos vasoactivos y endoscopia digestiva alta para practicar ligadura o, en su defecto, escleroterapia <sup>12,29,30</sup>. </p>     <p>La derivaci&oacute;n portosist&eacute;mica intrahep&aacute;tica transyugular queda como una segunda l&iacute;nea de manejo, con la alternativa de la derivaci&oacute;n quir&uacute;rgica cuando no se controla el sangrado por este m&eacute;todo. La derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular controla 90% de los sangrados varicosos, con tasas muy bajas de nuevo sangrado <sup>31</sup>, pero, no en todas nuestras instituciones se cuenta con esta tecnolog&iacute;a, los servicios de remisi&oacute;n muchas veces son demorados y, en ocasiones, no se tiene disponibilidad de este recurso las 24 horas. Los pacientes se deterioran y entran r&aacute;pidamente en falla multiorg&aacute;nica en cuesti&oacute;n de horas, por eso, cuando est&eacute; indicado se deben ofrecer otras alternativas de derivaci&oacute;n accesibles a nuestro medio y recursos, como son las t&eacute;cnicas quir&uacute;rgicas.</p>     <p> En nuestro arsenal quir&uacute;rgico disponemos de dos tipos de procedimientos, la &ldquo;desvascularizaci&oacute;n&rdquo; y la cirug&iacute;a derivativa. El primero, o procedimiento de Sugiura, consiste en una amplia &ldquo;desvascularizaci&oacute;n&rdquo; paraes&oacute;fago-g&aacute;strica, con secci&oacute;n tranversal de la mucosa del es&oacute;fago, vaguectom&iacute;a selectiva con piloroplastia y esplenectom&iacute;a. Ha mostrado buenos resultados en el control del sangrado en pacientes no cirr&oacute;ticos en Jap&oacute;n, pero este resultado no se ha podido reproducir en occidente donde se presentan tasas de recurrencia de 30 a 50% <sup>32,33</sup>. Este procedimiento se reserva para pacientes que no sean candidatos para derivaci&oacute;n porto-sist&eacute;mica, derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular o trasplante hep&aacute;tico <sup>34</sup>.</p>     ]]></body>
<body><![CDATA[<p>Con el procedimiento modificado de Sugiura, o sea, sin secci&oacute;n tranversal esof&aacute;gica o esplenectom&iacute;a, se obtuvieron altas tasas de mortalidad (31,2 %) y morbilidad (57,1 %), en cirug&iacute;a de urgencias para el control del sangrado <sup>32,35</sup>.</p>     <p> Romero Torres describi&oacute; otro procedimiento de &ldquo;desvascularizaci&oacute;n&rdquo; en Per&uacute; en 1981, luego de cirug&iacute;a experimental en perros mediante sutura circunferencial de los dos tercios superiores del est&oacute;mago, en la que observ&oacute; que controlaba el sangrado por v&aacute;rices esof&aacute;gicas. Este procedimiento lo practic&oacute; en 10 pacientes con resultados satisfactorios <sup>36,37</sup>. En 1989, public&oacute; los resultados de una serie de 96 pacientes sometidos a sutura circunferencial g&aacute;strica, ligadura de arteria, vena g&aacute;strica izquierda y arteria espl&eacute;nica, con una mortalidad operatoria de 12,5 % <sup>38</sup>. No hay nuevas series reportadas a la fecha. </p>     <p>Por otro lado, contamos con t&eacute;cnicas de cirug&iacute;a de derivaci&oacute;n parcial, cuyo objetivo es mantener alg&uacute;n grado de perfusi&oacute;n hepatorrenal y un gradiente de presi&oacute;n portal, con lo cual se disminuye el riesgo de encefalopat&iacute;a y aceleraci&oacute;n de la falla hep&aacute;tica, y se logra controlar el sangrado al disminuir la presi&oacute;n portal de forma inmediata. En general, la derivaci&oacute;n espleno-renal (Warren), la porto-cava en H (Sarfeh) y la porto-cava latero-lateral, ofrecen control del sangrado en 95% de los casos, con mortalidad a los 30 d&iacute;as de 6 a 20 % <sup>39</sup>. </p>     <p>En un metan&aacute;lisis, Whalen, et al., en pacientes con clasificaci&oacute;n de Child grado A y B, compararon estas t&eacute;cnicas de derivaci&oacute;n quir&uacute;rgica con la derivaci&oacute;n porto-sist&eacute;mica intrahep&aacute;tica transyugular para el control del sangrado varicoso. Concluyeron que la derivaci&oacute;n quir&uacute;rgica deb&iacute;a promoverse como primera l&iacute;nea de tratamiento en estos casos, ya que proporcionaba una significativa mayor supervivencia a los dos a&ntilde;os y menor falla del injerto <sup>29, 39, 40</sup>. Por consiguiente, la derivaci&oacute;n quir&uacute;rgica porto-sist&eacute;mica debe promoverse como un tratamiento de primera l&iacute;nea, cuando no se logra el control del sangrado varicoso con el manejo m&eacute;dico y endosc&oacute;pico; lo anterior, con base en la mejor informaci&oacute;n cl&iacute;nica disponible a la fecha. </p>     <p>Como ejemplo, se presenta el caso de un paciente con sangrado digestivo alto masivo, sometido a cirug&iacute;a para controlar el sangrado mediante una derivaci&oacute;n porto-cava, cuya evoluci&oacute;n fue favorable. Se trata de un hombre de 38 a&ntilde;os que ingres&oacute; al servicio de urgencias del Hospital Occidente de Kennedy por hematemesis de 12 horas de evoluci&oacute;n. Se reanim&oacute; en&eacute;rgicamente con 9.000 ml de soluci&oacute;n cristaloide en el sitio de remisi&oacute;n, donde se registr&oacute; una hemoglobina de 6,0 g/dl. Se transfundi&oacute; con gl&oacute;bulos rojos empaquetados y se solicit&oacute; una endoscopia digestiva alta, no disponible por ser fin de semana y en horas de la noche. Present&oacute; un nuevo episodio masivo de hematemesis y melenas, con r&aacute;pido deterioro cl&iacute;nico y shock hipovol&eacute;mico por lo cual se llev&oacute; de inmediato a laparotom&iacute;a. Era el primer episodio de sangrado digestivo en un paciente previamente sano, sin antecedentes de abuso de alcohol ni t&oacute;xicos. </p>     <p>Se procedi&oacute; a practicar una derivaci&oacute;n porto-cava latero-lateral de emergencia, con implantaci&oacute;n de endopr&oacute;tesis de 8 mm recubierta con politetrafluoroetileno (figuras <a href="#figura1">1</a> y <a href="#figura2">2</a>). Se dej&oacute; laparostom&iacute;a por el riesgo de s&iacute;ndrome de compartimentos. El tiempo quir&uacute;rgico de la anastomosis fue de una hora. La biopsia hep&aacute;tica demostr&oacute; cirrosis micronodular activa. El ultrasonido d&uacute;plex de porta demostr&oacute; permeabilidad del injerto a los 15 d&iacute;as y, las endoscopias de control, v&aacute;rices esof&aacute;gicas sin signos de sangrado y se remiti&oacute; al Grupo de Trasplante Hep&aacute;tico. El seguimiento a los tres meses demostr&oacute; una evoluci&oacute;n posoperatoria favorable y sin nuevos episodios de sangrado. </p>     <p>        <center>     <a name="figura1"></a>    <br>     <img src="img/revistas/rcci/v26n3/v26n3a7f1.gif">   </center> </p>     <p>        ]]></body>
<body><![CDATA[<center>     <a name="figura2"></a>    <br>     <img src="img/revistas/rcci/v26n3/v26n3a7f2.gif">   </center> </p> <h4>Discusi&oacute;n </h4>     <p>Lecciones aprendidas en el Hospital Occidente de Kennedy: de las 4 derivaciones portocava a la fecha, 3 se han realizado en el ultimo a&ntilde;o, todas por sangrado de v&aacute;rices secundario a hipertensi&oacute;n portal, tres de ellas sin respuesta al manejo m&eacute;dico instaurado en la Unidad de Cuidados Intensivos y sin respuesta al manejo endosc&oacute;pico. En la <a href="#figura3">figura 3</a> se muestra otro caso el cual se inici&oacute; como sangrado masivo de v&iacute;as digestivas altas. </p>     <p>        <center>     <a name="figura3"></a>    <br>     <img src="img/revistas/rcci/v26n3/v26n3a7f3.gif">   </center> </p>     <p>Es importante resaltar que el manejo derivativo quir&uacute;rgico es una opci&oacute;n valida, vigente y que cobra fuerza. Ante la evidencia disponible, el tipo de derivaci&oacute;n deber&iacute;a escogerse a la luz del pron&oacute;stico, variaciones anat&oacute;micas y experiencias quir&uacute;rgicas <sup>41</sup>; pueden salvar la vida del paciente y no contraindica posteriormente un trasplante hep&aacute;tico <sup>42</sup>.</p>     <p>La decisi&oacute;n de llevar a cirug&iacute;a depende de la respuesta al manejo m&eacute;dico y endosc&oacute;pico instaurado <sup>29</sup>, pero en caso de fallar, contemporizar su manejo de forma &ldquo;expectante&rdquo; usualmente conllevan a un deterioro cl&iacute;nico, hemodin&aacute;mico, empeoramiento de su clasificaci&oacute;n de Child, que influye directamente en morbilidad y mortalidad, por lo cual diferir este procedimiento disminuye la posibilidad de un desenlace favorable.</p> <h4>Bibliograf&iacute;a</h4>     <!-- ref --><p>1.
