Las diferentes estrategias para la reducción del dolor posoperatorio en una herniorrafia inguinal, demuestran variables grados de efectividad clínica. Se hace una revisión crítica de la literatura científica, evaluando la efectividad de la fijación de la malla con pegantes biológicos en cirugía laparoscópica y abierta (técnica de Lichtenstein y tapón de malla), frente a la no fijación o a la fijación convencional. Métodos. Se hizo una búsqueda de la literatura científica siguiendo la metodología Best BETs (Best Evidence Topics), para identificar artículos que compararan fijar la malla durante una herniorrafia inguinal con pegantes biológicos contra no fijarla. Los artículos se revisaron según los criterios de apreciación del Journal of the American Medical Association. Se extrajeron los datos de la efectividad de la intervención y se analizaron los desenlaces primarios, como el dolor posoperatorio, la reproducción, la infección, el seroma y el hematoma. Como desenlaces secundarios, se analizaron el costo, el tiempo de estancia hospitalaria, el retorno laboral y el tiempo quirúrgico. Resultados. Se identificaron once estudios de asignación aleatoria. En la herniorrafia transabdominal preperitoneal, la utilización de pegantes biológicos puede reducir el dolor posoperatorio al ser evaluado por la escala visual análoga. Esta ventaja no está completamente establecida en la herniorrafia abierta. En la herniorrafia con técnica extraperitoneal total, el dolor posoperatorio es mayor con el uso de pegantes biológicos. En todas las vías y técnicas de herniorrafia (técnica transabdominal preperitoneal, extraperitoneal total, o abierta) no se documentan diferencias en la tasa de reproducción ni de infección, y es variable la tasa del seroma y el hematoma. Es mayor el costo cuando se utilizan pegantes biológicos. Conclusiones. No se recomienda la utilización de forma rutinaria de pegantes biológicos para la fijación de la malla durante una herniorrafia inguinal.]]></p></abstract>

Methods: A search of the scientific literature following the Best BETs (Best Evidence Topics) methodology was carried out in order to identify articles that compare the mesh fixation during inguinal hernia repair with biological glue against non fixation or conventional fixation. The articles were reviewed according to the criteria for assessing literature of the Journal of the American Medical Association. We extracted data on the effectiveness of the intervention and analyzed primary outcomes such as postoperative pain, bleeding, hematoma, infection, and seroma. As secondary outcomes we analyzed the cost, length of hospital stay, return to work and operating time. Results: Eleven randomized studies were identified. In transabdominal preperitoneal herniorrhaphy (TAPP) the use of biological glue may reduce postoperative pain (assessed by visual analog scale). This advantage is not fully established in the open herniorrhaphy. In the total extraperitoneal herniorrhaphy (TEP), postoperative pain is greater with the use of biological glues. No documented differences appear in the rates of recurrence or infection, and the frequency of seroma and hematoma appeared variable in all techniques for hernia repair (TAPP, TEP and open). <body><![CDATA[<p><b>Resultados</b>. Se identificaron once estudios de asignaci&oacute;n aleatoria. En la herniorrafia transabdominal preperitoneal, la utilizaci&oacute;n de pegantes biol&oacute;gicos puede reducir el dolor posoperatorio al ser evaluado por la escala visual an&aacute;loga. Se han empleado en procedimientos vasculares, de trauma, de trasplantes, de t&oacute;rax, cardiacos, y de cirug&iacute;a anal y urol&oacute;gica, entre otros, demostrando variables grados de efectividad (1,2).</p>     <p>A partir de la creciente experiencia en la utilizaci&oacute;n de los pegantes biol&oacute;gicos en diversos procedimientos, durante la &uacute;ltima d&eacute;cada se han indicado en la herniorrafia inguinal, espec&iacute;ficamente en la fijaci&oacute;n de materiales prot&eacute;sicos (mallas). Los fundamentos cl&iacute;nicos que han justificado el desarrollo de alternativas experimentales para la fijaci&oacute;n de la malla, entre las cuales se encuentra el uso de pegantes biol&oacute;gicos (3), radican en la necesidad de modificar ciertos desenlaces, como el dolor y la inguinodinia posoperatoria, para algunos autores (4-6). Si bien las indicaciones precisas para la utilizaci&oacute;n de pegantes biol&oacute;gicos en herniorrafia inguinal no se han establecido completamente y a&uacute;n no se recomiendan rutinariamente (7,8), existen diversos experimentos cl&iacute;nicos que buscan establecer su efectividad en comparaci&oacute;n con la fijaci&oacute;n convencional de la malla, tanto en cirug&iacute;a abierta como laparosc&oacute;pica, con suturas y grapas.</p>     ]]></body>
<body><![CDATA[<p>Los pegantes biol&oacute;gicos que com&uacute;nmente se han utilizado para la fijaci&oacute;n de la malla, son el pegamento de fibrina (fibrin glue, FG) y el N-butil-2-cianoacrilato (N-butyl-2-cyanoacrylate, N-BCA). El primero de ellos es una mezcla de dos componentes principales: fibrin&oacute;geno y trombina. Ambos se cargan en jeringas separadas que desembocan en una v&iacute;a com&uacute;n durante la aplicaci&oacute;n. Cuando se conjugan, la trombina convierte el fibrin&oacute;geno en fibrina por acci&oacute;n enzim&aacute;tica, a una velocidad determinada por la concentraci&oacute;n de protrombina. La soluci&oacute;n m&aacute;s concentrada de protrombina produce un co&aacute;gulo de fibrina en diez segundos y la soluci&oacute;n m&aacute;s diluida de protrombina forma el co&aacute;gulo unos sesenta segundos despu&eacute;s de la aplicaci&oacute;n del pegante en el tejido (9-13). El segundo es un mon&oacute;mero sint&eacute;tico que se polimeriza r&aacute;pidamente al contacto con fluidos, sangre o tejidos, para formar un adhesivo fuerte y biodegradable (14,15).</p>     <p>Si bien esta t&eacute;cnica a&uacute;n no es aceptada de forma rutinaria, es una estrategia v&aacute;lida y promisoria en la herniorrafia inguinal. Frente a los avances quir&uacute;rgicos y la creciente literatura cient&iacute;fica disponible, en este estudio se eval&uacute;an los resultados de la fijaci&oacute;n con pegantes biol&oacute;gicos en comparaci&oacute;n con otros tipos de fijaci&oacute;n de la malla o la no fijaci&oacute;n durante la herniorrafia inguinal, laparosc&oacute;pica y abierta, y su impacto potencial en la reducci&oacute;n del dolor posoperatorio; adem&aacute;s, busca hacer una recomendaci&oacute;n para la pr&aacute;ctica cl&iacute;nica.</p> <h4>Materiales y m&eacute;todos</h4>     <p>El dise&ntilde;o de estudio se basa en la metodolog&iacute;a best evidence topics (The best BETs), desarrollada por el Manchester Royal Infirmary Emergency Department del Reino Unido (16).</p>     <p>Se llev&oacute; a cabo una b&uacute;squeda bibliogr&aacute;fica en las bases de datos Medline, Pubmed, Scopus, Embase, Cochrane y Ovid, con el fin de identificar revisiones sistem&aacute;ticas de la literatura o metaan&aacute;lisis y estudios de asignaci&oacute;n aleatoria publicados en idioma ingl&eacute;s o espa&ntilde;ol, entre enero de 2000 y junio de 2011, utilizando los t&eacute;rminos &ldquo;fixation&rdquo;, &ldquo;unfixation&rdquo;, &ldquo;mesh&rdquo;, &ldquo;groin hernia&rdquo;, &ldquo;inguinal hernia&rdquo;, &ldquo;surgery&rdquo;, &ldquo;glue&rdquo;, &ldquo;fibrin&rdquo;, &ldquo;cyanoacrilate&rdquo; y &ldquo;therapy&rdquo;. La b&uacute;squeda se extendi&oacute; en cada art&iacute;culo relevante utilizando operadores booleanos y referencias cruzadas para identificar otros art&iacute;culos.</p>     <p>Se incluyeron los estudios que compararon la fijaci&oacute;n de la malla con pegantes biol&oacute;gicos con la fijaci&oacute;n con suturas, tackers o grapas, o la no fijaci&oacute;n de la malla, con la herniorrafia laparosc&oacute;pica totalmente extraperitoneal (totally extraperitoneal, TEP), la transabdominal preperitoneal (transabdominal pre-peritoneal, TAPP) y la abierta, en pacientes adultos con hernia primaria unilateral, primaria bilateral, recurrente, directa, indirecta, femoral o mixta. Se excluyeron los estudios que comparaban la intervenci&oacute;n en hernia ventral y en poblaci&oacute;n pedi&aacute;trica.</p>     <p>Cada art&iacute;culo se evalu&oacute; cr&iacute;ticamente de acuerdo con las recomendaciones del Journal of the American Medical Association 17,18,19. Se definieron dos grupos de comparaciones: 1) fijaci&oacute;n con pegantes biol&oacute;gicos versus fijaci&oacute;n con suturas o tackers, y 2) fijaci&oacute;n con pegantes biol&oacute;gicos versus no fijaci&oacute;n.</p>     <p>Se definieron tres grupos de procedimientos para evaluar el impacto de la intervenci&oacute;n: 1) herniorrafia laparosc&oacute;pica totalmente extraperitoneal, 2) herniorrafia transabdominal preperitoneal y 3) herniorrafia abierta.</p>     <p>Los desenlaces evaluados fueron de dos tipos: primarios y secundarios. Los primarios fueron: tasa de reproducci&oacute;n, tasa global de complicaciones, incluida la de infecci&oacute;n del sitio operatorio, seroma, hematoma, dolor posoperatorio, evaluado mediante escala visual an&aacute;loga, y presencia de dolor cr&oacute;nico. Y, los secundarios fueron: costo, tiempo operatorio, sensaci&oacute;n de cuerpo extra&ntilde;o, estancia hospitalaria y de retorno laboral e incapacidad. Con la informaci&oacute;n obtenida se lleg&oacute; a una conclusi&oacute;n y se hizo una recomendaci&oacute;n para aplicarla en la pr&aacute;ctica cl&iacute;nica. </p> <h4>Resultados</h4>     <p>En la b&uacute;squeda se identificaron 11 estudios de asignaci&oacute;n aleatoria. No se encontraron revisiones sistem&aacute;ticas de la literatura ni metaan&aacute;lisis.</p>     <p><b><i>Fijaci&oacute;n con pegantes biol&oacute;gicos versus no fijaci&oacute;n</i></b></p>     ]]></body>
<body><![CDATA[<p>No se encontr&oacute; informaci&oacute;n disponible en estudios de asignaci&oacute;n aleatoria, revisiones sistem&aacute;ticas de la literatura ni metaan&aacute;lisis. </p>     <p><b><i>Fijaci&oacute;n con pegantes biol&oacute;gicos versus otros tipos de fijaci&oacute;n </i></b></p>     <p>Cirug&iacute;a abierta. T&eacute;cnica de Lichtenstein. En cinco estudios de asignaci&oacute;n aleatoria se evalu&oacute; esta comparaci&oacute;n (<a href="img/revistas/rcci/v26n4/v26n4a4t1.gif">tabla 1</a>). En tres estudios de asignaci&oacute;n aleatoria se compar&oacute; el N-BCA con la sutura absorbible sint&eacute;tica (20-22). En los otros dos estudios de asignaci&oacute;n aleatoria se compararon los pegantes biol&oacute;gicos con la sutura no absorbible; en uno de ellos se eval&uacute;o el pegamento de fibrina (23) y, en el otro, un nuevo compuesto llamado Gelatin Resorcin Formalin (GRF) (24). </p>     <p>S&oacute;lo en un estudio se evalu&oacute; el dolor posoperatorio con el uso de N-BCA mediante la escala visual an&aacute;loga (22), la cual indic&oacute; que fue menor al primero y s&eacute;ptimo d&iacute;as. Igualmente, el dolor posoperatorio fue menor al primero y s&eacute;ptimo d&iacute;as con el uso de GRF (24); adem&aacute;s, se observ&oacute; mayor incidencia de dolor cr&oacute;nico cuando se emple&oacute; sutura para la fijaci&oacute;n (12%). No hay informaci&oacute;n sobre el dolor en el estudio en que se utiliz&oacute; pegamento de fibrina (23). </p>     <p>En los estudios que compararon el N-BCA con la sutura, fue mayor la incidencia de dolor posoperatorio (hasta de 15,7 %), infecci&oacute;n del sitio operatorio y hematoma, con el uso de sutura en dos de ellos (20,21) y de seroma en otro (22). No hubo diferencia en la tasa de recurrencia en ninguno de los estudios. En el estudio en que se compar&oacute;el pegamento de fibrina con la sutura, no hubo diferencias en complicaciones como seroma y hematoma (23).</p>     <p>La estancia hospitalaria fue pr&aacute;cticamente igual en todas las comparaciones. En un estudio con NBCA (22,) fue menor la incapacidad. En un solo estudio 22 se evaluaron los costos y se demostr&oacute; que fueron iguales con N-BCA y con sutura (tablas<a href="img/revistas/rcci/v26n4/v26n4a4t2.gif"> 2</a> y <a href="img/revistas/rcci/v26n4/v26n4a4t3.gif">3</a>).</p>     <p> Tap&oacute;n de malla. En dos estudios se evalu&oacute; esta comparaci&oacute;n (<a href="img/revistas/rcci/v26n4/v26n4a4t1.gif">tabla 1</a>) (25,26). En el primero se compar&oacute; el pegamento de fibrina con la sutura absorbible y, en el segundo, el N-BCA y el pegamento de fibrina, con la sutura no absorbible. El dolor posoperatorio fue menor en un estudio con pegantes biol&oacute;gicos, al primero y s&eacute;ptimo d&iacute;as y a los seis meses de la cirug&iacute;a (25). En el otro estudio 26, no se documentaron diferencias en cuanto al dolor posoperatorio. </p>     <p>Respecto a las complicaciones, Testini encontr&oacute; una mayor tasa de hematoma (6,7% versus 0,26%) y de sensaci&oacute;n de cuerpo extra&ntilde;o, con el uso de suturas (26). No hubo diferencias en la tasa de recurrencia en ninguno de los estudios. En los dos estudios no se informa diferencia alguna en la estancia hospitalaria o en la incapacidad. No exist&iacute;a informaci&oacute;n sobre costos (tablas 2 y 3).</p>     <p> Herniorrafia laparosc&oacute;pica transabdominal preperitoneal. En tres estudios de asignaci&oacute;n aleatoria se compararon los pegantes biol&oacute;gicos con los tackers (<a href="img/revistas/rcci/v26n4/v26n4a4t1.gif">tabla 1</a>) (27-29). En todos se utiliz&oacute; pegamento de fibrina y, en uno de ellos, fibrina aut&oacute;loga (27). En uno de los estudios se compar&oacute; el pegamento de fibrina con tres diferentes dispositivos de sutura (28). </p>     <p>En cuanto a desenlaces primarios, el uso de pegamento de fibrina se asoci&oacute; con menor dolor postoperatorio evaluado mediante la escala visual an&aacute;loga, al d&iacute;a s&eacute;ptimo (27,28), al primer mes (27-29), al sexto mes (27,29) y al a&ntilde;o (29). En ning&uacute;n estudio se encontr&oacute; diferencia alguna en la tasa de seroma (27,28,29) y en uno (27) se inform&oacute; una menor tasa de hematoma con el uso de pegantes biol&oacute;gicos (0 versus 9,1%). La tasa de recurrencia fue similar en todas las intervenciones 27-29. En dos estudios de asignaci&oacute;n aleatoria, el n&uacute;mero de d&iacute;as de incapacidad fue menor con los pegantes biol&oacute;gicos en comparaci&oacute;n con los tackers 28,29. No se encontraron diferencias en la estancia hospitalaria. En cuanto al costo, un estudio (27) indic&oacute; que es mayor con el uso de fibrina aut&oacute;loga (tablas <a href="img/revistas/rcci/v26n4/v26n4a4t2.gif">2</a> y <a href="img/revistas/rcci/v26n4/v26n4a4t3.gif">3</a>).</p>     ]]></body>
<body><![CDATA[<p>Herniorrafia laparosc&oacute;pica totalmente extraperitoneal. En un estudio de asignaci&oacute;n aleatoria se evalu&oacute; esta comparaci&oacute;n (30) (<a href="img/revistas/rcci/v26n4/v26n4a4t1.gif">tabla 1</a>). Seg&uacute;n la escala visual an&aacute;loga, el dolor fue mayor con el uso de pegantes biol&oacute;gicos al primero y s&eacute;ptimo d&iacute;as, al igual que la incidencia de seroma. No hubo recurrencias con ninguna de las t&eacute;cnicas. En este estudio se inform&oacute; que el costo fue menor con el uso de grapadora laparosc&oacute;pica (US$ 130 Vs. US$ 270). No hubo diferencia en la estancia hospitalaria ni en la incapacidad (tablas <a href="img/revistas/rcci/v26n4/v26n4a4t2.gif">2 </a>y <a href="img/revistas/rcci/v26n4/v26n4a4t3.gif">3</a>).</p> <h4>Discusi&oacute;n </h4>     <p>La herniorrafia inguinal contin&uacute;a en permanente evoluci&oacute;n. Si bien la utilizaci&oacute;n de las t&eacute;cnicas libres de tensi&oacute;n y los avances de la herniorrafia laparosc&oacute;pica son aspectos decisivos, tambi&eacute;n lo son el tipo de materiales y la forma de utilizarlos durante la reconstrucci&oacute;n del defecto de la pared abdominal.</p>     <p>Al respecto, parte de las nuevas controversias radican en el tipo de pr&oacute;tesis que se debe utilizar (alta densidad, baja densidad o ultrabaja densidad), en la necesidad de fijar la pr&oacute;tesis y, si se fija, en el tipo de material utilizado para hacerlo (suturas, grapas o tackers, o pegantes biol&oacute;gicos). La v&iacute;a (abierta y laparosc&oacute;pica), la t&eacute;cnica (anterior o posterior al conducto inguinal, laparosc&oacute;pica totalmente extraperitoneal, transabdominal preperitoneal) y los materiales, son aspectos que se conjugan y modifican la satisfacci&oacute;n y la efectividad en la herniorrafia para el paciente y, simult&aacute;neamente, afectan la relaci&oacute;n costo-beneficio para el prestador de servicios y el sistema sanitario. En conformidad, la valoraci&oacute;n actual de un extenso grupo de resultados cl&iacute;nicos, entre los que se incluye el dolor posoperatorio y la calidad de vida, han permitido repensar los beneficios del procedimiento en relaci&oacute;n con los resultados convencionales, no menos importantes, como la reproducci&oacute;n de la hernia.</p>     <p>Como resultado, el tratamiento quir&uacute;rgico contempor&aacute;neo de la hernia inguinal es pol&eacute;mico en varios de sus aspectos estructurales, siendo uno de ellos el relacionado con la necesidad de fijar la pr&oacute;tesis. Varios experimentos cl&iacute;nicos que eval&uacute;an los resultados de fijar la malla contra no fijarla en cirug&iacute;a laparosc&oacute;pica totalmente extraperitoneal, en la transabdominal preperitoneal y en la abierta (Lichtenstein), indican que, en general, la reproducci&oacute;n de la hernia es igual con cualquiera de las t&eacute;cnicas, pero parece existir una ventaja cuando no se fija la malla, que se relaciona con la reducci&oacute;n del dolor posoperatorio tard&iacute;o (luego del sexto mes de la cirug&iacute;a) (31-36). No obstante, si bien esta informaci&oacute;n podr&iacute;a resultar contundente para apoyar que la pr&oacute;tesis no debe fijarse durante una herniorrafia inguinal, algunos estudios cl&iacute;nicos no experimentales indican que no fijar la malla puede aumentar la tasa de desplazamiento de la pr&oacute;tesis, lo cual incide en la tasa de recurrencia, en la sensaci&oacute;n de cuerpo extra&ntilde;o y, tambi&eacute;n, en la aparici&oacute;n de dolor posoperatorio (37,38).</p>     <p>Estos resultados y los avances tecnol&oacute;gicos han generado en la comunidad cient&iacute;fica, la necesidad de continuar evaluando la importancia de fijar la pr&oacute;tesis y, alternativamente, han planteado los beneficios de la fijaci&oacute;n con pegantes biol&oacute;gicos sobre las tasas de recurrencia, dolor y calidad de vida. En particular, parte del fundamento te&oacute;rico en contra de las suturas radica en los efectos secundarios; pueden causar estrangulaci&oacute;n de las fibras musculares, lesi&oacute;n o compresi&oacute;n de los nervios, y as&iacute;, producir dolor o disestesia hasta en 75% de los casos (39,40).</p>     <p>Como resultado, en los &uacute;ltimos a&ntilde;os se ha llevado a cabo un importante n&uacute;mero de experimentos cl&iacute;nicos, analizados en este documento, que comparan la fijaci&oacute;n de la malla con pegantes biol&oacute;gicos versus la fijaci&oacute;n convencional (suturas, grapas). Empero, en esta revisi&oacute;n no se encontraron experimentos cl&iacute;nicos que comparen el uso de pegantes biol&oacute;gicos para la fijaci&oacute;n de la pr&oacute;tesis contra la falta de fijaci&oacute;n, por lo que existe un vac&iacute;o de conocimiento en cuanto a la efectividad global de la intervenci&oacute;n, lo cual indirecta e hipot&eacute;ticamente podr&iacute;a apoyar los beneficios relacionados con la no fijaci&oacute;n de la malla demostrados en otros estudios.</p>     <p>Los resultados de los estudios de asignaci&oacute;n aleatoria evaluados en este estudio, permiten concluir que en la cirug&iacute;a abierta, fijar la malla con pegantes biol&oacute;gicos (como N-BCA y GRF) por la t&eacute;cnica de Lichtenstein, puede reducir la incidencia temprana de dolor posoperatorio (22,24). Sin embargo, en tres estudios (20-23) no se evalu&oacute; este desenlace, lo cual no permite establecer de forma contundente el resultado de la intervenci&oacute;n. En cuanto a la tasa de recurrencia y la de complicaciones, no demuestran diferencias significativas. Estos hallazgos son similares con el uso de tap&oacute;n de malla.</p>     <p>A la par, la informaci&oacute;n adicional de estudios no experimentales indica que, en cirug&iacute;a abierta (Liechtenstein) en atletas de alto rendimiento, la fijaci&oacute;n de materiales prot&eacute;sicos de alta densidad (polipropileno) con pegamento de fibrina puede tener un impacto positivo en la reducci&oacute;n del dolor posoperatorio y en la recuperaci&oacute;n funcional (41). Estos hallazgos han sido corroborados en estudios prospectivos en la poblaci&oacute;n general sometida a la misma t&eacute;cnica (42-44).</p>     <p>Actualmente se lleva a cabo un experimento cl&iacute;nico italiano multic&eacute;ntrico para evaluar el pegamento de fibrina versus la sutura, en la herniorrafia abierta (Lichtenstein), del cual a&uacute;n no hay resultados disponibles (45).</p>     <p>En cuanto a la herniorrafia laparosc&oacute;pica transabdominal preperitoneal, este estudio permite concluir que la incidencia de dolor posoperatorio y el tiempo de incapacidad son menores con el uso de pegantes biol&oacute;gicos, sin que exista diferencia en la frecuencia de recurrencia. En este sentido, diferentes estudios prospectivos sobre este tipo de herniorrafia informan que la fijaci&oacute;n de la malla con pegamento de fibrina es f&aacute;cil y efectiva (46-49).</p>     ]]></body>
<body><![CDATA[<p>En un estudio representativo con dise&ntilde;o prospectivo multic&eacute;ntrico de asignaci&oacute;n no aleatoria, realizado en Francia en 2010, en pacientes sometidos a diferentes herniorrafias por v&iacute;a laparosc&oacute;pica (transabdominal preperitoneal, laparosc&oacute;pica totalmente extraperitoneal), se inform&oacute; que la tasa global de complicaciones locales fue de 4,7%, la de hematoma, de 3,0%, la de seroma, de 1,4% y la de recurrencia, de 0,3%, cuando se fija la malla con pegamento de fibrina.</p>     <p>Adem&aacute;s, en todos los grupos de tratamiento el promedio de dolor evaluado por la escala visual an&aacute;loga antes del procedimiento fue de 3,2, y descendi&oacute; a 2,3 inmediatamente despu&eacute;s de la cirug&iacute;a y luego a 1,8 en el primer mes posoperatorio 50. Igualmente, en estudios retrospectivos que compararon la fijaci&oacute;n de mallas de alta densidad (polipropileno) con el pegamento de fibrina versus clips de titanio, se demostr&oacute; una recuperaci&oacute;n m&aacute;s r&aacute;pida y una menor tasa de dolor posoperatorio con esta t&eacute;cnica 51. Los resultados son favorables, aunque no conclusivos, con la t&eacute;cnica laparosc&oacute;pica IPOM (Intraperitoneal Onlay Mesh Technique) (52).</p>     <p>En cuanto al tipo de materiales prot&eacute;sicos utilizados en la herniorrafia laparosc&oacute;pica transabdominal preperitoneal, en un estudio experimental de asignaci&oacute;n aleatoria que compar&oacute; cuatro tipos de malla de diversa densidad fijada con 1 ml de pegamento de fibrina, no se demostraron cambios en la tasa de dolor cr&oacute;nico relacionado con el material empleado y solamente se inform&oacute; que la convalecencia fue m&aacute;s r&aacute;pida cuando se utilizaron mallas livianas (53).</p>     <p>En tercer lugar, en relaci&oacute;n con la herniorrafia laparosc&oacute;pica totalmente extraperitoneal, la literatura cient&iacute;fica evaluada muestra resultados opuestos a los de la transabdominal preperitoneal y la abierta, indicando mayor tasa de dolor posoperatorio, seroma y costos con el uso de pegantes biol&oacute;gicos, sin que se demostrara diferencia en la tasa de recurrencia. Estos hallazgos han sido similares en otro tipo de estudios no experimentales, de dise&ntilde;o retrospectivo y prospectivo, en los cuales se informaron diversos grados de eficacia en la reducci&oacute;n del dolor (54-57). Incluso, en uno de estos estudios, algunos pacientes refirieron molestias durante la micci&oacute;n que se resolvieron espont&aacute;neamente durante el primer mes posoperatorio (54).</p>     <p>En cuanto al tipo de materiales empleados en la herniorrafia laparosc&oacute;pica totalmente extraperitoneal, en un estudio retrospectivo se inform&oacute; que la utilizaci&oacute;n de mallas de baja densidad fijadas con pegante de fibrina, no se asoci&oacute; con reducci&oacute;n en la tasa de dolor posoperatorio (54). En otro estudio prospectivo, se evalu&oacute; la utilizaci&oacute;n de mallas biol&oacute;gicas fijadas con pegante de fibrina y se encontr&oacute; que este compuesto era eficaz y se asociaba con una recurrencia de la hernia de 2% durante el primer a&ntilde;o de seguimiento (58). Hasta el momento no se ha reportado la utilizaci&oacute;n de pegantes biol&oacute;gicos en otro tipo de herniorrafia laparosc&oacute;pica totalmente extraperitoneal, como el recientemente descrito en Colombia (e-TEP) (59).</p>     <p>Finalmente, un aspecto que se debe tener en cuenta cuando se emplean agentes biol&oacute;gicos, tiene que ver con las reacciones secundarias, el perfil de seguridad asociado y su relaci&oacute;n de costo-efectividad. Las reacciones secundarias informadas con el uso de pegantes biol&oacute;gicos en general, son taquicardia, bradicardia, hipotensi&oacute;n, complicaciones de trombosis, respiratorias, disnea, hipersensibilidad y par&aacute;lisis muscular, entre otras 59,60. Alternativamente, y frente a las reacciones secundarias de tipo inmunol&oacute;gico y al&eacute;rgico, se ha desarrollado otro tipo de materiales de origen aut&oacute;logo. Los estudios sobre el Autologous Platelet-Rich Fibrin Sealant y la fibrina aut&oacute;loga humana, utilizada en la herniorrafia laparosc&oacute;pica transabdominal preperitoneal, informan una eficacia cl&iacute;nica similar a la de los pegantes biol&oacute;gicos convencionales (60,61).</p> <h4>Conclusi&oacute;n </h4>     <p>La fijaci&oacute;n de la malla durante una herniorrafia inguinal puede ofrecer ventajas para el paciente en cuanto a la reducci&oacute;n del dolor posoperatorio en la herniorrafia laparosc&oacute;pica transabdominal preperitoneal. Esta ventaja no est&aacute; completamente establecida en la herniorrafia abierta, especialmente en el procedimiento de Lichtenstein, dado que en varios experimentos cl&iacute;nicos no se eval&uacute;a este desenlace. En la cirug&iacute;a laparosc&oacute;pica totalmente extraperitoneal, la informaci&oacute;n proveniente de los estudios de asignaci&oacute;n aleatoria indica que el dolor posoperatorio es mayor con el uso de pegantes biol&oacute;gicos.</p>     <p>En general, en todas las v&iacute;as y t&eacute;cnicas de herniorrafia (transabdominal preperitoneal, laparosc&oacute;pica totalmente extraperitoneal, abierta), no se documentan diferencias en la tasa de reproducci&oacute;n e infecci&oacute;n, y la tasa de seroma y hematoma es variable. No existe informaci&oacute;n sobre la comparaci&oacute;n del uso de los pegantes biol&oacute;gicos con la no fijaci&oacute;n de la pr&oacute;tesis. No obstante, existen experimentos que indican que puede existir beneficio cuando no se fija la pr&oacute;tesis, en comparaci&oacute;n con la fijaci&oacute;n con suturas o tackers, lo cual puede sugerir que no fijar la malla es una de las estrategias ideales para la reducci&oacute;n del dolor posoperatorio. Con la informaci&oacute;n obtenida, no se recomienda de forma rutinaria la utilizaci&oacute;n de pegantes biol&oacute;gicos durante la herniorrafia inguinal.</p> <h4>Conflictos de inter&eacute;s </h4>     <p>No se reporta ninguno.</p> <h4>Referencias</h4>     <!-- ref --><p>1.
