La presente revisión considera un consenso actual sobre las estrategias diagnósticas y terapéuticas más apropiadas para el manejo de estas frecuentes lesiones.]]></p></abstract>

This review considers the current consensus on the most appropriate diagnostic and therapeutic strategies for the management of these frequent lesions.]]></p></abstract>

<body><![CDATA[<center><font size="3" face="Verdana"><b>  Introducci&oacute;n</b></font></center></p>     <p> Durante las &uacute;ltimas tres d&eacute;cadas se han producido varias modificaciones en el comportamiento del trauma hep&aacute;tico, ilustradas claramente por Richardson en su estudio, que incluy&oacute; 1.842 pacientes con trauma abdominal en la Universidad de Louisville, entre 1975 y 1999; por una parte, encontr&oacute; una mayor frecuencia de trauma hep&aacute;tico durante el trascurso de los a&ntilde;os, probablemente relacionada por el aumento exponencial de las lesiones hep&aacute;ticas asociadas a accidentes de tr&aacute;nsito, y, por otra, evidenci&oacute; que la mortalidad global secundaria a estas lesiones present&oacute; un comportamiento contralineal al tener un porcentaje actual de supervivencia del 80 %, muy superior al de 55 % encontrando en el a&ntilde;o 1975 <sup>1</sup>.</p>       <p>Richardson propuso varias razones que podr&iacute;an explicar el descenso de la mortalidad asociada a trauma hep&aacute;tico en los &uacute;ltimos 25 a&ntilde;os: el uso progresivo de la cirug&iacute;a de control de da&ntilde;os y el mejoramiento de las t&eacute;cnicas quir&uacute;rgicas, conllevaron al mejoramiento del manejo en traumas hep&aacute;ticos mayores y a la disminuci&oacute;n del n&uacute;mero de lesiones vasculares asociadas a procedimientos quir&uacute;rgicos <sup>1,2</sup>. </p>       <p>No obstante, un factor fundamental que debe mencionarse en forma independiente es el cambio de la conducta frente al trauma hep&aacute;tico en los servicios de urgencias, que se dio desde la d&eacute;cada de los noventa, cuando se incorpor&oacute; de manera rutinaria la evaluaci&oacute;n de los grados de lesi&oacute;n imaginol&oacute;gica, con el fin de establecer un tratamiento m&aacute;s apropiado. Desde entonces, la tomograf&iacute;a computadorizada (TC) y la ecograf&iacute;a han sido pilares diagn&oacute;sticos fundamentales en el trauma abdominal cerrado, y fueron referenciados en los estudios de Richardson y Pachter, en los que las lesiones hep&aacute;ticas de grado IV o V estaban cercanas al 15 % y 14 %, respectivamente; adem&aacute;s, llevaron a abordar las lesiones hep&aacute;ticas de acuerdo con su estabilidad cl&iacute;nica <sup>2,3</sup>.</p>       <p>Estos cambios, as&iacute; como los grandes avances en el abordaje y manejo del trauma hep&aacute;tico, se han visto reflejados en el aumento de las publicaciones al respecto en revistas indexadas, pasando de 50 en 1970 a m&aacute;s de 300 en el 2011 y a m&aacute;s de 125 durante el primer semestre del 2012; Estados Unidos, Jap&oacute;n, China, Alemania y Francia, son los pa&iacute;ses que lideran este campo de la literatura cient&iacute;fica que se agrupa preferencialmente en revistas especializadas en trauma y cirug&iacute;a (figura <a href="#figura1">1</a> y <a href="#figura2">2</a>, <a href="#tabla1">tabla 1</a>). </p>       <p>    <center>     <a name="figura1"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8f1.gif"></center></p>       <p>    <center>     <a name="figura2"></a>        ]]></body>
<body><![CDATA[<br>   <img src="img/revistas/rcci/v28n1/v28n1a8f2.gif"></center></p>       <p>    <center>     <a name="tabla1"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8t1.gif"></center></p>     <p>En lo referente a nuestro pa&iacute;s, solo existe una publicaci&oacute;n sobre el tema en una revista indexada (Radiology) por el Servicio de Radiolog&iacute;a del Hospital San Vicente de Pa&uacute;l de Medell&iacute;n en el 2011, en la que comparan el ultrasonido y la TC abdominal en la evaluaci&oacute;n de pacientes estables con heridas abdominales penetrantes con arma blanca <sup>4</sup>.</p>       <p>    <center><font size="3" face="Verdana"><b>Mecanismo del trauma</b></font></center></p>       <p>El h&iacute;gado representa del 2 al 3 % del peso corporal, se encuentra ubicado en el cuadrante superior derecho del abdomen, divido en dos l&oacute;bulos por la l&iacute;nea de Cantlie y en ocho segmentos determinados por su irrigaci&oacute;n <sup>5</sup>. El sitio m&aacute;s frecuentemente comprometido en el trauma cerrado es el l&oacute;bulo derecho en su aspecto posterior, debido a su mayor tama&ntilde;o y fijaci&oacute;n anat&oacute;mica que, ante una desaceleraci&oacute;n brusca, provocan un desgarro intraparenquimatoso y fractura antero-posterior de este voluminoso l&oacute;bulo. Las fijaciones a la pared abdominal y al diafragma son las responsables de la alta frecuencia de lesiones de este &oacute;rgano (35-45 %) despu&eacute;s de un trauma cerrado del abdomen <sup>6</sup>.</p>       <p>En el contexto del trauma penetrante, el h&iacute;gado se ve comprometido en 40 % de las lesiones provocadas por arma corto-punzante y en 30 % de las lesiones por proyectil de arma de fuego. Las lesiones penetrantes usualmente causan un trauma vascular significativo en el territorio de la arteria hep&aacute;tica, la vena porta o la vena cava, adem&aacute;s de lesiones biliares complejas<sup> 7</sup>. </p>     <p>    ]]></body>
<body><![CDATA[<center><font size="3" face="Verdana"><b>Clasificaci&oacute;n de la lesi&oacute;n</b></font></center></p>       <p>Para el correcto abordaje del traumatismo hep&aacute;tico y teniendo a la TC como m&eacute;todo diagn&oacute;stico de referencia en la clasificaci&oacute;n de la gravedad de una lesi&oacute;n <sup>8</sup>, se han elaborado varias clasificaciones, de las cuales, la de la <i>American Association for the Surgery of Trauma</i> es la m&aacute;s utilizada y se&ntilde;ala c&oacute;mo las lesiones de grado I o II se consideran menores, mientras que las de grado III a V constituyen lesiones graves (<a href="#tabla2">tabla 2</a>) <sup>9</sup>. </p>       <p>    <center>     <a name="tabla2"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8t2.gif"></center></p>     <p>    <center><font size="3" face="Verdana"><b>Abordaje inicial en urgencias </b></font></center></p>       <p>El compromiso hep&aacute;tico debe sospecharse en todo paciente con trauma de la regi&oacute;n toraco-abdominal anterior y posterior, especialmente si se encuentra en estado de choque. La reanimaci&oacute;n inicial debe seguir los par&aacute;metros del Colegio Americano de Cirujanos, evaluando la respuesta del paciente a las medidas iniciales de reanimaci&oacute;n mediante una monitorizaci&oacute;n estricta y se deben incluir tempranamente las herramientas diagn&oacute;sticas que m&aacute;s adelante se describen, de acuerdo con el estado hemodin&aacute;mico. </p>       <p>Aunque una proporci&oacute;n importante de pacientes se benefician de un manejo no operatorio, aquellos con indicaci&oacute;n de cirug&iacute;a deben ser identificados tempranamente y llevados a cirug&iacute;a en forma urgente; el retraso en el tratamiento quir&uacute;rgico se asocia definitivamente a una alta mortalidad, que puede ser prevenible si se manejan estos pacientes con par&aacute;metros estrictos y bajo estricta vigilancia por personal con experiencia en trauma. </p>       <p>Seg&uacute;n los estudios de Richardson y Pachter <sup>10,11</sup>, el m&eacute;dico y los especialistas del servicio de urgencias deben tener en cuenta un par&aacute;metro fundamental para el manejo de estos enfermos: la estabilidad hemodin&aacute;mica, que determina en principio si se requiere una laparotom&iacute;a inmediata o se pueden completar los estudios para establecer si es apropiado un tratamiento no operatorio (<a href="#figura3">figura 3</a>).</p>       ]]></body>
<body><![CDATA[<p>    <center>     <a name="figura3"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8f3.gif"></center></p>     <p>    <center><font size="3" face="Verdana"><b>Manejo no quir&uacute;rgico </b></font></center></p>       <p>La aproximaci&oacute;n actual en el tratamiento no quir&uacute;rgico u observacional del trauma hep&aacute;tico se ha ido consolidando a medida que diferentes grupos de trauma han publicado los resultados favorables de este abordaje, que aparecen a partir de 1972 y han constituido uno de los cambios m&aacute;s importantes en el manejo del trauma abdominal. La l&oacute;gica que fundament&oacute; esta innovaci&oacute;n provino de la experiencia en cirug&iacute;a pedi&aacute;trica, en la cual los resultados han sido muy satisfactorios <sup>12,13</sup>, y en el hecho de que el sangrado hep&aacute;tico postraum&aacute;tico se resuelve espont&aacute;neamente en 50 a 80 % de los casos. </p>       <p>Los criterios para el tratamiento no quir&uacute;rgico u observacional de los traumatismos hep&aacute;ticos incluyen: estabilidad hemodin&aacute;mica, ausencia de lesiones abdominales adicionales que requieran laparotom&iacute;a, disponibilidad de recursos hospitalarios (disponibilidad de salas de cirug&iacute;a, monitorizaci&oacute;n y cirujano permanente) y disponibilidad de recursos imaginol&oacute;gicos (TC de buena calidad) <sup>14,15</sup>.</p>       <p>Se deben tener en cuenta algunos factores pron&oacute;sticos de falla en el manejo no operatorio, como el trauma de gravedad importante, de grado IV y V. La tasa de casos bajo tratamiento no quir&uacute;rgico en que se recurre a la cirug&iacute;a es significativamente mayor en las lesiones hep&aacute;ticas graves (grados IV y V). Sin embargo, la intervenci&oacute;n quir&uacute;rgica rara vez se ve relacionada con complicaciones hep&aacute;ticas <sup>16</sup>. La raz&oacute;n m&aacute;s com&uacute;n para la intervenci&oacute;n quir&uacute;rgica en pacientes con tratamiento conservador inicial, es que coexista una lesi&oacute;n abdominal que produzca sangrado del ri&ntilde;&oacute;n o del bazo; la hemorragia hep&aacute;tica es una causa en 0 a 3,5 % de los casos <sup>17</sup>. Fang y, posteriormente, Misselbeck, encontraron que los siguientes factores se comportar&iacute;an como indicadores de fracaso: hipovolemia al ingreso, lesi&oacute;n de alto grado en la TC (superior a IV), necesidad de transfusi&oacute;n de, al menos, cuatro unidades de gl&oacute;bulos rojos, lesi&oacute;n espl&eacute;nica o renal asociada o presencia de l&iacute;quido en cavidad de m&aacute;s de 300 ml; cuando se presentan, el porcentaje de falla es cercano a 96 %, frente a 2 % en su ausencia <sup>18-20</sup>. </p>       <p>Es importante destacar que entre las estrategias no quir&uacute;rgicas, existen alternativas para tratar lesiones vasculares y biliares mediante intervencionismo radiol&oacute;gico (procedimientos endovasculares, inserci&oacute;n de drenajes), con resultados muy satisfactorios. </p>       <p>Seg&uacute;n el estudio de Tinkoff del banco nacional de datos en trauma de los Estados Unidos, en el cual se demuestra que en la actualidad el 83,6 % de los traumas hep&aacute;ticos se tratan de forma no quir&uacute;rgica <sup>21</sup>, surge la necesidad de establecer un algoritmo de manejo para este tipo de lesiones, el cual fue creado por la <i>Western Trauma Association</i> y ajustado a la escala de gravedad del trauma hep&aacute;tico de la <i>American Association for the Surgery of Trauma</i> (<a href="#figura4">figura 4</a>) <sup>22</sup>. </p>       ]]></body>
<body><![CDATA[<p>    <center>     <a name="figura4"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8f4.gif"></center></p>       <p>    <center>     <a name="figura5"></a>        <br>   <img src="img/revistas/rcci/v28n1/v28n1a8f5.gif"></center></p>     <p>    <center><font size="3" face="Verdana"><b>Manejo quir&uacute;rgico</b></font></center></p>       <p>Cuando hay hipotensi&oacute;n o sangrado activo, el abordaje quir&uacute;rgico constituye la mejor opci&oacute;n terap&eacute;utica, en ocasiones combinado con una estrategia radiol&oacute;gica. </p>       <p>A partir de la experiencia en la segunda guerra mundial, se introdujo la cirug&iacute;a del control de da&ntilde;os que llev&oacute; a un importante descenso en la mortalidad operatoria de estos pacientes, de 30 % a 17 %, usualmente relacionada con sangrado intraoperatorio masivo e incontrolable <sup>23,24</sup>.</p>     ]]></body>
<body><![CDATA[<p>Las estrategias de mayor efectividad en la sala de cirug&iacute;a ante el trauma hep&aacute;tico con inestabilidad hemodin&aacute;mica han sido descritas por numerosos autores <sup>25-28</sup>:</p>       <p>- laparotom&iacute;a de reanimaci&oacute;n    <br>     - medidas hemost&aacute;ticas (electrocauterio, coagulaci&oacute;n con arg&oacute;n, taponamiento con balones)     <br>     - compresi&oacute;n y empaquetamiento perihep&aacute;tico    <br>     - maniobra de Pringle    <br>     - hepatotom&iacute;a y ligadura selectiva    <br>     - desbridamiento por resecci&oacute;n    <br>     - cirug&iacute;a de control de da&ntilde;os    <br>     - aislamiento vascular y derivaciones  </p>       <p>Dado que la hemorragia se comporta como un factor independiente de mortalidad, en el que el uso temprano de un componente de transfusiones y la elecci&oacute;n de cirug&iacute;a de control de da&ntilde;os en los casos de trauma hep&aacute;tico consigue el control de la misma en el 80 % de los casos <sup>29</sup>. Sin embargo, en los casos en que no se logra controlar el sangrado por estos m&eacute;todos, la realizaci&oacute;n de la maniobra de Pringle puede ser de utilidad en la localizaci&oacute;n del sitio de sangrado; en los casos en los que la maniobra de Pringle consiga controlar el sangrado se puede inferir que la lesi&oacute;n est&aacute; en la arteria hep&aacute;tica o en la vena porta, en caso contrario, el sangrado ser&aacute; debido a lesi&oacute;n hep&aacute;tica <sup>30</sup>; en estos &uacute;ltimos casos, la decisi&oacute;n cl&iacute;nica del cirujano de la resecci&oacute;n hep&aacute;tica debe ser tomada y, en caso de realizarse, la agilidad y prontitud de esta resulta vitales en el manejo.</p>     ]]></body>
<body><![CDATA[<p>En 1900 la mortalidad en resecciones hep&aacute;ticas era del orden del 80%, cifra que para 1965 disminuy&oacute; al 20% <sup>31</sup> y en la actualidad se encuentra entre el 8% <sup>32</sup>, y 20% <sup>33</sup>, con un promedio para lesiones hep&aacute;ticas grado IV o V que persiste en cifras muy elevadas entre el 15% y 50% <sup>34</sup>. </p>       <p>Sin embargo, cada vez se publican m&aacute;s experiencias alentadoras de grupos con experiencia en reseciones hep&aacute;ticas, como la de la Universidad de Pittsburgh, donde en un periodo de 15 a&ntilde;os y con 1.049 pacientes, de los cuales, 216 presentaban lesiones hep&aacute;ticas complejas superiores al grado III, se lograron unas cifras de mortalidad operatoria de 9 % y una global de 17,8 % <sup>35,36,37</sup>.</p>       <p>    <center><font size="3" face="Verdana"><b>Conclusiones</b></font></center></p>       <p>Con el aumento de los accidentes de tr&aacute;nsito, en los cuales se presentan de 65 a 70 % de casos de trauma hep&aacute;tico que, usualmente, afectan una poblaci&oacute;n joven y productiva entre la segunda y la tercera d&eacute;cadas de la vida, es fundamental desarrollar estrategias de manejo apropiadas y exitosas.</p>       <p>La estandarizaci&oacute;n del manejo en urgencias y la aplicaci&oacute;n de los algoritmos para seleccionar apropiadamente el manejo quir&uacute;rgico y no quir&uacute;rgico, han llevado a una disminuci&oacute;n importante de la morbilidad y la mortalidad de los pacientes con trauma hep&aacute;tico. Cuando se requiere tratamiento quir&uacute;rgico, las estrategias que se han impuesto en las &uacute;ltimas d&eacute;cadas, como la cirug&iacute;a de control de da&ntilde;os y el procedimiento endovascular adyuvante al manejo operatorio, han marcado un antes y un despu&eacute;s en la supervivencia de los pacientes sometidos a cirug&iacute;a, pero tienen como requisito fundamental la disponibilidad de personal entrenado en estos procedimientos. </p>       <p>Para el &oacute;ptimo tratamiento de las lesiones causadas por trauma cerrado, se requiere disponer de recursos imaginol&oacute;gicos, como TC de buena calidad, que permitan su clasificaci&oacute;n y seguimiento, e integrar esta informaci&oacute;n a la valoraci&oacute;n permanente del paciente por un equipo multidisciplinario con experiencia en la atenci&oacute;n del paciente politraumatizado, en una instituci&oacute;n hospitalaria que cuente con los recursos para atender casos de gran complejidad.</p>       <p>    <center>   <img src="img/revistas/rcci/v28n1/v28n1a8t.gif"></center></p>     <p>    ]]></body>
