El trabajo diagnóstico temprano y el tratamiento oportuno han permitido un mayor número de resultados exitosos. El tratamiento convencional ha sido la esplenectomía; sin embargo, por la importancia inmunológica del bazo y la morbimortalidad del tratamiento quirúrgico, hay una tendencia al tratamiento médico y de menor invasión. En este artículo se presenta una revisión narrativa del problema con base en la literatura médica con mayor 'evidencia', se discuten las diferentes alternativas de tratamiento y se concluye que el tratamiento debe individualizarse a cada paciente. El drenaje percutáneo es la primera línea de tratamiento; si este fracasa, debe considerarse la esplenectomía. Aunque con tiempos quirúrgicos más prolongados, la esplenectomía laparoscópica se asocia con una reducción significativa de la morbilidad posquirúrgica.]]></p></abstract>

<body><![CDATA[<p><font size="3" face="Verdana"><b>Introducci&oacute;n</b></font></p>     <p>El absceso espl&eacute;nico es una entidad poco com&uacute;n, su incidencia es de 0,2 a 0,7 %. Usualmente es resultado de bacteriemias en pacientes con enfermedades infecciosas concurrentes y, ocasionalmente, se produce por extensi&oacute;n de un foco infeccioso contiguo. En las &uacute;ltimas d&eacute;cadas se ha diagnosticado con mayor frecuencia en pacientes inmunocomprometidos<sup>1</sup>. Para hacer un diagn&oacute;stico temprano y oportuno, se requiere un alto &iacute;ndice de sospecha y el uso liberal de estudios radiol&oacute;gicos. </p>     <p>Despu&eacute;s de reconocer la importancia inmunol&oacute;gica del bazo, la existencia de sepsis despu&eacute;s de una esplenectom&iacute;a y la tasa de mortalidad del procedimiento quir&uacute;rgico (14 %), hay una tendencia a favorecer los tratamientos salvadores del &oacute;rgano, el tratamiento antibi&oacute;tico y el drenaje percut&aacute;neo, evitando la extirpaci&oacute;n del &oacute;rgano <sup>1,2</sup>.</p>     <p>En este art&iacute;culo se revisa el tema haciendo &eacute;nfasis en las estrategias terap&eacute;uticas. Para esto, se dise&ntilde;&oacute; una estrategia de b&uacute;squeda en las principales bases de datos disponibles, combinando t&eacute;rminos MeSH y texto libre, relacionados con absceso espl&eacute;nico, tratamiento o esplenectom&iacute;a laparosc&oacute;pica. Esta b&uacute;squeda no fue restringida por fecha de publicaci&oacute;n o idioma. Se incluyeron art&iacute;culos de revisi&oacute;n del tema y estudios en poblaci&oacute;n pedi&aacute;trica.</p>     <p><font size="3" face="Verdana"><b>Patog&eacute;nesis</b></font></p>     <p>Se conocen varios mecanismos que predisponen a la formaci&oacute;n de abscesos espl&eacute;nicos.</p>     <p><b><i>Infecci&oacute;n</i></b></p>     <p>Las infecciones distantes producen embolias al bazo, generando &aacute;reas de isquemia y posterior necrosis. Estos infartos espl&eacute;nicos pueden sobreinfectarse y desarrollar abscesos. En otras ocasiones, las bacteriemias son responsables de que los microorganismos se alojen en el bazo y generen un absceso <sup>2</sup>. </p>     <p>Las entidades m&aacute;s relacionadas son la endocarditis, responsable de 10 a 20 % de los abscesos espl&eacute;nicos, y la infecci&oacute;n de v&iacute;as urinarias <sup>3</sup>. En los pacientes inmunosuprimidos se est&aacute; reconociendo con mayor frecuencia la etiolog&iacute;a f&uacute;ngica <sup>3</sup>.</p>     <p><i><b>Trauma</b></i></p>     ]]></body>
<body><![CDATA[<p>El trauma espl&eacute;nico es un factor de riesgo para la formaci&oacute;n de abscesos espl&eacute;nicos; el tejido contuso o con hematoma puede sobreinfectarse <sup>4</sup>. Este factor etiol&oacute;gico se ha tornado a&uacute;n m&aacute;s importante con el manejo no operatorio del trauma espl&eacute;nico. El tiempo entre el evento traum&aacute;tico y el desarrollo del absceso, es generalmente de dos semanas <sup>3</sup>. Los procedimientos radiol&oacute;gicos de 'angioembolia' terap&eacute;utica utilizados como estrategia para controlar el sangrado activo en los pacientes con trauma espl&eacute;nico, pueden producir infartos espl&eacute;nicos con la consecuente formaci&oacute;n de abscesos <sup>5</sup>.</p>     <p><i><b>Enfermedades hematol&oacute;gicas</b></i></p>     <p>Se han relacionado con el desarrollo del 12 % de los abscesos espl&eacute;nicos. Las entidades asociadas son leucemia mielomonoc&iacute;tica cr&oacute;nica, drepanocitosis y d&eacute;ficit de piruvatocinasa <sup>3</sup>. </p>     <p><i><b>Compromiso contiguo</b></i></p>     <p>El &oacute;rgano aleda&ntilde;o que sufre un proceso infeccioso, inflamatorio u oncol&oacute;gico, que se extiende, erosiona y genera trombosis de la arteria espl&eacute;nica, desencadena el proceso de infarto, infecci&oacute;n secundaria y formaci&oacute;n de absceso <sup>4</sup>. Ejemplos de esto son la &uacute;lcera p&eacute;ptica perforada, la diverticulitis y el c&aacute;ncer de colon <sup>3</sup>.</p>     <p><i><b>Cirug&iacute;as g&aacute;stricas</b></i></p>     <p>Se han reportado infartos segmentarios espl&eacute;nicos que se abscedan, secundarios a cirug&iacute;as g&aacute;stricas laparosc&oacute;picas en las cuales se seccionan los vasos g&aacute;stricos cortos (manga g&aacute;strica y fundoplicatura de Nissen) <sup>6-8</sup>. </p>     <p><i><b>Inmunosupresi&oacute;n</b></i></p>     <p>Los pacientes inmunocomprometidos, particularmente aquellos con VIH y los adictos a las drogas intravenosas, tienen mayor riesgo de presentar abscesos espl&eacute;nicos. En algunas series llegan a contribuir con el 25 y el 28 %, respectivamente <sup>9,10</sup>. </p>     <p>Entre otros factores de riesgo se encuentran la diabetes mellitus, el alcoholismo, la artritis reumatoidea, la amiloidosis, el s&iacute;ndrome de Felty y la ictericia hemol&iacute;tica familiar, entre otros <sup>4,9</sup>.</p>     ]]></body>
<body><![CDATA[<p><font size="3" face="Verdana"><b>Presentaci&oacute;n cl&iacute;nica y diagn&oacute;stico</b></font></p>     <p>Las manifestaciones cl&iacute;nicas m&aacute;s frecuentes son la fiebre y el dolor abdominal <sup>11</sup>. La fiebre se presenta en 84 a 92,5 % de los enfermos <sup>3,9</sup>. El dolor en el hipocondrio izquierdo se presenta en 38,2 a 49,8 % de los casos, mientras que el dolor abdominal vago y difuso est&aacute; presente en 57,7 % de los pacientes. Generalmente, los pacientes presentan un s&iacute;ndrome de respuesta inflamatoria sist&eacute;mica, con la consecuente elevaci&oacute;n de los leucocitos; sin embargo, esto no es un hallazgo generalizado y se documenta en alrededor de 60 % de los enfermos<sup> 9</sup>. Se pueden presentar graves complicaciones, como ruptura, hemorragia, empiema y f&iacute;stulas. Eventualmente, puede presentarse como una emergencia abdominal con neumoperitoneo secundario a la producci&oacute;n de gas por g&eacute;rmenes <sup>5,12,13</sup>. </p>     <p>Los hemocultivos son positivos en 48,2 % de los casos. Los cultivos del absceso son positivos en el 72,9 % de los casos y, en 10,8 a 36 %, se cultivan m&uacute;ltiples microorganismos <sup>9,10</sup>. Se ha estimado que alrededor de 20 % de los abscesos espl&eacute;nicos tienen pus est&eacute;ril. Los g&eacute;rmenes que se a&iacute;slan con mayor frecuencia de los abscesos espl&eacute;nicos son <i>Streptococcus spp.</i>, <i>Staphylococcus spp.</i>, <i>Salmonella spp.</i> y <i>Escherichia coli</i>; algunos pueden ser polimicrobianos <sup>14,15</sup>. Se ha reportado <i>Entamoeba histolytica </i>como una causa rara de absceso espl&eacute;nico  <sup>16</sup>. Com&uacute;nmente no se a&iacute;slan las bacterias anaerobias. En pacientes inmunocomprometidos, se pueden aislar <i>Candida spp.</i>, <i>Aspergillus spp.</i>, <i>Mycobacterium tuberculosis</i>, <i>Mycobacterium avium complex</i>, <i>Leishmania spp.</i> o <i>Pneumocystis jirovecii</i><sup>17</sup>.</p>     <p>La tomograf&iacute;a computadorizada (TC) es el examen con mejor desempe&ntilde;o para el diagn&oacute;stico de absceso espl&eacute;nico. Tiene una sensibilidad de 96 % y una especificidad de 90 a 95 %. Puede mostrar un &aacute;rea de baja densidad con realce o sin &eacute;l, &aacute;reas radiol&uacute;cidas dentro del bazo con niveles l&iacute;quidos o de diferente densidad y formaci&oacute;n de gas intraespl&eacute;nico. Este gas puede estar disperso difusamente en el absceso o formar burbujas coalescentes en una o varias colecciones <sup>18</sup>. El 61 a 72 % de los abscesos son solitarios y, del 26,8 al 38,9 % son m&uacute;ltiples <sup>4,9,19</sup>. De manera caracter&iacute;stica, las lesiones m&uacute;ltiples menores de 1 cm de di&aacute;metro, hipointensas y con un halo perif&eacute;rico de realce, sugieren compromiso por c&aacute;ndida o tuberculosis <sup>20</sup>.</p>     <p>El diagn&oacute;stico diferencial del absceso espl&eacute;nico en las im&aacute;genes de tomograf&iacute;a y ecograf&iacute;a, son: el infarto espl&eacute;nico, el hematoma, la neoplasia, los quistes complicados, los abscesos subfr&eacute;nicos y el empiema pleural. Se debe tener cuidado con el diagn&oacute;stico diferencial de absceso subfr&eacute;nico, ya que 4 a 5,3 % de ellos pueden tener un absceso espl&eacute;nico concomitante. De la misma manera, el 4 % de los abscesos espl&eacute;nicos pueden acompa&ntilde;arse de empiema simult&aacute;neo <sup>4</sup>.</p>     <p>El ultrasonido y la TC permiten definir caracter&iacute;sticas relacionadas con el &eacute;xito del drenaje percut&aacute;neo, como ser uniloculares o biloculares, la presencia de tabiques gruesos, el contenido l&iacute;quido y el grosor de la pared. La localizaci&oacute;n del absceso tambi&eacute;n debe establecerse para determinar la mejor aproximaci&oacute;n terap&eacute;utica <sup>1</sup>.</p>     <p>En las im&aacute;genes por resonancia magn&eacute;tica (RM) se observan lesiones solitarias, m&uacute;ltiples o multiloculares. Generalmente, se presenta como una lesi&oacute;n con intensidad de l&iacute;quido, con baja intensidad en T1 y con alta intensidad en las secuencias T2. Cuando se desarrolla una c&aacute;psula, se observa un realce perif&eacute;rico. Los abscesos f&uacute;ngicos son hipointensos en T1 y en T2 <sup>21,22</sup>.</p>     <p>Meshikhes report&oacute; por primera vez el diagn&oacute;stico de tuberculosis espl&eacute;nica mediante biopsia percut&aacute;nea asistida por laparoscopia, evitando la esplenectom&iacute;a en una entidad de manejo no quir&uacute;rgico <sup>23</sup>. La biopsia laparosc&oacute;pica tiene la ventaja de controlar el sangrado y asegurar la hemostasia bajo visi&oacute;n directa, cuando se compara con la biopsia percut&aacute;nea asistida por im&aacute;genes.</p>     <p><font size="3" face="Verdana"><b>Tratamiento</b></font></p>     <p>El tratamiento convencional ha sido la esplenectom&iacute;a. La extracci&oacute;n del &oacute;rgano significa el control completo de los s&iacute;ntomas y la enfermedad. Sin embargo, la mortalidad puede ser muy alta puesto que se trata de pacientes muy enfermos con enfermedades concomitantes graves e inadecuada reserva fisiol&oacute;gica. </p>     ]]></body>
<body><![CDATA[<p>Se ha considerado el tratamiento antibi&oacute;tico como &uacute;nica modalidad terap&eacute;utica. El porcentaje de &eacute;xito es de 59,2 % y la mortalidad observada es de 24,5 % <sup>9</sup>.</p>     <p>El esquema antibi&oacute;tico inicial sugerido es de 2 g intravenosos de oxacilina cada 4 horas y, el alternativo, 1 g intravenoso de vancomicina cada 12 horas. La terapia emp&iacute;rica contra g&eacute;rmenes Gram negativos y enterobacterias, se apoya en las cefalosporinas de tercera generaci&oacute;n, aztreonam, quinolonas o aminogluc&oacute;sidos, asociadas a metronidazol si se sospechan anaerobios. Se contin&uacute;a de acuerdo con los resultados de aislamiento de los g&eacute;rmenes y los antibiogramas. Son cursos de tratamiento prolongados y, aparentemente, su &eacute;xito del tratamiento no depende de la presencia de m&uacute;ltiples abscesos ni del n&uacute;mero de g&eacute;rmenes asilados <sup>24</sup>. Como tratamiento &uacute;nico, estar&iacute;a indicado en pacientes seleccionados. Si no hay pronta mejor&iacute;a, se deben considerar el drenaje percut&aacute;neo, la esplenectom&iacute;a o ambos <sup>25</sup>. Si el germen responsable es <i>M. tuberculosis </i>o <i>Candida spp.</i>, puede ser suficiente la administraci&oacute;n del antibi&oacute;tico o antif&uacute;ngico correspondiente como &uacute;nico tratamiento.</p>     <p><i><b>Drenaje percut&aacute;neo</b></i></p>     <p>Recientemente, se ha demostrado la efectividad del tratamiento m&eacute;dico y el drenaje percut&aacute;neo del absceso  <sup>26</sup>. Sin embargo, varias condiciones interfieren con el pron&oacute;stico, como la enfermedad de base, el n&uacute;mero y el tama&ntilde;o del absceso, y el microorganismo responsable. La aspiraci&oacute;n del absceso con aguja fina es una alternativa intermedia entre el tratamiento antibi&oacute;tico y el drenaje con cat&eacute;ter; se practica por una sola vez cuando se hace el diagn&oacute;stico y se ordenan antibi&oacute;ticos dirigidos de acuerdo con el germen y el antibiograma. Cuando recurre, se puede intentar una nueva aspiraci&oacute;n o la colocaci&oacute;n de un cat&eacute;ter <sup>27,28</sup>. </p>     <p>El drenaje percut&aacute;neo es una excelente alternativa cuando el absceso espl&eacute;nico es unilocular o, bilocular sin tabiques, cuando se localiza en el centro del &oacute;rgano o en el polo inferior y cuando su contenido est&aacute; lo suficientemente l&iacute;quido para poder evacuarlo satisfactoriamente. El n&uacute;mero de colecciones es un importante factor predictor del &eacute;xito del drenaje percut&aacute;neo. Los abscesos multiloculares con tabiques gruesos o con tejido necr&oacute;tico, son menos aptos para el drenaje percut&aacute;neo, como tambi&eacute;n los abscesos m&uacute;ltiples peque&ntilde;os. La tasa de &eacute;xito del drenaje percut&aacute;neo es variable y depende del operador (47-65 %) <sup>29</sup>. </p>     <p>El drenaje percut&aacute;neo est&aacute; indicado especialmente en pacientes cr&iacute;ticos con alto riesgo de ser sometidos a anestesia general. Puede considerarse como un tratamiento inicial del paciente gravemente enfermo, antes de practicar la esplenectom&iacute;a <sup>24</sup>. </p>     <p>Algunos autores recomiendan la punci&oacute;n y el drenaje con aguja fina para abscesos menores de 50 mm de di&aacute;metro y, el drenaje con cat&eacute;ter, para los biloculares y los de mayor tama&ntilde;o <sup>30,31</sup>. </p>     <p>El cat&eacute;ter percut&aacute;neo se retira cuando el paciente se encuentre bien, tenga un drenaje m&iacute;nimo (menos de 5 ml al d&iacute;a) y los estudios imaginol&oacute;gicos no revelen una cavidad residual <sup>1</sup>.</p>     <p>Las ventajas del drenaje percut&aacute;neo comparado con la cirug&iacute;a, son: la preservaci&oacute;n del bazo con la conservaci&oacute;n de su funci&oacute;n inmunol&oacute;gica, las menores complicaciones posquir&uacute;rgicas, menos f&iacute;stulas y mayor aceptaci&oacute;n por el paciente, adem&aacute;s de estancias hospitalarias m&aacute;s cortas y, probablemente, disminuci&oacute;n en costos. Las complicaciones asociadas al drenaje percut&aacute;neo del absceso espl&eacute;nico, son: hemorragia, empiema pleural, neumot&oacute;rax y f&iacute;stulas <sup>32.36</sup>. </p>     <p><b><i>Esplenectom&iacute;a</i></b></p>     ]]></body>
<body><![CDATA[<p>Por a&ntilde;os, el tratamiento de elecci&oacute;n ha sido la esplenectom&iacute;a. Este procedimiento no solo retira el &oacute;rgano enfermo, sino que tambi&eacute;n permite explorar la cavidad abdominal y manejar adecuadamente la sepsis abdominal <sup>37</sup>. </p>     <p>La supervivencia de los pacientes a quienes se les practica esplenectom&iacute;a, es del 86 al 94 % <sup>19</sup>.</p>     <p>En general, el fracaso del tratamiento m&eacute;dico con drenaje o sin &eacute;l, es indicaci&oacute;n de tratamiento quir&uacute;rgico. En algunas series se recomienda la intervenci&oacute;n quir&uacute;rgica para bazos con m&aacute;s de dos abscesos <sup>38</sup>.</p>     <p>Por otra parte, a los pacientes con absceso espl&eacute;nico y anemia falciforme se les debe practicar esplenectom&iacute;a <sup>39</sup>.</p>     <p>El acelerado desarrollo tecnol&oacute;gico ha permitido la extirpaci&oacute;n del bazo por v&iacute;a laparosc&oacute;pica <sup>40</sup>. La esplenectom&iacute;a laparosc&oacute;pica se ha convertido en el m&eacute;todo de elecci&oacute;n para la extracci&oacute;n del bazo en otras indicaciones, dado que ha demostrado presentar menos complicaciones posoperatorias <sup>41-43</sup>. </p>     <p>Al analizar algunas series grandes de esplenectom&iacute;a laparosc&oacute;pica, la mayor&iacute;a de las cirug&iacute;as son por enfermedades hematol&oacute;gicas y los casos de abscesos espl&eacute;nicos son pocos <sup>44,45</sup>. En un reciente reporte de 300 esplenectom&iacute;as laparosc&oacute;picas practicadas en un solo centro, Corcione, et al., reportaron el tratamiento exitoso de un caso de absceso espl&eacute;nico y otro de tuberculosis espl&eacute;nica <sup>46</sup>.</p>     <p>Carbonel, et al., describieron el tratamiento exitoso por v&iacute;a laparosc&oacute;pica de cuatro pacientes con absceso espl&eacute;nico; el tiempo promedio quir&uacute;rgico fue de 200 minutos (rango, 160-220) y la p&eacute;rdida sangu&iacute;nea intraperatoria promedio fue de 220 ml (rango, 100-450). No hubo muertes y no se reportaron complicaciones. Concluyeron que, a pesar de ser un procedimiento t&eacute;cnicamente demandante y dif&iacute;cil, es un tratamiento seguro y efectivo <sup>47</sup>. Los principios quir&uacute;rgicos del procedimiento laparosc&oacute;pico para tratar el absceso espl&eacute;nico, no difieren de los usados para la esplenectom&iacute;a laparosc&oacute;pica por enfermedades hematol&oacute;gicas <sup>48</sup>. La presencia de adherencias firmes y un plastr&oacute;n inflamatorio, supone procedimientos t&eacute;cnicamente m&aacute;s demandantes y mayor tiempo quir&uacute;rgico invertido que, incluso, requieren resecciones parciales del epipl&oacute;n mayor y del fondo g&aacute;strico <sup>49</sup>. </p>     <p>Recientemente, se ha reportado el tratamiento quir&uacute;rgico conservador exitoso en algunos pacientes seleccionados, a quienes se les ha practicado resecciones parciales por v&iacute;a abierta y por v&iacute;a laparosc&oacute;pica <sup>50-54</sup>. </p>     <p>Al igual que para las otras indicaciones, la limitaci&oacute;n del abordaje laparosc&oacute;pico para esplenomegalias masivas (&gt;600 g o di&aacute;metro craneo-caudal &gt;15 cm) se est&aacute; reconsiderando, dado que hasta para bazos de 25 cm el abordaje laparosc&oacute;pico se asocia con una menor estancia hospitalaria <sup>42,45,55</sup>. La esplenectom&iacute;a laparosc&oacute;pica asistida con la mano, puede ser una buena alternativa para disminuir los tiempos quir&uacute;rgicos, como se ha demostrado en la extirpaci&oacute;n de bazos de gran tama&ntilde;o con enfermedades hematol&oacute;gicas <sup>56</sup>. </p>     <p>Se han reportado casos exitosos de esplenectom&iacute;as laparosc&oacute;picas en pacientes con absceso espl&eacute;nicos y esplenomegalia <sup>57</sup>. </p> </font>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana"><b><font size="3">&iquest;Qu&eacute; tratamiento elegir?</font></b></font></p> <font size="2" face="Verdana">     <p>El drenaje percut&aacute;neo es una alternativa segura y efectiva especialmente en abscesos uniloculares o biloculares, lo cual permite preservar el bazo. Por tanto, debe considerarse como primera l&iacute;nea de tratamiento; cuando fracasa, debe considerarse la esplenectom&iacute;a. En un an&aacute;lisis retrospectivo de 75 pacientes con absceso espl&eacute;nico, se observ&oacute; que aquellos sometidos a esplenectom&iacute;a presentaban mayores complicaciones y mayor tasa de neumon&iacute;as, y no encontraron diferencias en la estancia hospitalaria <sup>38</sup>. Dado que el pron&oacute;stico del paciente con absceso espl&eacute;nico depende de la etiolog&iacute;a, los microorganismos, las enfermedades concomitantes y el compromiso anat&oacute;mico de la infecci&oacute;n, el tratamiento de elecci&oacute;n se debe ajustar a cada paciente.</p>     <p>La revisi&oacute;n de la literatura recomienda la esplenectom&iacute;a laparosc&oacute;pica sobre la esplenectom&iacute;a abierta, independientemente de la indicaci&oacute;n de la esplenectom&iacute;a y la condici&oacute;n f&iacute;sica del paciente <sup>42</sup>. Aunque con tiempos quir&uacute;rgicos m&aacute;s prolongados, la esplenectom&iacute;a laparosc&oacute;pica se asocia con una reducci&oacute;n significativa de la morbilidad posquir&uacute;rgica <sup>41</sup>. </p>     <p>La esplenectom&iacute;a laparosc&oacute;pica puede cursar con menor morbilidad, aunque la evoluci&oacute;n posoperatoria puede diferir seg&uacute;n la causa del absceso, sus caracter&iacute;sticas y las de las enfermedades concomitantes. A pesar de que los resultados iniciales son promisorios, con potenciales ventajas, la experiencia actual es limitada para justificar su lugar en el tratamiento del absceso espl&eacute;nico.
