Materiales y Métodos: se realizó la búsqueda y selección de la literatura relevantes en Pubmed y Medline, usando los términos: "Medical Emergency Teams", "Rapid Response Teams", "Acute Teams"; combinándolos con "Emergency Medicine", "Emergency Physicians" y "Emergency Departments" desde 1990 hasta el año 2009. Resultados: Actualmente es recomendada la conformación de los EEM como una medida en pro de la seguridad intrahospitalaria del paciente; sin embargo la literatura es conflictiva ya que algunos estudios sugieren que las intervenciones de estos equipos no disminuyen la frecuencia de presentación del paro cardiorrespiratorio, ni las admisiones a las unidades de cuidados intensivos. No es posible justificar la presencia de urgentólogos en estos equipos desde el punto de vista terapéutico, pues no existe literatura que compare los resultados de estos equipos en cuanto a los miembros que los conforman, aunque desde el punto de vista administrativo la presencia de urgentólogos en los EEM potencialmente impacta negativamente en la dinámica del servicio de urgencias. En vista de lo anterior no es recomendable incluir médicos especialistas de medicina de urgencias en los EEM.]]></p></abstract>

Therefore, it cannot be recommended to include emergency physicians within those teams given that the economic overcost involved for health institutions in emergency services may not compensate for the results achieved by METs.     <p><b>KEY WORDS </b>     <p>  Hospital safety, Rapid response teams (RRTs), Emergency response teams (ERTs), Hemodynamic instability. <hr size="1" noshade></p>     <p>&nbsp;</p> </FONT>     <p><b><font face="Verdana" SIZE="3">INTRODUCCI&Oacute;N</font></b></p>      <p><font size="2" face="Verdana">Si bien es cierto que la existencia de los hospitales se justifica por el hecho de ser instituciones especializadas en el cuidado y atenci&oacute;n de personas enfermas, tambi&eacute;n es cierto que la seguridad de los pacientes que se encuentran en las salas de hospitalizaci&oacute;n general ha sido puesta en duda (1). Se ha llegado a sugerir que el simple hecho de estar hospitalizado puede representar un factor de riesgo para eventos adversos (2). Estos eventos se explican por situaciones como escasez de personal en algunos horarios espec&iacute;ficos (3), falta de monitoreo constante del paciente hospitalizado o traslados tempranos desde otras unidades como los servicios de urgencias, los servicios de recuperaci&oacute;n postquir&uacute;rgica o las unidades de cuidados intensivos (4). Este cuestionamiento a la seguridad hospitalaria del paciente ha llevado a discusiones sobre las alternativas sobre si es posible prevenir parte de estos eventos adversos (5) pues se ha propuesto que con ciertas medidas puede disminuirse hasta en un 37 &#37; (6).</font></p>     <p><font size="2" face="Verdana"> Lo anterior se ha materializado en numerosas   campa&ntilde;as como la de salvamento de vidas (7),   las cuales insisten en la prevenci&oacute;n y disminuci&oacute;n   de eventos adversos intrahospitalarios a   trav&eacute;s de procedimientos como lavarse las manos,   verificar las dosificaciones de medicamentos   e identificar correctamente los pacientes. En   el campo de la respuesta intrahospitalaria ante   el paciente que se reagudiza, se ha sugerido la   necesidad de implementar sistemas de respuesta   r&aacute;pida (RRS, por sus siglas en ingl&eacute;s), dentro   de los cuales uno de sus componentes o brazo   es el equipo de emergencia m&eacute;dica (EEM): un   equipo conformado por personal de salud que   debe acudir inmediatamente ante el llamado de   quien identifique pacientes con signos espec&iacute;ficos de deterioro hemodin&aacute;mico, con el fin de suministrar terapias de estabilizaci&oacute;n para evitar la progresi&oacute;n sintom&aacute;tica hacia la muerte o a un ingreso no planeado a la unidad de cuidados intensivos (8,9). </font></p>     <p><font size="2" face="Verdana">Los sistemas de respuesta r&aacute;pida est&aacute;n constituidos   por cuatro brazos: un brazo administrativo   que soporta administrativa y econ&oacute;micamente   el sistema; un brazo aferente que se encarga   de detectar los pacientes inestables hemodin&aacute;micamente   (cualquier personal de salud del hospital);   un brazo eferente que es como tal el EEM   quien interviene terap&eacute;uticamente al paciente; y   un brazo epidemiol&oacute;gico que analiza los datos obtenidos por el sistema y lo retroalimenta.</font></p>     <p><font size="2" face="Verdana"> El cuestionamiento mas grande que se ha realizado   al sistema de respuesta r&aacute;pida a reca&iacute;do en   los EEM, pues los miembros que lo conforman   no est&aacute;n estandarizados, lo que conlleva a que su   conformaci&oacute;n sea muy heterog&eacute;na, ya que en la   pr&aacute;ctica actual se encuentra gran variedad en los   miembros de estos equipos como: un fellow en   cuidado cr&iacute;tico y una enfermera (10); un grupo de   enfermeras de la unidad de cuidados intensivos   (11); un m&eacute;dico especialista en cuidados intensivos,   un m&eacute;dico general y una enfermera de cuidados   intensivos (12); m&eacute;dico especialista en cuidado   cr&iacute;tico, un m&eacute;dico especialista en medicina de   urgencias, un m&eacute;dico general y una enfermera (13);   o combinaciones que incluyen anestesi&oacute;logos (14)   terapeutas respiratorios (15) o farmaceutas (16);   esta heterogeneidad se ha postulado como una   de las causas de dificultad en la evaluaci&oacute;n de los   resultados obtenidos por los EEM (17). </font></p>     <p><font size="2" face="Verdana">El fundamento de los EEM comenz&oacute; en la d&eacute;cada   de los noventa, cuando se iniciaron los   an&aacute;lisis de los registros sobre las condiciones   hemodin&aacute;micas en las horas previas al paro cardiorrespiratorio   en las salas de hospitalizaci&oacute;n   general (18,19). Tales estudios sugirieron que   es posible detectar subgrupos de pacientes por   la homogeneidad de las caracter&iacute;sticas cl&iacute;nicas   que compart&iacute;an sin importar su diagn&oacute;stico   primario (20). La teor&iacute;a consiste en que en un   tiempo aproximado de entre seis y ocho horas   previas al paro circulatorio, el paciente presenta   signos de inestabilidad fisiol&oacute;gica, los cuales, si   son detectados y corregidos a tiempo, pueden   evitar el colapso hemodin&aacute;mico (21,22), De ah&iacute;   que, conformar grupos para intervenir oportunamente   a estos pacientes, podr&iacute;a disminuir la   frecuencia de presentaci&oacute;n del catastr&oacute;fico paro   cardiorrespiratorio (23-25). </font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">En el a&ntilde;o 1995 se public&oacute; el primer estudio sobre   la efectividad de implementar un equipo interdisciplinario   que respondiera ante el llamado   del personal asistencial por el s&uacute;bito deterioro   hemodin&aacute;mico de pacientes hospitalizados (26).   Este deterioro no implicaba el paro cardiorrespiratorio   sino criterios espec&iacute;ficos de inestabilidad   hemodin&aacute;mica (27). Subsecuentes reportes demostraron   que la implementaci&oacute;n de este tipo   de intervenciones pod&iacute;an reducir las muertes   hospitalarias o los ingresos inesperados a unidades   de cuidados intensivos, convirti&eacute;ndose   en una excelente alternativa para la seguridad   del paciente hospitalizado y por ende con un   impacto positivo en la calidad de la atenci&oacute;n que ofrece la instituci&oacute;n de salud (28).</font></p>     <p><font size="2" face="Verdana"> Lo anterior llev&oacute; a que 2 300 hospitales en Estados   Unidos implementaran los EEM entre los   a&ntilde;os 2001 y 2005 (17). En los &uacute;ltimos a&ntilde;os la   implementaci&oacute;n de estos equipos se ha vuelto   una pr&aacute;ctica cada vez m&aacute;s com&uacute;n a nivel hospitalario   (17,28), extendi&eacute;ndose a la poblaci&oacute;n   pedi&aacute;trica (10). </font></p>     <p><font size="2" face="Verdana">Subsecuentes publicaciones demostraron que   la creaci&oacute;n de EEM impactaba positivamente en   los ingresos no planeados a las unidades de cuidados   intensivos, la reducci&oacute;n de paro cardiorrespiratorio   y el costo total por hospitalizaci&oacute;n   (28-35). De ah&iacute; que se recomendara la inclusi&oacute;n   de estos equipos en la campa&ntilde;a de salvamentos   de vidas, sugiri&eacute;ndose incluso que la no conformaci&oacute;n   de ellos pudiese ser anti&eacute;tica (36,37). </font></p>     <p><font size="2" face="Verdana">El auge obtenido por las experiencias de estos   equipos gener&oacute; que en el a&ntilde;o 2006 se realizara el primer consenso sobre los sistemas de respuesta r&aacute;pida para impulsar y clarificar la terminolog&iacute;a a usar en la publicaci&oacute;n de estudios y para definir las estrategias para la normatividad general en la implementaci&oacute;n de ellos (17,38). Sin embargo es necesario resaltar que buena parte de las publicaciones que respaldan la conformaci&oacute;n de la estrategia de EEM est&aacute; basada en estudios pre y post intervenci&oacute;n, la cual se defiende por parte de algunos autores como la &uacute;nica metodolog&iacute;a posible en la realizaci&oacute;n de estudios sobre EEM (39,40), ya que a este tipo de pacientes no se les puede someter a un efecto placebo. A continuaci&oacute;n se presentan algunos de tales estudios que han mostrado resultados exitosos con la implementaci&oacute;n de los EEM (<a href="#t1">cuadro 1</a>).</font></p>     <p>&nbsp;</p>     <p align="center"><img src="/img/revistas/cesm/v23n2/v23n2a05t1.jpg"/><a name="t1" id="t1"></a></p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"> Los EEM fueron propuestos hace 10 a&ntilde;os como una estrategia que puede disminuir los ingresos no planeados a las unidades de cuidados intensivos y en igual medida la frecuencia de paro cardiorrespiratorio intrahospitalario, sin embargo, la efectividad de estos equipos no parece ser tan contundente. De ah&iacute; que, pese al entusiasmo inicial de la efectividad de los EEM, en la misma medida han surgido publicaciones que no han demostrado la reproductibilidad en los resultados de estos equipos. Estas publicaciones provienen igualmente de la experiencia pre y post intervenci&oacute;n de hospitales (41,42), revisiones sistem&aacute;ticas (43,44), estudios aleatorios (45) y metan&aacute;lisis (46); publicaciones que han creado dudas sobre la efectividad de estos equipos, sugiriendo que no se deber&iacute;a generalizar la recomendaci&oacute;n de implementar estos equipos en todos los hospitales con unidades de cuidados intensivos (44,46) (ver cuadro 2). </font></p>     <p><font size="2" face="Verdana">La eficacia de los EEM no es clara, se acepta (41)   que es una intervenci&oacute;n que no causa efectos   adversos, sin embargo actualmente la literatura   no reporta cu&aacute;l es la conformaci&oacute;n &oacute;ptima de   estos equipos. Los hospitales que deseen conformarlos,   pueden adaptar estos equipos de   acuerdo al personal con el que cuentan en la   instituci&oacute;n, como enfermeras y terapeutas respiratorios   y/o m&eacute;dicos generales bajo la supervisi&oacute;n   y comunicaci&oacute;n constante con los m&eacute;dicos   de las unidades de cuidados intensivos pues son estrategias reportadas en la literatura (10-12). </font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="center"><img src="/img/revistas/cesm/v23n2/v23n2a05t2.jpg"/><a name="t2" id="t2"></a></p>     <p align="center">&nbsp;</p>     <p><font size="2" face="Verdana"> Es necesario recordar que el concepto de los EEM est&aacute; ligado administrativamente a la unidad de cuidados intensivos, pues es all&iacute; donde reingresan los pacientes que son efectivamente reanimados despu&eacute;s de un paro o que persisten en deterioro hemodin&aacute;mico, pese a las medidas que se puedan tomar en las unidades de hospitalizaci&oacute;n. Lo anterior, con respecto a los m&eacute;dicos especialistas en medicina de urgencias, implica un sobrecosto para las instituciones de salud pues se concibe que estos equipos sean pagados por esperar la llamada de activaci&oacute;n del sistema sin tener otras labores simult&aacute;neas, pues se considera indeterminado el tiempo que lleva la estabilizaci&oacute;n del paciente al cual acude el EEM. </font></p>     <p><font size="2" face="Verdana">Si se incluyera el especialista en medicina de   urgencias en estos equipos, asumiendo que no   trabajar&aacute; en el servicio de urgencias por esperar   las llamadas de activaci&oacute;n del EEM, encontrarse   en entrenamiento del EEM o estabilizando pacientes,   implicar&iacute;a un m&eacute;dico especialista fuera   de su &aacute;rea de trabajo y a otra persona asumiendo   la ausencia en el departamento de urgencias,   quien, bajo las condiciones actuales de nuestro   medio, no ser&aacute; otro especialista en medicina de   urgencias por el escaso n&uacute;mero con el que se   cuenta actualmente en el pa&iacute;s. Al respecto, es   necesario recordar que Colombia tiene un sistema   de salud complejo, en el cual es claro que   el n&uacute;mero de especialistas a nivel nacional es   insuficiente; actualmente Colombia cuenta con   menos de 100 especialistas en medicina de urgencias   lo cual sugiere que ante la escasez de   este personal se debe buscar la mejor manera   de optimizar su rendimiento laboral. </font></p>     <p><font size="2" face="Verdana">Lo anterior puede alterar la realizaci&oacute;n del triage,   aumentar los tiempos de espera por parte de los   pacientes o impactar negativamente en la atenci&oacute;n   inicial de pacientes cr&iacute;ticos en este servicio,   lo cual se traduce en una dudosa intervenci&oacute;n   hacia el paciente que est&aacute; siendo evaluado por   el EEM y en una posible repercusi&oacute;n negativa   en la calidad de atenci&oacute;n del servicio de urgencias,   unidades de las cuales son operativamente   responsables los especialistas en medicina de urgencias (47-50)</font></p>     <p><font size="2" face="Verdana"> En esta revisi&oacute;n bibliogr&aacute;fica no se encontraron   estudios que compararan la efectividad de los   EEM con respecto a los miembros que los conforman,   por lo que realizar una recomendaci&oacute;n   exclusivamente sobre los resultados terap&eacute;uticos   contando con urgent&oacute;logos no es posible.   Sin embargo, s&iacute; parece ser posible hacerla desde   un punto de vista administrativo: con la evidencia   actual y con las dudas que se tiene sobre el   costo beneficio de los EEM, no es recomendable   incluir m&eacute;dicos especialistas en medicina de urgencias en dichos equipos.</font></p>     <p><font size="2" face="Verdana"><c> </font></p>     <p>&nbsp;</p>     <p><b><font face="Verdana" SIZE="3">REFERENCIAS</font></b></p> <FONT SIZE="2" FACE="Verdana"></FONT> <FONT SIZE="2" FACE="Verdana">     <!-- ref --><p>1.
