Método Foram selecionadas as bases de dados eletrônicas MEDLINE via Pubmed, Scielo, Web of Science e SCOPUS. A estratégia de busca incluiu os descritores propostos no Medical Subject Headings (MeSH): "Motor Activity", "Activities, "Nutritional Status", "Overweight", "Obesity", "Body Mass Index", "Child", "Brazil". Resultados A busca permitiu identificar 141 artigos, sendo que 16 estudos foram considerados potencialmente relevantes e foram incluídos na revisão. Conclusão Estudos com as temáticas: estado nutricional e nível de atividade física em crianças brasileiras ainda são escassos, mas vem aumentando nos últimos anos, especialmente aqueles com delineamentos transversais, bem como a utilização de questionários para mensuração da atividade física e do IMC para o estado nutricional ainda é amplamente utilizada. Além disso, nos estudos analisados a quantidade de horas atribuídas em comportamentos sedentários como: televisão, vídeo-game e computador foi superior a 2 horas diárias.]]></p></abstract>

La estrategia de búsqueda incluyó los descriptores propuestos en el Medical Subject Headings (MeSH): "Motor Activity", "Activities, "Nutritional Status", "Overweight", "Obesity", "Body Mass Index", "Child", "Brazil". Resultados: La búsqueda permitió identificar 141 artículos, de los cuales se consideraron potencialmente relevantes 16 estudios y se incluyeron en la revisión. Conclusiones Estudios temáticos: estado nutricional y los niveles de actividad física en los niños brasileños son todavía escasos, pero ha ido en aumento en los últimos años, especialmente aquellos con diseños cruzados, así como el uso de cuestionarios para medir la actividad física y el IMC para el estado nutricional todavía es ampliamente utilizado. O inadequado consumo alimentar aliado ao sedentarismo s&atilde;o fatores preditores do excesso de peso corporal (1,2). Em algumas regi&otilde;es do Brasil a preval&ecirc;ncia de excesso de peso corporal varia e pode atingir mais da metade das crian&ccedil;as e adolescentes e &eacute; agravada pelo crescente aumento do sedentarismo nestas fases et&aacute;rias (1,3), pois grande percentual n&atilde;o cumpre as recomenda&ccedil;&otilde;es m&iacute;nimas para pr&aacute;tica de atividade f&iacute;sica (4-6).</p>      <p>Deve-se salientar n&atilde;o somente a relev&acirc;ncia social do reconhecimento deste grave problema de sa&uacute;de na atualidade (6), mas principalmente a import&acirc;ncia cient&iacute;fica de pesquisas sobre n&iacute;vel de atividade f&iacute;sica e estado nutricional, principalmente porque houve crescimento substancial nas &uacute;ltimas d&eacute;cadas da produ&ccedil;&atilde;o cient&iacute;fica mundial (7,8) e brasileira (7,9,10); inclusive com aumento no n&uacute;mero de peri&oacute;dicos cient&iacute;ficos nacionais indexados nas principais bases de dados internacionais (8).</p>      <p>Contudo, apesar da relev&acirc;ncia de pesquisas sobre n&iacute;vel de atividade f&iacute;sica e estado nutricional em crian&ccedil;as brasileiras, deve-se destacar a escassez de estudos sobre estas tem&aacute;ticas, principalmente de revis&otilde;es sistem&aacute;ticas, demonstrando-se assim a necessidade de realiza&ccedil;&atilde;o de uma revis&atilde;o sistem&aacute;tica sobre o assunto. Assim, este estudo tem como objetivo revisar, de forma sistem&aacute;tica, a literatura sobre a preval&ecirc;ncia e fatores associados ao n&iacute;vel de atividade f&iacute;sica e ao estado nutricional de crian&ccedil;as brasileiras.</p>       <p align="center"><b>M&Eacute;TODO</b></p>      ]]></body>
<body><![CDATA[<p>Esta revis&atilde;o sistem&aacute;tica foi realizada de acordo com as recomenda&ccedil;&otilde;es da Colabora&ccedil;&atilde;o Cochrane (11) e Preferred Reporting Items for Systematic Review and Meta-analyses: The PRISMA Statement (12)<I>. </I></p>      <p><b>Crit&eacute;rios de elegibilidade</b></p>      <p>Para esta revis&atilde;o foram inclu&iacute;dos estudos originais de base populacional ou escolar; com amostra de crian&ccedil;as brasileiras; medida do estado nutricional efetuada atrav&eacute;s do IMC; com resumos dispon&iacute;veis e que foram acessados na &iacute;ntegra pelo meio on-line sem restri&ccedil;&atilde;o de ano e de idioma.</p>      <p><b>Estrat&eacute;gias de busca</b></p>      <p>Foram selecionadas as bases de dados eletr&ocirc;nicas <I>MEDLINE (Medical Literature Analysis and Retrieval System on-line)</I> via <I>Pubmed</I>, Scientific Eletronic Library Online (Scielo), Web of Science e <I>SCOPUS(Elsevier).</I> A estrat&eacute;gia de busca incluiu os descritores propostos no <I>Medical Subject Headings</I> (MeSH) seus correlatos: "Motor Activity", "Activities, Motor", "Activity, Motor", "Motor Activities", "Physical Activity", "Activities, Physical", "Activity, Physical", "Physical Activities", "Locomotor Activity", "Activities, Locomotor", "Activity, Locomotor", "Locomotor Activities", level of physical activity, "Nutritional Status", "Status, Nutritional", "Nutrition Status", "Status, Nutrition", "Overweight", "Obesity", "Body Mass Index", "Index, Body Mass", "Quetelet Index", "Index, Quetelet", "Quetelet's Index", "Quetelets Index", "Child", "Children","Brazil". Todas as estrat&eacute;gias de busca foram desenvolvidas no m&ecirc;s de Janeiro de 2014.</p>      <p><b>Sele&ccedil;&atilde;o dos estudos e extra&ccedil;&atilde;o dos dados</b></p>      <p>Os t&iacute;tulos e resumos de todos os artigos identificados pela estrat&eacute;gia de busca foram avaliados por dois autores deste trabalho, de forma independente. Nessa segunda fase, os revisores avaliaram independentemente os artigos completos e fizeram suas sele&ccedil;&otilde;es, de acordo com os crit&eacute;rios de elegibilidade pr&eacute;-especificados. As discord&acirc;ncias entre os revisores foram resolvidas por consenso.</p>      <p>Os dados extra&iacute;dos foram: identifica&ccedil;&atilde;o da publica&ccedil;&atilde;o, tipo de delineamento, faixa et&aacute;ria, tamanho da amostra, local da coleta dos dados, metodologia utilizada, as vari&aacute;veis associadas ao sedentarismo, obesidade e o crit&eacute;rio de ponto de corte do n&iacute;vel de atividade f&iacute;sica e do estado nutricional.</p>      <p align="center"><b>RESULTADOS</b></p>      <p><b>Quanto &agrave; busca sistem&aacute;tica</b></p>      ]]></body>
<body><![CDATA[<p>A busca permitiu identificar 141 artigos, sendo que ap&oacute;s a avalia&ccedil;&atilde;o geral, foram exclu&iacute;dos 68 estudos que demonstraram n&atilde;o se tratarem da tem&aacute;tica pelos t&iacute;tulos e 38 por seus resumos n&atilde;o contemplarem as tem&aacute;ticas investigadas no presente estudo.</p>      <p>Ap&oacute;s a avalia&ccedil;&atilde;o detalhada, foram exclu&iacute;dos 19 estudos pelos seguintes motivos: n&atilde;o apresentavam as informa&ccedil;&otilde;es quanto ao n&iacute;vel de atividade f&iacute;sica ou quanto ao estado nutricional; j&aacute; haviam sido analisados por outra base de dados; o tipo de pesquisa n&atilde;o era condizente com a tem&aacute;tica; ou a amostra n&atilde;o era composta por crian&ccedil;as. Deste modo, 16 estudos foram considerados potencialmente relevantes e foram inclu&iacute;dos na revis&atilde;o.</p>      <p><b>Quanto &agrave;s caracter&iacute;sticas sociodemogr&aacute;ficas e metodol&oacute;gicas</b></p>      <p>A faixa et&aacute;ria dos estudos abrangeu al&eacute;m de crian&ccedil;as tamb&eacute;m adolescentes (f=9) (13-21), sendo que as idades tiveram varia&ccedil;&atilde;o de 0 (22) a 19 anos (7), e que todos os estudos analisados foram realizados tanto com sujeitos do sexo masculino quanto do sexo feminino. A abrang&ecirc;ncia dos estudos investigados foi estadual (f=8) (14,15,18,19,21,23-25) e municipal (f=8) (13,16,17,20,22,26-28).</p>      <p>A maior parte dos estudos investigados apresentou delineamento transversal (f=15) (13-21,23-28), sendo que apenas um consistiu em um estudo de coorte (22).</p>      <p>O tamanho das amostras estudadas variou de 116 (13) a 5 249 (22). Foram encontrados estudos com mais de uma d&eacute;cada (25) e estudos com pouco  menos de dois anos (13,14,22), sendo que quando analisado o ano de publica&ccedil;&atilde;o verificou-se que oito estudos foram publicados nos &uacute;ltimos cinco anos, o que indica um aumento substancial de artigos nessa &aacute;rea (13-16,23,24,26,27).</p>      <p>No que se refere aos m&eacute;todos de detec&ccedil;&atilde;o do n&iacute;vel de atividade f&iacute;sica, pode-se verificar que em todos os estudos foi utilizada a aplica&ccedil;&atilde;o de question&aacute;rios e que em apenas um estudo (28) o question&aacute;rio n&atilde;o foi aplicado aos participantes da pesquisa, e sim, aos seus pais, os quais relataram sua percep&ccedil;&atilde;o quanto ao n&iacute;vel de atividade f&iacute;sica de seus filhos.</p>      <p>J&aacute; no que tange aos m&eacute;todos de detec&ccedil;&atilde;o do estado nutricional identificou-se que todos os estudos adotaram como padr&atilde;o de refer&ecirc;ncia o &Iacute;ndice de Massa Corporal (IMC), ou seja, baseado no peso corporal e estatura corporal. Contudo, verificou-se a utiliza&ccedil;&atilde;o concomitante ao IMC de medidas diretas de per&iacute;metro ou circunfer&ecirc;ncia de cintura, ou abd&ocirc;men ou pelve por dois estudos (21,26), dobras cut&acirc;neas por tr&ecirc;s estudos (16,21,25), e imped&acirc;ncia bioel&eacute;trica pelo estudo de Ribeiro (21). Ainda em rela&ccedil;&atilde;o &agrave; mensura&ccedil;&atilde;o do estado nutricional, cinco artigos utilizaram como ponto de corte para an&aacute;lise dessa vari&aacute;vel as recomenda&ccedil;&otilde;es da Organiza&ccedil;&atilde;o Mundial de Sa&uacute;de (OMS) (13,15,22,23,26), quatro estudos as do Centro de Controle de Doen&ccedil;as dos Estados Unidos (17,19,21,27), quatro estudos de Cole (29) (15,18,24,25) dois estudos de Must (30) (16,28) e um estudo a de Rolland-Cachera (31) (29).</p>      <p>Os resultados quanto &agrave;s caracter&iacute;sticas sociodemogr&aacute;ficas e metodol&oacute;gicas est&atilde;o apresentados na <a href="#t1">Tabela 1</a>.</p>     <p align="center"><a name="t1"></a><img src="img/revistas/rsap/v18n4/v18n4a11t1.jpg"></p>     ]]></body>
<body><![CDATA[<p><b>Quanto aos achados relevantes</b></p>      <p>Os principais resultados de cada estudo inclu&iacute;do na presente revis&atilde;o sobre atividade f&iacute;sica e estado nutricional est&atilde;o apresentados na <a href="#t2">Tabela 2</a>. Deste modo, pode-se constatar que no Brasil as preval&ecirc;ncias de atividade f&iacute;sica variam conforme a regi&atilde;o do estudo de 27,1 % (26) a 92,8 % (13). J&aacute; as preval&ecirc;ncias de inatividade f&iacute;sica variaram de 22,6 % (21) a 93,5 % (16). Em rela&ccedil;&atilde;o ao estado nutricional as preval&ecirc;ncias de sobrepeso e obesidade variaram entre 3,1% (21) e 38,9 % (23).</p>     <p align="center"><a name="t2"></a><img src="img/revistas/rsap/v18n4/v18n4a11t2.jpg"></p>      <p align="center"><b>DISCUSS&Atilde;O</b></p>      <p>Este estudo adotou procedimentos para busca e sele&ccedil;&atilde;o de artigos por meio de bancos de dados amplamente utilizados para realiza&ccedil;&atilde;o de uma revis&atilde;o sistem&aacute;tica visando reunir as publica&ccedil;&otilde;es e conhecimentos acerca da preval&ecirc;ncia, associa&ccedil;&otilde;es e ocorr&ecirc;ncias do n&iacute;vel de atividade f&iacute;sica e do estado nutricional de crian&ccedil;as brasileiras.</p>      <p>Os estudos levantados analisavam o n&iacute;vel de atividade f&iacute;sica e/ou do estado nutricional conjuntamente a outras vari&aacute;veis, sendo essas: sono e tempo e perman&ecirc;ncia sentado, sexo, horas de ado&ccedil;&atilde;o de comportamentos sedent&aacute;rios, idade, classe econ&ocirc;mica, meio de deslocamento para a escola, tempo de lazer, tempo no ambiente escolar, consumo alimentar e escolaridade dos pais.</p>      <p>No estudo de Giugliano (25) observou-se que cerca de 75 % da rotina di&aacute;ria das crian&ccedil;as est&aacute; distribu&iacute;da entre horas de sono e perman&ecirc;ncia sentada. Foram constatadas diferen&ccedil;as na rotina di&aacute;ria entre os grupos estudados quanto ao "tempo de perman&ecirc;ncia sentado", mais elevado no grupo com sobrepeso e obesidade que no grupo sem, em ambos os sexos. A diferen&ccedil;a torna-se significativa (p&lt;0,05) quando se comparam as crian&ccedil;as normais com as obesas. </p>       <p>Os estudos que investigaram o n&iacute;vel de atividade f&iacute;sica e sua associa&ccedil;&atilde;o com o sexo identificaram que os meninos s&atilde;o mais ativos quando comparados &agrave;s meninas (16,22,25). Ainda que &agrave;queles participantes que apresentavam maiores n&iacute;veis de obesidade/sobrepeso possu&iacute;am menor n&iacute;vel de atividade f&iacute;sica e mais horas em comportamentos sedent&aacute;rios (24,25), exceto para Nunes (17), que n&atilde;o demonstrou nenhuma rela&ccedil;&atilde;o, j&aacute; que 82,8 % dos participantes com peso normal foram considerados inativos, 75 % dos com obesidade e 78,4 % dos com sobrepeso. </p>      <p>Em seis dos sete estudos que mensuraram a quantidade de horas atribu&iacute;das em comportamentos sedent&aacute;rios como: televis&atilde;o, v&iacute;deo-game, computador, apresentaram apenas dados superiores a duas horas di&aacute;rias, chegando at&eacute; 5,5 horas, contrariando alguns artigos internacionais, como Proctor (32), que ao realizarem estudos com crian&ccedil;as e adolescentes com idade entre 4 e 11 anos dos Estados Unidos, constataram que o tempo m&eacute;dio em frente &agrave; televis&atilde;o era de apenas 1,6 horas. Assim como Giammattei (33) que ao realizarem estudos com crian&ccedil;as e adolescentes com idade entre 11 e 13 anos da Calif&oacute;rnia, Estados Unidos, verificaram 2,4 horas; e Lioret (34) que ao analisarem crian&ccedil;as e adolescentes de 3 a 17 anos na Fran&ccedil;a, verificaram 2,2 horas di&aacute;rias para ambos os sexos. </p>      <p>Segundo a literatura especializada, o tempo de assist&ecirc;ncia &agrave; televis&atilde;o em muitos pa&iacute;ses desenvolvidos &eacute; reduzido pelo fato de que a educa&ccedil;&atilde;o nesses pa&iacute;ses ocorre em tempo integral, diminuindo o tempo dispon&iacute;vel para esse comportamento (32,33,35,36). Acredita-se que o tempo em tela sejam indicadores de comportamento sedent&aacute;rio consider&aacute;veis e que est&aacute; associado a obesidade, principalmente por dois mecanismos: redu&ccedil;&atilde;o da atividade f&iacute;sica e aumento do consumo alimentar inadequado durante o tempo em que se dispende para tais atividades (37).</p>      ]]></body>
<body><![CDATA[<p>Em rela&ccedil;&atilde;o aos resultados encontrados quanto &agrave;s faixas et&aacute;rias, o estudo de Coelho (14) n&atilde;o demonstrou diferen&ccedil;as significativas entre os grupos de 6 a 9 anos (9,0 %) e 10 a 14 anos (10,4 %) em rela&ccedil;&atilde;o ao percentual de gordura corporal, assim como de Creem (23), entre os grupos menores de 6 anos (35,4 %) e de 6 a 10 anos (38,9 %) em rela&ccedil;&atilde;o ao excesso de peso e demonstrou que 81,1% das crian&ccedil;as menores de 6 anos eram inativas e 49,3 % das maiores de 6 anos, diferindo do estudo de Duncan (15), que constatou que as chances de sobrepeso e obesidade foram 0,36 vezes menor no grupo et&aacute;rio de 11 aos 14 anos, quando comparados com crian&ccedil;as de 7 a 10 anos e Siqueira (24) onde a faixa et&aacute;ria de 6 a 9 anos apresentou as maiores frequ&ecirc;ncias de sobrepeso e obesidade (48 % dos casos). </p>      <p>Quanto &agrave; classe econ&ocirc;mica, Nunes (17) demonstrou que a propor&ccedil;&atilde;o de crian&ccedil;as com sobrepeso e obesidade foi significativamente maior nas classes econ&ocirc;micas A1, A2 e B1 (31,4 %) do que nas classes C, D e E (18,1 %) (38). Ainda, estudos t&ecirc;m demonstrado que a ocorr&ecirc;ncia de obesidade entre os jovens brasileiros parece estar mais relacionada &agrave;s quest&otilde;es socioecon&ocirc;micas do que as quest&otilde;es biol&oacute;gicas (37).</p>      <p>No estudo conduzido por Su&ntilde;e (18) verificou-se um aumento superior a seis vezes no risco de apresentar sobrepeso ou obesidade nos participantes moderadamente ativos e superior a quatro vezes no risco de sobrepeso e obesidade em inativos. Ainda, a atividade f&iacute;sica esteve associada &agrave; ocorr&ecirc;ncia da obesidade, entretanto, os resultados parecem sofrer influ&ecirc;ncia do delineamento adotado (causalidade reversa), por se tratar de um estudo transversal, no qual n&atilde;o se podem definir com clareza as rela&ccedil;&otilde;es de causa e efeito. </p>      <p>Outro fator relevante referente &agrave;s diferen&ccedil;as encontradas nos estudos conduzidos no Brasil pode estar relacionado ao crit&eacute;rio de ponto de corte no que diz respeito ao estado nutricional, j&aacute; que cinco artigos utilizaram como ponto de corte para an&aacute;lise dessa vari&aacute;vel as recomenda&ccedil;&otilde;es da OMS, sendo esses mais atuais, enquanto quatro utilizaram Cole (29), dois utilizaram Must (30), quatro utilizaram as recomenda&ccedil;&otilde;es da CDC e um utilizou Rolland-Cachera (31).</p>      <p>Cabe destacar, que diante dos resultados apresentados, estudos com as tem&aacute;ticas: estado nutricional e n&iacute;vel de atividade f&iacute;sica em crian&ccedil;as brasileiras ainda s&atilde;o escassos, mas vem aumentando nos &uacute;ltimos anos, especialmente aqueles com delineamentos transversais, bem como a utiliza&ccedil;&atilde;o de question&aacute;rios para mensura&ccedil;&atilde;o da atividade f&iacute;sica e do IMC para o estado nutricional ainda &eacute; amplamente utilizada.</p>      <p>&Eacute; de relev&acirc;ncia o desenvolvimento de estudos a respeito dos determinantes quanto ao estado nutricional e o n&iacute;vel de atividade f&iacute;sica de crian&ccedil;as brasileiras, principalmente frente aos consider&aacute;veis n&iacute;veis de obesidade e sedentarismo encontrados nos estudos levantados, que tragam tanto a associa&ccedil;&otilde;es dessas vari&aacute;veis com outras como: sexo, idade, situa&ccedil;&atilde;o econ&ocirc;mica e classe social e que busquem informa&ccedil;&otilde;es complementares em rela&ccedil;&atilde;o &agrave; matura&ccedil;&atilde;o sexual, influ&ecirc;ncias gen&eacute;ticas, ambientais, culturais, socioecon&ocirc;micas e oportunidades para a pr&aacute;tica de atividade f&iacute;sica, considerando que o Brasil corresponde a um pa&iacute;s com diferen&ccedil;as regionais muito grandes e que determinantes relacionados &agrave; sa&uacute;de na inf&acirc;ncia est&atilde;o associados &agrave; obesidade e sedentarismo na adolesc&ecirc;ncia e consequentemente na vida adulta (37,39,40) * </p>      <p><B><I>Agradecimentos</I></B>: Agradecemos a Coordena&ccedil;&atilde;o de Aperfei&ccedil;oamento de Pessoal de N&iacute;vel Superior &ndash; CAPES.</p> <hr>      <p align="center"><b>REFER&Ecirc;NCIAS</b></p>      <!-- ref --><p>1.
