Los factores de riesgo implicados son múltiples; recientemente se ha reportado un incremento de los factores de riesgo cardiovascular en poblaciones jóvenes y se ha sugerido que el perfil de riesgo cardiovascular en esta población puede ser diferente al encontrado en el adulto. En esta revisión se examinaron artículos recientes y relevantes publicados e indexados en diferentes bases de datos que exploran los factores de riesgo más prevalentes en la población joven y se discuten sus causas. La mayoría de los factores de riesgo discutidos corresponden a factores modificables asociados con los estilos de vida. Definir el perfil de riesgo cardiovascular de la población joven aportará herramientas para diseñar estrategias de prevención de la ECV en esta población y podría tener un impacto favorable en la prevalencia de la enfermedad en los próximos años. El seguimiento del comportamiento de los factores de riesgo cardiovascular en el tiempo y de la eficacia de las estrategias preventivas brindará datos invaluables acerca de la etiología y prevención de la ECV.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Cardiovascular disease (CVD) involves several disorders related to the formation and development of atherosclerotic processes. Several risk factors are involved in CVD aetiology; some of them (i.e. age, hypertension, obesity, dislipidemia and diabetes) have been clearly associated, whereas others have a variable level of association. An increase in cardiovascular risk factors has been recently reported in the young population; studies of cardiovascular risk factors in this population have shown that its cardiovascular risk profile could be different from that presented by older populations. This review presents a summary of reported cardiovascular risk factors in the young population and their causes which have been released and indexed in different databases. Most factors discussed are life-habit risk factors and represent direct targets for clinical intervention. We propose that primary CVD prevention should include a more detailed knowledge of the nature of the risk factors concerning the young population and could have a positive impact on CVD prevalence during the next few years.]]></p></abstract>

La etiolog&iacute;a es compleja y    multifactorial, existen factores fisiol&oacute;gicos y bioqu&iacute;micos que sumados a factores    ambientales, act&uacute;an sin&eacute;rgicamente en el inicio y desarrollo de la ECV (2). Los factores    de riesgo (FR) cardiovascular descritos para poblaci&oacute;n adulta mayor (3) han    mostrado ser importantes en el desarrollo de ECV en la poblaci&oacute;n joven. Sin    embargo, se observan j&oacute;venes que desarrollan ECV en ausencia de &eacute;stos (4), lo    cual puede indicar que el perfil de riesgo cardiovascular para esta poblaci&oacute;n    podr&iacute;a ser diferente que el descrito para la poblaci&oacute;n adulta. Se ha propuesto que    la poblaci&oacute;n joven puede tener una respuesta de adaptaci&oacute;n r&aacute;pida a los factores    de riesgo que no permite cambios vasculares demostrables (5). Aunque los    j&oacute;venes presentan tasas de mortalidad menores en comparaci&oacute;n con ni&ntilde;os y    ancianos (6), esta poblaci&oacute;n presenta problemas caracter&iacute;sticos de salud, que sin el  debido tratamiento pueden constituirse en grandes problemas de salud p&uacute;blica.</P>     <P>En la presente revisi&oacute;n resume la informaci&oacute;n actual relacionada con    factores de riesgo para ECV en poblaciones entre los 4 y los 34 a&ntilde;os de edad.    Con base en las palabras clave: joven, cardiovascular y riesgo en espa&ntilde;ol y    <I>young, risk factor</I> y <I>cardiovascular</I> en ingl&eacute;s, se realiz&oacute; una b&uacute;squeda exhaustiva    en las bases de datos MEDLINE, Science Direct, OvidSP, Hinari, Proquest y    las publicaciones libres en la Internet. Se revisaron todos los resultados    encontrados, los cuales fueron aportados por los cuatro autores. Se seleccionaron 153    art&iacute;culos publicados desde el a&ntilde;o 2000 en adelante, que correspondieran a    ensayos cl&iacute;nicos, boletines electr&oacute;nicos o revisiones de tema publicados o en prensa    que estuvieran disponibles en forma impresa o en la Internet o que fueran    emitidos por organizaciones internacionales reconocidas. No se determin&oacute; el nivel de    evidencia de los documentos seleccionados por lo cual las afirmaciones    presentadas en el texto son aproximaciones. No se incluyeron art&iacute;culos no    publicados, tesis o comunicaciones personales. Los resultados se agruparon por factores  de riesgo descritos.</P>     <P>Factores asociados con la ECV en la poblaci&oacute;n joven</P>     <P>Dislipidemia</P>     <P>El colesterol LDL (del ingl&eacute;s, Low Density Lipoprotein) elevado es la    principal causa de EC (7).  Usualmente, las poblaciones j&oacute;venes con trastornos en    l&iacute;pidos y niveles muy altos de LDL tienen formas gen&eacute;ticas de hipercolesterolemia.    El colesterol total y el LDL elevados se han correlacionado con cambios en    la &iacute;ntima media (IMT) como tambi&eacute;n, en la distensibilidad arterial (8-10).    Estudios postmortem de ni&ntilde;os, adolescentes y adultos j&oacute;venes han mostrado la    presencia de estr&iacute;as grasas incluso desde los 3 a&ntilde;os de edad, evidentes desde la    segunda d&eacute;cada de la vida y con progresi&oacute;n gradual a la placa fibrosa a partir de los    20 a&ntilde;os (8). Por otra parte, los niveles bajos de HDL (del ingl&eacute;s, High    Density Lipoproteins) son un factor de riesgo para EC y hacen parte del perfil    aterog&eacute;nico en pacientes obesos (7). Tambi&eacute;n se ha descrito que se correlacionan con    la presencia de lesiones ateroscler&oacute;ticas en estudiantes entre los 13 y 18 a&ntilde;os  (10) y j&oacute;venes entre 25 y 34 a&ntilde;os (8). </P>     <P>Resistencia a la insulina  y Diabetes Mellitus Tipo 2</P>     ]]></body>
<body><![CDATA[<P>La presencia de diabetes mellitus tipo 2 (DMT2) en la poblaci&oacute;n joven se ha    visto incrementada proporcionalmente al aumento en la prevalencia de la    obesidad. Los factores de riesgo gen&eacute;ticos y ambientales para ECV y DMT2 son    similares y contribuyen independientemente a la enfermedad (11).  Se ha encontrado    evidencia que implica la resistencia a la insulina como un precursor de    s&iacute;ndrome metab&oacute;lico en el desarrollo de ECV (12). En la descendencia normoglic&eacute;mica    de pacientes con DMT2, son detectables disfunci&oacute;n endotelial y resistencia a    la insulina aun en ausencia de diabetes franca (13); lo que sugiere que los    factores gen&eacute;ticos que contribuyen a la resistencia a la insulina y la diabetes  tambi&eacute;n pueden influenciar el desarrollo de ECV (14). </P>     <P>Obesidad y S&iacute;ndrome Metab&oacute;lico</P>     <P>La prevalencia de la obesidad ha incrementado dram&aacute;ticamente tanto en    los pa&iacute;ses industrializados como en los subdesarrollados  hasta el punto que la    Organizaci&oacute;n Mundial de la Salud report&oacute; el sobrepeso  y la obesidad como una    epidemia mundial (15,16). La obesidad y la inactividad f&iacute;sica pueden predisponer    a hiperinsulinemia (17), hipertensi&oacute;n (18), ECV (19) y DMT2 (20).  Tambi&eacute;n    pueden causar una auto-imagen negativa y baja autoestima (21), depresi&oacute;n (22)  y problemas sociales o del comportamiento (23).  </P>     <P>Estudios en pa&iacute;ses en desarrollo han mostrado que en pocas familias de    ni&ntilde;os con sobrepeso, &eacute;ste es considerado un factor de riesgo (24) y de hecho, en    los grupos de bajo nivel socioecon&oacute;mico, el sobrepeso se considera un indicador    de buena salud en los ni&ntilde;os (25). Adicionalmente, la prevenci&oacute;n y el tratamiento    de la obesidad entre ni&ntilde;os y adolescentes se relacionan con el g&eacute;nero y con    la educaci&oacute;n materna (26).  La edad entre los 4 y 6 a&ntilde;os es un periodo cr&iacute;tico  para el desarrollo de obesidad (27) y una vez instaurada es de dif&iacute;cil tratamiento (28).</P>     <P>El S&iacute;ndrome metab&oacute;lico (SM), se refiere a un grupo de entidades cl&iacute;nicas    que incluyen  obesidad central, hiperglicemia, dislipidemia e HTA (29). La    definici&oacute;n y significancia nosol&oacute;gica de esta entidad son materia de debate, aunque se    ha aceptado que la expresi&oacute;n completa del s&iacute;ndrome depende de una    interacci&oacute;n compleja entre factores gen&eacute;ticos y modificables relacionados con el estilo    de vida (30). Se ha observado que los individuos con historia parental ECV    tienen mayor probabilidad de desarrollar SM o componentes individuales del SM    (31-33) que los que no los tienen (34) y tambi&eacute;n se ha reportado agregaci&oacute;n    familiar del SM (45). Factores psicosociales como rasgos de personalidad individual    (35,36) y estr&eacute;s (34), factores relacionados con el comportamiento frente a la    salud como el tabaquismo (37), el consumo de alcohol (38) y el ejercicio f&iacute;sico (39),    y factores socioecon&oacute;micos como la educaci&oacute;n y el ingreso familiar se han  asociado con el SM y con componentes individuales del SM y de la ECV (40-42).</P>     <P>La prevalencia del SM se incrementa paralelamente con el incremento en    el &iacute;ndice de masa corporal (IMC) y con el aumento en la prevalencia de la    DMT2 (43). A pesar de que los cambios fisiopatol&oacute;gicos relacionados con el SM    pueden ser detectados a edad temprana (44,45), pocos estudios se han enfocado    en poblaciones j&oacute;venes (46-48). La prevalencia del SM entre 12 y 19 a&ntilde;os en    los EEUU es del 4,2 % en mujeres y 6,1% en hombres, mientras que en Brasil,    la prevalencia en el mismo rango de edad es del 3,2 % para SM y del 30,0 %  para sobrepeso en adolescentes de ambos sexos (49). </P>     <P>Factores hormonales</P>     <P>En mujeres, la menarca y la menopausia son dos hitos importantes en la    vida reproductiva y la menarca temprana que se ha asociado con el IMC (50-53)    y con otros factores de riesgo CV (54,55). La ingesta cal&oacute;rica excesiva y la    actividad f&iacute;sica reducida en los a&ntilde;os tempranos de la vida puede ser causa de    menarca temprana (56), mientras que las ni&ntilde;as con menara temprana tienen unos    cambios mas delet&eacute;reos en la insulina, glucosa, presi&oacute;n arterial y l&iacute;pidos en la  ni&ntilde;ez, adolescencia y adultez temprana (54,55). </P>     <P>Estr&eacute;s y otros factores psicosociales</P>     <P>La poblaci&oacute;n joven vive un per&iacute;odo caracterizado por grandes    cambios psicosociales, por la b&uacute;squeda de un estilo de vida propio y por el rechazo    de modelos propuestos (6). El panorama de la situaci&oacute;n social de nuestro pa&iacute;s    plantea un agravante para la poblaci&oacute;n joven, aunque la poblaci&oacute;n adolescente    y joven presenta altas tasas de mortalidad relacionadas con violencia y suicidio,    un porcentaje de dicha poblaci&oacute;n podr&iacute;a estar siendo afectada por ECV al entrar  en la vida adulta luego de a&ntilde;os de estar expuestos a estilos de vida no saludables.</P>     ]]></body>
<body><![CDATA[<P>Los factores psicosociales pueden influenciar las salud f&iacute;sica (57). Entre    los factores psicosociales se incluye la exposici&oacute;n al estr&eacute;s, pobres soportes    sociales y redes sociales limitadas (58,59). El estr&eacute;s ha sido reportado como un    factor etiopatog&eacute;nico y pron&oacute;stico (60) que podr&iacute;a interaccionar con factores de la    dieta, del estilo de vida y otros factores biol&oacute;gicos. Aunque se han    relacionado diferentes factores estresantes tanto en estudios experimentales como en    estudios poblacionales (61,62), sigue siendo un problema la definici&oacute;n y    cuantificaci&oacute;n del estr&eacute;s (63). Los niveles de presi&oacute;n arterial se pueden elevar en estados    de estr&eacute;s agudo o persistir elevados por la acci&oacute;n de factores de riesgo    cardiovascular lo cual lleva a disfunci&oacute;n endotelial en el estr&eacute;s cr&oacute;nico (5).  La preocupaci&oacute;n    por las condiciones sociales se asocia con un riesgo doble de IAM y 1,5 de EC    en poblaciones estadounidenses (64) y se asocia con una mayor probabilidad    de involucrarse en comportamientos de alto riesgo como el tabaquismo,  consumo de alcohol, comer en exceso, entre otros. </P>     <P>Otro factor psicosocial importante es la influencia de los pares en la    adopci&oacute;n de h&aacute;bitos saludables. En la adolescencia, la influencia en los    comportamientos del joven cambia de la familiar a la de los grupos de pares. Se han reportado    que tener amigos que practiquen comportamientos saludables es un predictor    fuerte para que los adolescentes los adopten tambi&eacute;n (65). Tambi&eacute;n se han    descrito asociaciones fuertes entre afiliaciones al grupo de pares y su adopci&oacute;n del  tabaquismo, alcohol o ambos (66).</P>     <P>Herencia</P>     <P>La asociaci&oacute;n de los factores de riesgo cardiovascular y la historia parental    de DM (67), IAM (68-71), HTA y obesidad (67) es fuerte . Algunos estudios    han encontrado asociaci&oacute;n entre tabaquismo y historia familiar de ECV (71). El    SM muestra agregaci&oacute;n familiar (72), al igual que ocurre con la    EC, hipercolesterolemia, HTA y DM (73,74). La agregaci&oacute;n familiar puede    sugerir predisposici&oacute;n gen&eacute;tica, comportamientos aprendidos nocivos para la    salud cardiovascular, o ambas (72). Un estudio en gemelos mostr&oacute; que en la    juventud, la muerte por ECV es influenciada por factores gen&eacute;ticos, mientras que    los efectos gen&eacute;ticos disminuyen en edades avanzadas en las cuales los  factores ambientales pueden jugar un papel m&aacute;s importante (75,76)</P>     <P>Dieta aterog&eacute;nica </P>     <P>La interacci&oacute;n entre genes y nutrientes puede regular los niveles de    colesterol s&eacute;rico, pero los mecanismos de su interacci&oacute;n son insuficientemente    conocidos (77-79). La dieta DASH (Dietary assessment to Stop Hypertension) y la    dieta mediterr&aacute;nea han demostrado impacto ben&eacute;fico en la progresi&oacute;n de la ECV.    En un estudio en 134 cadetes j&oacute;venes (media de edad de 18,1 a&ntilde;os) con    niveles normales de colesterol, se describi&oacute; que el cambio del aceite de girasol por  aceite de oliva redujo un 12 % los niveles de colesterol s&eacute;rico a las 10 semanas (80).</P>     <P>La actitud, las pr&aacute;cticas y el estilo de alimentos pueden ser    influenciados desde la ni&ntilde;ez por los padres o abuelos. Ya en la adolescencia y la juventud,    el control parental de la comida es poco y hay una tendencia en el ni&ntilde;o a perder    la habilidad de atender a los sentidos internos de hambre y saciedad. Por otro    lado, la industria alimentaria no estimula el consumo de frutas y vegetales y    promueve el consumo de alimentos altos en az&uacute;cares y grasas en grandes porciones.    La disponibilidad de estas opciones alimentarias ha incrementado (81), los    determinantes individuales de los ni&ntilde;os y j&oacute;venes como son las preferencias    alimentarias y la percepci&oacute;n de la alimentaci&oacute;n saludable son influenciados comercialmente  y le dan prioridad a mensajes nutricionales distorsionados (65).</P>     <P>Tabaquismo</P>     <P>Es un fen&oacute;meno creciente entre adolescentes y j&oacute;venes (82). Puede    predisponer a infarto agudo de miocardio (IAM) prematuro posiblemente por la inducci&oacute;n    de espasmo de arterias coronarias sanas (83-85).  El tabaquismo ha sido    reportado extremadamente frecuente entre pacientes coronarios j&oacute;venes, ya que el 96    % de &eacute;stos fueron fumadores hasta la aparici&oacute;n del evento (86) y est&aacute; asociado    con aterosclerosis coronaria avanzada en individuos j&oacute;venes (87). Por otro lado,    el fumador pasivo se relaciona con s&iacute;ntomas respiratorios en el ni&ntilde;o y muerte    por c&aacute;ncer pulmonar en el adulto (88). En el joven produce disfunci&oacute;n endotelial    en arterias sist&eacute;micas dependiente de la dosis y equivalente al grado de    alteraci&oacute;n vascular encontrada en fumadores activos de la misma edad (88).  La    exposici&oacute;n corta al humo del tabaco ambiental tambi&eacute;n se ha asociado a    cambios fisiopatol&oacute;gicos (89). El consumo de tabaco puede modificar nocivamente    el perfil lip&iacute;dico (90,91), y puede estar asociado con una disminuci&oacute;n de la IMT  de la car&oacute;tida com&uacute;n (92).</P>     <P>Consumo de alcohol </P>     ]]></body>
<body><![CDATA[<P>El consumo de alcohol se incrementa durante la adolescencia y la juventud    (93), particularmente durante la transici&oacute;n fuera del colegio y disminuye a medida    que el adulto joven llega a la mitad de sus treinta a&ntilde;os (94). Se estima que en    el mundo cerca de 2 600 000 de personas lo consumen ya sea en forma    ocasional, habitual, abusiva o adictiva (95). En Colombia, el programa presidencial    RUMBOS estim&oacute; en 2001, que el 89,7 % de los estudiantes universitarios eran  consumidores habituales de alcohol et&iacute;lico (96). </P>     <P>Los efectos del alcohol en la salud dependen de la cantidad que se ingiere    y de los patrones de consumo, es t&iacute;pica la presentaci&oacute;n de curva J, que muestra    los efectos del alcohol en la salud (97). En dicha curva, se ha descrito que el    consumo bajo de alcohol est&aacute; asociado con una reducci&oacute;n de la mortalidad general    del 18,0 % y de enfermedad cardiovascular del 30,0 % (98). De otra parte, el    consumo excesivo puede conferir riesgo de ECV (99-101), se ha asociado    directamente con el tabaquismo (102) y est&aacute; asociado con una tasa mayor de  mortalidad general. </P>     <P>Sedentarismo</P>     <P>La OMS report&oacute; que el estilo de vida sedentario est&aacute; dentro de las 10    causas m&aacute;s importantes de muerte y discapacidad a nivel mundial. El    sedentarismo puede elevar los niveles lip&iacute;dicos al rango de riesgo para el SM y puede    actuar alterando la reserva cardiovascular mediada por el flujo sangu&iacute;neo    coronario. Los niveles saludables de actividad f&iacute;sica en la ni&ntilde;ez pueden prevenir la  obesidad en la ni&ntilde;ez y m&aacute;s adelante en la vida adulta (66).</P>     <P>Los computadores, los video-juegos, la internet y la televisi&oacute;n han    contribuido a la inactividad en los j&oacute;venes, junto con la falta de disponibilidad de tiempo    libre para hacer ejercicio debido a las obligaciones acad&eacute;micas, el mayor uso de    autom&oacute;viles y motos, en lugar de las caminatas o el uso de bicicleta para    desplazarse y el hecho de no tener de espacios urbanos adecuados para esto.    Raitakari report&oacute; que los ni&ntilde;os y adolescentes tienen una tendencia a adoptar un estilo    de vida sedentario que uno activo (103).  En 314 estudiantes universitarios    venezolanos entre 17 y 29 a&ntilde;os de edad, Moya-Sifontes describi&oacute; que el 31,5 %    eran insuficientemente activos, 47,5 % eran suficientemente activos y 21.02 %    ten&iacute;an un nivel de actividad f&iacute;sica elevado (104). Los resultados de su estudio    fueron comparables con el trabajo en la Universidad de Chile (105) y otro estudio  en poblaci&oacute;n colombiana (106).</P>     <P>La revisi&oacute;n presentada pretende resumir la informaci&oacute;n disponible respecto    a los factores de riesgo cardiovascular en poblaciones j&oacute;venes a nivel mundial.    A pesar de que se consultaron bases de datos reconocidas y de consulta    frecuente, esta revisi&oacute;n tiene la desventaja de no incluir otras bases de datos protegidas    por suscripci&oacute;n y otras trabajos no publicados. Sin embargo, puede verse que    los factores de riesgo tradicionales podr&iacute;an ser los principales responsables de    la ECV en el joven. Hacen falta m&aacute;s estudios dirigidos a determinar el perfil    de riesgo cardiovascular de esta poblaci&oacute;n y entrar a analizar factores propios    de nuestra poblaci&oacute;n joven. La intervenci&oacute;n para reducir los factores de riesgo    desde la ni&ntilde;ez ha mostrado ser segura y exitosa (107). La prevenci&oacute;n primaria en    la poblaci&oacute;n joven podr&iacute;a mejorar sus condiciones de salud en el futuro, ya    que muchos de los h&aacute;bitos de riesgo que conllevan riesgo para la ECV se    adquieren en edades tempranas y puede ser m&aacute;s f&aacute;cil inculcar h&aacute;bitos saludables en    los j&oacute;venes que modificar comportamientos en la edad adulta o luego del inicio de  la ECV &#168;</P>     <P align="CENTER"><B><FONT SIZE="3">REFERENCIAS</FONT></B></P>     <P>&nbsp;</P>     <!-- ref --><P>1.
