First surname, second surname, and given name, according to the order
displayed

Address: Please indicate the address to which
you want the journal to be sent: complete address and corresponding telephone
number.

Municipality: Please write the name of city/municipality of
your address.
•
State: Please write the name of the state/department of the address
where the journal will be sent.

Country: Please write the name of the country where you reside.
•
Date: Please write the date on which subscription is being completed,
starting with day and followed by the month and the year.
